[Transcript expression of the CPL 5x, BMD-3A, BMD-10 Interrogans leptospira].

Total RNA of Leptospira interrogans SV Lai strain 017 was prepared by the method of Licl-Urea, and was used in dot hybridization with biotin-labelled DNA probes. The probes included BMD-3A, BMD-10, which were the leptospirial protective antigen genes, and CPL 5x, which was the genus specific gene of interrogans Leptospira. All of the three probes have shown various degrees of hybridization signs, proving that they all have transcript expression in leptospira. The transcript expression is the main way of regulating the gene expression in procaryotae, and it is significant in molecular genetics of Leptospira. The results indicate that the antigens encoded by the BMD-3A, BMD-10 may play an important role in immune reaction against leptospiral infection and provide a clue to the development of gene-engineering vaccine. The results also suggest that the antibody against the antigen encoded by CPL 5x is a useful tool in the classification of Leptospira interrogans.

Patella and patellofemoral resurfacing (37 cases report).

Chondromalacia and patellofemoral osteoarthritis are common diseases that cause pain and disablement of the knee. Conservative therapy is not always effective. Since 1983 we have used isolated polyethylene patellar prosthesis for patella resurfacing of 19 patients with patellofemoral osteoarthritis. After an average of 44.9 months follow-up, we found the treatment was not as satisfactory as earlier trials. The main reason is the wide erosion of femoral condyle caused by the polyethylene patella. To overcome this shortness, we designed a new type of patellofemoral prosthesis which is named Y-L-Q. From January, 1991 to November, 1993, we used this prosthesis to treat 16 knees of 13 patients with patellofemoral osteoarthritis. Most of those patients improved both symptomatically and functionally. The good to excellent results rate was 87.5% (14/16 knees) at the time of an average 16 months follow-up. The early results of our experience with patellofemoral resurfacing are encouraging, and extended follow-ups are in progress.

Corneal topographic analysis in patients with cataract.

We studied the corneal curvature of 161 eyes in 101 patients, aged from 50 to 80 years (mean: 63 years), with senile cataract using the Topographic Modeling System (TMS) preoperatively. The results revealed that mean surface asymmetry index (SAI) was 0.36 and mean surface regularity index (SRI) 0.79. Simulated keratometry reading (Sim K) was 44.46 D, and 43.56 D. Minimum K reading was 42.60 D. The same patterns of corneal topography in both eyes were 50% in 60 patients who had binocular examination. Astigmatism less than 1 D was 23% in 161 corneas. Corneal patterns included round (21.2%), oval 16.1%), symmetric and asymmetric bow-tie (22.2%), and irregular (40.4%). SRI and SAI of the irregular pattern were statistically higher than those of the round, oval and bow-tie patterns in our patients (P < 0.01 and P < 0.05). The results of our study determined that the cornea of eye with senile cataract preoperatively is not a perfect spherocylinder. It is asymmetrically aspheric, with less curvature peripherally than centrally.

[Study on soluble proteins in human fetal lens].

Soluble proteins from corter of human fetal clear lenses were separated by column chromatography with Sepharyl S-300 (superfine) and determined by disc-PAGE and SDS-PAGE. The results documented from 18W, 21W (2 cases) and 24W fetal lenses suggested that a negative correlation tended to be presented between the age of fetus and fraction of alpha, beta 1 crystallins (r = -0.9542; r = -0.9674) and a positive correlation between the age of fetus and fraction of beta 2, gamma crystallins (r = 0.9666; r = 0.9970). There was no change in the alpha, beta 1, beta 2 crystallins on disc-PAGE. Among the fetuses of different ages, the blurred band of gamma crystallin seen in 18W and 21W fetal lenses was thickened in the 24W fetal lens. The SDS-PAGE showed that there was no significant difference among the polypeptides in each of alpha, beta 1, beta 2, gamma crystallins in the studied fetal lenses of different age groups. It is presumed that the normal lens is proportionally composed of four fractions of crystallin. This is hypothetically contributed to the maintenance of normal structure of the lens, why the proper proportion of four fractions of crystallins is depended either on proteins synthesis or on protein molecular assembly is not yet clear.

[Hair cell regeneration and recovery following gentamicin induced damage in the chick cochlea].

Two-day old chicks were given gentamicin (GM) and kanamycin (KM) for 10 days and then the cochleas were processed for both scanning electron microscopy and light microscopy on the same day and in 1, 2, 3, and 4 weeks following the last injection to examine hair cells regeneration and recovery. The result showed that hair cell loss was up to 40% in the base of the basilar papilla following GM and KM administration. One week after injection, however, most of the damaged region was replaced with regenerated hair cells. Three to four weeks later, the regenerated hair cells were developed into normal sensory hair cells. This experiment demonstrated that the hair cells in the chick cochlea were able to regenerate and recover following GM and KM induced damage.

