Digitalis combines a longstanding tradition with constant up-to-dateness. Kinetic maintenance therapy is very often required for heart patients with arrhythmia as one of their most striking clinical manifestations. The well-known questions of paroxystic tachycardia, flutter and atrial fibrillation are examined, and attention is concentrated on ventricular extrasystole, sinoatrial node disease, and Wolff-Parkinson-White syndrome. It is clear that combinations with recently introduced antiarrhythmic drugs, and administration of diuretics and electrolytes, and the employment of electrical stimulation have facilitated the long-term treatment of the chronic arrhythmic heart patient and brought about the better utilisation of digitalic substances. Their arrhythmogenic potential, however, cannot be overlooked.

Serum cholinesterase enzyme (E.C. 3.1.1.8) shows a considerable degree of genetically determined heterogeneity. In this communication we describe some kinetic properties of serum cholinesterase variants (Km and Vmax) from individuals predisposed to prolonged apneic periods after administration of the anesthetic, succinylcholine. Isoelectric focusing, in a narrow pH range (pH 4-6) of sera from normal and atypical phenotypes permitted the detection of differences in protein bands near the isoelectric pH of the enzyme.

We report the experience in Sicily, where more than 3000 pacemakers have been implanted in different Schools of Medicine and Hospitals (Agrigento, Catania, Messina, Palermo and Ragusa). Our personal experience (Cardiology Division--Catania) is 846 cases up to June 1977. We would like to stress that today the sick sinus syndrome and other pathological conditions not due to complete AV block are of increasing incidence as an indication to definitive electrostimulation. In addition, the average age of patients is getting lower, and more PM-demand and lithium powered PM are implanted. The most used implant technique has been through the cephalic vein. Today it is possible to have more distant controls mainly because of the oscilloscopic evaluation technique. Mortality during surgery and immediately post-surgery is very low, and the bioelectric complications are very rare. In conclusion, we would suggest: 1) a strict cooperation with a Cardio-surgery Division; 2) the utilization of more sophisticated techniques to make controls less frequent and more rapid and easy (e.g. telephone controls); 3) a more modern and correct law on this issue and a better information of the public, involving educational, social and medical structures so that the patient with a pacemaker can live in his family with a recovery for work and society.
Necrosis of the pouch containing the generator, due to non-traumatic breakage of the batteries, was observed in a patient with a permanent pacemaker, followed by rapid ingravescence. The cause of this complication is discussed. Chemical inflammation was noted soon after the pacemaker ceased to function. Initially this was similar to the not uncommon bacterial inflammation noted in such patients. Here, of course, the pacemaker usually continues to work, and its replacement is optional, whereas in the reported case is an emergency matter. In situations where tissue distress in the pouch is accompanied by inflammation, therefore, thought should be given to the possibility that, even if the batteries are still working, the cause may be chemical or electrochemical rather than septic, and than sudden breakdown of the generator may be expected.
The close links between the implantation of permanent pacemakers and radiological examination are described with particular reference to position of the the right ventricle. Here radiological examination offers the best chance of diagnosis. The need for close cooperation between specialists is emphasised.
