Efficacy of co-trimoxazole in Donovanosis. A preliminary report.

Co-trimoxazole (Septrin, Wellcome) was prescribed in a dose of two tablets twice daily for the treatment of 10 patients with Donovanosis. All the patients responded well to this treatment, and the ulcers healed completely within 10 days in eight patients and within 14 days in the remaining two. Ten days' treatment with 40 tablets of co-trimoxazole is suggested as sufficient to treat Donovanosis. No adverse reactions were noted in any patient.

Urinary bladder smooth-muscle electrical activity: response to atropine and bethanechol.

Fourteen patients with spinal cord injuries and lower motor neuron type neurogenic bladders and 12 patients with upper motor neuron type neurogenic bladders and spinal cord injuries, were evaluated before and after bethanechol or atropine, respectively. They were compared with 5 patients with benign prostatic hypertrophy or bladder tumors but without neurologic disease. Both drugs were found to modify partially smooth-muscle response. The implications of these results are consistent with the production of electrical activity by the smooth-muscle cells of the bladder wall.

A strategy to detect beta-thalassaemia minor.

Over the past three years 25 302 adults in Kentucky have been tested for haemoglobinopathies, and of these, haemoglobin A2 was measured on 3734, 1973 with microcytosis and 1761 within the normal range. The best methods of detecting beta-thalassaemia minor using red-blood-cell indices were compared. No method detected all heterozygotes. A new method was devised consisting of three parts: (1) haemoglobin electrophoresis, (2) calculation of the product of the square of the mean corpuscular volume (M.C.V.) multiplied by the mean corpuscular haemoglobin (M.C.H.) measured in units of one hundred, (3) A2 determination on all AA samples with (M.C.V.)2 X M.C.H. less than 1530 and on those with variant genotypes consistent with thalassaemia. In this series this new method detected 137 out of 138 heterozygotes with 4-4% false-positives.

Effect of phentolamine on perineal muscle EMG activity in paraplegia.

The effect of the alpha-adrenergic blocking agent, phentolamine, was studied on the perineal muscle EMG in 3 paraplegic patients. The drug reduced the EMG activity in these patients. This suggests that the normal suppression of external sphincter activity during micturition might be related to a reduction in the external sphincter activity supplied by the sympathetic system.

Dopaminergic function in two patients with catalepsy.

Apomorphine HC1 (2 mg subcutaneously), a dopamine receptor agonist, was administered to two schizophrenic patients with catalepsy. In one of these patients the clinical response to apomorphine was compared with that of sodium amytal and the growth hormone response apomorphine (0.75 mg subcutaneously) was compared with that of 25 control subjects. Apomorphine had no effect whereas sodium amytal caused rapid disappearance of catatonic symptoms including catalepsy. The peak growth hormone response to apomorphine was similar to that of controls. These data suggest that unlike experimental catalepsy in animals, catalepsy associated with schizophrenia may not be dependent on impaired dopaminergic function. Further case studies as well as the use of other dopamine receptor agonists are required before definite conclusions can be drawn.

Effect of monoamine receptor agonists and antagonists on cyclic AMP accumulation in human cerebral cortex slices.

In human cerebral cortex slices noradrenaline, isoproterenol (a beta-adrenergic agonist), dopamine, apomorphine (a dopaminergic agonist), and serotonin stimulated cyclic AMP formation: noradrenaline greater than or equal to isoproterenol greater than dopamine = apomorphine = serotonin. Clonidine (and alpha-adrenergic agonist) was ineffective in stimulating cyclic AMP formation in temporal cortex slices. The stimulatory effect of noradrenaline and isoproterenol was blocked by propranolol (a beta-adrenergic blocker) but not by phentolamine (an alpha-adrenergic blocker). Pimozide (a selective dopaminergic antagonist) inhibited the increase of cyclic AMP formation induced by dopamine or apomorphine but not that induced by noradrenaline, isoproterenol, or serotonin. Neither propranolol or phentolamine had any effect on dopamine- or serotonin-stimulated cyclic AMP formation. Chlorpromazine blocked the increase of cyclic AMP formation induced by noradrenaline, dopamine or serotonin, while cyproheptadine, a putative central serotonergic antagonist, was ineffective. These observations suggest that there may be at least two monoamine-sensitive adenylate cyclases in human cerebral cortex which have the characteristics of a beta-adrenergic and a dopaminergic receptor, respectively, and also possibly a serotonergic receptor.

Effect of methysergide and pimozide on apomorphine-induced growth hormone secretion in men.

The dopamine receptor agonist, apomorphine HCl (0.75 mg sc) increased serum growth hormone (GH) concentrations in each of 6 subjects. This increase was not inhibited by pretreatment with the serotonin antagonist, methysergide, but was completely abolished in the 4 individuals pretreated with pimozide, a selective dopamine receptor blocking agent. These results provide further evidence that apomorphine-induced GH secretion is mediated by a dopaminergic mechanism and is independent of a serotonergic mechanism.
