[A case of silicate urolithiasis].

We report a case of silicate calculi with no history of taking magnesium trisilicate. A 33-year-old woman was sent to our hospital as an emergency case because of severe right lower flank pain. Physical examination was unremarkable except for severe right cost-vertebral angle knock pain. She denied administration of a magnesium trisilicate anti-acid before. She was admitted to the urologic ward since the pain did not relieve in spite of several analgesics. The stone passed spontaneously on the third hospital day. Analysis by infrared spectrophotometry demonstrated the composition to be over 98% of silicate. A review of the literatures discloses only 21 cases of silicate stones.

[Angiotensin II decreases atrial natriuretic peptide-induced cyclic GMP accumulation in rat glomerular mesangial cells].

Glomerular mesangial cells are believed to contribute to regulation of glomerular filtration rate through their contractility, which is regulated by various vasoactive hormones such as angiotensin II (A II), and atrial natriuretic peptide (ANP). A II has been recently reported to inhibit ANP-induced cyclic GMP (cGMP) accumulation in vascular smooth muscle cells, and other types of cells, but the mechanism of this inhibitory effect of A II is still unclear. In order to know the interaction between A II and ANP in glomerular mesangial cells and to know the mechanism of the interaction, I examined the effects of A II on ANP-induced cGMP accumulation in cultured rat glomerular mesangial cells. ANP produced rapid increase in cellular cGMP in cultured rat glomerular mesangial cells, which was significantly inhibited by co-incubation with A II. A II also inhibited cGMP accumulation produced by sodium nitroprusside, soluble guanylate cyclase activator. This inhibitory effect of A II was completely blocked by 1 mM of 3-isobutyl-1-methylxanthine (IBMX), a cyclic nucleotide phosphodiesterase inhibitor. Thus, it seems that A II inhibits ANP-induced cGMP accumulation by activating phosphodiesterase rather than by inhibiting guanylate cyclase. Since the action of A II has been reported to be mediated by increase of cytosolic free Ca2+ secondary to inositol 1,4,5-trisphosphate (IP3) generation and activation of protein kinase C secondary to diacylglycerol (DG) generation, I investigated the effects of Ca ionophore (A23187), and 12-O-tetradecanoyl phorbol-13-acetate (TPA), protein kinase C activator, on ANP-induced cGMP accumulation.(ABSTRACT TRUNCATED AT 250 WORDS)

[Rapid prenatal diagnosis of familial amyloidotic polyneuropathy using DNA amplification].

We have used in vitro DNA amplification by the polymerase chain reaction for prenatal diagnosis of familial amyloidotic polyneuropathy (FAP) in a German family. Fetus DNA was prepared from chorionic villus sampling and specific prealbumin DNA sequence that may carry FAP type I mutation was enzymatically amplified. The mutant prealbumin gene could be distinguished by direct DNA analysis with Nsi I using amplified DNA. This method is sufficiently sensitive and rapid that prenatal diagnosis of FAP can be done on the same day when fetal DNA is available.

Value of tissue carcinoembryonic antigen in patients with colorectal carcinoma.

The tumours of 55 patients with colorectal carcinoma were evaluated for tissue carcinoembryonic antigen (CEA) by immunoperoxidase staining. It was shown that 33/35 patients with increased preoperative serum CEA levels above 5 ng/ml had positive tissue CEA. The other 17/20 patients who had serum CEA levels less than 5 ng/ml could be demonstrated CEA in tissue. The results of tissues CEA were compared with their preoperative serum CEA levels in the pathologic grading, histologic type and staging of cancer. It was found that tissue CEA was more sensitive than serum CEA and was correlated with serum CEA in all respects. The finding in this study suggests that tissue CEA should be performed along with preoperative serum CEA in all patients suspected of having colorectal carcinoma. The postoperative serum CEA should be determined serially in the patients who have more than 5 ng/ml serum CEA and/or tissue CEA positive although their preoperative serum CEA is less than 5 ng/ml.

[Tumor lysis syndrome at the induction therapy of the first remission in two cases of T-ALL].

The anti-tumor therapy followed by tumor lysis syndrome may cause the metabolic disorders including hyperkalemia, hyperphosphatemia and hyperuricemia. It should be known that it occurs frequently in lymphoproliferative diseases, especially in Burkitt's lymphoma. Two cases of T-ALL accompanied by this syndrome, from which the patients were recovered, at the induction therapy of the first complete remission are reported here. Case 1. A 28-year-old man received VP therapy under the diagnosis of T-ALL with massive hepatosplenomegaly and bilateral enlarged kidneys. During the therapy, metabolic disorders with both renal failure and ventricular tachycardia happened. They were resolved by certain series of treatments. The patient was brought to a complete remission with normal size of liver, spleen and kidneys. Case 2. A boy aged 15 having received the intrasubarachnoidal infusion of MTX and 1-Ad-VP therapy under the diagnosis of T-ALL accompanied by this syndrome which was improved by an appropriate treatment, and the patient was lead to the remission. The risk factors of this syndrome, such as 1-high drug sensitivity of the tumor; 2-renal dysfunction; 3-rapid cytokinetics of the tumor cell; 4-bigger size of the tumor, as well as the preventive treatment of this syndrome are reviewed.

