Symptomatic hyponatraemia due to inappropriate antidiuretic hormone secretion following minor surgery.

A rare case of the syndrome of inappropriate antidiuretic hormone secretion occurring after minor surgery is presented. A ten-year-old, previously healthy boy underwent general anaesthesia for detorsion and right orchiopexy. Throughout the operations, which lasted for one hour, he received 120 ml Ringer's lactate solution. The immediate postoperative period was uneventful. Twenty-two hours postoperatively he was found unconscious with generalized tonic-clonic seizures. Simultaneously obtained serum sodium concentration (121 mEq.L-1) serum osmolarity (265 mEq.L-1), urine sodium concentration (87 mEq.L-1) and urine osmolarity (525 mEq.L-1) suggested inappropriate antidiuretic hormone secretion which was confirmed by an elevated serum arginine-vasopressin (AVP) level of 14.5 pcg.ml-1 (normal 1-5 pcg.ml-1) measured by radioimmune assay. He was treated with a single iv dose of 30 mg furosemide and fluid restriction, which produced a gradual increase of his serum sodium concentration to normal within two days. He was well during the remainder of his hospitalization.

Safe epidural anesthesia performed during labor by an obstetrician.

One thousand seven hundred eighty-seven epidural anesthesias during labor were performed by obstetricians well trained in the technique by the anesthesiology department. The course of labor was accelerated compared to control group. Vacuum extraction rate was 11.9%. Fetal outcome was favorable. Incidence of major complications was 0.16%. We conclude that in countries where an anesthetist is not routinely available at delivery wards, a safe and efficient epidural anaesthesia could be performed by an obstetrician well trained in administration of epidural anesthesia.

Epidural anesthesia shortens duration of labor in singleton vertex presentation spontaneous delivery.

Influence of epidural anesthesia on the duration of labor was studied in 1,206 parturients having spontaneous singleton vaginal delivery. In primiparous women, the mean first stage was 3.52 hr in the epidural group, and 7.68 hr in the control group (p less than 0.001), the mean second stage was 25.78 min in the epidural group, and 43.58 min in the control group (p less than 0.001). In the multiparous women, the mean first stage was 2.06 hr in the epidural group, and 3.85 hr in the control group (p less than 0.001), the mean second stage was 17.06 min in the epidural group, and 25.42 min in the control group (p less than 0.0001). The conclusion is that epidural anesthesia shortens the duration of first and second stages of labor in singleton vaginal delivery.

Influence of epidural anaesthesia on the course of labour in patients with antepartum fetal death.

The course of labour in 22 patients with antepartum fetal death who received epidural anaesthesia was evaluated as compared to 22 controls matched for parity and gestational age, who received narcotic pain relief. Both groups had similar preinduction cervical dilatation and the induction was performed by amniotomy and oxytocin infusion. The mean first stage of labour was 5.4 hours in the epidural group, and 8.7 hours in the controls (p = 0.0192). The mean cervical dilatation rate was 3.3 cm/hour and 1.0 cm/hour respectively (p = 0.0142). The second stage was similar in both groups. We conclude, that parturients receiving epidural anaesthesia may benefit both emotionally and physically from excellent pain relief and a shorter delivery process when going through the distressing experience of delivering a dead fetus.

The ABO, Lewis or P blood group phenotypes are not associated with recurrent pelvic inflammatory disease.

An assumption that ABO, Lewis, or P blood group phenotypes are associated with recurrent pelvic inflammatory disease (PID) in a similar way as with recurrent urinary tract infection has been tried to establish. Of 20 patients with PID 9 (45%) had blood type A, 6 (30%) type B, 1 (5%) type AB and 4 (20%) type O; 14 (70%) had Le(a-b+), 5 (25%) had Le(a+b-), and 1 (5%) had Le(a-b-). Of the 20 controls 10 (50%) had blood type A, 3 (15%) type B, 1 (5%) type AB and 6 (30%) type O; 12 (60%) had Le(a-b+), 4 (20%) had Le(a+b-), and 4 (20%) had Le(a-b-). The difference in the proportions of the A, B, AB, and O phenotypes as well as the proportion of combined recessive and nonsecretor phenotype Le(a+/-b-) between patients with recurrent PID and controls were not statistically significant. The distribution was consistent with that in the general population. 2 of the patient group (10%) and 6 (30%) of the controls had positive blood type P1 (Fisher's exact probability = 0.0958). The distribution of P1 between the patients and controls was opposite to that in the general population. We could not demonstrate association of ABO, Lewis or P blood group phenotypes with recurrent PID.

