[Hybridoma Data Bank (HDB)].

A Hybridoma Data Bank (HDB) has been developed with the collaboration of ATCC (USA), Cerdic (Europe) and RIKEN (Japan). The database holds more than 23000 records of the monoclonal antibodies. HDB is designed to meet diverse research requirements. HDB describes the development, isotype, specificity, applications, availability and literature citations for an individual monoclonal antibody. The World Data Center (WDC) at RIKEN in Japan is currently giving users' access to HDB on INTERNET, packet switching system and telephone line. The procedures for access to WDC and the interactive information retrieval system in WDC are introduced. The extraction of HDB data to create the visual table, showing reactivity patterns of monoclonal antibodies, are also shown, as an example of intelligent usage of HDB.

Nucleolar organizer regions in human esophageal disorders: comparison with proliferating cell nuclear antigen by immunostaining.

A silver colloid technique to demonstrate nucleolar organizer region-associated proteins (AgNORs) was performed on sections of 15 samples of human esophageal tissue, including five nonpathological esophageal epithelium, two esophageal dysplasia of the squamous epithelium, and eight esophageal squamous cell carcinomas. Initially we examined various protocols for AgNOR staining. Staining performed on 4% paraformaldehyde-fixed paraffin-embedded specimens with an incubation time of 30 min yielded the most satisfactory results. In nonpathological esophageal epithelium, the mean number of AgNOR counts per nucleus in the four layers of esophageal epithelium was greatest in the parabasal layer and was statistically significant. No significant differences were observed among the mean number of AgNOR counts per nucleus in the nonpathological parabasal layer, dysplasia, and carcinoma. Positive correlation was observed between the PCNA labeling index of esophageal disorders and the mean number of AgNOR particles per nucleus. Therefore, in esophageal disorders, the AgNOR staining per nucleus appears to correlate with proliferative activity but is of little practical value in discerning malignancy and/or aggressive biological behaviors.

[A case of urinary bladder hernia].

A 77-year-old male visited our hospital complaining of swelling in the right scrotal and inguinal region, urinary urgency and difficulty of urination. Physical examination revealed an elastic soft mass in the right inguinal region toward the right scrotum. Rectal examination proved that the prostate was moderately enlarged. Cystogram and urethrography showed hernia of the bladder into the right scrotum and benign prostatic hyperplasia. Urodynamic studies demonstrated organic obstruction in the lower urinary tract. Transurethral resection of the prostate was done for benign prostatic hyperplasia. Later, surgical treatment for the bladder hernia was performed by replacing the bladder into the pelvic cavity and closing the hernial ring without resection of the bladder wall. The bladder hernia proved to be a para-peritoneal type. The postoperative course was uneventful. The previous reports of urinary bladder hernia are reviewed and the incidence, etiology, diagnosis the treatment are discussed.

[Clinical study of the epidermal growth factor contents in urine, plasma and tissue from the patients with urological diseases].

Contents of epidermal growth factor (EGF) in urine, plasma and tissues in urological diseases were estimated by enzyme immunoassay using beads bound to the anti-EGF antibody, and the clinical significance of the presence of EGF in the disease state was examined. There was no difference in EGF level between healthy male and female subjects (n = 22), and the level showed a tendency to decrease with age (p less than 0.05). The subjects were 19 cases of prostatic cancer, 7 of renal cancer, and 12 of urinary bladder cancer. The difference in EGF level between the healthy subjects and patients was not significant, and the levels were shown to be lower in 8 cases of renal insufficiency (including patients on hemodialysis:HD)(p less than 0.01). Plasma EGF levels in the 30 healthy subjects revealed no significant differences related to sex or age. Plasma EGF levels were lower in 42 cases of renal insufficiency (before and after HD), and in 7 cases of renal cancer (p less than 0.01); they ware significantly lower in 15 cases of prostatic cancer (p less than 0.05). In tissues including tumor sites, EGF levels were higher particularly in the prostatic gland tissue (hypertrophy and cancer regions). Thus, urinary and plasma EGF levels in urological diseases may be useful parameters of renal function, but its relationship with malignant diseases is still unknown. The EGF level should be explored in relation with the EGF receptor.

