Local inflammation caused by a monoclonal antibody that blocks the function of the rat membrane inhibitor of C3 convertase.

The participation of membrane inhibitors in C-mediated inflammation in vivo was analyzed in rats with the use of mAbs against membrane inhibitors of C. 512Ag inhibits C3 convertase of homologous rat C and rat CD59 prevents the formation of homologous membrane attack complexes. The histologic distributions of 512Ag and CD59 in rat skin were essentially the same. However, although intracutaneous administration of mAb against 512Ag (anti-512Ag) in the form of F(ab')2 induced a remarkable inflammatory response visualized by exudation of Evans blue that had been previously injected i.v., the mAb against CD59 caused no inflammatory exudate. Furthermore, anti-512Ag F(ab')2 caused deposition of C3 followed by intense polymorphonuclear leukocyte infiltration and hemorrhage at the injection sites within 24 h. This reaction was completely abrogated by depletion of C. Therefore, spontaneous C activation on self cell membranes requires amplification at the C3 convertase step, which is restricted by membrane inhibitors such as 512Ag, thus preventing inflammation. A type II allergic response might preferentially occur at sites that exhibit an impairment of membrane inhibitor(s) of C3 convertase. However, although depletion of leukocytes by preadministration of cyclophosphamide did not reduce C3 deposition by anti-512Ag. Evans blue exudation was appreciably reduced, indicating that infiltrated leukocytes play a role in enhancing and prolonging inflammation initiated by local C activation.

Effects of long-term enalapril treatment on persistent microalbuminuria in normotensive type 2 diabetic patients: results of a 4-year, prospective, randomized study.

The beneficial effect of long-term treatment with an angiotensin-converting enzyme (ACE) inhibitor on urinary microalbumin excretion (UAE) and renal function was investigated in a 4 year, randomized prospective study in normotensive patients with non-insulin-dependent (Type 2) diabetes mellitus. Sixty-two normotensive patients with Type 2 diabetes mellitus and microalbuminuria but normal renal function were randomized to receive either enalapril 5 mg day-1 or no treatment. In the enalapril-treated patients, UAE was reduced from 115.4 +/- 80.1 to 95.6 +/- 61.7 mg 24 h-1 after 12 months (p < 0.05) and to 75.3 +/- 44.8 mg 24 h-1 after 48 months (p < 0.001). In the untreated group, UAE increased slowly from 93.9 +/- 69.9 to 150.0 +/- 144.5 mg 24 h-1 after 48 months. No changes in creatinine clearance, blood pressure or HbA1C were seen in either group during the 4-year period. In normotensive Type 2 diabetic patients with early stage of diabetic microalbuminuria. This effect is long-lasting and probably independent of the antihypertensive action of the drug.

Specific N-ras mutation in bone marrow within 48 H of 7,12-dimethylbenz[a]anthracene treatment in Huggins-Sugiyama rat leukemogenesis.

7,12-Dimethylbenz[a]anthracene (DMBA)-induced leukemias in Long-Evans rats consistently have an A --> T transversion at the second base of codon 61 in the N-ras gene. This mutation is also detected in the preleukemic stage. To determine when this specific N-ras mutation occurs in the early stages of leukemogenesis, we designed the mutant allele-specific amplification method, which was sensitive enough to detect one mutant cell among 10(6) normal cells. In the study reported here, N-ras mutation was found in bone-marrow cells 2 d after a single DMBA injection and thereafter throughout the preleukemic stage. These results show that DMBA induces a specific N-ras mutation soon after one DMBA injection and that this mutation is probably the first event in DMBA leukemogenesis.

Effects of FK506 and cyclosporin A on proliferation, histamine release and phenotype of murine mast cells.

