[Environmental health risks in a marble-working shop].

With regard to the "Inquiry industrial accidents and respiratory pathology of marble workers", promoted by the "Regione Toscana" and being made by the preventive medicine services for workers acting in Massa Carrara and Lucca Provinces, the AA. relate about the sanitary environment conditions in a modern marble manufacture workshop and compare them with the ones in other workshop less advanced from the technological point of view.

[Pathogenetic and clinico-diagnostic considerations on a case of non-parasitic cyst of the spleen].

A rare case of non-parasitic cyst of endothelial type, recently observed in the spleen, is illustrated. The relevant literature is reviewed, Fowler's classification stressed, and the aetiopathogenetic and histopathological characteristics of such cysts are discussed in critical spirit. The diagnostic means are also fully described. With stress on the utility of the scintiscanning examination.

A sporadic, atypical case of desminopathy: morphological and immunological characterization.

Recently, abnormal expression of cyclin-dependent kinases was proposed as a possible cause of desminopathy. We describe an atypical case clinically characterized by severe respiratory distress. Muscle biopsy showed subsarcolemmal and intracytoplasmic accumulation areas, which intensively stained with anti-desmin antibodies and contained electrondense filamentous material at ultrastructural level. WB analysis showed 30% increased desmin signal compared to controls. Positive immunostain for CDC2 kinase, CDK2 and emerin and nuclear matrix-associated protein were, found in desmin-positive fibres.

[Malformations in the offspring of pregnant women with epilepsy. Presentation of an international registry of antiepileptic drugs and pregnancy (EURAP)].

The interaction between epilepsy and pregnancy has been studied for many years; nonetheless the risk associated with individual antiepileptic drug has not been adequately characterized up to date. Moreover, virtually nothing is known about the possible human teratogenicity of the newer antiepileptic drugs. Because of the complexity of the mechanisms involved, the crucial evidence needed can only come from very large population based studies, and a collaborative European multicentre investigation has been set up to this purpose. Specific objectives include the evaluation of the risk of major foetal malformations and of delay in prenatal growth following exposure to antiepileptic drugs, assessment of the pattern of congenital abnormalities associated with older and newer antiepileptic drugs and their combinations, and identification of possible relationships with dosage and with a variety of other risk factors. All women exposed to antiepileptic drugs at the time of conception are eligible for entry. The protocol is purely observational and does not entail any change in prescribing pattern or management policies, which are left to the discretion of the treating physician. Data obtained during prospective monitoring for up to 1 year after birth are regularly collected in especially designed forms and entered into Regional Registries prior to transfer to a Central European Registry of Antiepileptic Drugs and Pregnancy (EURAP). Evaluations of incidence and prevalence of teratogenic endpoints will be based exclusively on cases enrolled before foetal outcome is known and in any case not after the 16th week of pregnancy. Cases enrolled after birth, after the 16th week of pregnancy or after prenatal diagnosis will only be reported descriptively. The study is being implemented gradually in 19 countries in Western and Eastern Europe. Wide participation from interested physicians is essential for the achievement of the study objectives, which are expected to lead to important advances in pre pregnancy counselling and overall clinical management of women with epilepsy.

[Clinical and pathological characteristics of carcinoma of the kidney: computerized analysis].

To evaluate the significance of the clinical and pathological features and survivorship data, we analyzed 203 cases of renal carcinoma who underwent surgery at our department from 1974 to 1988. Tumor stages P2 and P3 and grades G2 and G3 were the most frequent. The series is large enough to permit classification according to tumor stage and grade and most of the possible associations. The diagnostic procedure and the surgical technique were consistent. Radical nephrectomy via the transperitoneal approach was performed in 153 patients. Transpleural thoracophrenolaparotomy was performed in 22 patients with a very large tumor mass in the upper pole. Extended classical nephrectomy via the lumbar approach was performed in 27 patients. Computer-assisted analysis of the clinical and pathological data showed tumor stage to be an important prognostic factor. Given a similar situation for tumor stage, the histologic grade has been shown to influence prognosis significantly. Other clinical or pathological parameters such as pseudocapsule, histological type and paraneoplastic biological and humoral changes often correlated with tumor stage or grade. Clinical staging is a point of reference for surgery. However, detection of microfoci of tumor metastasis that might be responsible for late reactivation of the disease is as yet not possible.

Effect of an inhibitor of PGE (flurbiprofen) on colony growth from normal and leukemic peripheral leukocytes in basal conditions and in presence of testosterone or cortisol.

The Authors have controlled the colony growth in coltures from peripheral leukocytes of normal subjects and from patients with CML and AML. The cultures were prepared on double agar layer in basal conditions and after adding flurbiprofen and/or cortisol and/or testosterone. The exam of the plates has shown a light similar stimulating activity on the growth of all substances separately considered, with and evidence ability of flurbiprofen of potentiate the action of the two hormones, both in coltures from normal and leukemic peripheral leukocyte CFU-GM.

[Primary sarcomas of the maxillofacial area. The clinical assessment of a case of angiosarcoma with a rare location].

Following a short critical review of the most recent literature, the present study reports a case of angiosarcoma of the gingiva, localized at the level of the vestibular surface of the lower incisor region. The intense positive reaction of neoplastic elements to factor VIII-correlated antigen, negative for cytokeratin and positive for vimentin, allowed the vascular nature of the neoplasia to be confirmed.

Critically ill patients: immunological evidence of inflammation in muscle biopsy.

AIM AND METHOD: To verify whether muscle necrosis in critically ill patients could be due to an inflammatory process, we tested muscle biopsies from five intensive care patients with different inflammation-specific immunocytochemical markers (antibodies anti-class I major histocompatibility complex products (class I MHCP or HLA I), membrane attack complex (MAC), T lymphocytes helper-inducer (CD4), cytotoxic (CD8) and pan-B-lymphocytes). RESULTS: In three patients muscle biopsy showed class I MHCP positivity on the surface membrane of several groups of fibres, mainly perifascicular, and scattered microvascular deposits of MAC. In the other two patients muscle biopsy did not show class I MHCP and MAC positivity. CONCLUSION: Our results suggest that inflammation may be a component of muscle damage in some critically ill patients.
