[Maintenance and the clinical evaluation of periodontal patients in Konus-Telescope denture].

In order to assess by periodontal evaluation the changes that might occur with time in the abutment teeth and periodontal tissues when Konus-Telescope dentures are used as that final treatment of periodontal disease, the dentures (15 units) were placed in 13 patients with missing tooth and periodontal disease and findings at the time of denture placement and 30 months after the placement were compared. The number of cases that exhibited significant changes in hygiene level, tissue inflammation and periodontal pocket depth of the abutment teeth after 30 months was very small, while as many as 85.2% of the abutment teeth showed decrease in tooth mobility. Increase in tooth mobility was not detected in any of the cases. In addition, X-ray examination revealed tendencies toward improvement of the periodontal ligament and remission of alveolar bone resorption in many of the cases. These results suggest that Konus-Telescope denture is highly offers protection of the residual periodontal tissues through its secondary splint action.

[Monoclonal antibody to desmosome associated 34 kD protein and immunohistochemical study in relation to the malignancy of transitional cell carcinomas of urinary bladder].

A mouse monoclonal IgG1 antibody (B-11) that recognized desmosomes in some human tissues was isolated and characterized. By Western blotting, B-11 recognized 34 kD protein only in squamous and transitional epithelium. By avidin-biotin complex (ABC) and immunofluorescence method, B-11 was reactive to squamous and transitional epithelium, but was not reactive to simple columnar epithelium from stomach and colon, simple cuboidal epithelium from urinary tubule and thyroid, and pseudostratified ciliated epithelium from trachea and ductus epididymidis. ABC method of B-11 was performed in 47 patients with transitional cell carcinoma of bladder. The higher positive staining was observed in well differentiated (83% in Grade (G) 1, 73% in G2 and 20% in G3), superficial (76% in non-invasive and 20% in invasive) and expansive growth (68% in Infiltration (INF) alpha, 24% in IFN beta and 18% in IFN gamma) carcinomas. 34 kD protein recognized by a new monoclonal antibody (B-11) is a new cell-type-specific desmosomal component which relates to the histological grade, stage and invasive manner of the transitional cell carcinoma of bladder. The transitional cell carcinoma of bladder obtained by operation or surgical biopsy from 47 patients was stained by ABC method using B-11.

[Sequential changes in plasma lipoprotein(a) as acute phase protein after myocardial infarction and surgery group].

The plasma lipoprotein(a), apo AI, apo B, and acute phase proteins, were studied in 21 patients with myocardial infarction and in 11 patients after surgery. In the both groups, C-reactive protein showed a rapid increase, and alpha 1 acid glycoprotein, alpha 1 antitrypsin and lipoprotein(a) followed by a moderate increase and restored to normal values after one month. Lipoprotein(a) increased to a maximum on day 11 in the myocardial infarction group, and on day 8 in the surgery group. Only slight changes in apolipoprotein AI and B were noted. We speculate that lipoprotein(a) is an acute phase protein that plays an important roles in recovery from trauma. Recently it was reported that the amino acid sequence of apolipoprotein(a) is partly identical to that of plasminogen. This sequence suggests that lipoprotein(a) and plasminogen are related immunochemically. We examined by immunoblotting technique whether our antibody for lipoprotein(a) is influenced by a plasminogen in plasma. By enzyme-linked immunosorbent assay, it was found that the purity of plasminogen does not influence determination of lipoprotein(a).

[Radiological and electron microscopic examination on epulis osteoplastica].

Epulis osteoplastica was examined with soft X-ray radiography, scanning electron microscopy (SEM) and electron probe X-ray microanalizer (EPMA). Extracted epulis tissue from the incisor region of mandible was offered to this examination. The soft X-ray radiograms showed well defined radiopaque trabecular structures in the soft tissue. The pathological findings with HE staining revealed that these structures were bone tissues. The SEM images demonstrated irregularly shaped trabecular structures with fine bone tissues. The EPMA images obtained from the same specimen supported the findings.

