Accelerated glycation of the aorta in diabetic rats.

Glycation of the aorta in rats with streptozotocin-induced diabetes was estimated by determining the early-stage product and the advanced product of the Maillard reaction. The early-stage product of the Maillard reaction was determined using furosine, which is derived from glycated lysine residues by acid hydrolysis. The advanced product was determined by fluorescence high-performance liquid chromatography. The levels of both early-stage and advanced products in diabetic rats were significantly higher than those in non-diabetic rats at the age of both 20 and 50 weeks. The levels of both early-stage and advanced products at 50 weeks in rats tended to be higher than those at 20 weeks. However, the level of glycated hemoglobin in both non-diabetic and diabetic rats showed no significant change between 20 and 50 weeks of age. These results suggest that tissue glycation may be involved in the development of diabetic complications and may be related to the aging mechanism.

Pseudomalignant myositis ossificans occurring in the hand.

This is a case report of a patient with pseudomalignant myositis ossificans of the hand. This entity is usually related to trauma, but there was no history of injury in this patient. Radiographic follow-up, histologic zone phenomena and computerized tomography are helpful in obtaining the correct diagnosis.

Chemotactic factors present in the supernatants of cultured sarcoid granulomas.

The present study was undertaken to test whether cultured lymphocytes and epithelioid cells from sarcoid granulomas obtained from patients with active stage 2 sarcoidosis produced chemotactic factors (CF) for leukocytes as assessed by modified Boyden's method. The results indicate that: (1) CF for mononuclear cells and for neutrophils were detected in supernatants from cultured sarcoid granulomas; (2) similar activity was detected in culture supernatants from both lymphocytes and epithelioid cells; (3) CF present in the supernatants of cultured epithelioid cells were similar physically to those which other investigators have disclosed to be released by alveolar macrophages, (4) CF in the supernatants of cultured lymphocytes were similar to the lymphokines based on an estimation of molecular size; (5) neutrophils from patients with sarcoidosis were less responsive to zymosan-activated sera and did not respond well to CF derived from their own granuloma, whereas mononuclear cells from sarcoid patients responded well to CF. In conclusion, CF are present in the supernatants of cultured sarcoid granulomas and they appear to preferentially attract mononuclear cells from sarcoid patients.

[Treatment of malignant fibrous histiocytoma of soft tissues].

Thirty-six patients with soft tissue malignant fibrous histiocytoma were followed up. At the time of this study, 14 of the 36 patients were alive and the average follow-up period was 59.3 months. The average age at initial diagnosis was 62.1 years. Of the 36 patients, 13 were initially treated by us. All of the marginal excision cases (17) showed recurrence. Five of 8 additional excision cases had residual tumor and three of these showed recurrence. Those cases given primary wide excision (5) showed no local recurrence. One out of 3 amputation cases showed recurrence five years after the operation. Thus, the total recurrence rate was 64%. The average period of recurrence was 9.3 months (n = 21). Radiotherapy was effective in four patients among 11 irradiated cases. Chemotherapy is difficult to apply, since patients are generally too old to tolerate intensive chemotherapy and also the drug-sensitivity is low. For this reason, we have insufficient clinical experience to make an evaluation. Five-year survival rate was 45% and 10-year survival, 30%. Early detection of the tumor and adequate initial surgery are the best ways of improving the results of treatment for MFH. Chemotherapy and radiotherapy might achieve adjuvant effects and produce better results.

[A long-surviving case of gastric cancer with liver metastasis without signs of recurrence for six years after palliative operation].

A 51-year-old man was admitted because of Borrmann 2 type gastric cancer located in the antrum. Since he had bilateral liver metastasis, palliative gastrectomy and liver biopsy was performed. Histopathologically, the primary cancer showed poorly differentiated adenocarcinoma and the liver biopsy showed the same. This patient has been alive without sign of recurrence for more than six years after he was given 40 mg of MMC, 12.5 KE of Picibanil and 528 g of PSK postoperatively. In addition, eight similar cases, with a survival rate of more than 5 years after resection of liver metastasis, have been reported in Japan and are reviewed.

Anorectal function after high sacrectomy with bilateral resection of S2-S5 nerves. Report of a case.

A 19-year-old man underwent resection at the S1-S2 interspace with sacrifice of bilateral sacral nerves below S2 for a sacral tumor. The postoperative anorectal function was evaluated periodically for one year using manometry and subjective findings. The rectoanal inhibitory reflex was intact, whereas a disturbance of anorectal sensation, a loss of anal squeeze pressure, a decrease of anal canal resting pressure, urinary incontinence, and impotency were apparent. These findings suggest that sacrifice of bilateral sacral nerves below S2 leads to a feeble anal canal basal tone with the rectoanal inhibitory reflex, and that a significant impairment of anorectal function is inevitable.

Mandibular ameloblastoma with intracranial extension and distant metastasis.

A case of mandibular ameloblastoma which invaded intracranially through the skull base and developed distant metastasis to the femur is reported. The intracranial extension of the tumor was partially removed three times and the metastasis of the femur was curetted. The remaining tumor temporarily responded to chemotherapy, but the patient died of intracranial tumor regrowth. The period from the initial treatment of the mandibular tumor to death was 21 years.
