Audiovestibular functions in rheumatoid arthritis.

Audiologic, middle ear impedance and vestibular function tests were administered to 25 patients with classical or definite rheumatoid arthritis and 25 normal control subjects of 30 to 50 years age group. Any possible influence of other common causes of hearing loss was avoided by carefully selecting the patients. Six patients had mild sensorineural hearing loss and another had bilateral mixed hearing loss. Results of tone decay test, speech reception threshold, speech discrimination score and short increment sensitivity index tests suggested cochlear pathology being responsible for the sensorineural hearing loss. There was no statistical correlation between hearing loss or otoadmittance abnormality and duration or activity of rheumatoid arthritis or positivity of rheumatoid factor. Electronystagmographic recording of saccades and bithermal caloric induced nystagmus revealed no significant abnormality.

Silent otitis media: an autopsy study in stillborns and neonates.

Sixty temporal bones of 30 stillborns and neonates who died of causes other than septicemia were examined by histopathologic methods for evidence of otitis media and persistence of mesenchymal tissue. The incidence of silent otitis media is 23.3% in this study. Mesenchymal tissue was observed in 6 temporal bones that showed evidence of otitis media. The role of mesenchymal tissue as a causative or immunologic factor, however, needs further evaluation.

Normokalaemic periodic paralysis as the presenting manifestation of hyperthyroidism.

We report a 50 year old male admitted with complaints of episodic weakness. During the attack of paralysis there was no hypokalaemia. There were no clinical signs of thyrotoxicosis, but since thyroid function tests revealed hyperthyroidism, he was treated as a case of thyrotoxic periodic paralysis, with no recurrence of symptoms over 6 months.

Transtracheal illumination for optimal tracheal tube placement. A clinical study.

The tracheas of 420 adult patients were intubated using the tip of a lighted stylet placed inside the lumen of the tracheal tube, just proximal to the tube cuff. The maximum point of transillumination was visible just distal to the cricoid cartilage, with proper cuff positioning. The lighted stylet was also introduced into the oesophagus to see whether transoesophageal illumination could be demonstrated. The intensity of transillumination was measured using a grading system. The distance between the bevel end of the tracheal tube and the carina was determined with a fibreoptic bronchoscope. Tracheal transillumination was graded as excellent in 81% of patients and as good in 19%, when the overhead lights were dimmed and cricoid pressure was applied. Transoesophageal illumination could not be demonstrated in any patient. The mean distance between the tip of the tracheal tube and the carina varied between 3.7 and 4 cm. Transtracheal illumination is a simple, effective and reliable method that can be used during intubation for the recognition of optimal tube placement.

Pulmonary infiltration with blood eosinophilia after scorpion sting.

A 30-year-old man developed bilateral pulmonary infiltrates with blood eosinophilia after receiving a scorpion sting. A short course of corticosteroids resulted in rapid improvement. The PIE syndrome after scorpiasis has not, to our knowledge, been described previously.

Congenital afibrinogenaemia.

A case of congenital afibrinogenaemia in a young female child is described. She had haemorrhagic tendency since birth in the form of markedly prolonged umbilical bleeding and easy bruising afterwards. Two of her brothers had bleeding tendencies, one died shortly after birth due to uncontrollable umbilical bleeding and other died at the age of 12 years from internal haemorrhage. The family study indicates the mode of inheritance to be probably autosomal recessive. The principal laboratory findings are complete non-coagulability of blood, grossly abnormal coagulation tests, zero ESR value, failure to detect fibrinogen by heat coagulation or chemical precipitation tests and biuret reaction and correction of thrombin time after fibrinogen infusion.

Effect of vitamin A deficiency on hepatic and intestinal drug metabolizing enzymes in rats.

Feeding of vitamin A-deficient diet to male weanling rats for 10 weeks caused significant reduction in the hepatic cytochrome P-450, cytochrome b5, aminopyrine N-demethylase and arylhydrocarbon hydroxylase activities. Contrary to this, the levels of these Phase I enzymes were found to be significantly elevated in all the 3 portions (proximal, middle and distal) of the intestine in deficient animals as compared to corresponding pair-fed controls. Of the Phase II enzymes studied, UDP-glucuronyltransferase showed a significant decrease whereas glutathione S-transferase showed a significant increase in vitamin A-deficient rat liver and small intestine. The study suggests that vitamin A deficiency causes an imbalance between the Phase I and phase II drug metabolizing enzyme systems which may decrease the capacity of the organism to withstand the neoplastic effects of chemical carcinogens in vitamin A deficiency.

Clinical and epidemiological features of acute gastroenteritis associated with human rotavirus subgroups 1 and 2 in northern India.

Rotavirus was detected in 111 (15.9%) of 694 children who presented to our hospital with acute diarrhoea over a period of 45 months (1982-1985). Subgrouping for rotavirus was done on 87 children by ELISA using specific monoclonal antibodies to find out any differences in the epidemiology and clinical profile of the two subgroups. Twenty six (29.9%) were found to belong to subgroup 1 and 61 (70.1%) to subgroup 2. Diarrhoea, vomiting and fever were present in both the subgroups in the same frequency. However, the severity of diarrhoea was more in children having subgroup 2 infection. Rotavirus infection showed two peaks, one during the early months of summer and the other during the early months of winter of each year. During most of the study period, infection was predominantly with subgroup 2, except for a few months in 1983 and 1985 when a majority of children had infection with subgroup 1.

Effect of dietary zinc and copper on peripheral blood plasma cholesterol, testosterone and histomorphology of testes in rats.

Total plasma cholesterol (mg/dl) significantly (P less than 0.01) decreased from 70.8 to 54.01 as the dietary Cu levels increased from 2.5 to 5 ppm at 12 pm Zn concentrations in male weanling rats. A similar trend was observed in the blood peripheral testosterone concentration at 12 ppm Zn and 2.5 ppm Cu. Histological examination of testes revealed smaller seminiferous tubules with atrophy of germinal epithelium. Also a marked loss of spermatogenic cells was observed in Zn and Cu deficient rats.
