Psychiatry in Saudi Arabia.

The author draws on personal experience and a review of the literature to summarize the current state of psychiatric practice and education in Saudi Arabia and its relationship to other medical specialties. Although psychiatry is not accorded the status that it has in the West and there is a drastic undersupply of psychiatrists, the specialist practicing in Saudi Arabia has a unique opportunity to integrate ancient beliefs, customs, and laws with modern knowledge and technology in a county where the emphasis is on a stable transition from being governed almost entirely by religious precepts to being a new world power.

Undergraduate education and recruitment into psychiatry.

From 1969 to 1979 senior medical students from the University of Colorado School of Medicine entered psychiatric residencies at an average rate of 3.8%. The university gradually instituted a series of changes in its undergraduate psychiatric education program for medical students, and the senior class of 1980 was the first to be exposed to all of the changes. In 1980-1982 7.7% of the seniors went into psychiatric residencies. The authors describe the curriculum changes and speculate that more attention to the quality of medical students' psychiatric education at the undergraduate level enhances recruitment into psychiatric residencies.

Intracellular calcium ions in affective disorders: a review and an hypothesis.

Recent advances in knowledge about calcium's role as an intracellular regulator promise to broaden our understanding of possible pathophysiologic mechanisms in affective disorders. In this paper, we review the influence of calcium on mood, the intracellular dynamics of the calcium ion, and the possible interaction of lithium with these actions. We then discuss similarities with lithium of calcium antagonists, a class of drugs that influences calcium's action within cells, and report preliminary findings of their possible usefulness in treating mania. Finally, we hypothesize ways in which alterations in intracellular calcium ion concentration may be associated with mania and depression, and suggest methods of studying this hypothesis.

Long-term lithium carbonate therapy causes hyperparathyroidism.

Twelve patients taking lithium carbonate for two to 13 years were compared with age- and sex-matched controls to determine whether long-term lithium carbonate therapy is associated with alterations in calcium metabolism. As a group and individually, patients had significantly higher levels of serum total calcium, ionized calcium, and parathyroid hormone. When compared with serum calcium levels, patients' parathyroid hormone levels were significantly more likely than those of controls to indicate hyperparathyroidism. Lithium carbonate-induced mild hyperparathyroidism appears to be more common than had previously been suspected.

Assessment of psychiatric emergencies in medical practice.

Acute psychiatric emergencies, often encountered in medical practice, demand rapid evaluation and treatment; routine measures are not enough. A scheme for acutely assessing such crises is outlined, and some of the more common emergencies are discussed.
