A case of pituitary acromegaly with multiple nonendocrine neoplasia is reported. The tumorogenic potential of growth hormone, prolactin and other growth factors is discussed.
An experimental rat model of established peritonitis was used to test the effect of intraperitoneal injection of platelet rich plasma (PRP) on blood and peritoneal fluid culture positivity and survival rates. Thirty animals divided into two groups of 15 each were studied. The first group served as control while animals in the second group received intraperitoneal injection of PRP. The use of PRP in established. Peritonitis was of no significant benefit.
Two cases with intestinal endometriosis, one with a right iliac fossa mass and the other with subacute intestinal obstruction, are presented. In both the cases, the diagnosis was not suspected clinically and was reached only on histological examination of the resected specimens.
Marlex mesh with zipper was used for abdominal closure in 5 of 147 patients with generalized peritonitis seen during a period of 2 years. Residual/recurrent intra-abdominal sepsis necessitating repeated explorations prompted use of this technique followed by frequent peritoneal lavages. Abdominal sepsis was successfully controlled in 4 of 5 patients, although we lost 3 of 5 patients due to multiple factors.
The drug vecuronium bromide, a short acting, non-depolarising agent with little side-effects, has brought much promise in the field of muscle relaxants. Fifth healthy patients were induced with injection thiopentone sodium 4 mg/kg and vecuronium bromide, 0.1 mg/kg was given IV. The earliest time at which the largest percentage of patients could be intubated satisfactorily was noted. The intubating conditions were estimated by scoring method. The duration of clinical relaxation was decided from the time of IV injection of vecuronium bromide to the return of muscle power of the non-respiratory muscles requiring repeat dose of the drug. The ideal intubating condition was achieved at 120 seconds and the duration of clinical relaxation ranged from 11-15 minutes.
Over a period of eleven years, eight patients were treated for duodenal ulcer perforation measuring more than one centimetre in diameter. In five of these patients, the perforation was sealed using a jejunal loop as serosal onlay patch; one patient underwent gastrectomy and in two patients catheter duodenostomy was done. Two patients died, both due to renal failure. The six surviving patients were discharged with the advice for a subsequent 'ulcer-curing' operation after six months. Two patients developed life threatening complications during this waiting period and one of these died. Giant perforations of duodenal ulcer can safely be closed using a jejunal loop as serosal patch. Delay in doing the second stage definitive surgery for the ulcer may be dangerous.

Distal ureteral atresia is a rare congenital anomaly of early childhood. Despite the present day diagnostic modalities, the pre-operative diagnosis continues to elude the clinicians. A greatly increased space between the bladder and the rectum in a lateral excretory urogram, in association with renal and bladder alterations, is highly suggestive of distal ureteral atresia. An interesting case in an adult is described: its embryogenesis and the diagnostic problems are discussed.

The activity of collagenase and certain lysosomal hydrolases (cathepsin B1, cathepsin D, beta-glucuronidase and beta-N-acetyl glucosaminidase) was studied in serum and tissues of rats with streptozotocin- or alloxan-induced diabetes. The activity of serum lysosomal enzymes was increased in both groups (p less than 0.05). Both streptozotocin- and alloxan-diabetic animals showed significantly higher dermal collagenase activity than those of controls (p less than 0.01), but the liver and spleen showed similar activities; there was a significant decrease in the renal collagenase activity of streptozotocin-diabetic rats (p less than 0.05). Comparison of the alloxan- or streptozotocin-treated groups with control animals showed an increase in lysosomal enzymes (cathepsin B1, cathepsin D, beta-glucuronidase and beta-N-acetyl glucosaminidase in skin, liver and spleen) (p less than 0.05) but beta-N-acetyl glucosaminidase was unchanged in the spleen of streptozotocin-diabetic rats. There was no difference in renal cathepsin B1 and D in control versus alloxan-diabetic rats, but there was an increase in beta-glucuronidase and beta-N-acetyl glucosaminidase (p less than 0.05). The streptozotocin-diabetic animals showed decreased activities of renal lysosomal enzymes (p less than 0.05), but similar activity of cathepsin D to the control animals.
In rats with adjuvant induced arthritis, the mechanical properties and thermal behavior of the skin and tendon were studied during the acute and chronic phase of the disease. Among the mechanical properties, the tensile strength and elastic modulus were found to decrease markedly both in the skin and tendon of arthritic rats while the extensibility did not show any significant change during the course of the disease. The thermal behavior, namely, shrinkage temperature, denaturation temperature, isometric tension and the temperature at isometric tension were decreased appreciably both in the skin and tendon of arthritic rats, the decrease being prominent during the chronic phase of arthritis. Both the mechanical properties and thermal behavior were correlated with biochemical parameters such as total collagen content and the ratio of the total activity of 14C-hydroxyproline of soluble collagens to insoluble collagen. The results support the concept of impaired collagen crosslinking in adjuvant induced arthritis. Prednisolone and L-thyroxine were administered to arthritic rats and the changes in the mechanical properties and thermal behavior of the skin and tendon of arthritic rats were studied. Both prednisolone and L-thyroxine increased the decreased values of tensile strength and elastic modulus of the skin and tendon in adjuvant induced arthritis. Similarly the decreased values in the thermal behavior of arthritic rats were found to be increased by treatment with prednisolone and L-thyroxine. Results indicated correlations with the beneficial action of prednisolone and L-thyroxine on the impaired collagen crosslinking in adjuvant induced arthritis.
