[Renal cell carcinoma with a long-term hemodialysis patient: a case report].

A 59-year-old man was found to have a renal cell carcinoma during about one hundred months of hemodialysis by means of an ultrasound tomography. Nephrectomy was done and renal cell carcinoma, clear cell type, with acquired renal cysts were observed, histologically. The usefulness and routine application of ultrasound tomogram are stressed for examination of the renal appearance during the management of hemodialysis.

[Studies of urinary proteins, FDP (fibrinogen degradation products), and NAG (N-acetyl-beta-D-glucosaminidase) in renal transplanted patients].

The urinary proteins, FDP (fibrinogen degradation products), and NAG (N-acetyl-beta-D-glucosaminidase) in renal transplanted patients were studied. SDS (sodium dodecyl sulphate) electrophoresis was used for the differentiation of urinary proteins according to their molecular size. In the azathioprine-treated patients with stable renal function, most of the urinary proteins were albumin. However, the low molecular weight (LMW) proteins, which were suggestive of tubular proteins, appeared in the urine of the ciclosporin-treated patients with stable renal function. During the rejection episodes of the ciclosporin-treated patients, the fraction of LMW proteins increased. The elevation of urinary FDP and NAG index (urinary NAG/urinary Cr) were detected in association with rejection episodes. Urinary NAG index increased in proportion to the elevation of serum Cr. However, the elevation of urinary NAG index was found in some ciclosporin-treated patients with normal serum Cr. The elevation of NAG index without the elevation of urinary FDP occurred in ciclosporin nephrotoxicity. The SDS electrophoresis of urinary proteins, urinary FDP, and urinary NAG index can be useful parameters for monitoring ciclosporin nephrotoxicity.

Renal transplant blood flow evaluation by ultrasonic duplex scanning.

The transplant blood flow was measured renal transplantation by ultrasonic duplex scanner composed of pulsed Doppler flowmeter and real-time B-mode linear scanner in 32 patients. The blood flow information could be obtained from anywhere of interest within the renal transplant. Then blood flow in 3 regions including renal hilum, central echoes and renal parenchyma were measured. The parenchymal peripheral blood flow was not always similar to the hilar central blood flow. Parenchymal blood flow was the most correlated with graft function and decreased remarkably during acute rejection episode. This method enabled detection of occurrence of acute rejection in the course of post-cadaver transplant ATN. Prolongation of delta t (acceleration time) in Doppler spectrogram from parenchyma was also reliable evidence for the deterioration of graft function. Ultrasonic duplex scanning is a useful method in managing post-transplantation patients because intrarenal hemodynamics can be evaluated by this method.

[True hermaphroditism with 46, XX karyotype: report of a case].

A case of true hermaphroditism, in a 4-year-old, legally female, is reported. The external genitalia showed clitorism and labial agglutinate, and her karyotype from peripheral lymphocytoculture was 46,XX. She received gonadectomy at both sides, clitorectomy and plasty of vaginal orifice. Histopathological examination showed that both gonads were ovotestis.

[Pathological study on reflux nephropathy].

Four hundred patients with primary vesicoureteral reflux were followed clinically in our department between 1975 and 1984. Histopathological studies including immunofluorescent study were carried out on 13 of them. Their renal function was followed by intravenous pyelography, and from serum creatinine values, proteinuria and radioisotope renograms. Renal deterioration was found in 4 patients. Proteinuria was observed since the first consultation in three of them. Microscopically, mesangial proliferation was observed in all patients but none of them showed focal glomerulosclerosis. An immunofluorescent study was carried out on 7 patients. IgM deposition was found in 3 patients (43%), C8 deposition in only 2 of 3 patients (29%), IgG deposition in 1 of them (14%), and IgA deposition in 1 patient (14%). Proteinuria, renal disorders and mesangial proliferation were correlated with reflux nephropathy. Especially, intrarenal reflux could cause reflux nephropathy but it is impossible to deny immunological factors.

[Complication during the period of anuria after cadaveric transplantation--with special reference to wound bleeding].

Some period of anuria is usually expected after cadaveric renal transplantation, and hemodialysis is necessary during the period. Postoperative bleeding in the anuric period is not uncommon and usually can be easily controlled with adequate conservative treatment. However, it becomes uncontrollable in some cases and causes a series of serious complications. Out of 22 patients having undergone cadaveric renal transplantation in our hospital, six patients had postoperative bleeding. On investigation of these cases, we found that not only a hemorrhagic tendency due to uremia but also hemodialysis is a risk factor of bleeding in the wound.

[Three cases of non-functioning adrenal tumor].

The nonfunctioning adrenal tumor is a rare disease. These tumors are difficult to diagnose because they usually remain asymptomatic. Recently computerized tomography (CT) scan has been used because it shows retroperitoneal anatomic details. Three cases of nonfunctioning adrenal tumors are reported. All were found by CT scan. The diagnostic methods and treatment are discussed. The most useful radiologic study of nonfunctioning adrenal tumor is CT scan and the early surgical approach remains the mainstay of treatment.

[Viscus perforation as an unusual complication of Politano-Leadbetter ureteral reimplantation].

Unusual complications of the Politano-Leadbetter ureteral reimplantation technique are reported in four ureters in three children with primary vesicoureteral reflux (VUR). These ureters were found to pass through an adjacent intraperitoneal viscus including the sigmoid colon, ileum or broad ligament. This complication occurred in 0.48% of the Politano-Leadbetter antireflux operations for primary VUR. Viscus perforation occurred only in children with a relatively high grade VUR. The prognoses of the hydroureter after ureteric reimplantation or ureterolysis, were good in all cases. Although there are several advantages in the transvesical procedure of the original Politano-Leadbetter method, we should not hesitate to go extravesically if there are difficulties in mobilizing the ureter or inducing it into the vesical cavity.
