[Intermediate- and high-dose cytosine arabinoside therapy in acute refractory leukemia].

Seven patients with refractory and/or relapsed acute leukemia were treated with intermediate- or high-dose cytosine arabinoside (ara-C). One patient obtained complete remission and two achieved partial remission. We advocate intermediate ara-C in combination with anthracyclines for the treatment of patients with refractory and/or relapsed acute leukemia, because the toxicity is less severe than that of high-dose ara-C and the therapeutic results are not inferior to those obtained with high-dose ara-C. Kinetic differences in leukemic cells should be considered in order to obtain the maximum effect in intermediate- and high-dose ara-C therapy.

Effects of riboflavin-2', 3', 4', 5'-tetrabutyrate on platelet function and plasma lipids.

Riboflavin-2', 3', 4', 5'-tetrabutyrate (B2-But) inhibited, in vitro, ADP (4 micrograms/ml)-induced platelet aggregation when added at the concentration of more than 1 microgram/ml and arachidonic acid (Aa) (1 mM)-induced platelet aggregation when given at the concentration of more than 0.1 microgram/ml. These inhibitions were dose-dependent. Both cyclic AMP and prostaglandin metabolites in the platelets, however, remained unchanged when the amount of B2-But added was between 0.1-10 micrograms/ml. In a volunteer study, 80 mg of B2-But taken in one bolus inhibited both ADP- and Aa-induced platelet aggregation 4 hr after oral administration. The administration of B2-But at this dose was associated with a significant decrease in malondialdehyde formation in plasma. The oral administration of 40 mg (t.i.d.) of B2-But daily did not result in significant changes in ADP- and Aa-induced platelet aggregation. B2-But showed no significant effects on either platelet adhesiveness or collagen-induced platelet aggregation, in vitro or in vivo. Our data suggest that a decrease of lipoperoxide in plasma, not in platelets, influences platelet aggregation induced by ADP and Aa.

[Clinical efficacy of a quadruple combination chemotherapy with ACNU, adriamycin, methotrexate, and prednisolone for patients with non-Hodgkin's lymphoma, who were refractory to VEPA (P) treatment].

A quadruple combination chemotherapy with ACNU, adriamycin, methotrexate and prednisolone (AAAP), was performed on 9 patients with non-Hodgkin's lymphoma, who had been previously treated with a VEPA (P) regimen. Complete remission rate was 44% (4/9) and, in four other patients, the tumor was decreased in size. Complete remission duration was from 30 days to over 390 days. Median survival time was 105 days. Myelosuppressive toxicity was severe. In eight patients, WBC counts were less than 3,000/mm3. In six patients, platelet counts were less than 100,000/mm3. An AAAP regimen is useful in relapse or for resistant forms of non-Hodgkin's lymphoma.

Factor XIII deficiency associated with Klippel-Weber disease, platelet dysfunction and cryofibrinogenemia.

A 23-year-old woman with factor XIII deficiency was presented. The patient had no consanguinity, but familial traits were present. A bleeding tendency and poor wound healing had been noted in the patient since birth. She had hemangiomas in the leg and vulva (Klippel-Weber disease). Hematologic studies revealed platelet dysfunction, cryofibrinogenemia and mild chronic disseminated intravascular coagulation with prolonged PT and PTT, hypofibrinogenemia, a high turnover rate of 125I-fibrinogen and mild elevation of fibrinogen-fibrin degradation products, beta-thromboglobulin and platelet factor 4. A decrease in clot retraction and a marked reduction in maximal amplitude of thrombelastogram were also found. The assay of the factor XIII level was 10% by the antiserum inhibition method, and the assay of subunits A and S were 16 and 29%, respectively, by the electroimmunoassay method. Transamidase activity of factor XIII was 26%. The level of factor XIII of her sister was low, similar to that of the patient. The concentration of cold-insoluble globulin in EDTA-plasma was 36.5 mg/dl.

Plasma beta-thromboglobulin and platelet factor 4 in patients with chronic renal failure and effect of hemodialysis.

Significantly increased levels of plasma beta-thromboglobulin (beta-TG) (76.8 +/- 25.5 ng/ml, p less than 0.01) were observed in 24 patients with chronic renal failure (blood urea nitrogen (BUN) greater than 20 mg/100 ml), as compared with normal subjects (13.2 +/- 5.6 ng/ml). The increase in beta-TG was highly correlated with BUN (r = 0.651, p less than 0.01), creatinine (r = 0.778, p less than 0.01) and creatinine clearance ( t = -0.723, p less than 0.01). Plasma platelet factor 4 (PF4) and normal 5.0 +/- 2.0 ng/ml) also increased significantly to 8.5 +/- 3.4 ng/ml (p less than 0.01). However, statistical correlation between beta-TG and PF4 was not found in these patients. The reason is thought to be due to differences in molecular weight (PF4 8,000 MW; beta-TG 36,000 MW) and half-life time (PF4 30 min; beta-TG 100 min), and due to the difficulty in calculating statistically the correlation because of the narrow distribution of PF4 levels. The high levels of beta-TG (89.4 +/- 3.4 ng/ml) showed a further increase (109.4 +/- 5.8 ng/100 ml, p less than 0.01) after dialysis. This is thought to be due to hemoconcentration, because other blood factors such as RBC, WBC, platelets, fibrinogen, etc were elevated by about 20% during hemodialysis and because no adhesion of platelets to the cellulose membrane did occur. The increase in PF4 levels at 15 min (55.2 +/- 19.6 ng/ml, p less than 0.01) and 1 hr (23.7 +/- 8.4 ng/ml, p less than 0.01) of hemodialysis from the level before it (7.7 +/- 1.3 ng/ml) is thought to be caused the effect of heparin infusion. The change in PF4 was not accompanied by the change in beta-TG. During hemodialysis the decrease of other platelet functions such as adhesiveness, aggregation induced by ADP, collagen and PF3 remained unchanged.
