RUGs and equity of access to nursing home care.

Case-adjusted prospective reimbursement systems, such as resource utilization groups (RUGs), may promote nursing home cost containment, but they may do no better than existing systems at encouraging homes to admit the more dependent and thus costlier Medicaid patients, if the homes' reluctance to admit these patients is due to a shortage of nursing home beds. Using 1983 data on Wisconsin nursing homes, this paper presents evidence that suggests that the presence of excess demand, rather than low reimbursement rates, causes nursing homes to exclude the more costly Medicaid patients. Consequently, to benefit fully from RUGs, it is necessary either to eliminate excess demand (by increasing supply or decreasing demand) or to decide which patients have priority for admission and assign reimbursement payments to these patients that significantly favor these patients. Health care of poverty and nonpoverty children in Iowa.

Responses to a survey of 637 Iowa children under age six years from Aid to Families with Dependent Children (AFDC) households was conducted through telephone interviews with parents and compared to a second sample of children in 760 randomly sampled households. Utilization of health services for children in the AFDC sample was similar to that of nonpoverty children and superior to that of other poverty children. Irrespective of Medicaid coverage, poverty children's health care differed from that of nonpoverty children in the following ways: less well care from a private practice or pediatrician, seeing the same health professional at each well visit less often, receiving immunizations at the same place as source of well care, examination by a physician at the most recent well visit, sick care at the same place as well care.

Synchronous testicular tumors in identical twins.

Testicular tumors are found in approximately 2 per 100,000 population. An extremely rare occurrence of synchronous testicular tumor in identical twins is presented. A complete review of the world literature with regard to testes tumors in brothers and twins is also given.

The long-term care marketplace: an analysis of deficiencies and potential reform by means of incentive reimbursement.

This study analyzes the institutional, economic, and regulatory deficiencies of the long-term care sector of the health industry and develops an incentive reimbursement model as a means for correcting or ameliorating some of the stated deficiencies. The reimbursement model presents a schema which attempts to match patients to facilities, needs to services, and services to cost. A capital expansion coupon program is proposed, both as a suitable vehicle for channeling incentive rewards accruing to nonprofit institutions, and as a mechanism for subsidizing the approved expansion and/or renovation plans of "efficient" providers of care.