Using mouse peritoneal cavity mast cells, we investigated the effects of FK506 and cyclosporin A (CsA) on cell proliferation and histamine release induced by anti-IgE antibody, calcium ionophore (A23 187), or neuropeptide (substance P). Both FK506 and CsA inhibited cytokine-dependent mast cell proliferation in a dose-dependent manner. The inhibitory effects of these compounds on mast cell proliferation was reversible; the removal of the chemicals from the incubation medium resulted in the reinitiation of mast cell proliferation. Flow cytometric analysis suggested that the inhibitory effect of FK506 and CsA was mostly due to G1/S boundary block, although a significant number of G2-arrested cells were also observed following FK506 treatment. Both FK506- and CsA-treated mast cells showed a similar inhibition of histamine release induced by A23187. However, CsA at higher concentrations inhibited the histamine release induced by anti-IgE antibody or substance P more markedly than FK506. Cellular histamine content was decreased by CsA treatment while FK506 had no effect. The staining properties of peritoneal mast cells changed from connective tissue-type mast cell-like to mucosal mast cell-like during CsA treatment but not during FK506 treatment. Thus FK506 and CsA have different effects on mast cell proliferation as well as histamine release, that might be associated with a phenotypic change of the cells during culture.

Influence of fluoride on secretory pathway of the secretory ameloblast in rat incisor tooth germs exposed to sodium fluoride.

Fluoride, which is an environmental toxicant, is a potent inducer of mottled enamel in humans and rats. To define the influence of fluoride on the secretory pathway in enamel fluorosis, mottled enamel was induced in the incisor tooth germs of rats by subcutaneous injections of sodium fluoride for 4 days, and then morphological and cytochemical changes of the secretory ameloblast were examined in the tooth germs with HRP-labeled lectin (Con A, GS-I, SBA and PNA) and En3 antibody labeling amelogenins. The accumulation of small vesicles on the route of the secretory pathway between the rER and the Golgi apparatus, disorder of Golgi stacks, and formation of abnormal large granules in distal cytoplasm were seen in the secretory ameloblast. Lectin staining patterns of the secretory ameloblast indicated the disturbance of the vesicular transport between the rER and the Golgi apparatus, and disorganization of the Golgi stack. Immunolabeling of the cell showed disruption of the sorting and fusion process on the secretory pathway. These results suggest that the fluoride disturbs the intracellular transport in the synthesis-secretory pathway of the ameloblast, and that this effect of fluoride on the synthesis-secretory pathway participates in the formation of enamel fluorosis.

Lingual factors enhance the increase of ornithine decarboxylase activity in rat jejunal mucosa after feeding.

Luminal nutrients are the main factors that stimulate ornithine decarboxylase (ODC) activity in rat intestinal mucosa following feeding. The aim of the present study was to determine whether lingual (oral) factors are related to the increase in jejunal ODC activity after feeding. ODC activity in the jejunum and liver was measured 3 hours after refeeding of 48-hour fasted rats. In the first experiment, rats were refed with a regular pellet, powder, or liquid diet. In the second experiment, rats were infused with the liquid diet through a gastric infusion tube following 48 hours' fasting. In the third experiment, the experimental rats had a gastric fistula that allowed free drainage from the stomach of all ingested liquid diet. In the fourth experiment, a truncal vagotomy was performed 1 week before the experiment. The increase of ODC activity in the jejunum of rats fed with the liquid diet was less than that of rats fed with the pellet diet or powder diet. The increase of ODC activity in the jejunal mucosa of rats infused through the gastric tube was less than that of rats fed per os, and the increase of ODC activity in the liver did not differ between these experimental groups. ODC activity did not increase in rats with a gastric fistula. Vagotomy did not affect the increase of jejunal ODC activity after feeding. In conclusion, the increase of ODC activity after feeding was attenuated in rats in which the diet was given by bypassing the mouth. This indicates that lingual factors enhance the increase of ODC activity in the jejunal mucosa after feeding, but the lingual factors alone do not increase ODC activity in the jejunum.

A case of Sézary syndrome associated with cutaneous transformation.

Sézary syndrome is a leukemic variant in the spectrum of cutaneous T cell lymphomas. In some cases of cutaneous T cell lymphoma, the skin tumors or lymph nodes show morphologic changes indistinguishable from those of a large-cell lymphoma; this has been termed transformation. We report a case of Sézary syndrome who developed multiple skin tumors that showed transformation, loss of multiple pan T-cell markers (CD3, 4, 5, 7, 8), and a concomitant expression of CD30 during the course of the disease. Some of the tumors spontaneously regressed following incisional biopsy or traumatic wounds. The patient died of pneumonia five years following the transformation.

Satisfactory remission achieved by PUVA therapy in Langerhans cell hisiocytosis in an elderly patient.

