[Regional therapy to prevent recurrence after surgery in hepatocellular carcinoma].

The significance of regional therapy to prevent recurrence after surgery for hepatocellular carcinoma (HCC) was evaluated. In 275 patients who underwent hepatic resection for HCC, 143 (52%) had recurrences. Post-recurrence survival of the patients with regional therapy for recurrent foci was significantly better than that of patients without such therapy. Five-year post-recurrence survival of the patients treated with second surgery (77%) was significantly better than that of patients treated with chemoembolization (17%). The difference in 5-year survival between the former and the latter seemed to be caused by the difference in the degree of tumor progression at the recurrence. A second hepatic resection was the treatment of choice for a solitary intrahepatic recurrent tumor. However, chemoembolization was recommended for two or more recurrent foci.

[Evaluation of hepatic resection for stage IV hepatocellular carcinoma-comparison with arterial embolization therapy].

To evaluate the application of hepatic resection (HR) for patients with Stage IV hepatocellular carcinoma (HCC), we compared the back-ground factors and survivals after therapies between Stage IV HCC patients who underwent HR (n = 38) and those who received arterial embolization (n = 186, group C). Of the patients who had HR, 17 (45.1) had relative noncurative (RN) resection (group A) and the remaining 21 (55%) had absolute noncurative (AN) resection (group B). Back-ground factors in relation to liver function were significantly better in both A and B than in C. On the other hand, tumor-relating factors were better in A than in both B and C, and were worse in B than in C. Five-year survival rates of group A and group C were 37.1 and 13.1 respectively (A to C; p < 0.05) and there were no longer survivors than two years in group B. These results suggested that the group which had better back-ground factors gained the better prognosis. To compare the survivals in the patients having well back-ground factors who were dominant in group A, the patients whose clinical stage was I or the number of tumors was two or less were selected from each group. Among these selected patients, the survivals of group A were significantly better than those of the other groups.

[Evaluation of radiation therapy combined with intra-arterial infusion chemotherapy in patients with non-resectable pancreatic cancer].

This is a retrospective study to evaluate arterial infusion chemotherapy and radiotherapy for non-resectable pancreatic cancer without liver metastasis. Intra-arterial chemotherapy of the pancreas was performed for 17 patients, and 5 additionally received irradiation therapy. Five patients received radiation therapy alone. Both pain relief and tumor-reduction were more successful compared to the systemic chemotherapy group. In the locoregional treatment group, the patient survival rate was 50% at 1 year and 16% at 2 years. These rates were significantly higher than in the systemic chemotherapy group (p less than 0.01). Among the locoregional treatment groups, the median survival period was 7 months in radiation alone, 12 months in intra-arterial chemotherapy group, and 11 months in intra-arterial chemotherapy plus radiation group. These results suggested that locoregional treatment for pancreatic tumor is effective in increasing survival period and improving the quality of life.

The partial characterization and clinical evaluation of pancreas cancer-associated antigen from the ascites fluid of a patient with pancreatic cancer.

Monoclonal antibodies against pancreas cancer-associated antigen (PCAAp) were produced by established hybridoma cells. Two monoclonal antibodies, 3F1 and 3B6, were selected and these two monoclonal antibodies were found immunohistologically to react strongly with cancer cells and intraductal mucin-like products in well-differentiated pancreatic cancer tissues, but weakly, if at all, with gastric, colorectal and other cancers and at all not with normal adult or fetal pancreatic tissue. PCAAp is usually expressed in normal colonic mucosa (PCAAc), but the two monoclonal antibodies scarcely reacted with normal colonic mucosa. A sandwich enzyme immunoassay was developed to measure circulating PCAAp. Thirty-two normal subjects and 271 patients comprised of 210 with malignant disease and 61 with benign disease were surveyed. The cut-off value of PCAAp levels in the 32 normal subjects was 2.06 micrograms/ml, being the mean + 2SD, while PCAAp levels of more than this were observed in 72 per cent of the patients with pancreatic cancers, 65 per cent of those with bile duct cancers, 60 per cent of those with hepatomas, 0-30 per cent of those with other malignant diseases, and 10 per cent of those with benign hepatobiliary diseases. The susceptibility of isolated PCAAp and PCAAc to various enzymes and chemical reagents were also studied. The results of this study suggest that the assay for PCAAp might be useful in the diagnosis of pancreatic cancer.

[Percutaneous ethanol injection therapy (PEIT) and transarterial embolization (TAE) for recurrent hepatocellular carcinoma--report of 2 cases].

Two patients with recurrent hepatocellular carcinoma underwent reoperation following percutaneous ethanol injection therapy (PEIT) and transarterial embolization (TAE). In the first case, although the nodules where the accumulation of Lipiodol (LpD) was seen showed complete necrosis, viable cancer cells were seen in the areas with slight accumulation of LpD. One nodule with no accumulation of LpD where PEIT seemed to have been performed, developed necrosis. In the second case, the nodules with LpI) accumulation also showed almost complete necrosis. The area where PEIT was performed was mixed with necrosis and bleeding, where a small viable cancer nodule existed. The reason for incomplete anticancer effects by PEIT seemed to be the too small ethanol injection.