[Iron deficiency anemia and hypoproteinemia induced by fresh cow's milk].

We reported two patients who developed severe iron deficiency anemia and hypoproteinemia which were induced by excessive drinking cow's milk. Case 1, a 23-month-old girl, had started drinking 800-1,000ml of cow's milk everyday since the age of 9 months. She developed generalized edema at the age of 23 months and visited Saga Medical School for Treatment. Case 2, a 14-month-old girl began to drink cow's milk at the age of 6 months. She drank 1,500ml cow's milk every day at the age of 14 months. She was, referred to our hospital by her physician because of severe anemia. Hypoproteinemia and severe iron deficiency anemia in both of these patients were improved by oral iron intake and prohibiting of cow's milk. Although many children at their age usually drink more than 500ml cow's milk every day, manifestations of symptoms such as iron deficiency anemia and hypoproteinemia do not occur frequently. Thus some investigations suggest that genetic defects as well as other factors related to this disease may be important factors.

[Initial and follow-up results of PTCA and PTMC].

On the bases of the initial and follow-up results of patients who underwent percutaneous transluminal coronary angioplasty (PTCA) and percutaneous transvenous mitral commissurotomy (PTMC), the proper applications of these interventional therapies were discussed. The efficacy of PTMC on valvotomy was comparable to open mitral commissurotomy and the procedural safety was noticed. Our results supported the use of PTMC for patients with plicable mitral valve. Despite the good and acceptable results of elective PTCA, low initial success rate and high incidence of major cardiac events including mortality was noted in the group of urgent PTCA for patients with multivessel lesions. The urgent use of PTCA for refractory unstable angina should be restricted to selected patients without high-risk multivessel lesions.

Microanalytical characterization and surface modification of TiNi orthodontic archwires.

Orthodontic archwires (equiatomic TiNi alloy) of both used (4 weeks) and unused conditions were microanalyzed by optical and scanning electron microscopes, energy dispersive X-ray spectroscopy, and electron diffraction to characterize the surface layers. They were also subjected to immersion and polarization corrosion tests in a 0.9% NaCl aqueous solution. Based on results obtained from these analytical and experimental studies, surfaces of TiNi archwires were further electrochemically treated to etch away nickel selectively and reform the surface morphology to uniform and porous surface layers. Main conclusions were: (a) surface layers of used archwires were covered contaminants causing the discoloration, and the contaminants were identified as mainly KCl crystals, (b) surfaces of both used and unused wires were observed to be irregular features characterized by lengthy island-like structures, where nickel was selectively dissolved, (c) corrosion tests in a 0.9% NaCl aqueous solution in immersion and polarization methods indicated that by increasing temperature from 3 degrees to 60 degrees C and acidity from pH 11 to pH 3, calculated corrosion rates increased, and (d) surface layers of TiNi archwires can be electrochemically modified to selectively etch nickel away, leaving a Ti-enriched surface layer and forming a uniformly distributed porous surface that may reduce the coefficient of friction against the orthodontic brackets.

A simple and practical method for evaluating overall measurement error of joint moments obtained by a force plate and a position sensing device.

Joint moments are now widely used in the field of gait analysis and sports biomechanics. They are usually calculated from the data obtained by a force plate and a position sensing device. Although separate assessment of the measurement error of a force plate and a position sensing device is possible, it can give only the worst case estimate of the joint moments. A simple and practical method is presented here which can give an estimate of the overall error of the combined system. The basic principle of this method is that if a lumped load is applied to a point where the joint center is supposed to be located and the moment is calculated about this point, then the moment should be zero. Thus, the deviation of the calculated moment from zero will give an estimate of the overall error. Examples of experimental data of a joint moment measuring system are given to validate this method. Various factors which contribute to the overall error are discussed and a method of reducing it by software compensation is presented.

