[Two cases of coexisting dysplasia fibrosa and mesenchymoma].

Two patients with dysplasia fibrosa who also had a tumor of mesenchymal origin are presented. One of them underwent one time resection of dysplasia fibrosa focus and the tumor, the other had the tumor resected 17 years after surgical resection of focus in the femur. The authors suggest an etiologic relationship between these two conditions based on mesenchymal changes--the occurrence never described previously. They also indicate efficacy of surgical treatment.

[Two cases of congenital defects within the transverse processes of the lumbar spine].

Two cases of congenital defects of transverse processes of lumbar spine causing lumbalgia are described. The first one was produced by direct contact of the elongated processes with subsequent pseudoarthrosis. Hypertrophied processes caused stenosis around intervertebral foramen and irritated posterior neural branch in the second case. Surgical resection of the hypertrophied transverse processes has brought the cure.

[Complications of vertebral disc and spinal diseases after manipulation therapy].

Complications caused by manipulation therapy in 3 cases of spine condition and 22 cases of vertebral disc disease observed in relatively short period of time are presented. In the first instance the patients were not diagnosed by physician. In the second one in all cases the nucleus pulposus protruded under the anterior ligament or into the spinal canal. It resulted in marked worsening of the patient's condition. The mechanism of disc damage has been determined intraoperatively. The authors conclude that manual therapy not preceded by precise diagnosis is highly hazardous.

[A case of posthemorrhagic cyst].

A case of posthaemorrhagical cyst around the right armpit in 13 years old boy is presented. The cyst has been removed surgically after conservative measures had failed.

[Management of old cervical spine injuries].

Fifty-two cases of inveterate cervical spine injury have been presented. Nearly all the injury types are represented. Neurological complications were present in sixty-nine percent of the cases. Specific surgical treatment aimed at restoration of anatomical relations within the spine, decompressing neural elements and stabilization of the injured segment. Good results have been achieved with neurological improvement, especially in the group of patients with paresis or root symptoms. Several paraplegic patients also benefited from the treatment. The authors conclude, that management employed offers a great chance for improvement to these patients.

[Three cases of thoracic spina fracture with displacement without spinal cord injury].

Three cases of extremely rare, thoracic spine displaced fractures without injury of the spinal cord are reported. An attempt to reconstruct injury mechanism and pathology of the fracture has been undertaken on the basis of radiographs analysis. Immobilisation treatment with no reduction yielded good results. The authors are convinced that this paper, if studied, will indicate the management of this type of injury to those come across it for the first time.

[History of lengthening and equalization of lower extremities].

Chronological history of development of methods for elongation and equalization of lower extremities is presented. An elongation by distraction of an osteotomized shaft and shortening by resection of a shaft fragment is discussed. An epiphyseal stimulation and arrest and non-aggressive method of epiphyseal distraction are shown. The author's intention was to present the development of this field of orthopedics and to prevent researching ideas known in the past.

[History of functional treatment of fractures].

A concise history of fracture treatment from ancient times to XIX century, when the old rules of management started to be modified are presented at the beginning. An isometric traction methods of treatment as well as isotonic ones both indirect and direct have been discussed. The shortcomings of fraction treatment induced application of functional treatment that was a combination of the traction and joint movements assisted by the mechanical devices. The author indicates that functional treatment methods were not known to polish orthopaedic surgeons up to the seventies. This empty space was filled only in the last decade. From historical analysis author concludes that external fixation is the future of fractures treatment.

[Nonspecific osteogenic arthritis of the hip].

Osteogenic arthritis of the hip in children depending on the localization of the primary focus of infection can be classified into three types of femoral, acetabular and pelvic origin. These types differ as to the pathology as well as clinical course of the disease. Diagnostics and treatment is discussed. It was found that improper treatment in the early stage led to the chronic form of the disease. Clinical and radiological picture of osteogenic hip arthritis as well as its treatment is discussed on the basis of 125 cases reviewed. Different sequel to osteogenic hip arthritis depending on pathological origin, stage of the disease and method of treatment were pointed out.

[Nonspecific arthritis of the hip in children].

Clinical and radiological picture as well as pathological classification of non specific hip arthritis in children were presented after reviewing 100 cases and on the base of literature. Different course and sequel of different pathological types of hip arthritis were pointed out with emphasis on suppurative arthritis of the hip. The management of all types of hip arthritis was discussed. The necessity of surgical drainage of the joint in the first 5-7 days of suppurative arthritis as the only measure to prevent irreversible hip destruction was underlined. The method should become popular among pediatric surgeons and pediatricians who are the first ones to take care of a child in the early stage of disease.

[Transiliac postero-lateral approach to the bodies of 2 lower lumbar vertebrae and the upper segment of the sacral vertebrae].

The author has described in detail the surgical procedure of a modification of the postero-lateral approach to the lumbar spine, which enables a wide access to the lower lumbar vertebrae and the upper sacral segment. The modification consists in osteotomy of a fragment of the lateral part of the ilium covering this region together with the origins of the dorsal muscles, isolating it from the sacral-pelvic ligaments and removing it posteriorly. This gives a good view into lateral parts of the arches and bodies. This approach has been used by the author--starting from 1964--in 78 operations in such diseases as: spondylolisthesis, spondylitis, stenosis of the root canals, disorders of growth of the sacral-lumbar junction, and tumors. The operation was tolerated well by the patients and no complications connected with osteotomy of the ilium were recorded.