[An autopsy case of extramural malignant leiomyoblastoma of the stomach with ovarian cancer: an immunohistochemical study].

The subject was an 85-year-old woman, who had been diagnosed as having an ovarian cancer and carcinomatous peritonitis and had been treated conservatively. She subsequently died from respiratory and renal insufficiency, and the autopsy that followed revealed that her pelvic cavity had been filled by a tumorous mass that size of a child's head. Histologically, the tumor was a serous cystadenocarcinoma of the ovary. Moreover another tumor, also the approximate size of a child's head, was found sited extramurally, beneath the posterior wall mucosa of the stomach body. Histological inspection of this tumor revealed a proliferation of round oval, and spindle-shaped tumor cells. A vacuolation of the cytoplasms and karyomitosis to the extent of 10/50 HPF also were observed. Based on the findings of being positive for Vimentin and a negative EMA, this tumor was diagnosed as being a malignant leiomyoblastoma of the stomach smooth muscle. The leioblastoma is a relatively uncommon neoplasm, and recent advances in immunohistochemical staining have indicated that some of these tumors are not only of smooth muscle derivation but also of nerve origin. Therefore, this tumor, given its morphological characteristics, had been generalized in this case as a gastric stromal tumor, and with negative findings for Desmin and S-100 protein, as well as positive for Vimentin.

[Intrapericardial bronchogenic cyst--a report of two surgical cases].

Two cases of intrapericardial bronchogenic cyst are reported. Case 1 was a 58-year-old man with a complaint of exertional dyspnea. A chest X-ray film showed a right hilar mass shadow. Echocardiographically the mass showed cystic pattern in the pericardium. A pulmonary angiogram revealed a mass displacing the right pulmonary artery superiolaterally and left atrium and pulmonary vein inferiorly. The preoperative diagnosis was intrapericardial cyst. Subtotal resection of intrapericardial cyst was performed via right thoracotomy. Histological diagnosis was bronchogenic cyst. The postoperative course was uneventful. Case 2 was a 41-year-old man. A displacement of the esophagus and duodenal ulcer was pointed out in the upper gastrointestinal series. A chest X-ray film showed a right hilar mass shadow. Since echocardiogram and pulmonary angiogram revealed similar findings to those in case 1, intrapericardial bronchogenic cyst was suspected preoperatively. Complete resection of the intrapericardial cyst was made via right thoracotomy. Histological diagnosis was bronchogenic cyst. The postoperative course was uneventful.

[Chromosome aberrations and genes in human and experimental leukemias].

Although determination of chromosomal abnormalities may be of limited usefulness for the diagnosis of leukemia, the recent advances in the molecular mechanism associated with chromosome aberration has been rapid. Chromosome translocation in Burkitt lymphoma and chronic myeloid leukemia was the most striking evidence for the oncogene activation. Other specific chromosome abnormalities for FAB-classified leukemias are also known. Translocated type of chromosome abnormalities between immunoglobulin or T-cell receptor genes and oncogenes may also affect the T and B-cell leukemogenesis. However, the role of trisomies found in human and experimental leukemias and the gene dosages had been thought to be most important until 1982, has not been unclear. Many types of phenotypically heterogeneous leukemias have been reported. t(4 ; 11) acute leukemia is one such leukemia which shows early B-cell and myelomonocytic nature. Heterogeneous leukemias have been called biphenotypic, hybrid and acute mixed leukemias. The terminology must be used the unified. Recent trials to use paraffin-fixed tissues and bone marrow smear for molecular analysis has been successfully reported. Basic analysis on the DNA degradation mechanism revealed the enzymatic activity might play an important role before the complete fixation.

[Multiple primary cancers of the head and neck].

From January 1985 to June 1988, the authors have treated 138 cases of head and neck cancers. Among this number, six cases (4.3%) involved patients with a multiple primary cancer. With the exception of 1 case, all patients were male with a history of heavy smoking and excessive drinking. The primary cancer of the head and neck was most frequently located in the hypopharynx, and a secondary multiple cancer in the upper digestive tract especially the esophagus. Although we were unable to detect an early esophageal cancer by an esophagogram in some cases, each lesion could be detected by using lugol stain and gastrointestinal endoscopy.

[Clinicopathological and immunohistochemical study on penetrating and superficial spreading type of early gastric cancers].

