[Left ventricular pressure-volume diagram determined by forward and backward formatting of radionuclide ventriculography and analog pressure data].

Pressure-volume (PV) loop is of great value for the assessment of left ventricular (LV) function, but its clinical application has been limited by methodological complexity. A new system was developed to make accurate loop with simplified procedure, and was applied to clinical and interventional study. The system constitutes of a mobile gamma camera, a poly-amplifier and a data processor (GMS-550U, Toshiba Medical) installed in cardiac catheterization labo for simultaneous raw data handling and successive analysis. Since LV time activity curve (TAC) obtained by usual ECG gating is not fully reliable for a entire cardiac cycle, radionuclide data acquired in list mode was formatted forward and backward from ECG trigger together with analog data of LV pressure, ECG and PCG. PV loops were drawn in 10 patients (OMI, AP, MR, HCM) and 5 normals before and after infusion of angiotensin-II (AII), and Emax and LV work (systolic; SW, diastolic; DW, net; NW = SW - DW) were measured. Radionuclide ventriculography was safely performed with cardiac catheterization even in patients with congestive heart failure. Satisfactory PV loops were obtained by the advantage of simultaneous acquisition of RNV and analog data. Changes of ECG, PCG, volume, pressure and derived indices through one cardiac cycle were readily comparable each other. Peak LV pressure (mmHg) increased from 134 to 159 and then 182 by infusion of AII, but no change in heart rate was observed Emax was higher in normals and AP (mean 1.96 mmHg/ml/m2) than in OMI and MR (range of 0.85-1.36). SW increased in response to rise of LV pressure in all subjects. NW increased in normals and AP, but decreased in OMI and MR with relative increase in DW. In conclusion, this new system is feasible for repetitive studies under drug intervention, since it makes accurate PV loop under physiologic state, i.e. without pacing and volume overloading. Variable changes of SW, DW, and NW in response to afterloading were clarified, which may be useful for the evaluation of cardiac reserve in normal and diseased heart.

Immunochemical characterization of the suppressor factor from early human decidual cells.

An immunosuppressive factor was obtained from culture supernatants of early human decidual cells. The suppressor factor was concentrated by gel filtration in a fraction with a molecular weight between 43,000 and 67,000 daltons. It was further purified by biochemical methods. Four peaks were obtained in the fraction with molecular weight between 43,000 and 67,000 daltons by anion exchange chromatography. Only the second peak had immunosuppressive activity in MLR. Lentil-lectin affinity chromatography of this suppressor factor showed that the suppressor factor had no affinity for lentil-lectin sepharose. Isoelectric focusing of the suppressor factor demonstrated four bands. The protein isoelectric (PI) point was approximately 7.50 in one band and between 6.85 and 7.35 in the other three bands. These results demonstrate that the suppressor factor is not glycoprotein but protein, whose PI is between 6.85 and 7.50. The suppressive effect of this purified factor on lymphokine production and lymphocyte activation was investigated. The addition of the purified suppressor factor to a culture of PBL stimulated with PHA suppressed not only IL-2 production and gamma-INF production, but also BSF-2 production. IL-2 receptor expression and transferrin receptor expression of PBL stimulated with PHA were also suppressed by addition of the suppressor factor. These results demonstrate that this suppressor factor inhibits lymphokine production and lymphocyte activation.

Generation of hypophosphatemia in rats by continuous oral administration of cadmium.

To obtain further information on the negative calcium balance caused by cadmium (Cd), the factors associated with serum calcium and phosphorus homeostasis other than inhibition of intestinal calcium absorption were studied by using parathyroid hormone (PTH) and 1 alpha-hydroxycholecalciferol (1 alpha-OH-D3). In rats exposed to Cd for 30 or 90 days, the concentrations of serum calcium after treatment with PTH, parathyroidectomy (PTX) or 1 alpha-OH-D3 showed almost the same patterns as those of control animals. It was considered that the mechanism of regulation of calcium in Cd-exposed rats was normal. The continuous oral administration of Cd generated hypophosphatemia in rats. On the other hand, in 30- and 90-day-treated rats, the low concentration of serum phosphorus caused by Cd was further decreased by administration of PTH, whereas it was increased by PTX. The hypophosphatemia found in rats exposed to Cd for 30 days, but not for 90 days, was reversed by treatment with 1 alpha-OH-D3. From these results, it was concluded that the hypophosphatemia caused by long term oral administration of Cd resulted from secondary hyperparathyroidism, due to inhibited calcium absorption from the intestine, as was demonstrated previously.

4-Hydroxyretinal, a new visual pigment chromophore found in the bioluminescent squid, Watasenia scintillans.

The bioluminescent squid, Watasenia scintillans has three visual pigments. The major pigment, based on retinal (lambda max 484 nm), is distributed over the whole retina. Another pigment based on 3-dehydroretinal (lambda max approximately 500 nm) and the third pigment (lambda max approximately 470 nm) are localized in the specific area of the ventral retina just receiving the downwelling light. Visual pigment was extracted and purified from the dissected retina. The chromophores were then extracted and analyzed with HPLC, NMR, infrared and mass spectroscopy, being compared with the synthetic 4-hydroxyretinal. A new retinal derivative, 11-cis-4-hydroxyretinal, is identified as the chromophore of the third visual pigment of the squid.

