Vulval Crohn's disease: difficulties in diagnosis.

Four patients with chronic vulval inflammation are described. The histological features of non-caseating granulomata and multinucleated giant cells are compatible with Crohn's disease, but only two patients had proven gastrointestinal involvement. The clinical and histological characteristics of Crohn's disease and other granulomatous inflammations of the vulva are discussed and the literature is reviewed.

Topical 5-aminosalicylic acid: a treatment for aphthous ulcers.

A double-blind placebo controlled trial of 5% 5-aminosalicylic acid (5-ASA) cream for the treatment of oral aphthous ulcers was carried out on 22 subjects. The cream or a placebo (11 patients each) was applied to the ulcers three times daily for up to 14 days. Daily discomfort was reduced by half (P less than 0.01) and less pain (P less than 0.05) was experienced by the treated group. Treatment with 5-ASA shortened healing time (7 vs. 11 days, P less than 0.01) and reduced the difficulty in eating (P less than 0.05). No significant side-effects were reported. We believe 5-ASA cream to be an effective treatment for aphthous ulcers.

Post-irradiation morphoea.

We present details of nine patients who developed morphoea after radiotherapy. In every patient morphoea began within the irradiated area and in four spread beyond it. We believe the irradiation triggered the morphoea despite the absence of any clear-cut relationship to dosage or severity of the acute reaction. Dermatologists and radiation oncologists should be aware that this condition may lead to the mistaken diagnosis of a local tumour recurrence.

Alcohol consumption and psoriasis.

A study has been undertaken of the relationship between the severity of psoriasis and alcohol consumption in 100 patients with chronic plaque psoriasis. In male patients, heavy drinking, at a level liable to be detrimental to health, was found significantly more commonly in those with severe psoriasis, and alcohol-related medical or social problems were frequent. By contrast alcohol excess and alcohol-related problems were significantly less common in women irrespective of the severity of their psoriasis.

A case of mycosis fungoides mimicking actinic reticuloid.

A 61-year-old man presented with erythema and induration of the skin on his face and neck. Initially a photosensitive dermatosis was suspected but this was excluded on further investigation. Mycosis fungoides was diagnosed and the lesions responded to photochemotherapy (PUVA).
