Diagnostic significance of anti-HBc IgM (RIA) in healthy HBsAg carriers and in chronic hepatitis B.

The diagnostic significance of IgM antibody against hepatitis B core antigen (anti-HBc) in healthy hepatitis B surface antigen (HBsAg) carriers and in subjects affected by chronic hepatitis B was evaluated. IgM anti-HBc was sought and found in all nine patients examined who were affected by acute HBsAg-positive hepatitis. It was also detected in 2 out of 18 patients with HBsAg-positive chronic persistent hepatitis and in 12 out of 42 patients affected by HBsAg-positive chronic active hepatitis. The absence of this marker was noted in all 26 HBsAg healthy carriers and in the subjects with HBsAg-positive cirrhosis. No relationship was found between the presence of IgM anti-HBc and the degree of inflammatory activity in the patients with HBsAg-positive chronic active hepatitis. A correlation was not found between the presence of IgM anti-HBc and the presence of hepatitis B e antigen (HBeAg) in the same patients. These data show that the absence of IgM anti-HBc may be useful in identifying healthy carriers of HBsAg. The presence of this antibody may be a suitable indication of acute HBsAg-positive hepatitis. In patients with chronic active hepatitis B the presence of IgM anti-HBc cannot be used as diagnostic tool in predicting the severity of liver disease.

Telethermography and axillary node status as predictors of early relapse in breast cancer: preliminary report.

The authors report on 141 primary, non-metastatic, breast cancer patients with a minimum follow-up of 12 months after surgery to a maximum of 5 years. All cases were studied according to the patterns of the preoperative telethermography (TH) and nodal status (N). Prognosis was evaluated in terms of incidence of relapse. A statistically significant correlation was found between TH+ and early relapse, and for N+, but there was no correlation between TH and N, both for negative and positive values. The results suggest a complementary prognostic value of the two parameters.

Cyclophenil, a non-steroidal compound with a higher central than peripheral oestrogenic activity: study of its effects on uterine growth and on some central parameters in castrated female rats.

The oestrogenic activity of cyclophenil, a non-steroidal compound which has structural analogies with both stilbene and triphenylethylene, has been reevaluated utilizing both central and peripheral parameters. The central parameters considered were LH, FSH, prolactin secretion and two enzymatic systems known to be oestrogen-sensitive: hypophyseal 5alpha-reductase and hypothalamic aromatase. The uterine growth test was used to determine oestrogenic peripheral activity. The compound was administered at various doses in comparison with oestradiol benzoate (EB) to long-term castrated female rats. Cyclophenil has an activity 1/8110 times that of EB on uterine growth, and 1/1660 and 1/550 times that of EB in inhibiting LH and FSH, respectively. The hypophyseal 5 alpha-reductase (expressed as DHT formation) was inhibited 1710 times less by cyclophenil than by EB. The other parameters considered were unsuitable to provide a statistically reliable estimate of the potency ratios between the two compounds. The data show that cyclophenil is an oestrogenic compound with peculiar characteristics. This substance is more effective in expressing its oestrogenic activity in central structures than in the peripheral ones.

Structural analysis of a set of socioeconomic indexes as an aid in defining the socioeconomic level of a family: results from an italian multicentric survey.

The study of possible associations between social and health factors was one of the purposes of the multicentric Italian survey of perinatal preventive medicine. To get a concise and coherent social indicator, which could be useful to this study, we explored the socioeconomic information systematically collected on the parents of 12,058 babies born in 1973-1975 in four centres (Trieste, Milan, Parma and Bari), which reflect some of the heterogeneous aspects of Italy. The Correspondence analysis indicated that the seven considered indexes, concerning both parents' education levels and occupations, dwelling quality and the length of father's residence in the area in which the baby was born, were highly and similarly correlated in each centre. The Dynamic cluster analysis enabled us to bring together the socioeconomic profiles of the families into six classes, on the basis of their similarities, which depend only upon the multivariate distribution of the seven categorical variables under study. Therefore, the attained classification not only is independent of whatever score is assigned to the socioeconomic categories but also allows for the strong interactions between the variables and exploits all the available information. Our six classes were found to be rather similar to the six socioeconomic groups (S.E.G.) of the U.K. Registrar General and to show some resemblance to certain social groups defined by other classifications adopted in obstetric and pediatric field. The socioeconomic indicator constructed here should enable us to find out how much, in different areas of Italy, the social classes may account for inequalities in health conditions and care of the mother and her baby.

Diagnostic value of the glycerol lysis test.

The diagnostic value of the standard glycerol lysis test (GLT) has been evaluated by studying 95 cases of hereditary spherocytosis (HS), 513 beta- and 25 alpha-thalassemia trait (Th) carriers, 170 iron-deficient patients (ID), and 217 patients with miscellaneous hematological conditions. The sensitivity of the test, at 95% specificity, was 97% for beta-Th, 53% for ID, and only 36% for HS, so that the predictive value of a positive result is practically nil for HS, 50% to 85% for Th, and 10% to 35% for ID, depending on the prevalence ratio of the disease in the examined population. The technical variability of GLT, explored by a proper experimental design, was found to be rather high, its main components being repeatability and variability of reagent preparations. The dependence of the results on operators was negligible. It is concluded that GLT is not suitable as a laboratory test for the detection of abnormalities in red cell osmotic fragility because of its very low sensitivity toward increased fragility. For decreased red cell osmotic fragility, the sensitivity of GLT is high; however, it is less valuable than the traditional 0.35% NaCl method as mass screening test for the detection of beta-Th, giving more false positive results.