Granular cell tumor of the stomach coincident with two early gastric carcinomas.

In a 64-yr-old Japanese man, granular cell tumor of the stomach plus two early gastric carcinomas were detected. The granular cell tumor was a 1.5 x 1.5-cm submucosal nodule located in the posterior wall of the fundus. The mucosa adjacent to the granular cell tumor showed two well-differentiated adenocarcinomas restricted to the mucosa, but there was no continuity among these three conditions. This seems to be the first report of this type of association. Immunohistochemical study revealed S-100 protein in the component cells of the granular cell tumor, thereby supporting the proposal that this granular cell is derived from Schwann cells.

Transgenic mouse model of familial amyloidotic polyneuropathy.

Familial amyloidotic polyneuropathy (FAP) is a dominantly inherited disorder, characterized by the extracellular deposition of amyloid fibrils composed of variant transthyretin (TTR), and by prominent peripheral nerve involvement. We demonstrate that the main cause of this disease is the presence of a point mutation in the TTR gene. However, neither the time of onset nor the clinical course is predictable. To elucidate the molecular pathogenesis of this disease, we constructed transgenic mice carrying and expressing the human mutant TTR gene. In these mice, amyloid is deposited in the alimentary tract as early as age six months, and becomes more remarkable with aging. These transgenic mice should be useful in elucidating factors which modulate the time of onset and the clinical course of FAP, and in establishing therapy for this intractable disorder.

Effects of d- and l-pentazocine on the release and uptake of norepinephrine in rat brain cortex.

Effects of d- and l-pentazocine on the release and uptake of norepinephrine and other neurotransmitters were examined in rat cerebral cortex. D- and l-pentazocine (10(-6)-10(-4) M) evoked the release of tritium from rat cortex slices preloaded with [3H]norepinephrine. Both isomers of pentazocine (3 x 10(-5) M) also evoked the release of tritium from slices preloaded with [3H]dopamine and [3H]5-hydroxytryptamine ([3H]5-HT) but did not from those preloaded with [3H] gamma-aminobutyric acid ([3H]GABA) and [3H]choline. The releasing effect of pentazocine was neither dependent on the extracellular calcium nor antagonized by naloxone. Both isomers inhibited the uptake of [3H]norepinephrine, [3H]dopamine and [3H]5-HT but not that of [3H]GABA and [3H]choline into synaptosomes prepared from the cortex. These results suggest that the effects of d- and l-pentazocine on release and uptake are relatively specific to monoamines and might explain some aspects of pharmacological actions of pentazocine.

[Influence of ischemic heart disease on early and late mortality after aortic reconstruction for abdominal aortic aneurysm and aortoiliac occlusive disease].

A retrospective study was undertaken to assess the influence of ischemic heart disease (IHD) on the early and late mortality of patients undergoing elective repair of an abdominal aortic aneurysms (AAA) or aortoiliac occlusive disease (AIOD). The patients were divided into IHD and non-IHD groups on routine clinical grounds. Among 157 patients with AAA, postoperative myocardial infarction occurred in 12.8% of the IHD group compared with 0.9% of the non-IHD group (P less than 0.05). The late mortality rate in the IHD group was higher than in the non-IHD group (p less than 0.05), and the mortality rate from myocardial infarction was 30% in the IHD group compared with 13% in the non-IHD group. Among 119 patients with AIOD, clinical evidence of IHD was found in 24 patients and extra-anatomical bypass was performed in 54% of these patients, compared with 35% of the patients in the non-IHD group. There was no occurrence of postoperative myocardial infarction. This study shows that an aggressive diagnostic approach should be taken for patients with AAA who have clinical evidence of IHD and that reevaluation of IHD should be performed in patients with AIOD after aortic reconstructive surgery.

[Prevention of spinal cord ischemia after cross-clamping of the thoracic aorta--monitoring of spinal cord perfusion pressure and somatosensory evoked potentials].