The effect of cyclosporine on mortality and renal function in living related pediatric kidney transplant recipients.

The outcome, incidence of acute rejection episodes, complications and cyclosporine (CyA) induced nephrotoxicity were studied in 10 pediatric kidney transplant recipients who were grafted from one-haplotype indentical parent with immunosuppression of CyA and prednisolone (Pred). Excellent patient and graft survival could be achieved in this population with low incidences of acute rejection or serious complications as when compared with the results of azathioprine (AZ) treated pediatric patients. With a mean follow-up of 12.9 months (range 1 to 50 months), the patient survival rate was 100 per cent and the graft survival rate was 100, 84, 84 and 84 per cent at 1, 2, 3 and 4 years post transplantation, respectively. Serum creatinine levels in the group were 0.97, 1.17, 1.14 and 1.2 mg/dl at 3, 6, 12 and 24 months post transplantation, respectively. The incidence of treated acute rejection episodes was 20 per cent (2 out of 10) in the CyA-treated children, whereas it was 53 per cent (9 of 17) in the Az-treated children. Five children who had undergone transplant surgery before they were 11 years old displayed linear growth in height after their transplantation. There have been no opportunistic infections, aseptic necrosis or peptic ulcers in this group and cyclosporine nephrotoxicity has not been a serious problem in the pediatric recipients. Only 10 per cent (1 out of 10) of the recipients displayed acute nephrotoxicity and only one recipient has converted from CyA + Pred to CyA + AZ + Pred (Three drug therapy) due to persistent nephrotoxicity.(ABSTRACT TRUNCATED AT 250 WORDS)

Infiltrating ductal carcinoma developing within cystosarcoma phyllodes--a case report.

Malignancy in cystosarcoma phyllodes is uncommon and often confined to the stromal element. An extremely rare case of infiltrating ductal carcinoma developing within the stroma of cystosarcoma phyllodes is reported herein. A breast tumor with a diameter of 15 cm, which was diagnosed as cystosarcoma phyllodes, was excised from the right breast of a 47-year old woman. The histopathological examination revealed that hyperplastic ductal epithelial cells with dark cytoplasm and enlarged hyperchromatic nuclei were infiltrating the stroma. Thus, a diagnosis of ductal carcinoma within cystosarcoma was made. Subsequently, a standard radical mastectomy was performed. No recurrence or metastasis has been observed over the post-operative period of 5 years and 6 months.

Effect of cyclosporine on the endocrine and exocrine pancreas in kidney transplant recipients.

In order to assess whether cyclosporine (CsA) affects the endocrine and exocrine pancreas, 105 patient courses comprised of 87 living related donor (LRD) and 18 cadaver donor (CAD) transplants treated with cyclosporine and prednisolone (Pred) were compared with the results of historical controls of 170 LRD and 10 CAD transplants treated with azathioprine (Az) and Pred. All of the recipients were followed for over 6 months after transplantation. There were no differences in age, sex, Broca index, family history, and preoperative evaluation on diabetic dispositions between the two treatment groups. The incidence of diabetes mellitus (DM) requiring insulin therapy was higher in CsA-treated recipients (18/105, 17.1%) than in Az-treated recipients (23/180, 12.8%; P less than 0.05), although both the daily Pred and cumulative doses of methylprednisolone (MP) at the onset of DM were significantly smaller in the CsA group than in the Az group (26.1 +/- 2.2 mg v 41.4 +/- 3.4 mg, P less than 0.01 and 3,086 +/- 626 mg v 7,133 +/- 1,129 mg, P less than 0.01, respectively). Diabetic patients with CsA showed higher levels of blood glucose (401 +/- 46 mg/dL), but lower amounts of urinary glucose (40 +/- 4.3 g/d) compared with patients treated with Az (239 +/- 31 mg/dL, and 61.4 +/- 4.6 g/d, respectively, P less than 0.05). In the CsA group, the onset of DM was related to high CsA plasma trough levels (greater than 350 ng/mL) in 23% of patients. Insulin could be withdrawn within 3 months in six of eight patients who had been converted from CsA to Az.(ABSTRACT TRUNCATED AT 250 WORDS)

Adaptation of a deep-sea cephalopod to the photic environment. Evidence for three visual pigments.

Watasenia scintillans, a bioluminescent deep-sea squid, has a specially developed eye with a large open pupil and three visual pigments. Photoreceptor cells (outer segment: 476 micron; inner segment: 99 micron) were long in the small area of the ventral retina receiving downwelling light, whereas they were short (outer segment: 207 micron; inner segment: 44 micron) in the other regions of the retina. The short photoreceptor cells contained the visual pigment with retinal (lambda max approximately 484 nm), probably for the purpose of adapting to their environmental light. The outer segment of the long photoreceptor cells consisted of two strata, a pinkish proximal area and a yellow distal area. The visual pigment with 3-dehydroretinal (lambda max approximately 500 nm) was located in the pinkish proximal area, giving high sensitivity at longer wavelengths. A newly found pigment (lambda max approximately 471 nm) was in the yellow distal area. The small area of the ventral retina containing two visual pigments is thought to have a high and broad spectral sensitivity, which is useful for distinguishing the bioluminescence of squids of the same species in their environmental downwelling light. These findings were obtained by partial bleaching of the extracted pigment from various areas of the retina and by high-performance liquid chromatographic analysis of the chromophore, complemented by microscopic observations.

