[Severe aplastic anemia remarkably improved by a treatment with antilymphocyte globulin, high-dose methylprednisolone and danazol].

Sixteen-years-old female with severe aplastic anemia received a therapy combined with antilymphocyte globulin (ALG), high-dose methylprednisolone (m-PSL) and danazol. At the hospitalization, hematological examination demonstrated as follows; reticulocyte 21,000/microliters, granulocyte 350/microliters, platelet 10,000/microliters and hypocellular bone marrow. Treatment schedule were 1) m-PSL 1,000 mg (day 1-4), 500 mg (5-8)--then tapered. 2) ALG lg/day (day 4-8) 3) danazol 600 mg/day. During ALG administration, leukocytopenia and thrombocytopenia appeared but thereafter hematological recovery was obtained and the patient was free from supportive care. She developed mild diabetes mellitus and moderate liver dysfunction, nevertheless, both of which were controlled. At 3 months after the beginning of the treatment, hematological examination demonstrated as follows; reticulocyte 236,000/microliters, granulocyte 1,900/microliters, platelet 56,000/microliters and normocellular bone marrow. Although this immunosuppressive therapy was remarkably effective to this patient, immunological relation to the onset of aplastic anemia was not demonstrated in in vitro examination. This combined therapy seems to be effective one for patients with severe aplastic anemia.

[Magnetic resonance imaging of the thorax].

We examined thirty-five patients with various chest diseases with cardiac gated Magnetic Resonance Imaging (MRI) at 1.5 tesla. Major advantages of MRI of the thorax are its abilities to display sagittal and coronal planes and to disclose vessels clearly without contrast media. We conclude MRI is useful in evaluating, 1) masses at the lung apex or base, 2) chest wall invasion, 3) hilar and mediastinal lymphadenopathy, 4) aortic aneurysm, and 5) bronchogenic cyst.

[Cancer curable by chemotherapy: acute myelogenous leukemia].

Acute myelogenous leukemia (ALM) can be cured by chemotherapy. We have treated 121 adult AML cases for the past 9 years. With BHAC-DMP therapy starting in 1979, CR was obtained in 82% of 51 consecutive AML and the predicted 8-year continuing CR (CCR). Survival of CR cases was, 15 and 21%, respectively. Multivariate analysis of the prognostic factors revealed that % of blasts in the bone marrow at 2 weeks after the start of induction therapy was the most significant factor for CCR. Thus, we started BHAC-DMP (II) therapy with highly intensive induction therapy in 1983, but were forced to cancel this because of an intolerably high incidence of severe infections during the induction therapy. The CR rate was 76% in 29 consecutive patients, and the predicted 5-year CCR and survival of CR cases are 27 and 47%, respectively. Then we started M-85 protocol with intermediately intensive induction followed by 3 courses of highly intensive consolidation therapy using non-cross resistant drugs in 1985. The CR rate was 71% in 41 consecutive patients. Although 4 patients died of infections or myocardial infarction during the consolidation therapies, the predicted 2.5-year CCR and survival of CR cases are 76 and 74%, respectively, with a median follow-up of 28 months.

[Lung lavage and artificial surfactant replacement therapy--animal experimentation and clinical application].

Lung lavage may cause a surfactant deficient condition. We examined the effect of artificial surfactant replacement on the post-lavage condition in rabbits and in a patient with alveolar pulmonary proteinosis. Adult rabbits (n = 21) were subjected to whole-lung lavage, and the PaO2 was made to decrease to less than 70 mmHg (FIO2 = 1.0). In the replaced group (n = 11), an artificial surfactant which was a saline suspension of lipids (mixture of dipalmitoylphosphatidylcholine, phosphatidylglycerol and tripalmitin) was administered into the airway. The PaO2 was elevated to a level higher than 400 mmHg at 30 min after the surfactant replacement. On the other hand, the PaO2 in the non-replaced group (n = 10) remained below 100 mmHg. In the patient with alveolar pulmonary proteinosis, we performed the artificial surfactant replacement after the therapeutic lung lavage. By the replacement, the PaO2 increased from 89 mmHg (FIO2 = 1.0) to 128 mmHg. These results demonstrate the therapeutic effects of surfactant replacement on the acute respiratory failure induced by lung lavage.

Effect of alpha-methyl-p-tyrosine and an antiserum to rat growth hormone (GH)-releasing factor (GRF) on plasma GH secretory profile during a continuous infusion of human GRF in rats.

