Postnatal change in lipid composition and nerve conduction of peripheral nerves of young rhesus monkeys.

Body weight, motor and sensory nerve conduction velocities of fore and hind limbs, and lipid composition were measured sequentially in peripheral nerves of 15 rhesus monkeys. Initially measurements were made with monkeys six to eight months of age. There were significant increases in body weight, motor, and sensory nerve conduction and myelin marker lipids over a five months period, but no change was observed in free fatty acids, triglycerides, and esterified cholesterol. These results indicate that myelination continues at least for 11 to 13 months of postnatal age in rhesus monkeys.

Skeletal manifestations in beta-thalassaemia intermedia.

Thalassaemia intermedia with distressing complications is reported from India. An 11 year old boy presented with recurrent fractures since 6 years of age. Investigations revealed that he had multiple healed fractures of long bones, hypersplenism, iron overload and a red blood cell half life (t1/2) of 17 1/2 days. Splenectomy was planned.

Effect of protein calorie malnutrition on in vitro incorporation of (U-C14)-glucose in brain of young rhesus monkeys.

Twelve young rhesus monkeys weighing 1.5-2.0 kg were acclimatised for a month in the animal house of the Institute. After acclimatization 6 monkeys were placed on a protein calorie deficient diet and remaining 6 on normal diet. The deficiency state was indicated by reduction in body weight, significant decrease in serum albumin, lack of physical activity and shedding of fur. At this stage, animals of both groups were sacrificed and brains were removed. Cerebrum was separated, blotted on filter paper and weighed. In vitro incorporation of (U-C14)-glucose was done for 2 hr in approximately 300 mg cerebrum. Incorporation of this precursor was determined in cerebrum, lipids, its fractions and proteins in both groups. There was increased incorporation of (U-C14)-glucose into tissue, gangliosides, free fatty acids and esterified cholesterol but no significant change in myelin marker lipids and proteins in PCM group was noticed as compared to age matched controls.

Electrophysiological and histopathological changes in peripheral nerves of postnatally undernourished and rehabilitated young rhesus monkeys.

Nerve condition velocity of ulnar and tibial nerves and qualitative histology of ulnar nerve were studied in young rhesus monkeys. Motor nerve condition velocity of both the nerves and amplitude of sensory response of ulnar nerve were significantly decreased in even moderate protein calorie malnourished (PCM) group of monkeys. Increased paranodal gap, segmental demyelination, thin myelinated fibres and prominent Schmidt Lanterman Clefts were also observed in PCM group. There was complete recovery in motor nerve conduction velocity in the nerves of both limbs while partial in amplitude of sensory response in proximal segment of ulnar nerve on nutritional rehabilitation of 10-12 weeks.

A study of relation between physical growth and sexual maturity in girls--V.

The findings indicate that sexual maturity would be initiated only after the attainment of requisite morphological body size such as weight on an average nearly 27 kg and height of 137 cm, irrespective of age and SES. The girls from LSES were chronologically older throughout the span of pubertal growth. However, they took lesser time to reach full maturity after the attainment of mid-pubertal growth compared to USES. Menarche appeared after attaining about a minimum weight of 35 kg, height of 143 cm and bicristal breadth is 24 cm. Its appearance followed the attainment of peak height velocity and 3rd developmental stage of breast and/or pubic hair. It, however preceded the attainment of weight and bicristal breadth peak velocities.

Pneumatic dilatation is a satisfactory first-line treatment for achalasia.

The results of 124 pneumatic dilatations done in 92 patients with achalasia cardia were assessed. Relief of dysphagia and other symptoms was obtained in 90 (97.8%) patients--in 68 (73.9%) after one dilatation, in 16 (17.8%) after two dilatations and in six (6.5%) after three dilatations. Two patients who did not obtain relief after three sittings of dilatation underwent surgery and both became totally asymptomatic thereafter. Most of the patients successfully treated with pneumatic dilatation remained asymptomatic during a follow up of 6 months--5 years. The few who did become symptomatic (8.01%) within a year after pneumatic dilatation responded well to the same procedure when repeated. Immediate and late complications of pneumatic dilatation occurred in 3.3% and 4.35% of patients respectively and were all medically manageable. In our assessment, pneumatic dilatation is a simple, quick, safe and effective method for treating achalasia. It should be used as the primary mode of treatment and surgery should be offered only to those patients who fail to respond to at least three attempts at pneumatic dilatation.

