A double blind comparison of lithium carbonate and ludiomil in the prophylaxis of unipolar affective illness.

A double-blind prospective study was carried out comparing the prophylactic effect of maprotiline and lithium carbonate over a period of 1 year in patients suffering from recurrent affective disorders. The average Affective Morbidity Index was lower, but not significantly so, in patients treated with lithium. A further analysis based on dividing patients into those with no affective morbidity and those who showed some affective morbidity during the study, demonstrated lithium carbonate to be significantly superior to maprotiline in both the group as a whole and in unipolar depressives.

Mianserin hydrochloride: a novel antidepressant.

The antidepressant activities of mianserin hydrochloride were investigated and compared with those of amitriptyline. The therapeutic efficacy of the two drugs appeared similar, but the incidence of side-effects was significantly higher with amitriptyline. Plasma levels of mianserin were determined during the trial and were not related to the therapeutic activity of the drug.

Autoregulation of renal blood flow in the puppy.

The ability of the immature kidney to autoregulate blood flow was investigated. Renal blood flow was measured by electromagnetic flowmeter. In six puppies, selective blockade of the intrarenal effects of angiotensin II (AII) by [1-sarcosine, 8-alanine]angiotensin II (anti-AII) administered into the renal artery did not change renal blood flow. During selective renal AII blockade, intravenous AII raised perfusion pressure from 76 +/- 2 to 100 +/- 6 mmHg. Renal blood flow increased from 1.59 +/- 0.29 to 1.98 +/- 0.59 ml/g kidney per min, but returned to control levels within 40 s in spite of persistent arterial pressure elevation. In another group of seven puppies, renal blood flow remained constant despite reduction of renal perfusion pressure by aortic constriction to 60 mmHg. In two of these seven puppies intrarenal anti-AII did not abolish autoregulation. Autoregulation of renal blood flow occurs in the puppy and is not influenced by inhibition of angiotensin. The renin-angiotensin system does not appear to be involved in the normal regulation of renal blood flow in the puppy.

Modification of Cotrel-Dubousset's original hook constructs for idiopathic scoliosis.

We performed a retrospective review of 41 patients (ages 9-18 years) who underwent posterior spinal fusion with either Isola or Cotrel-Dubousset (CD) instrumentation to determine whether the presence of an apical hook on the thoracic convexity affected initial and long-term sagittal and coronal correction in adolescent idiopathic scoliosis surgery. A study group of 38 female and three male patients was evaluated (2-5 years of follow-up). Twenty-three patients (Group A) were treated with an up-going hook at the convex apex of the thoracic curve, and 18 patients (Group B) with similar curves were instrumented without an apical hook. Results showed that Group A's average preoperative coronal curve of 48 degrees decreased to 17 degrees , whereas Group B's preoperative average of 52 degrees decreased to 25 degrees . At follow-up, no statistical significance was noted in either coronal curve correction (p = 0.203) or sagittal kyphosis (p = 0.38) between Groups A and B. We conclude that omission of the up-going hook at the apex of the thoracic convexity can reduce postoperative discomfort in patients undergoing posterior spinal fusion, without sacrificing curve correction or balance.

Radial keratotomy in a patient with keratoconus.

A 33-year-old patient had radial keratotomy performed on both eyes for relief of longstanding myopia. Minimal reduction of myopia was obtained, and the patient underwent a second radial keratotomy in both eyes. Best corrected visual acuity decreased, and the patient had a marked, irregular astigmatism postoperatively. Careful ophthalmologic evaluation revealed that the patient had keratoconus bilaterally. Following a failed trial of contact lens wear, the patient underwent a penetrating keratoplasty of the right eye. Pathologic evaluation revealed findings of keratoconus, as well as radial keratotomy scars. This case report illustrates the importance of a complete ophthalmologic examination possibly including videokeratography, prior to keratorefractive surgery.