The in vivo immunosuppressive action of suppressor cells from alloantigen-cyclosporine-treated mice and the capacity of spleen cells to release interleukins and gamma-interferon.

Antigen-nonspecific suppressor T cells were identified in spleens of mice rendered unresponsive by sensitization of allogeneic antigen in combination with cyclosporine (CsA) treatment. Suppressor cells were obtained from C57BL/6 (B6, H-2b) mice treated with a single i.p. injection of 1 x 10(7) allogeneic P815 (H-2d) cells combined with a five-day course of CsA, a group that did not show any cytotoxic activity of spleen cells against P815 targets. These noncytolytic spleen cells displayed suppressor activity on the induction of cytotoxic T (Tc) cells of normal lymphocytes against not only P815 stimulator (80.9% suppression, P less than 0.01, responder:additional cell ratio = 2.5:1) but also third-party BW5147 (H-2k) stimulator (68.2% suppression, P less than 0.01). The unresponsive state appears to be due to suppressor T (Ts) cells that are nonadherent to plastic or nylon-wool, 1500 rads-sensitive, and Thy-1-positive. Capacities of spleen cells from CsA-P815-treated mice to release cytokines (interleukin 1 [IL-1]), interleukin 2 [IL-2], interleukin 3 [IL-3], and gamma-interferon [gamma-IFN]) were examined. Spleen cells from CsA-P815-treated B6 mice displayed 84.1%, 91.7% and 90.8% inhibition (0.35 +/- 0.07 U/ml, 1.4 +/- 0.29 U/ml, and 7.0 +/- 0.9 U/ml) of IL-1, IL-2, and gamma-IFN production compared with normal mice (2.2 +/- 0.54 U/ml, 16.9 +/- 2.1 U/ml, and 76.0 +/- 3.1 U/ml, P less than 0.01), respectively. However, IL-3 production was significantly less inhibition (46.1%, 2.35 +/- 1.0 U/ml in CsA-P815-treated mice and 4.36 +/- 1.7 U/ml in normal mice) compared with other cytokines (IL-1, IL-2, gamma-IFN). Two systems were employed to assess the immunosuppressive efficacy of antigen-nonspecific Ts cells in vivo. First, adoptive transfer (i.p.) of spleen cells harvested from CsA-P815-treated mice ten days after treatment on 3 consecutive days (days 0, 1, 2) at a 3 x 10(7) cell dose into virgin B6 mice that were immunized with P815 cells (1 x 10(7), day 0) completely inhibited the development of Tc cells against P815 targets (5% specific cytolysis, effector:target ratio [E:T] = 200). The suppressor effect was immunologically nonspecific; adoptive transfer of Ts cells from CsA-P815-treated mice also abrogated the development of Tc cells against third=party BW5147 cells.(ABSTRACT TRUNCATED AT 400 WORDS)

Isolation of a bovine rotavirus with a "super-short" RNA electrophoretic pattern from a calf with diarrhea.

A rotavirus with a "super-short" RNA electropherotype was isolated from a calf with diarrhea and was designated VMRI strain. Segments 10 and 11 of this rotavirus migrated more slowly than did those of bovine rotavirus strains NCDV, B641, and B223. The electrophoretic pattern of the VMRI strain was similar to that reported for rotaviruses with super-short RNA electropherotypes from humans and rabbits. Northern (RNA) blot hybridization indicated that gene 11 of the VMRI strain was altered and migrated between gene segments 9 and 10. The subgroup of the VMRI strain was shown to be subgroup I. The VMRI strain of bovine rotavirus was neutralized by antisera containing polyclonal antibodies to rotavirus serotype 6 (bovine rotavirus serotype I) strains NCDV and B641 and by ascitic fluid containing monoclonal antibodies directed to VP7 of serotype 6 rotavirus. The VMRI strain was not neutralized by either polyclonal or monoclonal antibodies to strain B223 (bovine rotavirus serotype II). Collective data on the neutralization of the VMRI strain with monoclonal antibodies and polyclonal antibodies suggest that this virus is a member of the NCDV group (serotype 6) of rotaviruses (bovine rotavirus serotype I).

Pituitary and adrenal functions during late asthmatic responses.

The purpose of the present study was to examine the role of low cortisol levels which were observed during late asthmatic responses (LAR). Selective functions of the adrenals and pituitary glands were studied in three groups of asthmatics who showed LAR, immediate asthmatic responses (IAR), and DAR (dual asthmatic responses). Serial plasma ACTH levels in each group showed no apparent abnormalities when compared with diurnal levels. The cortisol response to ACTH changed within the normal range in all three groups with minor differences. We suggest that low cortisol levels that appear during LAR are not principally due to any dysfunction in the pituitary adrenal system.
