Joint motion of bipolar femoral prostheses.

From 1982 to 1992, 251 bipolar hip arthroplasties were performed on 213 patients. Among them, 117 bipolar femoral prostheses were randomly selected to examine the behavior of abduction motion under weight-bearing loads. Roentgenographic motion study was performed at an average of 46.5 months after surgery (range, 2-110 months). One hundred one prostheses used in dysplastic osteoarthritic, rheumatoid, and revised failed total hip arthroplasty patients moved 18.2% at the outer bearing and 81.8% at the inner bearing, while 16 prostheses used in femoral neck fracture and osteonecrosis of the femoral head patients moved 49.7% at the inner bearing and 50.3% at the outer bearing. There was a statistical difference in the motion pattern between the two groups. The abduction motion behavior of the bipolar femoral prostheses was not affected by the length of the follow-up period, the diameter of the outer heads, or the position of the prostheses on immediate postoperative roentgenograms.

Interleukin-1 receptor antagonist production in cultured synovial cells from patients with rheumatoid arthritis and osteoarthritis.

OBJECTIVE: To measure the amounts of interleukin-1 receptor antagonist (IL-1ra) protein produced by cultured synovial cells obtained from patients with rheumatoid arthritis (RA) and osteoarthritis (OA). METHODS: Synovial cells obtained from patients with either RA or OA were cultured and the supernatants were measured for IL-1ra by enzyme linked immunosorbent assay. RESULTS: The synovial cells obtained from patients with RA produced significantly smaller amounts of IL-1ra than did those obtained from patients with OA, in a late passage (third to fifth) without stimulation and a first passage both with and without stimulation (p < 0.025, respectively). In addition, when the patients with RA were divided into two groups according to the maximum number of lining cell layers, the amounts of IL-1ra produced by the proliferative type were smaller than those produced by the less proliferative type (p < 0.025). CONCLUSIONS: The above findings suggest that IL-1ra production in RA synovial cells is suppressed, and that reduced IL-1ra protein production is one of the causes which leads to the proliferation of lining cells and persistent joint inflammation.

Osteogenesis in xenogeneic bone transplantation, using an immunosuppressant. Rabbit-rat experiments.

We investigated osteogenesis and lymphocyte subsets in xenogeneic bone transplantation, using the immunosuppressant FK506 (FK). Iliac bones of rabbits were transplanted as fresh and frozen xenogeneic bone grafts into an intramuscular pouch of rats. FK was injected intramuscularly in half of the rats in a dose of 3 mg/kg/day for 14 days after transplantation. At 2, 4, and 8 weeks, transplanted grafts and the lymphocyte subsets of these rats were examined. In the group not given FK, the grafted bone became necrotic and infiltrated with small round cells around the trabeculae. In the FK group, at 2 and 4 weeks, new bone was formed in the fresh xenografts without infiltration of lymphocytes. At 8 weeks, the new bone became necrotic and lymphocytes were present. The percentage of T cells (CD 5), B cells and the ratio of CD 4 cells/CD 8 cells were smaller in the FK group. Using an immunosuppressant we concluded that xenogeneic bone has an osteogeneic potency.

Analysis of a Hind III site polymorphism in the type II collagen gene: it's location and frequencies in the Japanese population.

Hind III restriction fragment length polymorphism (RFLP) in the cartilage-specific type II collagen gene was studied in a sample of 135 persons randomly selected from the Japanese population. Localization of the Hind III site was determined by the dideoxy method. Based on this sequence, a pair of primers was designed and a polymerase chain reaction (PCR) was carried out. The Hind III allele had a frequency of 0.52 in the Japanese. The genotype digested by Hind III, (++) genotype was present with a frequency of 16.9%, (+-) was present in 70.0% and (--) was present in 13.1%, showing a lower rate of the (--) genotype in the Japanese compared to the Finnish. The difference was statistically significant, tested by chi 2 analysis (p < 0.0001). The present study is the first to report a race-dependent variance in Hind III RFLP on the type II collagen gene.

Bipolar hip arthroplasty without acetabular bone-grafting for dysplastic osteoarthritis. Results after 6-9 years.

