Heterogeneity of plasma and urinary immunoreactive calcitonin in patients with medullary carcinoma of the thyroid.

The immunological heterogeneity of calcitonin (CT) was examined in fractions separated by gel filtration (Bio Gel P-30) from plasma and urine samples of two patients with medullary carcinoma of the thyroid obtained after calcium infusion and after thyroidectomy. Five distinct forms of immunoreactive CT were observed by gel filtration of plasma before treatment; one was eluted with the monomer CT and the other four were of higher molecular weight. The concentrations of all five forms increased during calcium infusion (4 mg/kg, 10 min) and decreased after the infusion, accompanied by a rapid change in concentration of the monomer form. After thyroidectomy, the level of the monomer form decreased faster than the levels of other forms. The main form in the urine was eluted in the same portion as a bigger form (IIIb) in the plasma, not with the monomer form, after thyroidectomy. One antiserum (antiserum I) gave a higher estimation for the IIIb form and the predominant form in the urine than another antiserum (antiserum II). These findings suggest the identity of these two forms. Blood CT in various forms, including the monomer, may be converted to form IIIb in the kidney and excreted in that form.

Morphologic and biochemical characteristics of human kidney cells in vitro.

In vitro cultivated cells derived from normal human renal cortex were characterized morphologically and biochemically. Although the epithelial monolayer was composed of heterogeneous cells, it included cells with a surface structure similar to microvilli as well as some resembling the desmosome between neighboring cells. Enzymatic studies revealed a marked decrease in alkaline phosphatase activity, and the activity of gamma-glutamyl transpeptidase was also reduced to about one-fifth of that in the original tissue. The electrophoretic mobility of the enzyme was not identical with that of normal kidney or of the novel enzyme in renal neoplastic tissue. Lactate dehydrogenase activity was similar to that of normal kidney tissue but the isozyme pattern was completely inverted. These cells responded to the addition of 10 ng per ml of parathyroid hormone in culture medium and there was a 33 fold increase in intracellular cyclic adenosine monophosphate. Estrogen specific binding protein was not detectable in the monolayer cells. These results clearly indicated that the biologic transformation observed in the cultivated normal cells was not attributable to simple fetalism or dedifferentiation, but was a more complicated process.

[Reliability and limitation of various diagnostic methods including nuclear medicine in myocardial disease (author's transl)].

Electrocardiography (ECG), echocardiography, nuclear method, cardiac catheterization, left ventriculography (LVG) and endomyocardial biopsy (biopsy) were performed in 40 cases of cardiomyopathy (CM), 9 of endocardial fibroelastosis (EFE) and 19 of specific heart muscle disease (SHMD), and the usefulness and limitation of each method was comparatively estimated. In CM, various methods including biopsy were performed. The 40 patients were classified into 3 groups, i.e., 1) hypertrophic (17), 2) dilated (20) and 3) non-hypertrophic . non-dilated (3) on the basis of left ventricular ejection fraction (LVEF) and hypertrophy of the ventricular wall assessed by LVG and/or echocardiography. The hypertrophic group was divided into 4 subgroups: 9 septal, 4 apical, 2 posterior and 2 anterior. M-mode scan was useful for detecting hypertrophy at the site of the ventricular septum and posterior wall, but not at the site of the anterior wall and apex. The hypertrophy was detected in 18 out of 20 cases using nuclear method. The posterior wall was hypertrophic but the septum was normal in 2 cases. In 2 of 3 non-hypertrophic . non-dilated cases, the left ventricle was oval in shape by LVG, echocardiography was normal, but significant pathological changes were seen in the biopsy, and there were abnormal ECG findings. There was no correlation between the ECG findings and the site of ventricular hypertrophy. Left ventricular ejection fraction measured by LVG (Kasser) had a closer correlation to LVEF obtained by nuclear method (multigated blood pool scan; r = 0.97) than LVEF by echocardiography (Teichholz; r = 0.79), although LVEF obtained by nuclear method was slightly higher than LVEF obtained by LVG. Myocardial perfusion defect was found in 10 of 20 cases of dilated cardiomyopathy (DCM) and the site of relative hypokinesis coincided with the site of the defect in 6 of 7 cases. A semi-quantitative myocardial perfusion defect index (PDI) and histo pathological contractility failure index (HCFI) obtained by the biopsy were devised. HCFI is the score of myocardial degeneration, fragmentation and fibrosis. The PDI plus HCFI had a close correlation with LVEF (r = -0.898). In 9 cases of DCM, LVEF was more reduced than right ventricular ejection fraction. The perfusion defect was also found in 4 cases of EFE and 4 cases of SHMD, i.e., sarcoidosis, postmyocarditis, Kugelberg-Welander disease and cardiac tumor. We conclude that the nuclear study is useful in assessing the site of the abnormal ventricular thickening, perfusion defect and ventricular function. Echocardiography is most useful in detecting ASH. The biopsy gives the sole diagnostic clue, especially in non-hypertrophic . non-dilated cardiomyopathy. ECG is useful in all cases but correlation with the site of disproportional hypertrophy was not obtained.

