Comparison of breath-hold multishot echo-planar and respiratory triggered fast-spin-echo sequences for T2-weighted MRI of liver lesions.

The purpose of this study was to evaluate the usefulness of multishot echo-planar imaging in detecting liver tumors in comparison with respiratory triggered T2-weighted fast-spin-echo (FSE) imaging. Thirty-two patients with 70 focal liver lesions were imaged using a 1.5-T high speed MR imager. Eight-shot echo-planar images covering the whole liver were acquired during a single breath-hold period. FSE images were acquired with respiratory triggering in approximately 4 minutes. Lesion detectability and image quality of the two pulse sequences were analyzed qualitatively. Quantitative analysis was performed by means of signal-to-noise and tumor-liver contrast-to-noise analysis. Lesion detectability was comparable in both solid (86.3% vs 90.2%: .3 < P < .5) and nonsolid lesions (89.5% vs 100%: .3 < P < .5) between echo-planar and FSE images. Echo-planar imaging provided significantly reduced image artifact, better lesion conspicuity, and anatomic detail compared with FSE imaging. The signal-to-noise and contrast-to-noise ratios of echo-planar images were significantly higher than those of FSE images. Breath-hold eight-shot echo-planar imaging can be an alternative to T2-weighted FSE imaging because it can provide comparable image quality in a substantially decreased acquisition time.

Application of a self-expanding metallic stent to a strictured esophagojejunostomy.

We placed a Gianturco self-expanding metallic stent across the recurrent stricture of an esophagojejunostomy in a patient with gastric cancer. Though excellent passage of food resulted, intractable reflux occurred. Two months later the patient succumbed to recurrent tumor. At autopsy, the stent was patent and was partially covered by esophageal mucosa. There were narrow but deep ulcers around the stent hooks. The Gianturco metallic stent may provide an additional option for treating recurrent enteric strictures after other methods fail. Further refinements of the technique appear necessary.

Plasma thrombomodulin values and hepatorenal function in the elderly.

To define the clinical significance of plasma thrombomodulin (TM) values in elderly, we examined plasma TM in healthy young subjects, healthy elderly subjects and patients with cerebral infarction sequelae. We also studied the relationship with effective renal plasma flow (ERPF) and with the liver's protein-production ability. The TM values of healthy elderly subjects were higher than those of healthy young subjects. There existed an inverse correlation between TM values and ERPF. Accordingly, high TM values might significantly influence renal arteriosclerosis. From the inverse correlation identified between TM and serum cholinesterase, it was estimated that high TM might appear in conjunction with the liver's protein production ability. Patients with cerebral infarction showed higher plasma TM values. It is thought that angiopathy has been maintained in patients as the anamnesis of cerebral infarction even though it occurred in the past. The TM values of patients with diabetes mellitus (DM) were higher than those without it. Moreover, the TM values of patients with DM complicated by retinopathy were higher than those uncomplicated by retinopathy. It is therefore estimated that increased TM might occur with angiopathy resulting from DM. A possibility thus exists that plasma TM could be utilized as one of the markers for endothelial injury.

Superselective bronchial artery embolization for hemoptysis with a coaxial microcatheter system.

PURPOSE: To compare the effectiveness and safety of superselective bronchial artery embolization with that of nonsuperselective embolization in the control of hemoptysis. MATERIALS AND METHODS: Retrospective case analysis was done for 47 patients with hemoptysis originating from a variety of causes. In 22 patients, embolization was performed superselectively using a microcatheter inserted into the bronchial artery beyond the spinal or mediastinal branches (superselective group). In the remaining 25 patients, embolization was performed at the opening of the bronchial artery with a 5-F catheter (nonsuperselective group). RESULTS: Initial hemoptysis control rates were 96% (21 of 22) in the superselective group and 88% (22 of 25) in the nonsuperselective group. Cumulative hemoptysis control rates of the superselective and nonsuperselective groups were 80% and 67% at 6 months, 79% and 56% at 1 and 2 years, and 79% and 48% at 3 years, respectively (not significant; generalized Wilcoxon test). One major complication (spinal infarction) occurred in the nonsuperselective group. CONCLUSIONS: Superselective embolization is safer and more effective way to control hemoptysis than the ordinary (nonsuperselective) method.
