[Prognosis of the patients with prostate cancer clinically confined within the pelvis].

Between 1975 and 1989, 90 patients with prostate cancer in clinical stage A2 to C underwent pelvic lymphadenectomy. Median follow-up period was 38 months. Almost all of the patients with pN0-1 (49) and 4 of pN2 were treated by curative treatment, such as radical prostatectomy (7) or radiation therapy (45). The remaining pN2 (26), pN3 (4) and pM1 LYM (6) received endocrine therapy. Pelvic lymph node metastasis were noticed in 50 cases (56%). Rates of positive node and degree of nodal extension were related to clinical stage and histological grade. Disease-free survival of the patients with pN0-1 was better than that of the patients with more than pN2. There was no difference in disease-free survival between the patients with pN0 and pN1. We concluded that the patients with pN0 and pN1 were the candidates for curative therapy and recommend that the patients with more than pN2 be treated with endocrine therapy.

[Effect of combination therapy with carumonam and clindamycin on severe infections in patients with hematologic malignancies].

We evaluated the efficacy of a combination therapy with carumonam (CRMN) and clindamycin (CLDM) on severe infections in patients with hematologic malignancies. Fifty three patients were included in this study. The efficacy of the combination therapy was evaluated according to the criteria by TAKAKU et al. Fourteen cases were evaluated as excellent, and 24 cases were as good, with a total rate of effectiveness of 71.7% (38/53). It should be noted that the rate of effectiveness in patients having less than 100/microliters neutrophils was 77.3% (17/22). Adverse effects were observed in 2 patients (3.7%). One case was hepatotoxicity and the other was nephrotoxicity. Both were mild and transient, however. These observations suggested that the combination therapy with CRMN and CLDM was effective and safe for the treatment of severe infections in patients with hematologic malignancies.

[The ornithine decarboxylase activity of the gastric remnant mucosa: the effect of the duodenal juice with bile on the gastric mucosa].

Using male Wister rats, the ornithine decarboxylase (ODC) activity in the fundic mucosa has been determined from the duodenal juice with bile of a reflux model. The activity at 24 hours postoperatively presented a value approximately 18 times higher than the preoperative level. Though this level declined subsequently, it still was approximately two times higher four weeks later. The ODC activity in the corporal mucosa of humans was compared in 25 normal persons, in 21 patients with a gastric adenoma in 29 patients with a gastric cancer, in 20 patients who had undergone a Billroth I operation and in 20 patients who had undergone a Billroth II operation. No differences in ODC activity were observed among those with a gastric adenoma, a gastric cancer, and normal cases but significantly higher values were seen in cases with a remnant stomach, particularly those who had undergone Billroth II reconstructive surgery. Further, this activity tended to be especially high from 5 to 15 years postoperatively in cases with a gastric remnant.

[Malignant lymphoma associated with Addison's disease].

A 62-year-old man was admitted to our hospital in Oct. 1986, because of easy fatigability, generalized pigmentation and lymph node swelling. Radiological examination showed massive adrenal tumor and two lung tumors. Endocrinological findings showed cortisol 8.3 micrograms/dl, aldosterone 35.3 ng/dl, ACTH 348 pg/ml, and non reacting pattern was observed in ACTH-overloading test, suggesting a presence of Addison's disease. A diagnosis of malignant lymphoma (Non-Hodgkin's lymphoma, diffuse large cell type) was made by lymph node biopsy, and radiotherapy to bulky bilateral adrenal tumors and cervical lymph nodes was performed with some response. Then, treatment with VEPA and PEP-VP regimens were carried out, and adrenal tumor became remarkably small in size. However, adrenal function did not recover. He relapsed in Jan. 1987 and died of multiple organ failure.

[Anesthetic management for resection of cardiac pheochromocytoma].

Cardiac pheochromocytoma is a rare disease, and there have been few reports concerning anesthetic management for the resection. This is a report of a 46-year-old man with cardiac pheochromocytoma with the perioperative changes in plasma catecholamine concentrations. Anesthesia was maintained with enflurane (0.5-2.0%) and 50% nitrous oxide in oxygen. Additional fentanyl (900 micrograms) was given as required. Intravenous administration of nitroglycerin in a dose of 5-20 micrograms.kg-1.min-1 was effective for prevention of intraoperative hypertension. After cardiopulmonary bypass and cardioplegic arrest were instituted, the manipulation of the tumor had no effect upon the blood pressure. A serious increase in plasma norepinephrine concentration occurred during both bicaval cannulation and manipulation of the tumor. Although the anesthetic management for resection of adrenal pheochromocytoma has been well-documented, that of cardiac pheochromocytoma requires following specific considerations. 1) Serious hypertension and arrhythmias may occur during manipulation of the heart. 2) It is possible to dissect the tumor safely after isolation of the heart from systemic circulation. It is of great interest that the blood pressure remained almost unchanged during cardiopulmonary bypass despite a marked increase in catecholamine concentrations. 3) Operative mortality rate is high.