The authors report the clinical and radiographic results of 36 bipolar hip arthroplasties after performing excavation of the steep and shallow acetabulum without acetabular bone-grafting for dysplastic osteoarthritis. The procedures were carried out between 1981 and 1985. Survivorship analysis showed that 84.6 of the bipolar hip arthroplasties were retained for 8 years. Twenty-nine patients, which were followed for 6-9 years after surgery, were reviewed. Severity of acetabular dysplasia was classified according to the method of Crowe. Class 1 included 17 hips and class 2 included 12 hips. The average preoperative clinical score was 49 points. The average postoperative clinical hip score improved to 87 points after 6 years. Twenty-seven of the 29 hips assessed were classified as either excellent or good by Charnley's function score. Stress fracture, due to excessive acetabular excavation at the time of surgery, and femoral component loosening were major symptomatic complications. The cessation of radiographic evidence of migration of the bipolar socket was recognized in 25 of 29 procedures at 6 years after surgery (86.2%). Cineradiographic study demonstrated that the abduction motion under standing position for 20 of 24 hip joints functioned dominantly at the inner-bearing and metallic-stem interface.

Inhibition of tubular morphogenesis in human microvascular endothelial cells by co-culture with chondrocytes and involvement of transforming growth factor beta: a model for avascularity in human cartilage.

Tube formation in collagen gel was induced in human omental microvascular endothelial (HOME) cells in the presence of epidermal growth factor (EGF) or transforming growth factor-alpha (TGF-alpha). TGF-alpha enhanced the expression of the tissue type plasminogen activator (t-PA) gene, whereas TGF-beta increased the expression of the PA inhibitor-1 (PAI-1) gene and inhibited that of the t-PA gene. TGF-beta inhibited the tube formation of HOME cells in type I collagen gel that was enhanced in response to TGF-alpha. We have recently established an angiogenesis model in vitro in which vascular endothelial cells on type I collagen gel in an inner chamber are co-cultured with other types of cells in an outer chamber. Here we examined whether the EGF/TGF-alpha-induced tube formation in HOME cells was modulated by human chondrocytes co-culture in the outer chamber. TGF-alpha-dependent tube formation of HOME cells was inhibited when human chondrocytes were co-cultured in the outer chamber. This chondrocyte-induced inhibition of tube formation was partly abrogated by co-administration of anti-TGF-beta antibody. These findings suggest that TGF-beta is partly involved in the human chondrocyte-dependent inhibition of tube formation by human microvascular endothelial cells. This is the first model system demonstrating that avascularity of human chondrocytes is partly due to TGF-beta family produced from them.

Association of HLA-DR and HLA-DQ antigens with congenital dislocation and dysplastic osteoarthritis of the hip joints in Japanese people.

OBJECTIVE: To investigate the frequency of HLA-DR and DQ antigens among patients with congenital dislocation of the hip joint or dysplastic osteoarthritis (OA) and to determine whether tissue typing might contribute to better understanding of these disorders. METHODS: HLA-DR and DQ typing was undertaken in 18 patients with congenital dislocation of the hip joints, 65 patients with dysplastic OA of the hip, 42 patients with primary OA of the knee, and 40 normal controls. Typing for HLA-D antigens was performed using the standard microlymphocytotoxicity method. RESULTS: The frequency of HLA-DR4 in patients with congenital dislocation of the hip joint (61.1%) and patients with dysplastic OA of the hip (58.5%) was significantly increased compared with normal controls (27.5%) (Pcorr < 0.05 and P < 0.01, respectively). The relative risk for HLA-DR4 was 4.1428 and 3.7104, respectively, in these 2 disease groups. The frequency of HLA-DR4 in the patients with primary OA of the knee (35.7%) did not differ significantly from that in normal controls. CONCLUSION: The results strongly indicate that development of congenital dislocation of the hip joint and progression of dysplastic OA of the hip are associated with genes of the HLA-D region.

Quantitative analysis of revascularisation and new bone formation in experimental bone grafts. The use of an image input and processing system.

Revascularisation and new bone formation in bone grafts has been extensively studied by histological techniques, but the images obtained are very complex and consequently difficult to evaluate quantitatively. We report similar analyses in experimental bone grafts using newer computerised digital techniques. Fresh autografts and fresh and frozen allografts were used. The histological analogue images from them were input digitally into an image processor. The amount of revascularisation and new bone formation could then be quantified in the different grafts. This system has proved to be a useful method of evaluation which could be applied to the quantitative analysis of other histological images.

Effect of dental metals on magnetic resonance imaging (MRI).

The aim of this research was to examine the effects of metals used in dentistry upon magnetic resonance imaging (MRI). Ten disk-shaped samples of metals have been used in this study. Each sample has been set into a volunteer who had produced a normal MR image when tested. The subject was imaged in the sagittal, frontal and coronal plane with a 0.1 T MRI device and results examined for MR defects. Au, Ag, Au-Ag-Pd and amalgam produced no defect. Ni-Cr, Co-Cr and SUS304 expressed small amounts of MR defects, but SUS405, Pd-Co-Ni and Sm-Co expressed large defects. Those metals which create MR image degradation must not be used for fixed prosthesis or orthodontic devices both of which could easily be made from other metals. If the MR image degrading metals are to be used in dentistry, their application should be restricted to removable prosthesis.

