Sixteen cases of either a pityriasiform or discoid eczematous rash occurring in patients with rheumatoid arthritis receiving treatment with gold (sodium aurothiomalate and auranofin) were studied. The results suggest that this is a dose related, not allergic, reaction to gold. The development of this rash is not an absolute indication to stop treatment with gold. Control can often be effected with potent topical steroids or a reduction in the dose or frequency of treatment with gold.
We have observed a high incidence of pemphigus foliaceous, in the absence of therapy with penicillamine, within a small population of patients with rheumatoid arthritis. We suggest that penicillamine as well as inducing autoimmune disease might exacerbate subclinical pemphigus foliaceous in this group, accounting for those few patients whose skin disease fails to resolve following drug withdrawal. Pemphigus and rheumatoid arthritis have both been associated with HLA DR4, which was present in all three of our patients who were tested.
We have found a high incidence (4.8%) of allergy to hydrocortisone in patients with suspected allergic contact dermatitis. In view of this finding we would suggest that, in any condition that does not improve or that deteriorates after administration of hydrocortisone, allergy to this compound should be considered.
Two cases of keratoacanthoma arising within organoid naevi are presented. The possible aetiology of keratoacanthoma in relation to organoid naevi is discussed.
Immunosuppression is recognized to predispose to the development of disseminated superficial actinic porokeratosis and to cause the spread of pre-existing lesions. Porokeratosis of Mibelli has been less frequently described following immunosuppression and we report a further case.

We report an outbreak of occupational allergic contact dermatitis from 1,6-diisocyanatohexane (old name hexamethylene diisocyanate or HDI), in the anti-pill finish Evafanol AS-1, in 2 clothing factories. The 19 operatives from 2 dressmaking mills all complained of work-related dermatitis. 5 of the subjects had positive patch tests to the fabric with which they were working, and when all 19 were patch tested to Evafanol AS-1 (1% aq.), 11 were positive. 6 of these 11 were patch tested to HDI (1% pet.) and all 6 were positive. It appears that HDI in our group of clothing workers acted as a potent sensitizer affecting around 10% of the workforce.

11 patients, positive to tixocortol pivalate on patch testing, were tested intradermally with a variety of hydrocortisone analogues. Substitution at the C21 position had no effect on the occurrence of positive reactions to hydrocortisone, whereas alteration of any of the carbon rings significantly reduced the number of positive reactions. We suggest that the C17 side chain is involved in protein binding and subsequent presentation of antigenic sites on the carbon rings.
A one in two alternate sample (N = 200) from a population-based register of 411 people with multiple sclerosis (MS) was studied. Out of this sample, 147 people with MS and 34 people with rheumatoid arthritis were interviewed at home and completed a battery of neuropsychological tests. Cognitive impairment was found in 46 per cent of those with MS, with memory impairment in 34 per cent and failure on tests of frontal lobe function in 33 per cent. Physical disability was associated with cognitive impairment. Memory impairment was more common in those who had had MS for 10 years or more. A significant minority of people with mild physical disability and some who had had MS for less than a decade nevertheless had cognitive impairment. Relationships between cognitive impairment, other disease variables and psychosocial factors were examined. Counselling and rehabilitation programmes for people with MS and their families should take account of cognitive deficits that may be present.
The subjective narrative employed by the little girl to account for her femininity draws on her primary sense of femaleness, as well as on any secondary elaborations. The secondary nature of femininity is highlighted by the Freudian view wherein femininity is thought to be achieved through changes in libidinal aims and objects. The primary nature of femininity is highlighted by current views elaborating on ego development. An examination of the subjective role of the phallus in a number of clinical reports reveals that envy of the penis may represent the intersection of several associative chains. For example, the girl's castration reaction could result from a condensation of the libidinal theme of not having a penis as Freud argued. However, Freud's reliance on the phallus as a metaphoric representation of instinctual vicissitudes is not an end point. In addition to Freud's elaborations of the phallic theme, the desire to possess the penis may represent the girl's associations to contiguous events. That is, through the process of displacement, the phallus also functions as a metonymic symbol. To fully appreciate penis envy, both the metaphoric and metonymic meanings assigned to femininity must be analysed.
We report a case of necrolytic migratory erythema in association with a neuroendocrine tumour producing predominantly insulin. As far as we know this has not been observed before.
