Thin basement membrane nephropathy as a cause of recurrent haematuria in childhood.

A survey of 69 children presenting with recurrent or persistent haematuria and submitted to percutaneous renal biopsy at this hospital over a 17-year period, was performed to establish the incidence of thin basement membrane nephropathy (TBMN). A diagnosis of primary glomerular disease was established in 44 (IgA nephropathy in 16, Alport's syndrome in 13 and other varieties of glomerulonephritis in 15). Of the remaining 25 patients in whom light microscopical and immunochemical examination revealed no abnormalities, material for electron microscopy was available in 11. In eight of these (five of whom had a family history), TBMN was diagnosed on the basis of ultrastructural morphometric evaluation of glomerular basement membrane thickness. Assuming a similar proportion of the remaining 14 patients with renal biopsy specimens normal by light microscopy had TBMN, the probable frequency of this abnormality in the whole series would be 26%, very similar to that of IgA nephropathy. In the eight TBMN patients the mean glomerular basement membrane thickness ranged between 181 and 236 nm, whilst in 'control' biopsies from children with 'minimal change' nephrotic syndrome or IgA nephropathy, the mean thickness ranged between 242 and 333 nm.

[Electron microscopic observations on Pseudomonas cocovenenans].

A strain of food-poisoning bacterium has been isolated by Jin Jiexiang (1963) in China from the fermented cornflour that has gone bad. These pathogenic microorganism has been identified and named Pseudomonas by Zhao Naixin in 1988, which is the same species as P. cocovenenans. The characteristics of them were conformed to these of the species P. cepacia of section 2 of the genus Pseudomonas. In view of the fact that the fine structures of the above mentioned three strains of Pseudomonas have not been described yet, we decided to observe them with electron microscope. Results indicate there are many things in common among the three strains, such as: appearing short rods, 0.6-0.8 microns in diameter by 1.5-2.0 microns in length, one polar multiflagella; non-pili, non-capsules, non-endospores; containing intranuclear inclusions (electron-dense bodies or concentric laminae bodies), accumulating intracytoplasmic PHB granules; forming filaments, minicells and bizarrecells; producing extracellular cellulose-like materials by the three strains have not been reported previously.

The investigation of antiepileptic action of qingyangshen (QYS)--influences of QYS on the development of rat brain and memory behavior.

As an anticonvulsant, the influences of Qingyangshen (QYS) on the development of brain and memory behavior were observed in four-week-old male rats. The dosage of QYS was 100 mg/kg/QOD and the period of administration was one month. The results showed that QYS had less influence on the development of body weight and brain wet weight in young-aged rats than diphenylhydantoin (DPH). However, QYS inhibited the establishment of passive avoidance conditional reflex and had influence on memory behavior in young-aged rats.

Infant proneness-to-distress temperament, maternal personality, and mother-infant attachment: associations and goodness of fit.

66 mother-infant pairs were examined when the infants were 9 and 13 months. The purpose of this report was to examine relations between infant proneness-to-distress temperament, maternal personality characteristics, and mother-infant attachment. There were no main-effect relations between infant proneness-to-distress temperament as assessed at 9 months and infant attachment classification at 13 months. This was true whether security of attachment (A and C vs. B) or proposed temperament (A1-B2 vs. B3-C2) groupings of attachment classifications were examined. Infant proneness-to-distress temperament, however, was associated with maternal behavior and personality. Furthermore, security of attachment could be predicted by an interaction between maternal personality and infant proneness-to-distress. The importance of considering goodness-of-fit relations in predicting attachment security is discussed.

Difficult tracheal extubation.

We describe a case of nasotracheal tube fixation with a screw. A second case is described in which a broken drill bit was found to impinge on the wall but not penetrate into the lumen of a nasotracheal tube. Possible sequelae of this complication include airway leak, aspiration, tube obstruction, and trauma from attempts at forceful extubation. We recommend the routine intraoperative testing for tracheal tube movement and routine fibreoptic bronchoscopy through the tube when blind surgical procedures occur in the vicinity of a tracheal tube.

Changes within the "normal" cerebral white matter of multiple sclerosis patients during acute attacks and during high-dose cortisone therapy assessed by means of quantitative MRI.

Changes in the apparently unaffected cerebral white matter of multiple sclerosis (MS) patients were studied during acute attacks as well as during high-dose prednisolone therapy. Serial MR scans of patients with a clinically definite diagnosis were performed on four defined occasions: before an episode, within three days after its onset, after 10 days of therapy as well as four weeks later. Thirteen patients agreed to cooperate in forming a MRI data base and to be rescanned immediately after the onset of an acute relapse. Within one year, six patients had such episodes, one of them had a second bout. Both T1 and T2 relaxation times within the apparently normal white matter were significantly prolonged in all cerebral lobes compared to a control group of healthy volunteers. During the acute attacks as well as during therapy the T1 values remained as before. The T2 values were elevated only in two out of six cases during the episode. After therapy a considerable clinical improvement was seen in all cases, but a significant T2 decrease as a possible effect of cortisone was noted in only one case. We conclude that the prolonged relaxation times T1 and T2 within the apparently normal cerebral white matter of MS patients are the result of a number of molecular events differing considerably among individual patients and that serial measurements of these relaxation times do not consistently change during an acute relapse and do not reflect clinical improvement after high dose prednisolone therapy.