The pressure difference between the mean distal aortic pressure (MDAP) and the cerebrospinal fluid pressure (CSFP), defined as the spinal cord perfusion pressure (SCPP), as well as somatosensory evoked potentials (SEP) were monitored intraoperatively to detect and prevent intraoperative spinal cord ischemia in 24 patients who required cross-clamping of the descending thoracic aorta. A temporary axillo-femoral shunt, utilizing a 10 mm woven Dacron tube graft, was employed in 10 patients and partial cardiopulmonary bypass was employed in fourteen. Ischemic SEP changes were seen in six patients. Two patients, whose SCPPs were 32 and 35 mmHg, showed a complete loss of SEP and subsequently developed paraplegia. In the other four cases, increase of the MDAP and/or withdrawal of cerebrospinal fluid were performed to increase the SCPP to more than 60 mmHg when ischemic SEP changes occurred. The SEP gradually recovered in two of these cases. The ischemic SEP changes seen in one patient, who had the longest aortic cross-clamping time, (175 minutes) returned to normal immediately after unclamping. In another case, who had a thoracoabdominal aortic aneurysm, the intercostal arteries were reimplanted since the ischemic SEP changes did not revert. These four patients recovered without any neurological deficit. In the other 18 cases without ischemic SEP change, SCPP was kept at more than 40 mmHg during aortic cross-clamping. We conclude that the SCPP should be maintained at more than 40 mmHg during aortic occlusion, and increased to more than 60 mmHg when ischemic SEP changes occur, by increasing MDAP and/or withdrawing cerebrospinal fluid in order to prevent postoperative paraplegia.

Papillary adenoma of the gallbladder associated with regurgitation of pancreatic juice through abnormally shaped union. Case report.

In a 52-year-old woman papillary adenoma of the gallbladder was associated with regurgitation of pancreatic juice into the biliary tract via an abnormally shaped union, not a long common channel. The condition was demonstrated by endoscopic retrograde cholangiography and histologically confirmed. Extended cholecystectomy and lymph-node dissection were performed.

[Studies on cholangiographic and endoscopic findings of primary intrahepatic cholesterol stones].

Cholangiographic investigations in 16 patients with primary intrahepatic cholesterol stones yielded some characteristic findings as follows. 1. Several numbers of stones were filled within locally dilated portion of the subsegmental or more peripheral intrahepatic bile duct branches, with no or minimal dilatation of the more peripheral portion and no stenosis-like lesions of the more hilar portion. 2. The lesion was single or multiple scattered in the liver and intrahepatic cholangiogram excluding involved segments was almost normal. 3. Although extrahepatic bile duct and/or gallbladder stones were often associated, the normal gallbladder, cystic duct and papillary portion and slightly dilated common bile duct were demonstrated. These cholangiographic findings were compatible with the endoscopic findings that inflammatory changes of the bile duct were rarely observed. According to above mentioned findings, it is suggested that the etiology of this disease differs from intrahepatic stones of calcium bilirubinate and dissolution therapy with bile acid may be available.

[Malignant melanoma of male urethra: a case report].

A case of male urethral melanoma is reported. A 85-year-old male with a 2-month history of progressive, severe obstructive urinary symptoms and bloody urethral discharge was referred to us after an unsuccessful management at a local doctor. Physical examination revealed an ill looking old man with no evidence of nevi or other cutaneous pigmentation looking like malignant melanoma. Neither palpable periurethral mass nor inguinal lymphadenopathy was noted. RUG showed an irregular shadow defect in bulbous urethral regions. In cystourethroscopy, a raised nodular reddish black lesion in the urethra without adjacent satellite lesions was found. Histologic examination revealed that the tumor was made up of closely spaced, anaplastic, spheroidal or polyhedal cells. Intracellular brown pigment was richly present, gave a negative reaction for iron, but stained black with Masson-Fontana's method. Further examination for evaluating metastases including bone scintigraphy, computer tomographic scan, chest X-ray film were negative. Due to his poor risk, radical operation such as cystourethrectomy might be undesirable. We performed TUR to relieve urethral obstruction, because the patient refused cystostomy. He died of wide spread metastases at 6 months after the operation. This case seems to be the second report in the Japanese literature.