Neuroendocrine responses to methylphenidate and d-amphetamine: applications to attention-deficit disorder.

The authors reviewed the studies of the neuroendocrine secretion of growth hormone, prolactin, and cortisol as they relate to attention-deficit hyperactivity disorder (ADHD). Studies to date have focused on the hormonal responses to the stimulants (amphetamines and methylphenidate) in children and adults. The authors discuss the methodologic limitations of current studies, the implications of this research for an understanding of the pathophysiology of ADHD, as well as the mechanisms of action of the stimulants and the side effect of stimulant-related growth suppression. They also suggest directions for future research.

Curettage after midtrimester termination of pregnancy. Is it necessary?

We conducted a prospective study to assess the need for curettage after spontaneous expulsion of the placenta in midtrimester termination of pregnancy. The rate of early complications (9.4%) was consistent with previously reported data. A higher incidence of intrauterine adhesions, detected on hysteroscopy, was observed in patients who underwent curettage (38.5% vs. 7.7%, P = .0727). In order to prevent intrauterine adhesions, curettage should be restricted to cases in which retained products of conception are suspected. It should not be performed routinely in every case of midtrimester termination of pregnancy after spontaneous expulsion of the placenta.

Placental abruption associated with air travel. A case report and an overview on safety of air travel in pregnant women.

A case of placental abruption during air travel is described. None of the known predisposing factors could be demonstrated. Although it may be a true coincidence, the connection between the abruption and the flight could not be totally excluded. Safety of air travel in pregnant women is discussed, and it may be summed-up that flying is not contraindicated in uncomplicated pregnancy.

Nifedipine in the treatment of severe preeclampsia.

We conducted a randomized clinical trial in which patients with severe preeclampsia between 26-36 weeks of gestation received either nifedipine (10-30 mg sublingually, then 40-120 mg/day orally; N = 24) or hydralazine (6.25-12.5 mg intravenously, then 80-120 mg/day orally; N = 25). Effective control of blood pressure was achieved with nifedipine in 95.8% of subjects and with hydralazine in 68%, a statistically significant difference (P less than .05). Maternal side effects were minor in both groups. Acute fetal distress developed in one nifedipine subject and in 11 treated with hydralazine. Mean prolongation of gestation was 15.5 +/- 10 days with nifedipine and 9.5 +/- 11 days with hydralazine, a difference that did not reach statistical significance (P less than .07). Infants born to women treated with nifedipine were delivered at more advanced gestational ages (34.6 +/- 2.3 versus 33.6 +/- 2.4 weeks; statistically not significant), weighed more (1826 +/- 456 versus 1580 +/- 499 g; statistically not significant), and tended to have fewer, mainly minor, complications. The average number of days spent in the neonatal intensive care unit was significantly lower in the nifedipine group (15.1 versus 32.7 days; P less than .005), leading to an average 31% reduction in total (maternal and neonatal) hospitalization-related charges for each nifedipine-treated pregnancy. We conclude that nifedipine is an effective, convenient, and low-cost treatment for patients with severe preeclampsia, and is not associated with undesirable side effects.

Unilateral congenital short femur: a case report.

A case of congenital short femur in an otherwise healthy infant is described. Antenatal sonographic diagnosis was made at 26 weeks of gestation. The left femur was shorter than the right (ratio 0.82-0.85). In this case, the occurrence of congenital short femur was sporadic. The diagnosis was confirmed after delivery. This case illustrates the importance of ultrasound as an early detector of certain congenital anomalies and as a useful tool in their follow-up.

The use of calcium channel blockers in obstetrics and gynecology; a review.

Since their introduction, calcium channel blockers gained wide use in the treatment in various fields of medicine. Calcium channel blockers were shown to inhibit contractions of the pregnant and nonpregnant uterus, and lately their properties have made it, theoretically, an ideal candidate for the use in severe pregnancy-induced hypertension: it is effective in lowering blood pressure, has a fast and long-standing action, may be conveniently administered via the oral route, and it is not associated with serious side effects.
