A new model of brainstem ischemia in dogs.

A new model of brainstem ischemia in dogs is described. The perforating arteries arising from the posterior cerebral arteries as far as the bilateral junctions with the posterior communicating arteries were occluded using a subtemporal approach to the region of the interpeduncular cistern. Infarction of the posterior thalamus, subthalamus, midbrain, and upper pons was consistently produced. The dogs survived for greater than 1 week while exhibiting such clinical symptoms as disturbance of consciousness, tetraparesis, oculomotor paralysis, respiratory abnormalities, bradycardia, and arrhythmia. The clinical features and extent of infarction observed in this model closely resemble those of rostral brainstem infarction in human stroke. The model may be useful in studying the pathophysiology of brainstem ischemia.

The basic study on kappa-lambda imaging by delta-curve for the detection of a monoclonal B-cell population in the peripheral blood.

Recently, kappa-lambda analysis with the "D" value was developed by Ault to detect a minor population of malignant B cells in peripheral blood. This analysis is based on the Kolmogorov-Smirnov test, and the D value is calculated by a flowcytometer and a computer. We have recently devised a more sensitive parameter for the kappa-lambda analysis than the D value called the delta-curve (delta c); the delta c applies the same principle as that of the D value. Mixing experiments with kappa-type and lambda-type chronic lymphocytic leukemia cells revealed that the delta c could not only detect a minor population of malignant kappa-B cells, but also that of malignant lambda-B cells using more sensitivity than the D value. A total of 49 blood samples obtained from 27 patients with various B-cell malignancies were investigated. D values were abnormal in 37% of all samples, while abnormal patterns of the delta c were recognized in 71%. On the other hand, 59% of samples obtained from the patients with B-cell lymphoma in aleukemic phase showed abnormal delta c, whereas D values exceeded the upper limit of the normal value in only 15% of the samples. It was suggested that the delta c could detect 3% to 7% of malignant B cells that were mixed with a population of normal lymphocytes.

[Experimental study of brain stem infarction in dogs--effect on BAEP, SSEP, blink reflex and EEG of perforator occlusion].

Assessment of the lesion in the brain stem by evoked potentials has not been well established. We have already developed a model of brain stem ischemia by occluding the perforators of the posterior cerebral arteries of the dog. The ischemic lesions locates mainly in the ventral side of the midbrain. Using this model, we assessed brain stem function by brain stem auditory evoked potential (BAEP), surface- and depth-recorded (in medial lemniscus) short latency somatosensory evoked potential (SSEP), blink reflex (BR) and electroencephalography (EEG), and investigated the correlation between the electrophysiological abnormalities and the lesion in the brain stem. The studies were performed for 6 hours after perforator occlusion. Furthermore, depth-recorded SSEP and regional cerebral blood flow (rCBF) were measured under induced hypotension by withdrawal of arterial blood. BAEP did not change in 13 of 16 animals. Surface-recorded SSEP remained unchanged in all 6 animals. The results are probably due to the fact that the lesion does not involve the auditory and somatosensory pathways and the accompanying events such as edema does not affect the both pathways. Depth-recorded SSEP remained unchanged after occlusion and did not disappear even when rCBF fell below 10 ml/100 g/min. It may be suggested that the threshold for electrical failure in the brain stem is much lower than that in the cortex. In BR, R1 did not change but ipsilateral R2 became nearly invisible immediately after perforator occlusion in all animals. The fact that the ischemic lesion did not involve the pons and disturbed reticular formation in the midbrain may probably account for the remaining of R1 and the disappearance of ipsilateral R2.(ABSTRACT TRUNCATED AT 250 WORDS)

[Malignant lymphoma of the ileum].

A 53-year-old man with malignant lymphoma of the ileum is reported. He was admitted to our hospital because of lower abdominal discomfort. 67Ga citrate radionuclide imaging revealed an abnormal round uptake in his lower abdomen. CT scans showed an abnormal soft tissue density mass and a remarkably dilated small intestine in the pelvis. X-ray studies of the small intestine revealed a giant diverticulum-like lesion. Angiograms showed a hypovascular tumor with irregularity of the small vessels. A resected specimen revealed non-Hodgkin's malignant lymphoma, diffuse large-cell type. 67Ga citrate radionuclide imaging was useful in screening for neoplastic lesions in the small intestine because of its easiness and safety.

[Tissue penetration of aztreonam in obstetrics and gynecology].

The following results were obtained by measuring serum and tissue concentrations of aztreonam, a new monobactam antibiotic. Penetration into each tissue was favorable, particularly, to the portio vaginalis, but there was no conspicuous difference of the penetration among other tissues. Serum concentration tended to decrease with the lapse of time, but the concentration in each tissue did not show a specific pattern of change due to a considerable irregularity of the measured concentrations. No specific side effect was noted.

[A case report of early rectal carcinoma, IIa + IIc type, with no invasion of the submucosa].

A 66-year-old woman was admitted to the Hakodate Chuo Hospital because of abdominal discomfort. A barium enema study revealed a deformity of the middle Houston's valve of the rectum and granular lesions at the Rb region of the rectum. A colonoscopic examination showed the IIa + IIc type of early rectal carcinoma. A resected specimen also revealed IIa + IIc type early rectal carcinoma, 8 X 13 mm in diameter, and well-differentiated adenocarcinoma with no invasion of the submucosa. Infiltration into the lymphatic vessels and regional lymph node metastasis were not encountered. The deformity of the rectal Houston's valve was checked by a barium enema study. It should be emphasized that early detection of colorectal carcinoma with no invasion of the submucosa, requires careful barium enema studies and colonoscopic examination.

Emergence of permanent teeth and onset of dental stages in Japanese children.

The purpose of the present cross-sectional study was to provide mean times of emergence of permanent teeth and onset of dental stages in Japanese children. A total of 1819 schoolchildren aged 6-15 yr were recorded. The results demonstrated specific sequences of emergence in the two arches and earlier emergence times in girls than in boys, both findings in keeping with investigations on other ethnic groups. Secular trend in tooth emergence was studied by comparing the results with data on Japanese children in 1934. The comparison revealed earlier mean times of emergence in general in contemporary Japanese.