A case of pseudothrombophlebitis due to inguinal synovial cyst.

A case of pseudothrombophlebitis of the whole right leg by an inguinal synovial cyst that had a compressed femoral vein is presented. Differential diagnosis is important in order to avoid hemorrhagic complications. The usefulness of ultrasonography and necessity of phlebography are emphasized. Radical excision of the cyst is recommended.

Specific binding of a hepatoma nuclear factor to the NF.kappa B/H2TF1 recognition motif found in the C4 promoter, but not in the Slp promoter.

The fourth component of C (C4) and sex-limited protein (Slp) genes of FM strain mice show more than 95% nucleotide identity both in the coding and the about 2 kb of 5' flanking region, but are distinct in their regulation. Although C4 is a constitutive gene, Slp is an androgen-dependent gene and shows negligible basal level expression. We have previously shown that this difference in basal expression is determined by a region of about 400 bp immediately 5' of the transcriptional start site of the C4 and Slp genes. In this report, we demonstrate a specific binding site for a HepG2 nuclear extract in this region of the C4 gene by gel retardation and DNase I footprinting experiments. Nucleotide sequence of this binding site is very similar to the recognition sequence of well characterized transcription factors, NF.kappa B and H2TF1. Synthetic oligonucleotides representing the binding sites of the H-2Kb class I and IL-6 genes for these factors compete with the C4 promoter for the complex formation with a HepG2 nuclear factor. Due to the nucleotide deletion and substitution, the corresponding region of the Slp gene lacks this sequence motif as well as binding activity to the HepG2 nuclear factor.

Neuronal death in the developing sexually dimorphic periventricular nucleus of the preoptic area in the female rat: effect of neonatal androgen treatment.

Neonatal treatment of female rats with androgen decreases the nuclear volume of the anteroventral periventricular nucleus of the preoptic area (AVPv-POA). In order to examine the effect of androgen on the neural substrates in the developing AVPv-POA which show a sexual dimorphism in nuclear volume, the cell death pattern in the AVPv-POA was compared between normal females and androgenized females. Wistar female rats were treated with 50 micrograms of testosterone propionate (TP) for 5 days from the day of birth. Degenerating cells (pyenotic cells) and normal cells were counted in every third section from days 1 to 13 of life. The rate of pycnotic cells to 1000 cells in TP-treated females sacrificed at days 7 and 10 was significantly higher than that in normal females. These results may suggest that neonatal androgen regulates neuronal death in the AVPv-POA, decreasing the number of neurons in the nucleus.

Composite motifs and repeat symmetry in S. pombe centromeres: direct analysis by integration of NotI restriction sites.

S. pombe centromeres are large and complex. We introduced a method that enables us to characterize directly centromere DNAs. Genomic DNA fragments containing cen1, cen2, or cen3, respectively, are made by cleaving NotI sites integrated on target sites and are partially restricted for long-range mapping in PFG electrophoresis. The 40 kb long cen1 consists of two inverted approximately 10 kb motifs, each containing centromeric elements dg and dh, flanked by a central region. In cen2, three motifs are arranged in inverted and direct orientations with flanking domains, making up the approximately 70 kb long repetitious region. In cen3, approximately 15 copies of dg-dh constitute a region longer than 100 kb. A set of inverted motifs with an approximately 15 kb central region might be a prototype for the S. pombe centromeres. The motifs appear to play a role in chromosome stability and segregation. Their action may be additive, and the mutual directions of dg and dh inside a motif may not be essential for function.

The coexistence of substance P- and glutamic acid decarboxylase-like immunoreactivity in entopeduncular neurons of the rat.

The coexistence of substance P- and glutamic acid decarboxylase-like immunoreactivity in cell bodies was investigated in the entopeduncular neurons of the rat by the double-immunofluorescence method using species-specific antibodies. Over half of the substance P-like immunoreactive cells were GABAergic. Double-labeled neurons were generally seen in the rostral to the middle region of the entopeduncular nucleus. Since the rostral portion of the entopeduncular nucleus mainly projects to the lateral habenular nucleus, it is suggested that double-labeled neurons in the entopeduncular nucleus largely project to the lateral habenular nucleus which is involved in the limbic system.

Chromosomal assignment of woodchuck hepatitis virus (WHV) DNA integration sites in a woodchuck hepatocellular carcinoma-derived cell line (WH257GE10).

The chromosomal sites of woodchuck hepatitis virus (WHV) DNA integration were identified in a woodchuck hepatocellular carcinoma-derived cell line (WH257GE10) by the in situ hybridization technique using 3H-labelled WHV whole genome (WHV 2) as a probe. The G-banded chromosome spreads from WH257GE10 were identified and diagrammed schematically according to their band patterns. WHV DNA was integrated into 2 sites: 33 region of the long arm of chromosome 6 (6q33) and 31 region of the long arm of chromosome 8 (8q31). Chromosomal sites of WHV DNA integration were stable during successive passage periods analyzed in vitro.