The effects of alpha-methyl-p-tyrosine (alpha-MT) and an antiserum specific to rat growth hormone-releasing factor (GRF) on growth hormone (GH) secretory profile during a 6-h continuous infusion of human GRF(1-44) NH2 were observed in unrestrained adult male Wistar rats. All rats were provided with two indwelling cannulae; one in the right atrium for undisturbed blood collection and the other in the inferior vena cava for 0.9% NaCl or GRF infusion. GRF was administered by an infusion pump at a dose of 50 ng/kg b.wt./min ma GH levels during baseline period were low with little fluctuation. GH secretion was augmented significantly during continuous GRF infusion in control rats but interpeak intervals remained unaltered. When an antiserum specific to rat GRF was administered, episodic GH secretion was abolished. In these rats, pulsatile GH secretion indistinguishable from that of control rats was observed in the continuous presence of human GRF. Although alpha-MT inhibited episodic GH secretion, alpha-MT-treated rats exhibited high-frequency, low-amplitude episodic GH secretion and elevated baseline levels during the stimulation. There were no differences in the amount of GH secreted during GRF infusion between rats that had received either alpha-MT or antiserum to rat GRF. Since GH secretion to GRF is determined largely by somatostatin, the results suggest that phasic release of somatostatin plays an important role in determining the rhythmicity of episodic GH secretion, and that it is modulated by alpha-MT but not by the immunoneutralization of GRF.

Effects of trisodium nitrilotriacetate monohydrate, nitrilotriacetic acid and ammonium chloride on urinary bladder carcinogenesis in rats pretreated with N-bis(2-hydroxypropyl) nitrosamine.

The effects of trisodium nitrilotriacetate monohydrate (Na3 NTA.H2O) nitrilotriacetic acid (H3NTA) and ammonium chloride (NH4Cl) on two-stage urinary bladder carcinogenesis were examined. Carcinogenesis was initiated by administration of 0.2% N-bis (2-hydroxypropyl)-nitrosamine (DHPN) to male Wistar rats in the drinking water for 2 weeks, and then the animals were treated with basal diet containing Na3NTA.H2O, Na3NTA.H2O plus NH4Cl, H3NTA, H3NTA plus NH4Cl, or without these chemicals for 28 weeks. Na3NTA.H2O increased significantly the resultant incidence of neoplastic and preneoplastic lesions of the urinary bladder. Moreover, treatment with Na3NTA.H2O, without the initiation, itself induced papillary or nodular (PN)-hyperplasia. H3NTA produced only a slight increase in the incidence of preneoplastic urinary bladder lesions (PN-hyperplasia) in rats initiated by DHPN, and this was not statistically significant. Elevation of both pH and sodium ion concentration in the urine were correlated with promotion of tumor development. These data showed that Na3NTA.H2O was more effective than H3NTA with regard to promoting potential, and that changes in both urinary pH and concentration of sodium played important roles in enhancement of urinary bladder tumorigenesis by these chemicals.

Promotive effect of 4,4'-methylenebis(N,N-dimethyl)benzenamine on N-bis(2-hydroxypropyl)nitrosamine-induced thyroid tumors in Wistar rats.

The promotive effect of 4,4'-methylenebis(N,N-dimethyl)benzenamine (MDBA) on 2-stage thyroid tumorigenesis induced by N-bis(2-hydroxypropyl)nitrosamine (DHPN) in Wistar male rats was investigated. Animals were given i.p. injections of DHPN, and then a basal diet containing 0.0375% MDBA for 19 weeks. In addition, localization of the thyroid-stimulating hormone (TSH) in the pituitary gland was investigated. Levels of TSH and T4 in serum were measured by radioimmunoassay (RIA). The addition of MDBA to the diet significantly increased the incidence and numbers of preneoplastic lesion (focal hyperplasia), adenoma and carcinoma of the thyroid. TSH was usually localized in the thyroidectomy-cell in the anterior lobe of the pituitary gland, and the number of TSH-positive cells increased in the group treated with MDBA. Mean circulating levels of TSH were elevated in all MDBA-treated groups, and mean T4 levels in groups treated with MDBA were significantly lower different (P less than 0.05) than those in control groups.

School screening for scoliosis by the Chiba University Medical School screening program. Results of 1.24 million students over an 8-year period.