[The assessment of preoperative transcatheter arterial embolization (TAE) for hepatocellular carcinoma (HCC)--the comparison between "whole-liver" TAE and "lobar or segmental" TAE].

In 180 patients with hepatocellular carcinoma (HCC) who underwent radical hepatectomy, 52 patients were received preoperative arterial chemoembolization (TAE) for the whole liver (whole-liver TAE group: group A), 39 for the limited area of the liver (lobar or segmental TAE group: group B) and the remaining 89 had no treatments before surgery (control group: group C). In order to evaluate the significance of preoperative TAE, long-term prognoses were compared among the three groups. Although there were no significant differences in survivals between A and C, the 2- and 6-year survivals in group B were significantly better than those in group C (P less than 0.05). With regard to reduction rates of tumors and necrotizing effect for daughter nodules after TAE, the lobar or segmental TAE was significantly superior to the whole-liver TAE. Moreover, the lobar or segmental TAE deteriorated the liver function significantly less than the whole-liver TAE. These findings suggest that the lobar or segmental TAE is more advantageous than the whole-liver TAE as a preoperative adjuvant therapy.

[Hepatic arterial infusion chemotherapy and loco-regional treatment and irradiation of pancreas tumor in non-resectable pancreas cancer with liver metastases].

This study seeks to evaluate arterial infusion chemotherapy and radiotherapy for non-resectable pancreatic cancer with liver metastases. Arterial infusion to the liver was performed in 24 patients, 15 of whom received arterial infusion to the pancreas and 9 of whom underwent irradiation for pancreas tumor (40-50 Gy). However, arterial infusion to the liver alone did not prolong survival, but loco-lesional therapy for the pancreas tumor improved quality of life and resulted in good local control. The survival of the two treatment groups (arterial infusion to the liver combined with loco-regional treatment to the pancreas versus systemic chemotherapy) was statistically different (median 7 months versus 3 months, p less than 0.01). Arterial infusion to the pancreas decreased liver metastases as the first site of failure. These results suggested that arterial infusion to both liver and pancreas combined with irradiation for the pancreas tumor are effective in increasing survival time and improving the quality of life.

[Prevention of peritoneal dissemination after colorectal surgery--significance of MMC administration in abdominal cavity].

Peritoneal lavage cytology during surgery was done in 287 patients with colorectal cancer. Positive cytologic specimens were obtained from 21 patients. Of 13 patients who had free cancer cells in peritoneal cavity but no peritoneal dissemination macroscopically, 5 patients received intraperitoneal administration of 20 mg of MMC dissolved in 500 ml of saline. Peritoneal dissemination occurred in 0/5 (0%) of the MMC treated group and in 5/8 (63%) of the untreated group (p less than 0.05). Our results indicate that intraperitoneal administration of MMC (20 mg) is an effective method of preventing peritoneal dissemination after resection of colorectal cancer.

Preparation and characterization of monoclonal antibodies to an N-linked oligosaccharide.

Two monoclonal antibodies to an N-linked oligosaccharide, MT-5 and MT-9, have been prepared by immunization with a pyridylaminated, asialylated, galactosylated, fucosylated, bisected biantennary sugar. The reactivity of these antibodies was monitored by their reaction with human asialoglycophorin in a solid-phase enzyme-linked immunosorbent assay. Both antibodies reacted with the sugar chains of various human glycoproteins such as immunoglobulin G, transferrin, gamma-glutamyl transpeptidase, alpha 1-acid glycoprotein, and alpha-fetoprotein. Treatment of asialoglycophorin with beta-N-acetylhexosaminidase or alpha-mannosidase resulted in reduction of the binding to these antibodies. The reactivity of MT-5 to asialoglycophorin was slightly inhibited by D-mannose and N-acetylglucosamine, whereas that of MT-9 was inhibited by D-mannose, N-acetyl-D-glucosamine, chitobiose, and L-fucose. The epitope specificity of MT-5 appears to be a sugar chain containing biantennary N-acetyl-D-glucosamine residues, the bisected N-acetyl-D-glucosamine residue, and a trimannosyl core. The epitope to which MT-9 is directed may be a complex made up of beta-mannose, chitobiose, and L-fucose. These studies indicate that immunization with pyridylaminated sugars can produce antibodies that recognize N-linked oligosaccharides. Monoclonal/polyclonal antibodies to the N-linked sugar chains of glycopeptides would be useful in such studies of proteins.

[Pharmacokinetics and prophylactic effect of intraperitoneal administration of mitomycin C resulting in IC50 of gastric cancer].

