Congenital megalourethra.

Congenital non-obstructive dilatation of penile urethra (megalourethra) can result from absence of corpus spongiosum alone (scaphoid) or along with the absence of corpora cavernosa (fusiform). Associated urogenital or other systemic anomalies are usually present and require detection and appropriate management. Urethroplasty (Nesbitt) gives desirable results. Condition has been reviewed with report of a case.

Prenatal diagnosis and assessment of fetal malformations by ultrasonography in India.

A multicentre study to assess the status of prenatal diagnosis of fetal malformation in India was conducted. Questionnaires were sent to the teaching hospitals and to centres in the private sector involved with ultrasonography in all the four zones. Data were obtained from 13 centres. Basic level I scans were performed on all pregnant women in 64% centres, whereas level II or targeted scans were performed as routine in 42% centres. Obstetricians performed level II scans in only 35% of cases while rest were performed by radiologists. Malformations of the central nervous system were commonest, accounting for 35-69% of all malformations, followed by genitourinary and gastrointestinal malformations. Malformations of the cardiovascular system were detected in 9.3% as abnormal four chamber view and outlet tract abnormalities, subsequently confirmed on fetal echocardiography. Invasive prenatal diagnosis by fetal blood sampling, chorion villus sampling and amniocentesis for chromosomal analysis of malformed fetuses was performed at 40% centres. At the All India Institute of Medical Sciences, approximately 11,042 ultrasound scans were performed by obstetricians as routine in all pregnant women. A total of 543 malformations were detected accounting for a 4.9% incidence of structural anomalies. A total of 98 cardiovascular malformations were detected in high and low risk pregnant women on fetal echocardiography. No database or registry exists in India to give an actual insight into the problem of congenital malformation and their prenatal diagnosis. It is important to train obstetricians in level I and II ultrasonography, to recognise centres and institutions at national and regional level, and to integrate a network of diagnostic testing. Close liaison between geneticists, radiologists, pediatric surgeons, pediatricians and obstetricians is a must for prenatal diagnosis and management of fetal malformations.

Role of computers in learning--application in formative assessment.

Computers can help to liberate students from the burden of rote learning of facts and enhance the role of reason and imagination in the learning process. Arguably, the computers constitute the most important single resource ever to become available to the teachers and students since the invention of the printing press, and may well have a similar revolutionary effect on the way education is carried out, bringing about the massive shift from conventional expository teaching to the mediated individualized learning which is interactive, stimulating and rewarding. Soon, computers will be involved in almost all teaching. Apart from acting as data bases for information, computers will guide students through hypertext and will provide assessment at critical junctures, giving immediate feedback.

Recognition of the avirulence gene avrB from Pseudomonas syringae pv. glycinea by Arabidopsis thaliana.

The response of Arabidopsis thaliana land race Columbia to the bacterial pathogen Pseudomonas syringae pv. maculicola 4326 harboring cloned avirulence genes avrB and avrC from P. syringae pv. glycinea and avrA and avrD from P. syringae pv. tomato was examined. Only avrB was recognized by Columbia, as evidenced by attenuation of disease symptoms, slower bacterial multiplication in planta, and differentially greater induction of mRNA for several defense-related genes. This contrasts with two A. thaliana land races where P. s. pv. maculicola strains containing avrB were not recognized. These plants showed typical disease symptoms, and bacterial multiplication in planta was not reduced in response to P. s. pv. maculicola containing avrB. In addition, there was no differential induction of defense-related mRNAs in these susceptible land races after infiltration with bacteria containing or lacking avrB. These results extend previous observations that avirulence genes from pathogens of one host plant can be recognized by "nonhost" plants and provide the genetic framework for analysis of the plant-specified response to the bacterial avrB gene product in A. thaliana.

Role of the phytotoxin coronatine in the infection of Arabidopsis thaliana by Pseudomonas syringae pv. tomato.

