[Antitumor effector cells in recombinant IL-2 activated tumor infiltrating lymphocytes from human solid cancers].

Tumor infiltrating lymphocytes (TIL) isolated from 46 human solid cancers, including 24 renal cell carcinoma (RCC), 15 cancers of digestive organs and 7 other malignancies, were cultured with 1000 units/ml of recombinant interleukin-2 (rIL-2). Induction of activated TIL was achieved in 41 cases (89.1%). Their antitumor characteristics and cell surface antigen expressions were examined using 4 hr 51Cr release assay and flow cytometric analysis in long-term culture. Cytotoxic activity of rIL-2 activated TIL derived from RCC was significantly higher than that from other tumors, but the TIL could lyse various types of cells and specificity for autologous tumor cells could not be demonstrated in any of the cultures. Though CD3+ lymphocytes were predominant in all fresh TIL preparations, antitumor activity of rIL-2 activated TIL correlated with the increase of a cell population expressing the Leu19 antigen without the CD3 antigen. Depletion of selected cell types from TIL cultures using direct complement dependent cytolysis followed by cytotoxicity tests identified the Leu19+ CD3- CD16- cells as the major effector population. CD3-, CD16+ NK cells held minor antitumor cytotoxicity and there was no detectable cytotoxic activity in the CD3+ T lymphocytes.

Surgical treatment of the Wolff-Parkinson-White syndrome in children.

From 1986 to 1989, seven children ranging in age from 5 months to 16 years underwent surgical treatment for the Wolff-Parkinson-White syndrome at the Shiga University of Medical Science. None of the patients had any other associated congenital heart disease. There was a right free wall accessory pathway in four patients and a left free wall accessory pathway in three. Surgical ablation of these accessory pathways was performed on eight occasions, using the endocardial approach three times and the epicardial approach five. All the children are alive and none has since had episodes of tachycardia. Only one patient had a recurrent delta wave, which was noted 18 months after the operation. Surgical ablation of the accessory pathway for the Wolff-Parkinson-White syndrome can be performed safely, even in infants and children; it is concluded that this useful procedure is capable of improving a patient's quality of life.

Acute extensive necrosis of the visceral organs after repair of a ruptured thoracic aortic aneurysm.

A 47-year-old woman was admitted to our hospital for repair of a ruptured thoracic aortic aneurysm. Her post-operative course was uneventful, but she had persistent complaints of anorexia and general fatigue that failed to improve. Thirty-one days after surgery, she complained of severe abdominal pain. Abdominal computed tomography (CT) and X-ray revealed extensive necrosis of the visceral organs. She underwent emergency abdominal exploration and was found to have necrosis of the liver, gallbladder, stomach, entire small bowel and colon. The extensive necrosis made resection of the involved organs unfeasible. The patient died one day after exploratory laparotomy.

[Pacemaker implantations in children with regard to the site of implantation].

We reviewed thirty eight children, ranged 3 days to 15 years (mean, 7.2 years) of age, who underwent permanent pacemaker implantations in our hospital. Long-term results including the site of generator pocket were discussed. They are alive and well except two early and three late deaths that were not related to the pacemaker implantation. Twenty-seven epicardial and 11 endocardial electrodes were implanted at the initial operations. Thirteen pulse generators implanted in subaxillar position had no complications, otherwise, 12 subcostal implantation resulted 4 skin necrosis and 3 lead fractures. Eleven anterior chest implantations used for relatively older children resulted in one skin necrosis. We recommend subaxillar implantation for children, especially for small infants. The replacements of pulse generators due to battery depletion were done between 1.7 to 5.5 years (mean 4.0 years). Because epicardial implanted electrodes often show higher stimulation threshold than epicardial ones, pacing rate should be as lowered as possible if the patient had no symptom. Stab-in leads placed on the atrial wall sometimes caused the elevation of stimulation threshold and undersensing. We conclude that the development of more reliable and stable electrode is desired to improve the long term results of pacemaker therapy in children although the improvement of pulse generator itself and the operative procedure.

Impact of stress on serum gastrin in Zollinger-Ellison syndrome.

