An abnormal response of the adrenal cortex to insulin: glucose in essential hypertension.

This investigation demonstrates an abnormal response to the adrenal cortex to modulation of the potassium metabolism by an intravenous infusion of insulin-glucose in patients with essential hypertension. In response to insulin-glucose there was a transient decline of plasma aldosterone and plasma cortisol concentrations in control subjects with normal blood pressure, whereas a temporary increase of both hormones was found in patients with essential hypertension. Treatment with thiazide diuretics further magnified the different aldosterone response between the two groups. It is suggested that the abnormal response of patients with essential hypertension may be of significance to the understanding of the pathogenesis of this important disease.

Immunocytochemical visualization of taurine: neuronal localization in the rat cerebellum.

A novel technique for immunocytochemical demonstration of taurine (Tau) is presented. Antisera raised against Tau conjugated to protein by glutaraldehyde (GA) react selectively with similar conjugates in model systems and in tissue fixed with GA. In rat cerebellum, Tau-like immunoreactivity is high in the Purkinje cells but low in other cell types, including the stellate cells for which Tau has been proposed as transmitter.

Taurine in the hippocampal formation of the Senegalese baboon, Papio papio: an immunocytochemical study with an antiserum against conjugated taurine.

An antiserum raised against taurine conjugated to bovine serum albumin by glutaraldehyde produced intense staining of hippocampal pyramidal neurons at the CA1/CA3 transition (including CA2) and of a small proportion of the granule cells. Strongly immunoreactive neurons were also found in a zone overlapping the second reflected blade in the hilus. Most glial cells were unlabeled.

Data entry by light pen.

Collection of data is often difficult in medical and nursing applications of computers. A system has been developed using an inexpensive light pen to enter most of the required information. This method of data entry has proved to be acceptable to medical and nursing staff in clinical situations. The software places light blocks adjacent to a list of items available for selection. When the pen is applied to a light block, the item is selected with visual and audible confirmation being given. Corrections may be made by re-applying the pen to the light block.

Aldosterone response to modulation of potassium in patients on dialysis or with essential hypertension.

This investigation demonstrates in patients with essential hypertension an abnormal response of the adrenal glands to modulation of potassium metabolism by infusion of insulin-glucose. Similar results have been reported in anephric patients, while the inverse response of non-nephrectomised patients on dialysis corresponded to that of normal subjects. It is suggested that the abnormal response of patients with essential hypertension may be of importance to the understanding of the pathogenesis of this important disease.

Clinical results and cyclosporine effect on prednisolone metabolism of cadaver kidney transplanted patients.

In the first randomised study of cyclosporine in Denmark 43 cadaveric kidney recipients were treated either with cyclosporine (Cys) and prednisone or azathioprine (Aza) and prednisone. The degradation of prednisolone was examined in 18 of these patients. The 12 month graft survival of the Cys group was not significantly greater than that of the Aza group (68% and 60%, respectively), but fewer rejection episodes occurred in the Cys group (p less than 0.05). The degradation of prednisolone was significantly decreased in the Cys group compared to the Aza group. Accordingly, plasma half-lives of prednisolone were increased in the Cys group compared with the Aza group. As a decreased degradation of prednisolone during Cys treatment may lead to increased steroid activity, we recommend that a reduction of the prednisone dosage during Cys treatment is safe.

A record system for the Apple II microcomputer.

A suite of programs for the Apple II microcomputer is described. The programs allow a clinician with no programming knowledge to set up a record system suitable for storing and analysing clinical information. The system requires an Apple II or Apple IIE microcomputer, a printer, and a simple light pen.

Vaginal spermicides and miscarriage seen primarily in the emergency room.

Among 813 women who had obtained a vaginal spermicide within 48 weeks of the estimated date of fertilization (EDF), 47 (5.8%) had an early miscarriage. Among women who had obtained oral contraceptives, 35/1,127 (3.1%) miscarried, and among women who obtained neither, 140/4,231 (3.3%) miscarried. The risk ratio estimate comparing spermicide users with nonusers was 1.8 (90% confidence interval 1.4, 2.3). The association was strongest among women who had obtained a spermicide within 12 weeks of the EDF. Examination of abortus material revealed that the association with spermicides was strongest among those where an abnormal fetus was present.

Nonestrogenic drugs and breast cancer.

The relation between breast cancer and selected nonestrogenic drugs was evaluated in the Group Health Cooperative of Puget Sound, Seattle, Washington, a prepaid health care organization with computerized information on diagnoses and outpatient drug use. No important positive associations with breast cancer were found in a follow-up study of 302 women aged 35-74 years. These women were newly diagnosed with breast cancer in 1977-1980 and were studied in relation to exposure in the six months prior to diagnosis to one or more of the following drugs: diazepam, digitalis glycosides, medroxyprogesterone acetate, methyldopa, metronidazole, phenothiazines, tricyclic antidepressants, thiazides, thyroid/levothyroxine sodium, or spironolactone. A modest association between recent reserpine use and breast cancer was present (risk ratio = 1.7, 90% confidence interval 0.9-3.3).