Two steps forward--and one back?

In Croatia, as in Yugoslavia as a whole, the immense economic progress achieved since the Second World War has been accompanied by a marked decline in the prevalence of infectious and parasitic diseases. On the other hand there has been an upsurge in chronic diseases, attributable to changes in life-styles. Third World countries now undergoing rapid development should see this as a timely warning.

[Involvement of apparently normal white brain substance in the disease process of multiple sclerosis].

In a group of patients with a clinically definite diagnosis of multiple sclerosis (MS) the cerebral white matter was investigated by means of quantitative nuclear magnetic resonance imaging (MRI). Measurements were taken from plaque-free regions and compared to a control group of healthy volunteers. A significant prolongation of T1 and T2 values was seen for all regions investigated (frontobasal, temporal, temporoparietal, and upper parietal white matter). In a comparison of MS patients with a short duration of the disease (mean: 1.7 years) with MS patients with a long-standing course of the disease (mean: 15.2 years) a significant prolongation of T2 relaxation times was found in the upper frontal and upper parietal white matter only for the latter group. In keeping with neuropathological findings the noted prolongation of T2 values might most probably be due to an increase of extracellular water secondary to astroglial proliferation. The increase of relaxation times could either be due to degeneration of commissural fibers or be the result of an exhausted capacity for remyelination. Both events are known to occur predominantly in the later stage of the disease. It is concluded that MS represents a disease which is not restricted to limited areas of demyelination but rather involves the entire cerebral white matter. These findings imply possibilities for the assessment of disease progression as well as for therapy.

The quality of life of elderly people on the island of Iz.

The results obtained by polling 105 elderly people, aged 75 to 95 years, on the Island of Iz are presented. Several groups of questions were asked to obtain information on the demographic characteristics, economic and social status, functional and physiologic disorders, health care needs, utilization of medical services and subjective evaluation of the quality of life of the persons polled. Compared to other studies of the quality of life of the elderly, our results point to some specific characteristics of this group of people, such as functional and physiological disorders and health risks. The results have shown that health risks and their frequency depend on the life style, self-help and mutual help. Despite the high criteria set for the evaluation of individual factors determining the quality of life, the population studied was found to have a high quality of life thanks to the operation of mechanisms which improve the quality of life.

[Isolation and identification of an amoeba of the genus Naegleria].

One pathogenic free-living amoeba, strain W042302 (Fig. 1.), was first isolated from the cooling water of a power plant in Shanghai, in April, 1986. It could result in the deaths of mice, and recovered from the brain tissue of the dead mice. This pathogenic free living amoeba could be induced into trophozoite with two flagellae (Fig. 2.). Protein patterns obtained by PAGE showed twelve bands stained by Coomassie brilliant blue R250. The isoenzyme patterns of both acid phosphatase and alkaline phosphatase of the amoebic extract exhibit only a single band, while lactate dehydrogenase is devoid of noticeable activity. From the results obtained in the present study, it is inferred that strain W042302 may be belong to Naegleria australiensis.

Infective endocarditis. Current recommendations for prophylaxis.

Antibiotic prophylaxis is indicated for any patient with a predisposing cardiac lesion who undergoes a procedure likely to produce bacteraemia with an organism having the propensity to cause bacterial endocarditis. Cardiac abnormalities have been ranked according to their approximate risk and it is known that the organisms most likely to cause endocarditis are viridans streptococci, Group D streptococci and staphylococci. The procedures likely to induce bacteraemia with each of these are, respectively, dental and upper respiratory with bleeding, urinary and gastrointestinal, and cardiac valve surgery. Antibiotic prophylaxis is impractical when bacteraemia cannot be anticipated and is unnecessary when it is due to organisms such as anaerobes and Gram-negative bacilli which rarely colonise the endocardium. A variety of prophylactic antibiotic regimens, directed against the common aetiological organisms, have been evaluated in animal models of infective endocarditis and it is on the basis of this kind of indirect evidence that several expert committees have made and regularly update their recommendations. Because infective endocarditis is an uncommon disease, a controlled clinical trial to prove the efficacy of prophylaxis would require the enrolment of a prohibitive number of patients. Consequently, there is room for differences of opinion over what constitutes optimum prophylaxis in any particular situation. This review examines the rationale for prophylaxis and compares and contrasts several authoritative recommendations, among which the trend in recent years has been towards simpler oral regimens.