Langerhans cell histiocytosis is currently regarded as a reactive proliferative process of Langerhans cells rather than a malignancy. The disease is characterized by Langerhans cell infiltration of skin, lung, bone and other organs. We report a 74-year-old man with Langerhans cell histiocytosis who had generalized hemorrhagic and crusted papules. He also had diabetes insipidus. Because he did not have any severe constitutional symptoms or failure of vital organs, we applied topical PUVA treatment to his skin lesions, which responded well to the therapy. Diabetes insipidus, however, remained, in spite of X ray radiotherapy for the pituitary lesion.

[Detection and identification method of three mycobacterium species and genus specific detection by polymerase chain reaction and DNA hybridization with alkaline phosphatase labeled oligonucleotide probe].

Rapid detection and identification method of mycobacteria from specimen with infected mycobacteriosis was established by the combination of polymerase chain reaction and alkaline phosphatase labeled oligonucleotide probe (PCR-ALPDH). We prepaired four kinds of probes: Mycobacterium tuberculosis complex, M. avium, M. intracellulare, and mycobacterial-probe other than tuberculosis. PCR-ALPDH was compared with conventional methods using 234 specimens which suspected mycobacteriosis. In culture, 68 specimens (29.1%) were positive and 166 specimens (70.9%) were negative. In PCR-ALPDH, 88 specimens (37.6%) were positive and 146 specimens (62.4%) were negative. In 68 specimens which were positive in culture, the agreement of results of conventional identification and PCR-ALPDH for each probes were: 39/40 (PCR-ALPDH/Culture, 97.5%) in M. tuberculosis 5/9 (55.6%) in M. avium, 6/6 (100%) in M. intracellulare and 22/28 (88.6%) in MOTT isolation. Among 166 negative culture specimens, 27 specimens were positive in PCR-ALPDH. The results indicated that PCR-ALPDH method was applicable for the establishment of rapid and sensitive mycobacterial diagnostic system.

Autonomic dysfunction in hemodialysis patients with persistent hypotension.

To investigate autonomic mechanisms underlying persistent hypotension in long-term hemodialysis patients, high-frequency (HF, > 0.15 Hz) and low-frequency (LF, 0.04-0.15 Hz) components of heart rate variability and plasma noradrenaline were analyzed in 10 persistently hypotensive hemodialysis patients (group H), 11 normotensive patients (group N) and 10 healthy controls (group C). The HF amplitude, an index of cardiac parasympathetic activity, and LF-to-HF ratio, an index of sympathetic predominance, were in the order of groups C > N > H (p < 0.01). While the HF amplitude decreased with standing in all three groups (p < 0.05 for all), the LF-to-HF ratio increased only in groups N and C (p < 0.05 for both) but not in group H. Conversely, plasma noradrenaline level was in the order of groups C < N < H (p < 0.001). Furthermore, while the LF-to-HF ratio correlated positively with the plasma noradrenaline level in group C (r = 0.73, p < 0.01), it correlated negatively in group H (r = 0.69, p < 0.05). These results indicate that an impairment in both parasympathetic and sympathetic functions exists in hemodialysis patients with persistent hypotension, and that the apparent sympathetic dysfunction could result from a reduction in cardiovascular responsiveness to sympathetic stimulation.

Polymerase chain reaction of Borrelia burgdorferi flagellin gene in Shulman syndrome.

A 49-year-old man presented a progressive swelling and induration of the skin resulting in flexion contracture. He had a history of two tick bites at the age of 17 and 47 years. Serum anti-Borrelia-burgdorferi antibody was positive; isolation of B. burgdorferi from the skin lesion was unsuccessful. He had eosinophilia (white blood cells 8,300/microlitre, 33% eosinophils) and hypergammaglobulinemia. The diagnosis of Shulman syndrome (eosinophilic fasciitis) from clinical and histological findings was established. A part of the flagellin gene of B. burgdorferi was detected in a skin biopsy sample by using the polymerase chain reaction method. To the best of our knowledge, this is the first report of detection of B.-burgdorferi-specific DNA from a skin sample of Shulman syndrome.

Interferon therapy for chronic hepatitis C in habitual drinkers: comparison with chronic hepatitis C in infrequent drinkers.