The effect of oestrogens on human calcitonin secretion after calcium infusion in elderly female subjects.

The effect of oestrogen administration (4-6 weeks) on the response of human calcitonin (hCT) secretion to 5 min calcium infusions was studied in ten elderly women. There was no significant difference in mean basal plasma hCT levels before and after oestrogen administration. However, the mean increment in plasma hCT in response to calcium infusion (delta hCT) increased significantly (P < 0.001) from 21.9 +/- 6.6 (mean +/- SE) before treatment, to 79.6 +/- 15.5 ng/l after oestrogen administration. Mean serum calcium levels decreased significantly (P < 0.001) from 2.42 +/- 0.06 before to 2.19 +/- 0.07 mmol/l after oestrogen treatment. Mean plasma immunoreactive PTH (iPTH) levels increased significantly (P < 0.05) from 521 +/- 41 before to 696 +/- 96 ng/l after oestrogen treatment. To exclude out the possibility that the decreased serum calcium level itself might have influenced delta hCT, 1 alpha-hydroxycholecalciferol (1 alpha-OH-D3) was administered with oestrogens. While this resulted in a slight increase in serum calcium level, there was no significant difference in delta hCT in response to calcium infusion following oestrogen treatment alone, and after combination therapy of oestrogen and 1 alpha-OH-D3. The primary action of administered oestrogen may be in stimulating hCT secretion which results in a decrease in plasma calcium concentration and an increase in plasma iPTH level.

Role of the renin-angiotensin system in enhancing the aldosterone response to adenocorticotropin during acute sodium depletion in normal subjects.

The role of the renin-angiotensin system in enhancing aldosterone responsiveness to ACTH during acute sodium depletion was studied in 14 healthy medical students. Acute sodium depletion was achieved by oral treatment with 80 mg furosemide and 200 mg SQ 14,225 for 1 day. The im administration of 250 micrograms alpha ACTH-(1-24) or vehicle was performed at 0800-0900 h both on the day after ad libitum diet (control) and 1 h after the oral administration of 50 mg SQ 14,225 on the day after acute sodium depletion. Treatments with furosemide and SQ 14,225 before both ACTH and vehicle administration induced a reproducible sodium depletion, accompanied by a marked increase in PRA and no significant increase in plasma aldosterone. The administration of ACTH, but not of vehicle, produced significant increases in plasma aldosterone in both control and acute sodium-depleted subjects. However, the ACTH-induced increases in plasma aldosterone and their maximal net and percent increments during acute sodium depletion were significantly greater than control values. It is concluded that angiotensin II does not play an important role in enhancing the aldosterone-stimulating activity of ACTH during acute sodium depletion and that sodium depletion per se may be responsible for this enhancement.

Ionic mechanism of the resting membrane depolarization of frog sartorius muscle induced by dimorpholamine.

Ionic mechanism of the resting membrane depolarization of frog sartorius muscle induced by dimorpholamine was studied in a variety of ionic conditions. The resting membrane was depolarized in a dose-dependent manner reaching a maximum with a dose of 6 X 10(-5) M. The amount of the depolarization (1.4 mV) was suppressed with doses over 10(-4) M. This dose-dependent response was accompanied by change in the membrane conductance. The chloride conductance was increased by 16% and the potassium conductance by 7%, however the sodium conductance did not significantly change with a dose of 2 X 10(-5) M. The slight depolarization induced by the drug was well explained by the changes in the ionic conductance. The effects of these actions on the membrane action potentials were simulated, suggesting that this drug possesses the nature of a stimulant on excitable membranes yet does not produce any appreciable depolarization blockade.

Bacterial contamination in feed ingredients, formulated chicken feed and reduction of viable bacteria by pelleting.

The number of bacteria in steam-pelleted or crumbled chicken food was compared with the number in individual ingredients and in mash. A few bacteria were detected in white-fish meal processed Alaska pollack. Highest yields were from alfalfa meal. Colonies numbering 3.5 x 10(3)-4.2 x 10(5) were obtained by heart-infusion agar culture, and 2.1 x 10(2)-7.0 x 10(4) with deoxycholate hydrogen sulphide lactose agar, from 1 g mash. After pelleting or crumbling the number of colonies in 1 g was reduced to 0-1.9 x 10(3) and 0-3.0 x 10(0) respectively. Bacteria in the lactose medium, mostly coliform, were observed in only 1 of 18 samples.