Primary pigmented nodular adrenocortical disease (PPNAD): immunohistochemical and in situ hybridization analysis of steroidogenic enzymes in eight cases.

Primary pigmented nodular adrenocortical disease (PPNAD) is a rare but an interesting adrenocortical disorder associated with ACTH-independent hypercortisolism. We have studied eight cases of the adrenals with PPNAD by immunohistochemistry of all steroidogenic enzymes involved in cortisol biosynthesis (P-45scc, 3 beta-HSD, P-450c21, P-45017 alpha, and P-45011 beta) and also by performing in situ hybridization of P-45017 alpha in seven cases in order to localize the sites of specific steroidogenesis in this unique disorder. Immunoreactivity of all the enzymes examined was intense in almost all of the cells in adrenocortical nodules, especially the cells with abundant eosinophilic cytoplasm in all the cases examined. The internodular cortex, which demonstrated atrophy in five cases, normal appearance in two cases and hyperplasia in one case, was negative for the enzymes with an exception of 3 beta-HSD. Hybridization signals of P-45017 alpha were condensed over the nodules in in situ hybridization study, suggestive of an increased production of the enzyme itself in cortical cells of the nodules. These results may be consistent with autonomous cortisol production by the nodular cells and indicate that almost all of the cells in the nodules produce cortisol, which can also explain the presence of hypercortisolism despite small sizes of adrenals in PPNAD. Immunoreactivity of steroidogenic enzymes is observed in a small cluster of cortical cells with abundant eosinophilic cytoplasm located at the zona reticularis but not in adjacent non-nodular cortex, which may support an abnormal development of the zona reticularis as a possible pathogenesis of this disorder.

Temporal pattern of survival and dendritic growth of fetal hippocampal cells transplanted into ischemic lesions of the adult rat hippocampus.

Cell suspensions obtained from the fetal hippocampus were transplanted into the adult rat hippocampus at 1 or 4 weeks after transient forebrain ischemia. Only when the ischemia induced death of most of the CA1 pyramidal cells of the host hippocampus and transplantation was performed at 1 week after the ischemia, did a large number of transplanted cells survive and the most extensive dendritic growth was demonstrated by microtubule-associated protein 2 immunohistochemistry. The dendrites of the cells located in the ventral part were oriented ventrally, lining up similarly to the parallel arrangements of apical dendrites of normal CA1 pyramidal cells. These findings suggest that certain forms of trophic factors, which appear to occur in association with the presence of free terminals of afferent fibers during the earlier period after ischemic insult, are involved in the survival of and dendritic growth from transplanted hippocampal cells.

Analysis of a variant form of platelet glycoprotein (GP) IIb: a second patient with abnormal molecular weight GPIIb.

In 1988, we reported a thrombasthenic patient whose platelets contained an abnormal molecular weight GPIIb (Blood, 71, 915, 1988). Here we describe another patient whose platelets contain a variant GPIIb with properties similar to the previously reported abnormal GPIIb. This patient has a small amount of the abnormal GPIIb and was suggested to be a heterozygote characterized by deficient and abnormal GPIIb genes. The abnormal GPIIb was suggested to be related to the precursor form of GPIIb because the molecular weights of both proteins are the same. The abnormal GPIIb may lack a peptide region around the cleavage site of the two chains, as suggested from its lack of reactivity towards antibodies against these epitopes.

Retinoic acid stimulates peptide leukotriene-syntheses in rat basophilic leukemia-1 (RBL-1) cells.