A mixed-longitudinal study on the pattern of pubertal growth: relationship to socioeconomic status and caloric-intake--IV.

The pubertal growth pattern was observed on 791 girls belonging to upper and low SES. These girls ranged between 7-16 years. The effect of calorie intake on the pubertal growth was also ascertained. The development of breast was first to appear at the age of 8.25 years. It was followed by pubic and axillary hair development. The mean age of menarche was 12 years and 12.8 years for USES and LSES, respectively. The onset of menarche, breast and pubic hair was significantly delayed in LSES girls by 0.8 years. Menarche was found to correlate better with breast development than pubic or axillary hair. The girls on adequate calories showed early onset of breast, pubic hair and axillary hair development and of menarche. Similarly, these girls attained mature stage (adult) of these variables earlier compared to those who were on inadequate calories. However, intermittent developmental stages of pubic hair and axillary hair showed no consistency with intake of calories. The girls on inadequate calories showed approximately one year late onset of breast and pubic hair development. The present observations suggest that the onset of puberty is strongly influenced by environment but its attainment is under the genetical control.

Fungal diarrhoea: association of different fungi and seasonal variation in their incidence.

A total of 854 patients (640 children and 214 adults) admitted with acute or chronic diarrhoea suspected of non-invasive overgrowth of fungi in intestine were screened during a period of 3 years. Fungal proliferation was noted in 54.8% of these patients (53.6% in children, 58.4% in adults). The predominant fungal species isolated were Candida albicans (64.5%), followed by C. tropicalis (23.3%) C. krusei (6.9%). Torulopsis glabrata (1.6%). Trichosporon sp. and Geotrichum sp. were found to be responsible in 2.3% of adults. As seen in bacterial diarrhoea, higher incidence was noted in children from April to August. No such seasonal variation was noted in adults.

Induction of membrane proliferation in mouse CNS by gold sodium thiomalate with reference to increased virulence of the avirulent Semliki Forest virus.

Separation of smooth membrane vesicles from whole mouse brain by isopycnic centrifugation in discontinuous sucrose density gradients show an increased membrane proliferation in gold sodium thiomalate (GSTM) treated mice. Induction of membrane proliferation by GSTM seems to be an important factor in converting the avirulent Semliki Forest virus infection into a lethal one.

Effect of unprocessed wheat bran on calciuria and oxaluria in patients with urolithiasis.

Seventeen hypercalciuria patients (8 control, 9 treatment) with a history of urolithiasis were randomly selected to receive low-calcium, low-oxalate diets with or without the addition of 30 g of dietary fiber as unprocessed wheat bran. Diet alone resulted in a 5.6 percent decrease in calciuria compared with a 23.5 percent decrease with the addition of the fiber. The addition of hydrochlorothiazide and potassium citrate further reduced calciuria by 40.4 percent and 34.5 percent, respectively. Oxaluria was decreased 21.4 percent by diet alone compared with 3.9 percent in the diet and fiber treatment group. Patient compliance to diets was good, and no complications resulted from fiber intake.

Endoscopic sclerotherapy of esophageal varices in infants and children.

Thirty children, aged 7 months to 13 years, with bleeding esophageal varices were managed by endoscopic sclerotherapy (EST). Of these children, 73.3% had extrahepatic portal vein obstruction (EHPVO), 16.6% had cirrhosis of the liver, and 10% had noncirrhotic portal fibrosis. EST was done with fiberoptic pediatric upper gastrointestinal endoscopes and a flexible sclerotherapy needle under sedation with intravenous diazepam and pentazocine. The sclerosants used were ethoxysclerol 1% and absolute alcohol. Ten injections were given to control active variceal bleeding, and 145 injections were given on planned basis at 2-3-week interval. An average of five injections was required to obliterate the esophageal varices. In 90% of cases, an avariceal state was achieved; 10% had decreased size and number of varices. Emergency EST was effective to control bleeding in all sessions. Complications, including retrosternal discomfort, esophageal ulceration, stricture formation, and aspiration pneumonia, occurred in 60, 20, 20, and 6.6% of cases, respectively; complications were managed conservatively. Strictures were dilated with Eder-Puestow's olive dilators. Recurrence of esophageal varices, gastric varices, and rebleeding was seen in 13.3, 13.3, and 10% of cases, respectively. Shunt surgery was performed in 13.3% cases, who had developed gastric varices and were having EHPVO.