The screening program for scoliosis started by Chiba University in 1979 consists of using moiré topography, low-dose roentgenography and a final ordinary x-ray examination. The number of children screened through this Chiba University Medical School (CUMS) screening program to 1986 amounted to 1,246,798. The incidence of scoliosis of more than 15 degrees increased linearly according to age from the fifth grade primary school children (0.07% in boys, 0.44% in girls) to the second grade junior high school students (0.25% in boys, 1.77% in girls). The female predominance of scoliosis cases with curvatures of more than 20 degrees detected during the total period was 10:1 and this female predominance was the same for primary school children and junior high school students. According to a study of the incidence of scoliosis by districts, (areas were divided according to population density and urbanization) there were no significant differences in the fifth grade primary school children between the sparsely and densely populated areas. In the cases of children beyond the fifth grade primary school level, however, the incidence in the densely populated areas were significantly higher than those in the sparsely populated districts. The incidence of scoliosis of more than 20 degrees decreased significantly every year among junior high school students, because they were screened periodically in school and the scoliotic students who had already been detected were left out of the next screening. This study establishes that screening for scoliosis by the CUMS screening program is cost-effective with a low risk of radiation hazards.

The promoting effects of food dyes, erythrosine (Red 3) and rose bengal B (Red 105), on thyroid tumors in partially thyroidectomized N-bis(2-hydroxypropyl)-nitrosamine-treated rats.

The effects of erythrosine (Red 3), rose bengal B (Red 105) and thyroidectomy on the development of thyroid tumor were examined in male Wistar rats treated with N-bis(2-hydroxypropyl)nitrosamine (DHPN). Red 3 and Red 105 were used at 4% in the basal diet and were administered for 19 weeks from week 2 to 20. Thyroidectomy was performed by resection of the left lobe at week 4. Single injection of DHPN was performed intraperitoneally at 280 mg per 100 g body weight at the beginning of the experiment. Red 3 and Red 105 significantly promoted the development of thyroid tumors in thyroidectomized rats given DHPN, but had no significant effect in non-thyroidectomized rats. The incidence of thyroid tumors was 91% in rats with partial thyroidectomy, Red 3 and DHPN, 100% in rats with partial thyroidectomy, Red 105 and DHPN, and 64% in rats with partial thyroidectomy and DHPN. Serum TSH was 5.5 +/- 3.1 ng/ml in rats with partial thyroidectomy, Red 3 and DHPN, 2.1 +/- 2.2 ng/ml in rats with partial thyroidectomy, Red 105 and DHPN, and 1.5 +/- 0.5 ng/ml in rats with partial thyroidectomy and DHPN.

Thyroid masses: MR imaging and pathologic correlation.

The authors explored the capability of high-field-strength surface coil magnetic resonance (MR) imaging in disclosing the gross pathologic characteristics of thyroid masses (especially pseudocapsular and hemorrhagic degeneration). Twenty-four patients were examined, including 12 with papillary carcinoma, eight with adenoma, and four with adenomatous goiter. All patients underwent surgery within 2 days after MR imaging. Specimens were cut and correlated directly with MR images. The appearance of the pseudocapsule was classified into four types: A, intact and even thickness around the tumor; B, only partially present or even absent; C, intact but with uneven thickness; D, partially destroyed by tumor. MR imaging findings corresponded precisely with those of gross pathologic examination in all cases but four of adenoma. Type A pseudocapsules were found only in adenoma and type D only in papillary carcinoma. Although the number of cases in the series was limited, the authors conclude that MR images reflect gross pathologic findings well and that some characteristic findings are suggestive of benign or malignant disease.

Spinal arteriovenous malformation: MR imaging.

Ten patients with spinal arteriovenous malformations (AVMs) were examined with high-field-strength (1.5-T) magnetic resonance (MR) imaging and a surface coil (eight patients) or head coil (two patients). Four AVMs were intramedullary, and six were extramedullary. There was one case of Foix-Alajouanine disease (subacute necrotizing myelitis; thrombosed AVM). Three important pathologic findings--myelomalacia, total thrombosis, and wall thickening of the draining vein--were clearly demonstrated at MR imaging and confirmed at autopsy. A flow-sensitive sequence was valuable in the depiction of one intramedullary AVM as hyperintense, the postoperative evaluation of AVM, and the differentiation of nidus from old intramedullary hematoma. In other AVMs, cord edema, periradicular hematoma, and reversible changes of cord scalloping after surgery were demonstrated. MR imaging demonstrated various pathologic changes of the spinal cord that could not be detected with any other imaging method.

Effect of testosterone on growth hormone secretion in female rats during a continuous infusion of growth hormone releasing factor.

