Alternating non-cross resistant chemotherapy for small cell lung cancer.

After stratification for the extent of disease, previously untreated patients with small cell lung cancer randomized to receive therapy with the four-drug combination of cyclophosphamide, oncovin, nimustine hydrochloride (ACNU), and procarbazine (CONP) every four weeks (continuous regimen) or to receive CONP alternating with the three-drug combination of etoposide (VP-16), adriamycin and cisplatin (VAD) at four-week intervals (alternating regimen). Sixty-nine patients were entered in the study. Of 34 evaluable patients receiving the continuous regimen, six (17.6%) achieved complete response (CR) and 16 (47.1%) achieved partial response (PR). Of 31 evaluable patients receiving the alternating regimen, 10 (32.3%) achieved CR, and 16 (51.6%) achieved PR. There was a tendency in favor of the alternating regimen in CR and over-all response rates (0.05 less than p less than 0.1). There were no significant differences between the regimens in response duration or survival. The projected median survival times were 9.2 months and 9.4 months for the continuous and alternating regimens, respectively. One patient receiving the continuous regimen and three receiving the alternating regimen have been living for more than two years. The major toxicity was myelosuppression in both regimens. One patient died of hemorrhage due to thrombocytopenia during induction with CONP, and one patient died of cisplatin-induced renal failure. We conclude that alternating non-cross resistant chemotherapy leads to improved CR and response rates, but does not improve survival.

Defective expression of HLA class I antigens: a case of the bare lymphocyte without immunodeficiency.

A case of the bare lymphocyte without apparent immunodeficiency was observed in a 33-year-old woman who had no history of severe infections but suffered from sino-bronchial disease. No HLA-A and -B antigens (class I antigens) were detected at the cell surface of lymphocytes, granulocytes, and platelets, but they were expressed, although at a reduced level, on the cultured B lymphoid cell line. T lymphocytes were normal in number and in the relative proportion of T4/T8 and responded to mitogens but not to PPD and candida. HLA-DR antigens (class II antigens) were present on B lymphocytes and showed intermediate MLR-stimulatory capacity, which made it possible to deduce the patient's HLA genotype. She was found to be homozygous at consanguinity for HLA-A, -B, and -DR antigens. The numbers of B lymphocytes, immunoglobulins, and complements were all in the normal range; there was, however, a low level of IgM. Two-dimensional gel analysis of class I antigens revealed the presence of normally expressed beta-2 microglobulins (B2M) and an apparently single set of class I heavy chains, allowing us to consider two alternative cellular mechanisms in this defect; the presence of one abnormal class I structural gene and the regulatory mechanism that acted in cis were suggested.

Synergistic combination therapy of 5-fluorouracil, vitamin A, and cobalt-60 radiation for head and neck tumors--antitumor combination therapy with vitamin A.

Using cultured transformed fibroblasts, an antitumor agent, 5-fluorouracil (5-FU), was found to be potentiated in its inhibitory action on RNA formation by vitamin A. 5-FU in combination with vitamin A enhanced more effectively the cellular sensitivity to irradiation than when 5-FU was used alone. So we have applied a triple combination of 5-FU, vitamin A, and cobalt-60 radiation (FAR therapy) to clinical treatment of head and neck tumors since 1972. The treatment of 192 patients with cancer of the larynx, 60 patients with hypopharyngeal and cervical esophageal cancer, and 23 patients with epipharyngeal cancer with FAR therapy during 6 years from 1972 to 1977 resulted in highly effective synergism, and the improved points of this therapy were considered.

Metastases to the Rouviere nodes and headache.

This paper presents two cases of Rouviere node metastasis in carcinoma of the hypopharynx as confirmed by gallium scintigraphy. Invasion of the Rouviere nodes was clarified within six months of the onset of symptoms in both cases. One patient eventually died of intracranial cancerous invasion and the other remains in the terminal stage of cancer with multiple bone metastases. It was hitherto believed that metastases to the Rouviere nodes were extremely difficult to diagnose in the early stages. However, it is now apparent that the neurological signs and symptoms which commonly appear in patients with cancerous invasion of the jugular foramen syndrome facilitate early diagnosis of the disease, the most common symptom being a headache.

[Response and pharmacokinetics of cisplatin instilled into the pleural cavity].

The response and pharmacokinetics of cisplatin instilled into the pleural cavity were studied in 11 patients with malignant pleural effusion; 10 patients had primary lung cancer and one had breast cancer. All of them were adenocarcinoma histologically. In five of the 11 patients effusion disappeared and its cytology became negative for malignancy after four weeks. In the other six patients effusion was reduced and its cytology became negative for malignancy after four weeks. Toxicity was almost similar to that in systemic administration of cisplatin but a few patients had chest pain and fever possibly due to local irritation. The pharmacokinetics showed that a high concentration of cisplatin (free-form, 48.9 micrograms/ml) was maintained over a long period (free from (t 1/2) beta = 33.6 hours) in the pleural cavity. This was regarded as the reason for the high response to this therapy. The intrapleural instillation of cisplatin into the pleural cavity therefore seems to be an effective modality for malignant pleural effusion.

Crackles in asbestos workers: auscultation and lung sound analysis.

We surveyed 270 asbestos factory workers and 222 control subjects matched for age to assess the usefulness of listening for fine discontinuous adventitious lung sound (fine crackles) in detection of the early stages of interstitial fibrosis of asbestosis. Fine crackles at both bases were heard more frequently in asbestos workers (32.2%) than in controls (4.5%) (P less than 0.01). The presence of bilateral basal crackles was related to the duration of asbestos exposure and occurred before abnormalities could be detected by chest radiography. Two observers examined 74 workers randomly chosen from among 270 asbestos workers and found inter-observer agreement to be high (91.9%). There was close agreement between findings on chest auscultation and sound recordings.