We report an impressive case with Zollinger-Ellison syndrome (ZES), in which stress-induced sympathetic discharge influenced serum gastrin. Our patient was a 35-yr-old female who complained of frequent and massive vomiting (more than 4000 ml of gastric juice) which was aggravated especially by psychosocial stress. Basal hypergastrinemia (1900 pg/ml) was found after the admission. The most striking finding was that laboratory stress dramatically increased serum gastrin (from 1900 to 5400 pg/ml) and plasma noradrenaline (from 180 to 1130 pg/ml). Mental arithmetic stress further enhanced hypergastrinemia (5800 pg/ml) with a concomitant increase in plasma noradrenaline (1240 pg/ml). Neostigmine (10 micrograms/kg im) also increased serum gastrin up to 6100 pg/ml but propranolol (40 micrograms/kg i.v.) reduced these elevations (noradrenaline: 990 pg/ml, gastrin: 5000 pg/ml). In this case, the effect of stress on serum gastrin mimicked the effect of catecholamine infusion in ZES. These findings suggest that psychological stress induces serum gastrin secretion via beta-adrenoceptor with exacerbation of symptoms in some cases with ZES.

[Post-gastrectomy gallstone disease].

Biliary tree of gastrectomized patients was examined with ultrasonography to investigate the factors affecting post-gastrectomy gallstone formation. Only gallbladder stone was found in 57 (18.9%). In 11 (20.0%) of 55 patients, stones had developed within 2 years after surgery, but no difference in incidence were observed by sex and age. Sixty four gastrectomized gallstone patients were examined in respect to the kind of gallstone. Patients with cholesterol gallstones were 18 (28.1%), patients with black gallstones were 29 (45.3%) and patients with calcium bilirubinate gallstones were 17 (26.6%). The incidence rate of cholesterol gallstones was lower and the rate of black gallstones and calcium bilirubinate gallstones was higher than that of non-gastrectomized patients. In conclusion, the incidence of pigment gallstones is very high in gastrectomized patients.

[Functional and phenotypic analysis of tumor infiltrating lymphocytes derived from solid tumors and effusion associated lymphocytes in malignant ascites cultured in recombinant interleukin 2].

Tumor infiltrating lymphocytes (TIL) and effusion associated lymphocytes (EAL) were isolated from 7 human solid tumors and 6 malignant ascitic fluids respectively, and cultured in rIL2 (700JRU/ml)-containing medium. Long-term culture (> 14 days) of separated lymphocytes with exponential increase in cell number was achieved in 5 EAL-cultures, whereas in only 2 TIL-cultures. rIL2-expanded TIL and EAL manifested significant cytotoxicity in a 4-hrs chromium release assay. The maximum NK and LAK activity were reached 21 days and 14 days after starting incubation with rIL2 respectively, followed by a rapid decrease in cytolytic potential without declining growth rate of the cells. Phenotypic analysis showed the majority of the freshly isolated TIL and EAL were CD3+ T cells, and CD16+ NK cells were rarely identified in TIL. With induction of LAK cell activity, CD8+ T cells predominantly increased in TIL-cultures, while both CD4+ and CD8+ T cells and CD16+ NK cells were increased in EAL cultures. In activated TIL most cytolytic activity was found in CD8+ T cells, in contrast CD16+ NK cells were responsible for it in activated EAL. These results indicated that EAL and TIL have similar properties in which they coexist with cancer cells at tissue level and were capable of expanding and acquiring LAK cell activity in the presence of rIL2, but apparently differ in their mechanism of rIL2-mediated activation.

[Effect of intra-venous hyperalimentation on hepatic energy metabolism after hepatic resection in the cirrhotic rat].

We evaluated experimentally the effect of administration of glucose on hepatic energy metabolism, mitochondrial functions and lipid metabolism after hepatic resection. The 75% hepatic resection was performed on thioacetamide induced cirrhotic rat. Glucose solution with 30 Cal/kg/day (group I) or 200 Cal/kg/day (group II) was administrated intravenously for 48 hours after hepatic resection. Blood levels of ketone bodies and glucagon, tissue activities of ATP synthesis and tissue content of adenine nucleotides were measured in both groups. Hepatic energy charge was significantly decreased in group II, comparing with group I. When fatty acid was used as a substrate for respiratory system, the activity of ATP synthesis on mitochondria after hepatic resection rose, while in case pyruvate was used as a substrate the value was declined. On the levels of serum free fatty acids and ketone bodies, there were no remarkable changes in group I, while in group II, obvious declines were investigated. It is suggested that the overload of glucose in the early stage after hepatic resection is unfavorable to hepatic energy metabolism because of decline of serum free fatty acid by hyperinsulinemia and suppression of fatty acid oxidation by decline of blood glucagon.