The effect of testosterone on growth hormone (GH) secretory pattern during a 6-hour continuous infusion of human GH-releasing factor (GRF) (1-44) NH2 was observed in unrestrained adult female Wistar rats. Rats had been ovariectomized or sham operated 6 weeks previously. Three weeks after the ovariectomy, the rats received sesame oil or testosterone propionate at a dose of 1 or 2 mg s.c. daily for 21 days. All rats were provided with two indwelling cannulae: one in the right atrium for undisturbed blood collection and the other in the inferior vena cava for vehicle or GRF infusion. Vehicle or GRF was administered by an infusion pump at a dose of 50 ng/kg/min for 6 h. Serial blood specimens were obtained every 20 min. Sham-operated adult female Wistar rats exhibited a high-frequency, low-amplitude pulsatile GH secretion during a 6-hour vehicle infusion. When they received a 6-hour continuous infusion of GRF, the amplitudes of GH pulses and baseline GH values were markedly augmented, but the pulse frequency remained unaltered. The GH secretory pattern during a 6-hour vehicle infusion among ovariectomized rats was similar to that of sham-operated female rats, whereas the magnitude of elevation of GH pulse and baseline level in ovariectomized rats were significantly lower than in sham-operated rats. The ovariectomized female rats that had received 2 mg testosterone for 21 days showed a low-frequency, regularly timed, high-amplitude pulsatile GH secretion, and GH values during the intervening period were low. This GH secretory pattern was indistinguishable from that in adult male rats.(ABSTRACT TRUNCATED AT 250 WORDS)

Recurrent attacks of vomiting, hypertension and psychotic depression: a syndrome of periodic catecholamine and prostaglandin discharge.

A syndrome of periodic catecholamine and prostaglandin E2 discharge is described in 2 patients aged 17 and 3 years. They had recurrent attacks of vomiting, hypertension and psychotic depression for several years with a fixed periodicity. At initiation of the attack, plasma ACTH, AVP, norepinephrine and prostaglandin E2 were markedly elevated, whereas dopamine was undetectable. This resulted in hypercortisolemia, hyponatremia and oliguria, which were completely normalized when the attack subsided. Dopaminergic inhibition by metoclopramide injection induced a sustained rise in plasma bicyclo-prostaglandin E2 in the patients, a transient rise in 4 controls, and no response in 8 control children. The 4 control responders had significantly higher plasma norepinephrine levels and aldosterone responses than the non-responders (P less than 0.001). There was a linear correlation between peak values of bicyclo-prostaglandin E2 and basal norepinephrine levels (r = 0.990, P less than 0.001). The patients released bicyclo-prostaglandin E2 and aldosterone more easily than the control responders in terms of plasma norepinephrine and dopamine levels. Treatment of the patient with clonidine was partially effective, whereas administration of indomethacin completely suppressed recurrence of the attacks for 1 year. These results suggest the etiologic possibility that the patients have a decreased dopaminergic inhibition of prostaglandin E2-mediated norepinephrine secretion, which causes periodic discharge of norepinephrine and concomitant release of ACTH and AVP.

Use of cyclophosphamide and total lymphoid irradiation combined with cyclosporine in bone marrow transplantation for transfused severe aplastic anemia.

Between February 1984 and August 1987, 10 patients with severe aplastic anemia were treated with bone marrow transplantation from HLA-identical sibling donors after preparation with cyclophosphamide (CY) 200 mg/kg and total lymphoid irradiation (TLI) 750 cGy. Ages ranged from 5 to 28 years (median 14 years). All patients were previously transfused. Median number of transfusions was 16 (range, 3-886). For post-transplant immunosuppression all patients were given cyclosporine and the last three patients received additional immunosuppression with short-term methotrexate. All patients had initial engraftment and survived for more than 3-46 months after transplantation. One patient developed significant acute graft-versus-host disease (GVHD) and three of nine recipients who survived more than 100 days developed chronic GVHD. One male patient who had received 21 transfusions from his marrow donor before transplantation suffered from persistent granulocytopenia. Otherwise all have Karnofsky performance scores of 90-100%. Although the number of patients is small, it appears that allogeneic bone marrow transplantation with the regimen of CY + TLI for preparation combined with cyclosporine (+ short-term methotrexate) for post-transplant immunosuppression is a promising modality for treatment of previously transfused patients with severe aplastic anemia.

[Clinical experience of enoxacin complicated urinary tract infection].

Enoxacin (ENX) was administered to 69 patients with complicated urinary tract infections (UTI). Clinical efficacy and safety were evaluated by the criteria proposed by the UTI Committee, Japan. The overall clinical efficacy was excellent in 60.9%, moderate in 10.1% and poor in 29% of the patients. Of the 76 strains isolated from the patients 61 strains (80.3%) were eradicated. Subjective side effect was observed in one patient who complained of slight nausea. No drug-related aggravation in the laboratory test was observed. These results showed that ENX was effective and safety for the treatment of complicated urinary tract infection.