The epidermal melanocyte population in the skin of ultraviolet-irradiated crested newt.

The response of the epidermal melanocyte population to repeated ultraviolet (UV) exposure (wavelength spectrum 275-350 nm) has been investigated in the crested newt, Triturus cristatus carnifex. The effects of different doses of UV light were studied. The animals were killed 7 months after the first UV exposure. Only a slight decrease in the number of pigment cells was found after 85 sequential irradiations with a total dose of 1.3 x 10(5) J/m2, whereas striking decreases were observed when the same total dose was fractionated into 14 exposures or when a double dose was given in 57 exposures. The relationship between the square roots of the epidermal melanocyte densities and single doses appeared to be roughly linear, at least over the range of doses administered. The main factor in melanocyte damage seemed to be the single dose of irradiation rather than the cumulative dose administered. Decreased melanin content of the keratinocytes was observed in most irradiated animals.

[S-adenosylmethionine as a regulator of protein synthesis. Influence on plasmatic fibrinogen].

S-adenosyl-L-methionine (SAMe) is a physiological compound which plays an important role in numerous biological processes, such as transmethylations, transulphurations and aminopropylations. Starting from a study about the usefulness of the SAMe to normalize protein synthesis in old age, the drug was found to have an interesting action with respect to fibrinogen. Later research regarding a large number of patients, led us to discover after treatment for 40 days with 60 mg/die of SAMe, highly significant variations in fibrinogen values with constant normalization of this parameter, in both hypo- and hyper-fibrinogenemia. So far we don't know anything about the action of the drug on various metabolic stages of the fibrinogen; interestingly however our data exclude a rise in fibrinolysis.

Linear models for the analysis of variability in factorial designs: an application to anthropometric indicators.

This paper has two aims. One is to evaluate, in a set of 9740 'reference' neonates born in five Italian centres (Trieste, Milan, Parma, Rome, Bari) during 1973-1976 the effects of gestational age and birthplace on the variability of crown-heel length and head circumference at birth; this is a preliminary step in the construction of 'Italian' intrauterine growth standards for such anthropometric measures. The other aim is to put the various techniques for the analysis of variability into a common framework of Linear Models, either Generalized or Classical, to compare their main features and provide some indications about their application.

Role of hepatitis B virus infection in alcoholic patients.

The aim of this study was to determine the incidence of HBV markers in patients with alcoholic liver disease and to compare the results with those of patients with non-alcoholic liver disease and control subjects. We tried to determine whether the association between alcohol intake and HBV infection increases the risk of developing severe liver disease. The results showed an increased incidence of HBsAg in alcoholic patients when compared with controls as well as an increased incidence of severe chronic liver disease in HBV-positive groups when compared with HBV-negative groups. We conclude that HBV infection is an important additional risk factor for the development of severe liver disease in alcoholic patients.

A computerized microassay for complement hemolytic activity using an automated spectrophotometer.

A microassay carried out with V-bottomed microplates was developed as a simplified procedure to estimate the complement hemolytic activity. This microassay has the advantage of permitting the evaluation of the extent of lysis without the need to separate the supernatants from the unlysed erythrocytes. This is accomplished by calculating the ratio between the absorbance at 405 nm and that at 450 nm. The results, read automatically on a Dynatech MR580 spectrophotometer, are transformed into CH50 U by means of a computer program. Finally, a kinetic assessment of complement hemolytic activity, easily carried out by this simple procedure, may reveal complement abnormalities otherwise undetected by the standard hemolytic assay.

Italian cross-sectional growth charts for height, weight and BMI (2 to 20 yr).

The aim of this study is to extend to pre-school ages the Italian Society for Pediatric Endocrinology and Diabetes (SIEDP)-2002 growth charts for height, weight and body mass index (BMI), to obtain charts (SIEDP-2006) that apply to the Italian population from 2 to 20 yr of age, taken as a whole, or separately in two geographical areas (Central-North Italy and South Italy). The charts are based on a sample of about 70,000 subjects attending infant, primary and secondary schools, between 1994 and 2004. The distribution of the sample by gender, age and geographic area was roughly similar to that of Italian school population in the last decade of the 20th century. Height and weight were measured using portable Harpenden stadiometers and properly calibrated scales, respectively. SIEDP-2006 references are presented both as centiles and as LMS curves for the calculation of SD scores, and include the extra-centiles for overweight and obesity. Large differences in BMI growth pattern emerged between the SIEDP-2006, 2000 CDC and UK90 references: in Italy, BMI is higher and its distribution is more skewed during childhood and adolescence. At the end of growth, median values of the three references are similar, but the 97th centile of 2000 CDC charts is much higher and increases more steeply than that of SIEDP-2006 charts, which on the contrary reach a plateau. SIEDP-2006 references intend to supply pediatricians with a tool that avoids the use of charts that are outdated or that refer to other populations, and thus should be suitable for adequately monitoring the growth of their patients.