In order to prevent peritoneal metastasis after gastrectomy for gastric cancer, we conducted a study to find a clinical dose of MMC which would result in IC50, the drug concentration which inhibits the growth of colonies of cells by 50%. After closure of the abdomen, MMC dissolved in 1,000 ml of physiological saline was administered. In the 40 mg group, the mean intraperitoneal MMC concentration after 60 min was 8.2 micrograms/ml, which exceeded IC50 of 7.6 micrograms/ml. In the group, peak serum concentration of MMC was 0.29 micrograms/ml. Seventy to 80% of intraperitoneally administered MMC was transferred in blood. Serious adverse effects were not observed. One-year survival rate of absolute noncurative resection with peritoneal metastasis was 33% in the control group and 78% in the treated group. Two-year survival rate of curative resection with free cancer cells in the peritoneal cavity was 11% in the control group and 60% in the treated group.

[Long-term postoperative courses of hepatocellular carcinoma with and without cirrhosis].

Until the end of August 1988, 158 patients with hepatocellular carcinoma underwent curative hepatic resection in our department. One-hundred and twenty-one patients (77%) had liver cirrhosis (Group A), and the remaining 37 (23%) did not (Group B). Background factors and postoperative courses were compared between Group A and Group B. With regard to clinical stage and curability, there were no significant differences between the two groups. On postoperative follow-up on 1- to 5-year survival, no differences were recognized between the two groups, but on and after 6-year survival, the former were significantly worse than the latter. The main cause of death was cancer in both groups. In group B, all intrahepatic recurrences happened within 3 years after surgery. On the other hand, intrahepatic recurrences of Group A were also found on postoperative 4- and 5-year to be as numerous as the recurrences found at 1-, 2- and 3-year after surgery. Moreover, with recurrent cases on and after 4 years in Group A, the frequency of solitary focus was high. The differences of postoperative courses between these two groups might be caused by metachronous carcinogenesis of cirrhotic liver.

[Chemoembolization for liver metastasis from colorectal cancer].

Chemoembolization using Lipiodol, cisplatin, angiotensin II and Gelfoam (modified sandwich therapy) was carried out for the patients with liver metastasis from colorectal cancer. 1. Ten of 30 patients who underwent hepatic resection received TAE before operation, and the remaining 20 underwent surgery without preoperative TAE. Three-year survival of the former was 66%, and that of the latter was 44% (not significant). 2. Twenty-two patients who were assessed as non-resectable were surgically catheterized into the hepatic artery. Thirteen patients received TAE (totally 38 times) and intra-arterial infusion chemotherapy, and the remaining 9 underwent intra-arterial chemotherapy alone. The 50% survival of the former and the latter was 545 and 285 days, respectively. One year survival of the former was significantly better than that of the latter. 3. Fourteen patients had intrahepatic recurrences after hepatic resections. Eight patients received TAE and the remaining 6 did not. The 50% survival of the former was 615 days and that of the latter was 190 days. For one-year survival, the former was significantly better than the latter. These results suggested that TAE is an effective modality for liver metastasis from colorectal cancer.

[Intraarterial and intraportal chemotherapy combined with decollateralization for cholangiocellular carcinoma and metastatic liver cancer].

Two patients with cholangiocarcinoma and one patient with liver metastasis from inflammatory breast cancer underwent catheterization into both hepatic artery and portal vein following decollateralization using silicone rubber sheeting. They received arterial chemoembolization and chemotherapy and portal chemotherapy through the catheters repeatedly. Two patients with cholangiocarcinoma are still alive more than one year after the beginning of the treatments without regrowth of the tumors. The patient with metastatic liver cancer died of lung metastasis, although the liver foci were controlled by procedure. Thus, both intraarterial and intraportal chemotherapy combined with decollateralization by silicone rubber sheeting seems to be effective for advanced cholangiocarcinoma and metastatic carcinoma.

[Intraarterial and intraportal infusion chemotherapy to prevent hepatic metastasis after curative resection of pancreatic cancer].

In order to prevent postoperative hepatic recurrence after curative resection of pancreatic cancer, we developed a new method of intra arterial and intraportal infusion chemotherapy. This method involves placing a catheter in the portal vein and another catheter in the hepatic artery, before closing the abdomen. Postoperatively, 5-FU (125 mg/day) was infused continuously into these two catheters, for more than 28 days. Twelve patients received this therapy, without liver dysfunction or myelosuppression. During this therapy, the concentration of 5-FU was more than 0.14 micrograms/ml in the portal blood and 0.02 micrograms/ml or less in the systemic blood. The cumulative survival rate was 77% at 18 months and cumulative rate of hepatic metastasis was 25% at 18 months. These data were better than those in the control group (45 curative resections without chemotherapy). Therefore, this method seems beneficial for patients who have received curative resection for pancreatic cancer, since lowers the incidence of hepatic recurrence.

Margin quality and microleakage of Class II composite resin restorations.

Margin quality and isotope microleakage analyses of Class II restorations placed in extracted human molars were compared using various composite resins and placement techniques. At occlusal margins, the traditional (experimental) composite resin restoration placed by the incremental technique showed less microleakage than did the traditional (commercial) [corrected] composite resin restoration placed by the bulk technique. In each group, the occlusal and proximal adaptations had significantly higher "excellent margin" than did the cervical adaptation. Thus, the marginal adaptation at the cervical aspect of conventional Class II composite resin restorations may present a problem with respect to microleakage.