Do recent advances in diagnosis and operative management improve the outcome of gallbladder carcinoma?

To study the effect of recent advances in diagnosis and operative management on survival of patients with gallbladder carcinoma, records of 26 patients who underwent resection between 1960 and 1978 (group 1) and 25 patients who underwent resection between 1979 and 1991 (group 2) were compared. The operative mortality rate was lower in group 2 (0%) than in group 1 (12%). The 5-year survival rate was higher in group 2 (60.8%) than in group 1 (30.4%; p < 0.01). Although preoperatively undiagnosed cases were not less in group 2 than in group 1, cases diagnosed with certainty before surgery were increased significantly and suspected cases of gallbladder carcinoma were decreased in group 2 (p < 0.05) as a result of recent advances in hepatobiliary imaging techniques. Twenty patients in group 2 (80%) and 13 in group 1 (50%) had tumors less than 30 mm in diameter (p < 0.05). Nine patients in group 2 (36%) had tumors at an early stage (tumors limited to mucosal or muscular layer), and only two (8%) in group 1 had early-stage tumors (p < 0.05). The incidence of curative resection was higher in group 2 (76%) than in group 1 (27%; p < 0.001). Earlier diagnosis and a higher chance of curative resection, together with improved surgical techniques, may contribute to the better survival seen in the recent 12-year period.

A computer simulation of the plasma leakage through a vascular prosthesis made of expanded polytetrafluoroethylene.

We explored the blood-retaining mechanism of a vascular prosthesis made of expanded polytetrafluoroethylene through analysis of its structure and physicochemical properties. Plasma leakage through this vascular prosthesis was simulated by computer to explore its etiology. These examinations disclosed that leakage is dependent upon the inner pressure and the density of fibers. In other words, the study revealed that the mean distance between fibers constituting the wall of the expanded polytetrafluoroethylene vascular prosthesis is increased by tension (that is, inner pressure), resulting in an increased probability of leakage. It was additionally found that a thin membrane is formed on the polytetrafluoroethylene surface if blood in contact with the surface is dried. This membrane was found to reduce the water-repelling property of polytetrafluoroethylene and to make it impossible to preserve the inter-fiber liquid surface, thus causing leakage through the expanded polytetrafluoroethylene vascular prosthesis.

[Removal of infected transvenous electrodes requiring cardiopulmonary bypass or extraction sheath].

Infection of four transvenous pacemaker electrodes were experienced in three patients and removed them using different techniques. The first patient had two infected electrodes. One was removed by direct traction and the other required cardiopulmonary bypass for its removal. In the second patient, one electrode was removed using a new extraction system called 'Lead Removal Kit' without thoracotomy. In the third patient, the kit was used during cardiopulmonary bypass. All these patients survived and none had recurrent infection. Our experiences proved that this Lead Removal Kit can minimize the risks for the removal of the infected pacemaker electrodes.

The mucus-hypersecreting tumor of the pancreas. Development and extension visualized by three-dimensional computerized mapping.

BACKGROUND: Mucus-hypersecreting tumor of the pancreas appears as dilated ducts and cystic spaces filled with mucus. To determine where such tumors arise and how they extend, computer-aided three-dimensional reconstruction was done of the ductal system. This also was used to visualize the spatial relationships among epithelial hyperplasia, dysplasia, and carcinoma in situ (CIS). METHODS: Surgically removed pancreases were studied from 12 patients with mucus-hypersecreting tumors. The specimens were fixed in buffered formaldehyde solution 10%, embedded in paraffin and semiserially sectioned at 3 microns at an interval of 60 microns. The ductal contours were differentiated among ducts lined by ordinary epithelia, hyperplastic epithelia, dysplastic cells, or CIS and were inputted into a computer system that integrated a three-dimensional image of ducts in the display. RESULTS AND CONCLUSIONS: (1) The tumors arose in the main pancreatic duct or its subbranches, and the cysts corresponded to segments expanded by the superficial growth of tumor cells; (2) areas of CIS arose in zones of preceding dysplasia, suggesting a dysplasia-carcinoma sequence; and (3) dysplastic or cancerous cells often extended intraductally over the dilated segments of ducts.