[The use of internal fixation in the treatment of spinal tuberculosis].

We have applied debridement with grafting and internal fixation techniques (including Luque and Dick procedures) in the treatment of tuberculosis of the spine in the past few years. Analysis of 18 patients showed good prognosis after the Luque or Dick procedure. The patients are able to walk around within a few days after operation. One to five years of follow-up showed no recurrence or kyphotic deformity.

[The prevention and treatment of complications with pedicle screws].

Pedicle screws are more commonly used by spinal surgeons recently. Although Roy-Camile, Dick, Steffee, R-F etc., are all different surgical procedures, all need pedicle screw techniques. Up to March 1991, 61 cases with 334 screws inserted by Steffee and Dick operations had been performed in our hospital. Neurological symptoms, among all complications, are the most serious problems associated with these procedures. By analyzing the insertion point, transverse screw angle, sagittal angle and other causes of complications of pedicle screws, we present here our experiences for prevention and treatment of these complications.

[Effects of Tripterygiitotorum in the treatment of insulin dependent diabetes mellitus with islet transplantation].

UNLABELLED: The therapeutical efficacy of islet transplantation in treating insulin-dependent diabetes mellitus (IDDM) patients is marked for a short time and the long-term efficacy is unsatisfactory. So 30 IDDM patients given tripterygiitotorum (T II) were compared with 24 IDDM patients without using any immunosuppressive agents after islet transplantation. RESULTS: prior to transplantation both the numbers of T lymphocyte subpopulations such as CD2, CD4, CD8 and the concentrations of C-peptide in all IDDM patients were lowered; after transplantation the numbers of CD2, CD4 were significantly elevated (P < 0.01), the ratio of CD4/CD8 in control group was higher than that in TII group (P < 0.01), while the concentration of C-peptide were greatly increased (normal: 2.24 +/- 0.34; before transplantation: 0.21 +/- 0.01; 15 days after transplantation: 3.24 +/- 1.2 ng/ml). The peak value of C-peptide in TII group began decreasing half a year after transplantation and it gradually dropped to the baseline level. The dose of insulin all were significantly reduced, and 3 patients stopped altogether. Half a year after transplantation TII group remained stable for the requirement of insulin, whereas the control group gradually increased the dose of insulin. (1) Islet transplantation could adjust the immunological disorder in IDDM patients, increase the numbers of T lymphocyte subset such as CD2, CD4, CD8 while the chronic immuno-rejective response occurred. (2) T II inhibited both the numbers and function of T lymphocyte subpopulation and normalized the ratio of CD4/CD8. (3) TII prolonged the survival time of grafts in IDDM patients and suppressed immunological rejection.(ABSTRACT TRUNCATED AT 250 WORDS)

The biological study of the cultured human lens epithelial cells in vitro.

The human lens epithelial cells (HLE) cultured in vitro was established in normal and cataractous lenses. The biological feature, histological characteristics and the ultrastructure of the cultured HLE cells were investigated. The results reveal that the proliferative capacity of the cultured HLE cells is reversely proportional to the donor age; the cultured HLE cells has the limited proliferative capacity in vitro. The relieve of the contact inhibition is the effective trigger of the HLE cell proliferation. This research work is possible to make a basis for the further study of the regeneration of the lens and the roles of the lens epithelial cells in the development of the cataract.

The comprehensive control of astigmatism during and following intraocular lens implantation.

The operating corneoloscope and Terry operative keratometer were used respectively in 29 and 34 eyes during the intraocular lens implantation to measure the corneal astigmatism qualitatively or quantitatively, so that the tension of incision closure could be adjusted. The surgically induced astigmatism in qualitative group two weeks after the operation was 3.5 +/- 1.70 D and that in quantitative group was 2.56 +/- 1.60 D. There were 55.17% and 38.24% of the eyes with over 2.00 D corneal astigmatism in qualitative and quantitative group two months after the surgery. The astigmatism of both groups at the early stage after the operation was significantly lower than that of the control group (p < 0.05). Argon laser, Nd:YAG laser or razor-blade were used to cut 1 to 3 limbal sutures in 64 eyes with over with-the-rule astigmatism 2.25 D at the early stage (2 months) after the operation. One hour after suture cutting, the with-the-rule corneal astigmatism reduced significantly with an average of 2.61 D. The astigmatism continued to reduce and stabilized one month after the suture cutting. The arcuate keratotomy was performed in 21 eyes, of which the postoperative astigmatism was over 2.25 D (with the average of 3.34 D) more than 6 months after IOL implantation. The mean astigmatism was 0.82 and 1.18 D one day and 6 months after the keratotomy respectively. The uncorrected visual acuity improved significantly after the keratotomy. These results indicate that applying corneoloscope or Terry keratometer to adjust the tension of incision closure during operations, selected suture cutting at the early postoperative stage, and performing arcuate keratotomy at the late postoperative stage be the effective methods to reduce surgically induced corneal astigmatism.