[Infection after implant arthroplasty].

From 1982 to 1990, ten patients (twelve joints) who developed deep infection after total joint replacement were analysed. An average follow-up period after the surgical management was two years and eight months (range, one year and seven months to three years and ten months). Synovectomy and closed irrigation were carried out in seven of ten patients without waiting the results of cultures. In six of seven patients, the infection was successfully eradicated without removal of the components. Antibiotics were administered orally for two years after the closed irrigation. There was no recurrence. In four of ten patients, the components had to be removed. In one of four patients, synovectomy and closed irrigation failed, and arthrodesis was carried out. In three patients, loosening of the components were radiographically noticed.

[The signification of total elbow replacement in the treatment of rheumatoid arthritis].

The purpose of this paper is to emphasis the signification of total elbow replacement in the treatment of rheumatoid arthritis. Total elbow replacement were performed in twenty elbows in fourteen patients who had rheumatoid arthritis from 1982 to 1990. The follow-up period ranged from 2 to 9 years. Pain relief was complete in twelve patients. Preoperation flexion averaged 119 degrees, increasing to 127 degrees. Preoperation extension averaged -34 degrees, increasing to -24 degrees postoperation. There were no infection and fracture postoperatively. Radiolucent lines were seen in four humeral components, in one ulnar component. Proximal subsidence of the humeral component were found in two elbows. Posterior dislocation occurred in three elbows. In two cases, reduction was successful by three weeks immobilization in a plaster cast with the elbow in the flexion position. Transient ulnar nerve palsy was evident in two elbows. Revision surgeries were performed in two cases, which occurred dislocation and progressive subsidence. Multiple replacements of major joints were performed in ten patients. Two joints were replaced in two patients, three joints were replaced in six patients, four joints were replaced in two patients. Four total elbow replacements were performed accompanied with other joints replacement in the same anesthesia. We recommended two joint replacements in the same anesthesia in patient with good general condition. All patients had functional improvements after total elbow replacement, especially reaching of the arm. Four patients improved the walking by being able to use stich after total elbow replacement.

[Effect of arthroplasty of the first metatarsophalangeal joint and shortening valgus osteotomy of the first metatarsus on forefoot surgery in rheumatoid arthritis].

Results of forefoot surgery with rheumatoid arthritis were evaluated in 19 feet of 11 patients. Clayton's operation was performed on 12 feet of 7 patients. The follow-up averaged 3 years and 10 months (range, 2 years and 1 month to 8 years). 1 foot (8.3%) recurred hallux valgus deformity and 3 feet (25%) recurred hammer toe deformity. But all patients complained of difficulties to push off action on the forefoot. Interpositional arthroplasty of the first metatarsophalangeal joint with shortening-valgus osteotomy through the first metatarsal base was done on 7 feet of 4 patients. The follow-up averaged 1 years and 8 months (range, 8 months to 2 years and 2 months). 2 feet recurred hammer toe deformity. But, this group had no problem to push off action on the forefoot compared with Clayton's operation. As regard the relief of pain, all patients had satisfactory results from both methods. These results clarified that the preservation for the first metatarsophalangeal joint function was superior for push off action on the forefoot.

Removable magnetic dental attachment that permits magnetic resonance imaging.

Magnetic dental attachments cause magnetic resonance image (MRI) degradation. A new magnetic dental attachment that allows removal of magnetic parts to exchange them for nonmagnetic parts was developed and tested. It allows high quality MRI with no change in occlusal vertical dimension. A volunteer test subject with combinations of attachments in place was examined by MRI. The images produced showed no degradation or distortion. Magnetic attachments for overdentures or maxillofacial prostheses should be removable to permit use of MRI.

A supernumerary muscle between the adductors brevis and minimus in humans.