Effectiveness of planar image and single photon emission tomography of thallium-201 compared with gallium-67 in patients with primary lung cancer.

A comparative study of planar images and single photon emission tomography (SPET) of thallium-201 chloride and gallium-67 citrate was performed in 38 patients with proven primary lung cancer to detect the primary lung tumour and to establish the presence of metastasis in the lung hilum and mediastinum. The findings of planar images and SPET were compared with the pathological findings after thoracotomy. It was shown that 201Tl studies were superior to 67Ga studies for evaluation of the primary lesion and lymph node metastases.

An experimental study on the gastric acid and gut hormone secretion after pylorus preserving duodenectomy in dogs.

To clarify changes in gastric acid and gut hormone secretion after pylorus-preserving pancreaticoduodenectomy (PPPD), an experimental study was performed using a model of pylorus-preserving duodenectomy in dogs previously provided with Heidenhain pouch (HP). The duodenectomy involves resection of the duodenum and 10 cm of the proximal jejunum preserving 2 cm of juxtapyloric duodenum and round-shaped duodenal wall around pancreatic papilla. Reconstruction was done by anastomosing the rho-shaped jejunal loop to gallbladder, juxtapyloric duodenum and peripapillar round-shaped duodenal wall with ligation of the common bile duct. For these dogs, intravenous glucose tolerance test (IVGTT), oral glucose tolerance test (OGTT), meal ingestion test (TM) and histological studies of pancreatic specimen obtained at autopsy were performed investigating pancreatic, gastric acid and gut hormone secretion. Preservation of endocrine and exocrine pancreatic secretion after operation demonstrated our experimental model to be adequate for evaluation of the factor of duodenectomy in PPPD on gastric acid and gut hormone secretion avoiding the influences of changes in pancreatic secretion. Postprandial gastric acid secretion from HP did not change significantly after operation. Postprandial secretion of gastrin, glucagon, GIP and enteroglucagon did not alter significantly after operation. These results indicated that in the clinical PPPD procedure, preservation of more than 2 cm of duodenum from the pylorus produced neither postprandial gastric acid hypersecretion, which might be cause of postoperative stomal ulcer, nor any change of related gut hormone secretion.

De novo expression of aromatase in gastric carcinoma. Light and electron microscopic immunohistochemical and immunoblot study.

We have performed immunohistochemical and immunochemical studies of steroidogenic enzymes involved in estrogen biosynthesis in 30 cases of gastric carcinoma in order to investigate possible in situ production of estradiol (E 2) in carcinoma cells. Positive incidence of immunoreactivity for E 2, testosterone (T), cholesterol side-chain cleavage enzyme (P-450 scc) and aromatase (P-450 arom) were 17/30 (56.7%), 11/30 (36.7%), 3/30 (10.0%) and 23/30 (76.7%), respectively on light microscopy. Estrogen receptor (ER) immunoreactivity was not observed in any of the 30 cases examined. Normal gastric mucosa was negative for P-450 arom and P-450 scc. Examination of serial sections revealed that immunoreactivity of E 2 and P-450 arom were located in the same cells of carcinomatous glands. Immunoelectron microscopy demonstrated that E 2 and P-450 arom were located along the membrane and cisternae of smooth endoplasmic reticulum (sER). Western blot analysis showed one major band of 55 kDa of P-450 arom in the gastric carcinoma tissues examined. Retrospective analysis of immunohistochemistry of E 2 in 108 cases of gastric carcinoma revealed that E 2 positive carcinoma cases were likely to demonstrate better survival rate than negative cases. These results above strongly suggest that E 2 is produced by de novo expressed aromatase in gastric carcinoma cells and is possibly involved in the biology of gastric carcinoma cells.