OBJECTIVE: To assess the efficacy of interferon therapy for chronic hepatitis C among habitual drinkers. METHODS: Ninety-five hepatitis C virus ribonucleic acid-positive patients with chronic hepatitis were treated with four standardized regiments of interferon. Patients were divided into four groups based on the degree of daily alcohol consumption and duration of abstinence before treatment: 47 infrequent drinkers, 12 moderate drinkers who had consumed more than 23 gm but less than 69 gm of ethanol daily but stopped drinking for 39 +/- 18 months before therapy, 19 heavy drinkers I who had consumed more than 69 gm of ethanol daily but stopped drinking for 38 +/- 37 months before treatment, and 17 heavy drinkers II who consumed more than 69 gm ethanol daily soon before treatment. RESULTS: The rate of responders; in whom serum ALT levels remained normal for 6 months after the end of treatment, was in a decreasing order of: infrequent drinkers (36.2%), moderate drinkers (33.3%), heavy drinkers I (26.3%), and heavy drinkers II (5.9%) (p < 0.05, infrequent drinkers vs heavy drinkers II). The negative rate of serum hepatitis C virus ribonucleic acid 6 months after the end of treatment was in a similar order (27.7%, 25.0%, 15.8%, and 0%, respectively) (p < 0.05, infrequent drinkers vs heavy drinkers II). CONCLUSION: Heavy drinking will reduce the efficacy of interferon therapy for chronic hepatitis C, and the adverse effect of drinking on efficacy might be reversed, partly, by abstinence for a long period before treatment.

[Characteristics of regional sympathetic innervation in diabetic patients with silent myocardial ischemia assessed by 123I-metaiodobenzylguanidine imaging].

The purpose of this study was to clarify any association between clinically detectable silent myocardial ischemia (SMI) and myocardial 123I-metaiodobenzyl-guanidine (MIBG) uptake. Subjects of this study were (1) patients with SMI with diabetes (n = 15), (2) patients with angina pectoris with diabetes (n = 15), (3) patients with SMI without diabetes (n = 8) and (4) normal subjects (n = 23). Subjects underwent planar and single photon-emission-computed tomography (SPECT) imaging 15 min and 3 hours after injection of 123I-MIBG. H/M ratio was significantly lower in diabetic SMI (2.1 +/- 0.3) and non-diabetic SMI (2.3 +/- 0.3) than control subjects (2.6 +/- 0.3). The inferior-to-anterior wall count ratio (I/A) in diabetic SMI group was the lowest among all groups (p < 0.05). A significant difference was observed in relative regional uptake in the inferior segment of the distal left ventricle between SMI and angina group in patients with diabetes mellitus (p < 0.05). The decreased MIBG uptake in the inferior wall may be an important sign of cardiac sympathetic dysfunction, suggesting the abnormalities in cardiac nervous system play an important role in the mechanism of diabetic silent myocardial ischemia.

[Hereditary cardiac arrhythmia].

Although most of arrhythmia are secondary to cardiac disease, for example, ischemic heart disease and valvular heart disease, some types of arrhythmia are hereditary. Long QT syndrome (LQTS) is a hereditary cardiac disorder that causes syncope and sudden death from ventricular tachyarrhythmia. A boy eight years old, who had the familial history of LQTS in his sister and mother, admitted in our hospital. He had some episodes of syncope and markedly prolonged QT interval of 0.58 seconds in electrocardiogram. After follow-up of 12 years, he died suddenly because of interrupted medication including beta-blocker. Another patient with ventricular tachyarrhythmia also admitted. His family tree in which three young men died suddenly, revealed hereditary cardiomyopathy. Hereditary cardiac arrhythmia as shown the above clinically important because of prevention from sudden death, especially in young patients.

[Influence of hyperkalemia on clinical decision making].