In order to elucidate the nerve supply of a supernumerary muscle observed between the adductors brevis and minimus in humans and to investigate its true nature and mechanism of formation, 100 body halves from 50 adult Japanese cadavers were subjected to gross anatomical examination. A supernumerary muscle was noted in 33 (33.0%) out of the 100 thighs. In each of these thighs, it arose from the upper part of the inferior ramus of the pubis and ran obliquely downwards and laterally. It was inserted into the anterior surface of the insertion aponeurosis of the adductor minimus (17/33 thighs, 51.5%), the upper part of the pectineal line (9/33 thighs, 27.3%), or the posterior side of the base of the lesser trochanter (7/33 thighs, 21.2%). It was supplied, from its posterior aspect, by a filament from the twig originating from the posterior branch of the obturator nerve and being distributed to the superficial fasciculus of the obturator externus (18/33 thighs, 54.5%) or by a twig directly originating from the posterior branch (15/33 thighs, 45.5%). The obturator nerve received fibers from L1234 (4/33 thighs), L234 (25/33 thighs), L2345 (2/33 thighs) or L34 (2/33 thighs) and, moreover, its posterior branch ran through (25/33 thighs, 75.8%) or over (8/33 thighs, 24.3%) the obturator externus to emerge into the thigh. Based on the topographical-anatomical relationships of this muscle to its nerve supply, it seems probable that it is formed by separation from the superficial layer of the obturator externus and changes into an independent structure during the process of ontogeny. Furthermore, from a statistical standpoint, the segmental composition or course of the obturator nerve is not related to either the formation or incidence of this muscle.

A novel antibody from rheumatoid arthritis patients.

A 106 base-pair length DNA sequence was isolated from rheumatoid synovium. Base sequence analysis showed this fragment to correspond to sequences already reported as the non-transcribed spacer of ribosomal DNA. Base sequence analysis also revealed that a fused protein has a unique five amino acid sequence. This five amino acid sequence was considered to be an epitope for an antibody. This antibody was named BUNGO antibody (BUNGO is the old name for Oita prefecture). An antigen peptide was synthesized chemically in accordance with the amino acid sequence of the epitope. Using this synthetic peptide, BUNGO antibody in serum was measured. Twelve of 32 patients (37.5%) with RA (rheumatoid arthritis) tested positive for the antibody. Five of 32 (15.6%) age and sex-matched control subjects were positive, indicating a significant difference from the RA group (chi 2 = 3.9, p < 0.05).

Human T cell leukemia virus-I carriers and gonarthrosis. A preliminary report.

Gonarthrosis in 11 human T cell leukemia virus-1 (HTLV-I) carriers were studied for sera, synovial fluids (SF) and cerebrospinal fluids (CSF). Atypical lymphocytes similar to adult T cell leukemia cells were noticed in the SF of all the HTLV-I carriers and ranged from 19 to 1%, while those of peripheral white blood cells ranged from 3 to 0%. Anti-HTLV-I antibodies in the SF were equal to or lower than those in the sera. The CSF from 7 of them demonstrated positive titers of anti-HTLV-I antibodies, while no evidence of neurological disorders manifested. The histopathological findings of synovial membranes on hematoxylin and eosin staining method showed no statistical difference between those of gonarthrosis of HTLV-I carriers and noncarriers.

Oncogenic vitamin D resistant hypophosphatemic osteomalacia (benign ossifying mesenchymal tumor of bone): case report.

A 53-year-old Japanese woman suffered hypophosphatemic osteomalacia secondary to bone tumor. Her clinical symptoms did not improve for a long time following the oral administration of Alfacalcidol (vitamin D3). A bone survey using radiographs revealed a small tumor located in the proximal head of the left fibula. Hypophosphatemia and low levels of 1-alpha,25-dihydroxycholecalciferol (1-alpha,25(OH)2D3) returned towards normal levels soon after an en-bloc tumor resection, and bone pain and muscle weakness gradually disappeared. The histology, especially of the trabecular bone of the left fibula, showed typical features of osteomalacia with demineralization. The tumor comprised numerous small blood vessels, spindle shaped or oval neoplastic cells, a few multinucleated giant cells, osteoid tissue with or without calcification, chondroid tissue and hemangioma. The spindle shaped or oval cells contained a round nucleus with no mitotic figures. These clinical and histological findings coincide well with a diagnosis of benign ossifying mesenchymal tumor of the bone with vitamin D resistant hypophosphatemic osteomalacia.

[Roentgenographical evaluation of physiological bow-leg and the infantile type of Blount's disease in children].

Roentgenographical examinations were carried out in 41 joints of 22 cases with physiological bowleg and 7 joints of 5 cases with the infantile type of Blount's disease. The observation periods were from 1 year and 4 months to 8 years and 1 month with an average of 4 years and 2 months. The femorotibial angle, the proximal tibial metaphyseal diaphyseal angle, the distal tibial metaphyseal diaphyseal angle and the tibial metaphyseal metaphyseal angle were measured, and evaluated statistically. The measurement of the proximal tibial metaphyseal diaphyseal angle and the tibial metaphyseal metaphyseal angle were more significant than that of the femorotibial angle for early differentiation of the infantile type of Blount's disease from physiological bowleg. The degree and it's change of the distal tibial metaphyseal diaphyseal angle show no difference between physiological bowleg and the infantile type of Blount's disease.