Specific type of early gastric cancer based on the cancer surface area and the degree of invasion were studied by measuring digital values of the cancer surface area in early gastric cancer patients. The results indicated that the pen and super type of early gastric cancer had many clinicopathological characteristics as compared with common type of early gastric cancer. An immunohistochemical study also revealed that the pen type of early gastric cancer had a high growth activity. Moreover, it was suggested that EGF was involved in its specific invasion and growth, and that EGF and TGF-beta affected its scirrhous growth. The possibility that the poorly differentiated pen type is an early lesion of linitis plastica type gastric cancer was also considered. From these findings, it was assumed that the immunological staining of EGF and TGF-beta in biopsy specimens might be useful in the diagnosis of the pen type of early gastric cancer and also in diagnosis of early lesion of linitis plastica type gastric cancer.

[Rationale of paraaortic lymph nodes dissection for advanced gallbladder cancer].

Paraaortic lymph nodes dissection was performed routinely in 28 patients with advanced gallbladder cancer who underwent curative or relative non-curative resection. In 10 patients out of 28 (36%), metastasis was found in the dissected paraaortic nodes. The incidence of paraaortic lymph nodes metastasis was 50% when the gallbladder serosa or adjacent organs were involved by cancer. If routine paraaortic lymph nodes dissection had not been carried out, the operation would be regarded as curative in 4 patients out of 10 with paraaortic lymph nodes metastasis. The incidence of paraaortic lymph nodes metastasis was twice higher than that of the mesenteric root and the metastatic route was possibly derived directly from the retroportal and/or retropancreactic nodes. Therefore, we emphasize that the paraaortic nodes should be regarded as the nodes between the 2nd group such as the retroportal or retropancreatic nodes and the 3rd group including the nodes of mesenteric root. In spite of positive paraaortic nodes, mean survival time of the patients without other non-curative factors was 26 months and the longest survival was 42 months. It seems that paraaortic nodes dissection has palliative effects to prolong the survival period of those patients. Paraaortic lymph nodes dissection should be employed in radical operative procedures for advanced gallbladder cancer.

Combined resection of the inferior vena cava for hepato-biliary and pancreatic malignancies.

Seven cases of hepato-biliary and pancreatic malignancies that underwent partial resection of the inferior vena cava) were reviewed. Histological findings of inferior venca cava involvement were direct invasion in 5 cases, tumor thrombus in 1 case, and adhesion in 1 case. Correct preoperative diagnosis of inferior vena cava involvement was made in only 2 cases. A retrospective study on enhanced CT revealed that irregular deformity of the inferior vena cava had suggested inferior vena cava involvement. Total occlusion of the inferior vena cava was employed temporarily for inferior vena cava resection in 3 cases. A saphenous vein graft was used for reconstruction in 2 cases. Complications due to inferior vena cava resection are, as yet, unknown. One patient is alive, without recurrence, 24 months after the operation. One case underwent re-resection of liver metastasis, and is alive 17 months after the operation. Although advanced hepato-biliary and pancreatic malignancies involving inferior vena cava have been regarded as having a poor prognosis, an aggressive surgical approach may be applicable in some cases.

[Developmental changing of rat taste responses from chorda tympani nerve].

In order to clarify developmental changing of gustatory system, histological and electrophysiological experiments were performed in the rat. Histological examination on the anterior tongue innervated by chorda tympani nerve showed that the ratio of matured taste buds which possess a taste pore were only 9% of all taste buds observed at 1 week of postnatal age, and 81.3% at 3 weeks of age. Recording integrated responses from the chorda tympani nerve reveals that taste buds with a pore at 1 week of age responded to NaCl, HCl, and quinine-HCl as well as in adult rats, which suggests that these relatively young taste buds are matured functionally for these three stimuli. However, the response magnitudes for various sugars at 1 week of age were smaller compared to those in the adult rat, reached to the maximum at 3 weeks of age, then decreased gradually with age. Also, results from the experiment of cross-adaptation among different sugars, effects of pronase-E treatment of the tongue, analysis of correlation between on- and off-responses to sugars, showed that qualitative changes for sugar responses continues after 3 weeks of age. These results suggest that functional changes occur in the gustatory processing of sugars during postnatal development in the rat chorda tympani nerve.

[Bone marrow necrosis in thymic T cell lymphoma].

A 29 year-old male patient was admitted because of exertional dyspnea, due to the pleural effusion on chest X-P on June 18 of 1988. On admission, enlargement of left supraclavicular and mediastinal lymph nodes were found. Cytological studies of pleural exudate cells showed the proliferation of atypical lymphoid cells with a few eosinophiles. Surface markers of the tumor cells were just positive for both CD5 and CD38, thus indicated that they resided as early thymocyte along T cell differentiation. Moreover, according to Southern blotting analysis, these DNA's showed rearrangement of TCR-beta genes. Based on these results, he was diagnosed to be suffered from thymic T cell lymphoma. Bone marrow biopsy on left posterior iliac bone showed tumor necrosis owing to extensive tumor which were diffusely infiltrated among collagen fibers with reticulin fibrosis. He was treated with aggressive combination chemotherapy but achieved no remission and died of cerebral vascular bleeding on Jan. 7, 1989.