The role of the phytotoxin coronatine in the virulence of Pseudomonas syringae pv. tomato in Arabidopsis thaliana was evaluated by comparing symptom development, in planta bacterial multiplication, and the induction of defense-related genes in Arabidopsis plants inoculated with the coronatine-producing (Cor+) P. s. pv. tomato strain DC3000 and the coronatine-defective (Cor-) strain DC3661 by either infiltration or dipping methods. The Cor+ strain, P. s. pv. tomato DC3000, caused severe disease symptoms and multiplied by 4-6 logs after inoculation by either infiltration or dipping. P. s. pv. tomato DC3661 failed to produce any disease symptoms and multiplied by only 1-1.5 logs in dipped plants, whereas it caused mild symptoms and multiplied 6 logs over the 4-day experimental period in plants inoculated by infiltration. Parallel experiments using a natural host, tomato, yielded similar results. Analysis of the accumulation of mRNAs encoded by several distinct defense-related genes in Arabidopsis leaves infiltrated with either DC3000 or DC3661 demonstrated that the Cor- strain consistently induced higher levels of these transcripts. These results demonstrate that coronatine production is required under more natural inoculation conditions for the successful infection of Arabidopsis by DC3000, and that coronatine may play a critical role during the early stages of infection by suppressing the activation of defense-related genes.

Emerging indications for cardiac pacing.

Substantial data have been accumulated and indications have been well delineated for pacemaker implantation in the treatment of sinus node dysfunction and heart block. However, many other indications have been proposed for pacemaker implantation. In this review, the authors examine available data regarding pacemaker implantation for new indications: neurally mediated syncope, hypertrophic obstructive cardiomyopathy, congestive heart failure, prevention of atrial fibrillation, and the relative merits of single-chamber and dual-chamber pacemakers.

Ulcerative keratitis associated with contact lens wear.

To review the microbiological profile of ulcerative keratitis associated with contact lens (CL)-wear, 35 patients with culture-proven CL-associated microbial keratitis were studied between September 1999 and September 2002. Corneal scrapes and CL-care products were collected and were subjected to microbiological evaluation. Gram-negative bacilli alone were recovered from the corneal scrapes of all 35 (100%) patients, all 70 (100%) CL storage case wells and also from CL-care solution of six (17.14%) of the 35 patients. There was a significantly higher number of Pseudomonas aeruginosa (71.43%) isolated from eyes with CL-related keratitis than other bacterial isolates (28.57%) (P<0.001). Microbial contamination of CL storage cases was a great risk for gram-negative bacterial infection among soft CL-wearers.

Association of Helicobacter pylori with peptic perforation in Chattisgarh region of India.

BACKGROUND: Helicobacter pylori is present in over 90% cases of peptic ulcer. There are very few reports regarding prevalence of H. pylori infection in peptic ulcer perforation which is the commonest complication of peptic ulcer disease. In the present study we have drawn this association. PATIENTS AND METHODS: 44 cases of peptic perforation were included in this study. Per-operative biopsy was taken from the margin of perforation. The criteria for H. pylori positivity were, Positive Rapid Urease Test and evidence of H. pylori infection on histological examination using Modified Giemsa and H&E stains. RESULTS: Prevalence of H. pylori infection in peptic perforation was 61.4%. This is well above the usual prevalence in normal population of 45%. H. pylori infection was significantly more common in cases of chronic dyspepsia (> 6 months) and not significantly related to sex, blood group, addiction, family history of acid peptic disease and previous H2 blocker therapy. INFERENCE: H. pylori is found more commonly in patients with peptic ulcer perforation than in those without.

The role of nonlinear dynamics in cardiac arrhythmia control.

The field of nonlinear dynamics has made important contributions toward a mechanistic understanding of cardiac arrhythmias. In recent years, many of these advancements have been in the area of arrhythmia control. This paper reviews the literature on analytical, modeling, and experimental nonlinear dynamical arrhythmia control with a focus on stimulation and pharmacologic techniques that have been developed, and in some cases used in experiments, to control reentrant rhythms (including spiral and scroll waves) and fibrillation. Although such approaches currently have practical limitations, they offer hope that nonlinear dynamical control techniques will be clinically useful in the coming years.