Differential colocalization of neuropeptide Y- and methionine-enkephalin-Arg6-Gly7-Leu8-like immunoreactivity in catecholaminergic neurons in the rat brain stem.

The present study, using a combination of catecholamine (CA) histofluorescence and peptide immunocytochemistry in the same tissue sections, investigated the coexistence of neuropeptide Y (NPY) and methionine-enkephalin-Arg6-Gly7-Leu8 (MEAGL)-like immunoreactivity (LI) in catecholaminergic neurons of colchicine-treated rat brain stems. Of the total number of catecholaminergic neurons in the A1/C1, A2/C2, A3, A4, and A6 regions approximately 83, 28, 98, 76, and 36%, respectively, contained both NPY-LI and CA. Of the total number of catecholaminergic neurons in A1/C1, A2/C2, A3, and A5 regions, approximately 47, 4, 8, and 17%, respectively, contained both MEAGL-LI and CA. Moreover, about 24% of the catecholaminergic neurons in the A1/C1 region contained both NPY- and MEAGL-LI. Neither the noradrenergic neurons (A7) in the pons nor any of the dopaminergic neurons in the midbrain (A8, A9, A10) contained NPY- or MEAGL-LI. Neurons containing both NPY- and MEAGL-like immunoreactive peptides without CA were not found in the rat brain stem. These findings indicate that catecholaminergic neurons in the brain stem of the rat can be subdivided into distinct subgroups on the basis of the coexistence of specific peptides.

Human skin histology using high-resolution echography.

Normal and abnormal human skin echograms were made using new 25-MHz and 40-MHz ultrasonic sector scanners, and were compared with histologic sections. The malpighian layer of the epidermis was demonstrated as an echolucent zone under a strongly echogenic, linear corneo-malpighian junction in normal palms and soles. Cross sections and longitudinal sections of hair follicles and hypertrophic sebaceous glands were also demonstrated as echolucent structures.

Purification of angiotensin-converting enzyme from human intestine.

Angiotensin-converting enzyme (ACE) activity in the intestinal whole homogenate was showed as three peaks on a column of Sephacryl S-300 HR gel filteration. Over 90% of total ACE activity was found in a soluble fraction separated with an ultracentrifuge of the intestinal homogenate, and the ACE activities were detected as two peaks on the same column. On the other hand, two peaks of ACE activities were found in a membrane-bound fraction of treated with trypsin on the Sephacryl column and confirmed with the two peaks of the soluble fraction, while the fraction extracted with Triton X-100 of the membrane-bound fraction showed only one peak as major peak. All ACE peaks were inhibited by addition of EDTA or captopril and by absence of chloride ion completely. We purified one ACE from the soluble fraction by lisinopril-linked Sepharose 6B affinity column chromatography and Cellulofine GCL-200 gel filteration. This enzyme was a 1323-fold purification and its final recovery was 25%. The molecular weight of this enzyme (180,000) was larger than that of ACE from human kidney (170,000), estimated by 7.5% SDS-PAGE. The Km value of the enzyme for HHL was 2.1 mM. The enzyme activity was competitively inhibited by captopril.

A full length cDNA clone for the alkaline protease from Aspergillus oryzae: structural analysis and expression in Saccharomyces cerevisiae.

We have cloned and determined the nucleotide sequence of a cDNA fragment for the entire coding region of the alkaline protease (Alp) from a filamentous ascomycete Aspergillus oryzae. According to the deduced amino acid sequence, Alp has a putative prepro region of 121 amino acids preceding the mature region, which consists of 282 amino acids. A consensus sequence of a signal peptide consisting of 21 amino acids is found at the N-terminus of the prepro region. The primary structure of the mature region shares extensive homology (29%-44%) with those of subtilisin families, and the three residues (Asp 32, His 64 and Ser 221 in subtilisin BPN') composing the active site are preserved. The entire cDNA, coding for prepro Alp, when introduced into the yeast Saccharomyces cerevisiae, directed the secretion of enzymatically active Alp into the culture medium, with its N-terminus and specific activity identical to native Aspergillus Alp.

Coexistence of catecholamine and methionine enkephalin-Arg6-Gly7-Leu8 in neurons of the rat ventrolateral medulla oblongata. Application of combined peptide immunocytochemistry and histofluorescence method in the same vibratome section.

An overlapping distribution of catecholamine-containing cells and proenkephaline-A derived peptide-containing neurons have been identified in the rat medulla oblongata. However, it is not evident whether the coexistence of these bioactive substances occurs in the same neurons or not. Therefore, we examined the coexistence of catecholamine and methionine-enkephalin-Arg6-Gly7-Leu8 (MEAGL), a proenkephaline-A derived peptide, using a combination of histofluorescence and peroxidase-anti-peroxidase (PAP) immunohistochemical (modified formaldehyde-glutalaldehyde (Faglu)) methods on the same tissue sections. We found one third of A1/C1 catecholamine fluorescent cells show MEAGL-like immunoreactivity.