INTRODUCTION: In our hospital, extreme values are promptly reported in routine test, one of three kinds of reporting system. We investigated the influence of hyperkalemia on clinical decision making and discussed critical levels of hyperkalemia and the significance of prompt report of it. MATERIALS AND METHODS: One hundred eighty six patients with more than 6 mmol/l of blood potassium detected during 1993 were studied. Each episode in a patient with different causes was classified as a separated case and finally 194 cases were studied. Cause of hyperkalemia, clinical symptoms, electrocardiogram, outcome and treatment were investigated. RESULTS AND DISCUSSION: The mortality was 14% in 6 approximately 7 mmol/l, 42% in 7 approximately 8 mmol/l and 100% in more than 8 mmol/l. Electrocardiograms showed high peaked T wave in 6 approximately 7 mmol/l, widening of QRS complex and bradycardia in 7 approximately 8 mmol/l, and ventricular fibrillation and flatter in more than 8 mmol/1. These results supported that 7 mmol/l was acceptable as a critical value. One hundred percent of outpatients and 79% of inpatients undergoing emergency tests were promptly treated by doctors, and 61% and 35% undergoing urgent tests, and 52% and 30% undergoing routine tests (prompt report) were done respectively. Prompt reports of extreme values were more useful in decision making for outpatients than inpatients. Urea-N was more important than potassium as a factor influencing decision making in both patients, because most patients had renal failure or heart failure. CONCLUSION: The mortality and electrocardiogram studies supported that 7 mmol/l was an acceptable critical value. Prompt reports of hyperkalemia were more significant to clinical decision making for medical care for outpatients than for inpatients.

Evaluation of cardiac sympathetic innervation with iodine-123-metaiodobenzylguanidine imaging in silent myocardial ischemia.

UNLABELLED: An abnormality of the autonomic nervous system caused by diabetic neuropathy is one of the major causes of silent myocardial ischemia (SMI). This study evaluated quantitatively the association between clinically detectable SMI and myocardial 123I-metaiodobenzylguanidine (MIBG) uptake in patients with diabetes. METHODS: Patients with SMI with diabetes, with SMI without diabetes, with angina pectoris with diabetes and normal control subjects participated in this study. Subjects underwent planar and SPECT imaging immediately and 3 hr after injection of [123I]MIBG, and exercise thallium scintigraphy. MIBG was quantified based on the myocardial-to-mediastinal ratio (H/M) of [123I]MIBG count, the inferior wall to anterior wall count ratio (I/A), the relative regional uptake (RRU), washout rate and corrected [123I]MIBG defect score. RESULTS: H/M ratio was significantly lower in diabetic SMI (2.1 +/- 0.3) and nondiabetic SMI (2.3 +/- 0.3) than in control subjects (2.6 +/- 0.3). RRU was the lowest in inferior wall of the left ventricle in the normal group (78 +/- 7). Compared with anterior segments, the uptake of [123I]MIBG was lower in inferior and apex segments in normal subjects (p < 0.05). A significant difference was observed in RRU in the inferior segment of the distal left ventricle between SMI and angina groups (p < 0.05) among patients with diabetes mellitus. I/A ratio in the diabetic SMI group was the smallest (p < 0.05). However, no significant difference was observed in the corrected [123I]MIBG defect score among all groups. CONCLUSION: The MIBG uptake was heterogeneous in normal subjects. Decreased MIBG uptake in the inferior wall may be an important sign of cardiac sympathetic dysfunction, suggesting that the abnormalities in cardiac nervous system play an important role in the mechanism of silent myocardial ischemia.

Radioimmunoprecipitation assay for glutamic acid decarboxylase antibodies evaluated clinically with sera from patients with insulin-dependent diabetes mellitus.

We evaluated a new, commercially developed radioimmunoprecipitation assay for measuring glutamic acid decarboxylase (GAD) antibodies by using recombinant human GAD65. The intra- and interassay CVs were 8.0% (n = 20) and 8.6% (n = 15), respectively. We found GAD antibodies in 74% (23 of 31; 95% confidence interval 55-88%), 70% (14 of 20; 46-88%), and 65% (28 of 43; 49-79%) of patients at, respectively, < or = 1 year, 1-2 years, and 2-4 years after the onset of insulin-dependent diabetes mellitus (IDDM) and in 30% (30 of 99; 21-40%) of patients with long-term diabetes (4-22 years). We also detected GAD antibodies in 8% (9 of 106; 4-16%) of patients with non-insulin-dependent diabetes mellitus (NIDDM). The frequency of GAD antibodies in the NIDDM group was markedly higher in the insulin-deficient patients [67% (6 of 9; 30-93%)], who initially were nonketotic and non-insulin-dependent for > or = 6 months but later became insulin dependent, than in the non-insulin-deficient patients [3% (3 of 97; 1-9%)]. This new commercial assay is easy to use and provides a specific and sensitive method for evaluating GAD antibodies in IDDM.