PVEP in patients with different types of congenital cataract.

The preoperative and postoperative PVEP in 79 eyes of 45 patients with three types of congenital namely lamellar cataract, nuclear cataract and total cataract was studied. The results showed that visual acuity of patients who could be cooperatively examined was improved significantly after operation. The examination of PVEP revealed that more abnormal waveforms were found in total cataract group before operation. P1 latencies of low and medium spatial frequencies (140', 70', and 35') became much longer in total cataract group than those in lamellar and nuclear cataract groups. The abnormal changes of P1 latency of high spatial frequency (17.5') showed similar in three groups. It is suggested that time for surgery should be based on visual function in the different types of congenital cataract. The total congenital cataract and unilateral cataract should be extracted as early as possible.

The clinical analysis of secondary glaucoma following posterior chamber intraocular lens implantation.

We present 14 patients with secondary glaucoma following the implantation of a posterior chamber intraocular lens (IOLs). All patients were unilateral glaucoma, which developed within 1 month following the IOL implantation in 10 cases, and from 1 to 3 years in 4 cases. The angle of anterior chamber was open in 8 patients, and close in 6 ones. Seven patients required treatment of antiglaucomatous medicine; 4 patients underwent laser iridectomy and 3 patients required antiglaucoma surgery. The results showed that the corrected visual acuity was more than 0.5 in 28% of the patients before antiglaucoma treatment and in 50% of the patients after treatment. The main cause of poor vision is glaucomatous optic nerve atrophy. It suggests that it is important to find glaucoma early.

[Comparison of corneal topography and keratometer in patients with cataract preoperatively and postoperatively].

The comparison of corneal topography with keratometer in 200 patients (278 eyes) was made. The results revealed that Sim K values of corneal topography were 44.22D and 44.26D, mean diopters of keratometer were 44.16D and 44.23D in 150 eyes of 116 patients with cataract preoperatively. There was no significant difference between Sim K values of topography and diopters of keratometer (P > 0.05). The Sim K values of the topography were 46.37D and 43.06D and diopters of keratometer were 46.09D and 42.88D in 90 eyes of 67 patients who received cataract extraction with IOL implantation or second IOL implantation. No significance was found in results between Sim K values and diopters of keratometer in patients postoperatively (P > 0.05). Our investigation demonstrated that Sim K values of corneal topography were in agreements with diopters of keratometer in patients with cataracts preoperatively or postoperatively. However, corneal topography has great values in diagnosis of corneal diseases, traumatic astigmatism, severe non-rule stigmatisms, in which keratometer is often useless.

[The study of the dangerous factors of the simple heart disease in children and its action mode].

The simple congenital heart disease is a multiple genetic disease. Its environmental factors are still unclear. The mode of action between the causes and congenital heart disease hasn't reported yet. This study used 1:1 case-control study and multiple factors were investigated in 168 pairs of parent before conception and the period of conception. First, using single factor analysis, the 19 suspicious risk factors were obtained from 55 factors(alpha = 0.10). Second, using conditional logistic regression selected 8 risk factors from 19 factors(alpha = 0.05). Finally, analysis of 8 risk factors were carried out by path analysis to explore the mode of action between the causes and congenital heart disease.

Application of internal fixation in the treatment for tuberculosis of the spine.

This is a study of 18 patients who had surgical treatment for spinal tuberculosis. All patients were admitted with the spinal tuberculosis and treated with a combination of anterior debridement and bone graft with posterior internal fixation using the Luque or Dick technique. The patients were able to walk within a few days postoperation. Three to seven years follow-ups show no recurrence, kyphosis and/or solid fusion.

[Long-term follow-up of patients with patella resurfacing].

Patellar resurfacing was performed by the authors. From 1983 to 1988, 19 cases (22 knees) with patello-femoral osteoarthritis were treated with this procedure in our hospital. Excellent and good results were recorded in 13 cases (59.1%) one year after operation, but were zero in the long-term follow-up (average 8.7 years) because of recurrent knee pain due to the wearing of the cortex of the femoral groove. Further operation was necessary for those cases, such as patellectomy or total knee replacement. Since 1991 Y-L-Q's type of patellofemoral replacement has been used in 11 cases in our department to alter the isolated resurfacing of the patella. The femoral groove was replaced with a metallic implant that allows sliding of a polyethylene patella to prevent the wearing of the femoral cortex. Preliminary results are good.
