[Aminoglycoside for the infectious disease associated with acute leukemia. Gentamicin intravenous drip administration (author's transl)].

In 29 patients of infection diseases associated with acute leukemia, 60 to 160 mg/day of gentamicin was injected intravenously drip infusion. The results showed that 69.0% of the cases were clinically effective (excellent+good+fair), and 31% ineffective. Abnormal GOT and GPT level was temporarily noted in 2 cases and audiovestibular dysfunction in 1 case, which improved after the discontinuation of the injection. Safety of the intravenous drip administration of gentamicin was discussed. The patients with acute leukemia might easily have opportunistic infection by Gram negative rods and it is more advantageous to administer intravenously drip infusion than intramuscularly because of hemorrhagic tendency.

[Antihypertensive and related hemodynamic effects of a thiazide in desoxycorticosterone (DOC)/saline hypertensive rats].

Antihypertensive and related hemodynamic effects of trichlormethiazide (TCM), a thiazide, in DOC/saline hypertensive rats were investigated. To prepare hypertensive rats, 7-week-old male Wistar rats were subjected to nephrectomy of the left kidney, subcutaneously administered DOC (5 mg/kg X 3 times/week, or a 25-mg tablet), and given 1% saline solution as drinking water. When the systolic blood pressure of the rats reached about 200 mmHg, once-a-day oral administration of an arabic gum suspension of TCM was started. The results were: 1) Oral administration of TCM at no less than 3 mg/kg/day showed a dose-dependent, lasting antihypertensive effect throughout the experimental periods. 2) TCM administration effectively prevented the appearance of malignant hypertensive symptoms such as body weight loss, ischemia of the ocular fundus, and slight paralysis of extremities with the following pathological changes: hyalinization and fibrinoid necrosis of renal arteries and glomeruli, degeneration of renal tubules, and cardiac hypertrophy. 3) Although TCM had no effect throughout the experimental periods on the cardiac output (CO), which was determined by a modified dye-dilution method, total peripheral resistance (TPR) decreased from the first week of TCM administration. The antihypertensive effect of TCM in DOC/saline hypertensive rats is presumed to be associated with the decrease of TPR from early stages of therapy.

Smooth muscle tumors of the stomach.

In retrospect, we analyzed 12 cases of gastric smooth muscle tumor, nine cases of leiomyomas and three of leiomyosarcomas. In only half the number of these patients was an adequate preoperative diagnosis made. Ulceration of the tumor apparently led to the misjudgment in some cases. Features more relevant to leiomyosarcoma than to leiomyoma were findings of a larger size, exogastric growth, ulceration and tumor in patients over 50 years of age. It is stressed that enucleation is the treatment of choice for leiomyoma and an extensive gastrectomy for cases of leiomyosarcoma.

Purification and properties of D-beta-hydroxybutyrate dehydrogenase from Zoogloea ramigera I-16-M.

D(-)-beta-Hydroxybutyrate dehydrogenase was purified from Zoogloea ramigera I-16-M to electrophoretic homogeneity. The molecular weight of the enzyme as determined by Sephadex G-200 gel filtration was 112,000, and the monomer molecular weight estimated by polyacrylamide gel electrophoresis in the presence of sodium dodecyl sulfate was 28,000, indicating that the native enzyme is a tetramer with four identical subunits. The enzyme showed a pH optimum at 8.0 in the oxidation reaction, and a broad pH optimum (5.5-7.5) in the reduction reaction. The Km values for D(-)-beta-hydroxybutyrate and NAD in the oxidation reaction were 3.2 X 10(-4) M and 5.7 X 10(-5) M, respectively. The Km value for acetoacetate in the reduction reaction was 1.5 X 10(-4) M and that for NADH was 1.5 X 10(-5) M. Acetyl CoA, D-lactate, and 2-hydroxybutyrate were effective inhibitors for the oxidation of D(-)-beta-hydroxybutyrate. The enzyme was sensitive to the inhibitory actions of sulfhydryl reagents such as p-chloromercuribenzoic acid, 5,5'-dithiobis(2-nitrobenzoic acid) and HgCl2.

Effect of dibutyryl adenosine 3',5'-monophosphate administration on plasma concentrations of 1,25-dihydroxyvitamin D in pseudohypoparathyroidism type I.

The effect of dibutyryl cAMP on plasma concentrations of 1,25-dihydroxyvitamin D [1,25-(OH)2D] was studied in two patients with pseudohypoparathyroidism (PHP) type I, five normal adults, and three normal children as controls. In normal adults, plasma 1,25-(OH)2D tended to increase 3 h after the infusion of 2.5 mg/kg dbcAMP and was significantly increased after 6 h. In normal children, plasma 1,25-(OH)2D increased 3 h after the infusion, then gradually decreased; in the patients with PHP type I, it increased greatly from 6.8 and 11.2 pg/ml to 264.6 and 128.2 pg/ml 3 h after the infusion. These results suggest that the renal mechanism for the response to parathyroid hormone is intact distal to the renal adenylate cyclase in patients with PHP type I.

The effect of RES blockade on red blood cell survival.

The effect of RES blockade on the survival of heat-damaged and antibody coated red blood cells was evaluated in C3H mice. RES function of C3H mice was determined by the survival of 59Fe-labeled, heat-damaged red blood cells at 49 degrees C for 30 min. When animals were treated with an intravenous injection of non-labeled, heat-damaged red blood cells for the blockade of RES, red blood cell destruction was depressed in non-splenectomized and splenectomized mice. In the survival or organ distribution of 59Fe-labeled, antibody-coated red blood cells, no difference was noted between animals with and without blockade. These findings suggest that heat-damaged red blood cells were destructed predominantly in the splenic RES and antibody coated cells were not broken down only by the splenic RES.