Overnight incubation of rat basophilic leukemia-1 (RBL-1) cells with retinoic acid enhanced calcium ionophore-stimulated syntheses of LTC4 by more than 28-times (from 5.91 +/- 0.31 to 168.31 +/- 22.66 ng/2.10(6) cells) nd LTD4 by more than 7-times (from 5.27 +/- 0.12 to 39.38 +/- 14.89 ng/2.10(6) cells). The stimulatory action first appeared after a 10 h incubation with retinoic acid and was completely abolished by concomitant presence of low concentration cycloheximide (0.5 micrograms/ml) in the medium. However, LTB4 synthesis was dose-dependently inhibited by incubation with retinoic acid. Reduced form of glutathione significantly increased the synthesis of LTC4, but showed no action on the syntheses of LTD4 or LTB4. These results indicate a new enzyme synthesis of LTC4 synthetase.

[A case of undiagnosed pheochromocytoma caused by negative metoclopramide test].

Metoclopramide (MCP), a dopamine antagonist, was recently used as the pharmacological test for the diagnosis of pheochromocytoma. There have been no reports involving false negative cases in the MCP test. We experienced a rare case of pheochromocytoma which showed a negative MCP test, and it caused a failure of the diagnosis. A 51-year-old man visited our hospital with a sudden onset of headache and palpitation. Blood pressure was 218/98 mmHg at another hospital. When he came to our hospital, blood pressure returned to normal (120/80 mmHg), and both serum adrenaline (E) and noradrenaline (NE) were within normal limits. A computed tomography, magnetic resonance imaging, and angiography demonstrated a 1.8 x 1.8 cm right adrenal mass. No changes in blood pressure and plasma catecholamine were observed following the injection of 10 mg of MCP. The pathologically resected right adrenal gland contained a typical pheochromocytoma which was 1.0 x 1.0 cm in size and weighed 8 g. The detailed mechanism of the negative MCP test in this case was not known but might be related to the small size of the tumor.

Unilateral interhemispheric keyhole approach for anterior cerebral artery aneurysms.

A special midline interhemispheric keyhole approach to the anterior communicating artery aneurysms is described. A small trephine opening of 3 cm in diameter is used in most cases. The detailed microsurgical technique of the unilateral interhemispheric exposure of the anterior cerebral artery complex is presented. For the past 10 years, a total of 138 patients, 112 with Acom AN and 26 with distal ACA AN, were operated upon through this approach. There were 16 cases with non-ruptured aneurysms and the postoperative results were excellent in all of them. The overall results in 122 cases with ruptured aneurysms were excellent and good (working) in 95 cases, fair in 10 cases, poor in 9 and death supervened in 8 cases. The mortality rate in 71 acute operation cases was 8% and 4% in 51 delayed cases. The advantages of this approach include simple and rapid craniotomy, minimum brain retraction, accurate midline exposure of all parent arteries and the aneurysm. This anterior interhemispheric approach is, in our experience, much superior to the conventional pterional approach.

Growth of totally thrombosed giant aneurysm within the posterior cranial fossa. Diagnostic and therapeutic considerations.

We report a case in which growth of a totally thrombosed giant aneurysm of the posterior cranial fossa was demonstrated by computed tomography (CT) scans repeated after 4 years. A magnetic resonance (MR) image demonstrated an onion-skin-like, laminated structure within a calcified wall. The laminated structure had developed around an old thrombosis, without any communication to the flowing blood. It showed intensities indicating recent clots, revealing that the giant aneurysm had grown by recurrent intramural hemorrhage rather than by intraluminal accumulation of thrombotic materials. This case illustrates that totally thrombosed giant aneurysms still have the potential of growth.

Effect of chemicals on glutathione peroxidase of chick liver.

Chick liver glutathione peroxidase activity was separated into selenium-dependent and selenium-independent enzyme fractions and the effects of chemicals on the activities of each fraction were examined. Clofibrate induced the selenium-dependent enzyme, while phenobarbital, butylhydroxyanisole and trans-stilbene oxide elevated selenium-independent peroxidase activity. A third enzyme, which showed glutathione peroxidase activity toward both hydrogen peroxide and cumene hydroperoxide, was found.