[Primary osteogenic sarcoma of the breast. Report of a case].

Primary osteogenic sarcoma (POS) of the breast is very rare. Only eleven cases have been previously reported in Japan. In this paper, we report an additional case of POS. A 30 year old female was admitted to our hospital with chief complaint of rapidly growing mass of the right breast. She was diagnosed carcinoma of the right breast and underwent a radical mastectomy (Br + Ax + Mj + Mn). Histological findings of the resected tissues revealed POS of the breast. After three months from the operation, local recurrence was demonstrated by an incisional biopsy. One more months later, chest X-P revealed bilateral lung metastases. Irradiation with 60Co for recurrence and chemotherapy using vincristine, methotrexate for distant metastases were not effective. However, cisplatin caused the necrosis of local recurrent tumor. She died nine months after operation. At autopsy, extended metastasis of tumor cells diffusely to the pleura, lungs, liver, adrenal glands, pancreas, peritoneum, fifth lumbar vertebra, skull, dura mater and left pulmonary hilar lymph nodes was observed.

[Two autopsy cases of prune belly syndrome in stillborn and its pathogenesis].

Two autopsy cases of prune belly syndrome are reported. In Japan, 111 cases have been reported. Abdominal muscle deficiency or hypoplasia, urinary tract abnormalities and cryptorchidism are usually recognized. Case 1 was a female stillborn at 33 weeks' gestation and case 2 was a male stillborn at 21 weeks' gestation. Both cases had anomalies of genitourinary and digestive system. There was anatomic obstruction of external urethral orifice in case 1 and overriding urethra in case 2 which meant functional obstruction. In relation to the pathogenesis, we propose the important role of the urethral obstruction, whatever anatomic or functional. Further examination of the lower urinary tract including serial section analysis might be necessary to identify the pathogenesis of prune belly syndrome.

[An autopsied case of malignant paraganglioma of the posterior thoracic cavity].

An autopsied case of a malignant paraganglioma of the posterior thoracic cavity is reported. A 68-year-old man had complained of chest discomfort, and serial examinations revealed a functioning paraganglioma with bone metastasis. After death a pathological examination revealed that the tumors consisted of alveolarly arranged cells and well developed capillary vessels. Numerous neurosecretory granules were observed on viewing by electron microscopy. An immunohistochemical examination showed that most of the tumor cells were positive for NSE, while only a few cells were positive for the S-100 protein. These results indicate that a paraganglioma originating from the aortic sympathetic paraganglia had similar features of a carcinoid and a neuroblastoma.

[Evaluation of cancer treatment by autopsy].

Various indices such as tumor markers, X-ray figures and variation of leucocyte count and body weight, are used for objective evaluation of cancer treatment in clinical medicine. On the other hand, autopsy does not provide exact evaluations in this sense. However, our analysis of 105 cancer autopsy cases revealed that autopsy helped to resolve 94.9% of the problems raised in clinical medicine: 94.9% having regard to primary and metastatic tumors, 45.7% with complicating diseases and cause of death 13.3% with appropriateness of diagnosis, and another 13.3% with suitability of treatment. To recover the position of autopsy in medicine, it is important to restore "science" in pathology by incorporating modern scientific methods and by logical analyses of exact findings obtained at autopsy. For this purpose, autopsy findings must be categorised and coded rather than descriptive. On the other hand, it is important to satisfy the clinical side by responding to the needs of rapidly advancing clinical medicine. For this purpose, especially in Japan where there is a shortage of pathologists in medical schools and hospitals, it is essential to involve clinical staffs, postgraduate students, and clinical residents in analysis of autopsy cases.
