Alkaline phosphatases, which had a unique electrophoretic mobility on polyacrylamide gel electrophoresis, were found in hepatic tissue of a patient with liver cirrhosis. Enzymic and immunological properties of the enzymes examined on electropherogram were similar to those of a fetal intestinal-type alkaline phosphatase in hepatoma with respect to sensitivity to amino acids, heat stability, sensitivity to sodium dodecyl sulfate, and reactivity to anti-intestinal alkaline phosphatase antiserum. The enzymes seem to be a variant of a fetal intestinal alkaline phosphatase. The significance of occurrence of the enzymes in cirrhotic liver is discussed.

Myocardial contractility was estimated in 27 patients with hyperthyroidism (10 men and 17 women, aged from 16 to 38) by echocardiography. Echocardiographic examination was performed before and on the 7th day after the subtotal thyroidectomy. Thyroid functions (PBI, T3, and T4) decreased significantly after the operation (p less than 0.01). Heart rate and systolic blood pressure showed a slight decrease (N.S.), while diastolic blood pressure showed a slight increase (N.S.), while diastolic blood pressure showed a slight increase (N.S.). Mean AWVs decreased from 35.8 +/- 7.0 mm/sec to 27.6 +/- 5.7 mm/sec and correlated wtih T3 (r=0.4008, p less than 0.05). Mean PWV's also decreased from 53.2 +/- 12.2 mm/sec to 45.2 +/- 11.4 mm/aec and correlated with T3 (r=0.5237, p less than 0.02). DDR decreased from 108.0 +/- 36.8 mm/sec to 85.7 +/- 31.7 mm/sec and correlated wtih T3 (r=0.4008, p less than 0.05). Above results suggest that enhanced myocardial contractility and elevated rate of ventricular filling in hyperthyroidism are suppressed after the operation, and that echocardiography is useful for the evaluation of the clinical course after the operation.
We examined 19 hepatoma tissues for alkaline phosphatase isoenzyme and found that six have both the Kasahara isoenzyme and an alkaline phosphatase with a unique electrophoretic mobility, in addition to the liver-type enzyme. From two of six carcinoma tissues, the abnormal enzyme was partly purified and subjected to a detailed analysis, which clarified that the abnormal enzyme resembled a fetal intestinal alkaline phosphatase in most of its enzymic and immunologic properties and also in properties that reflect enzyme structure. This fetal intestinal-type alkaline phosphatase was not found in 24 specimens of normal liver from adults. The relevance of fetal intestinal-type alkaline phosphatase to Kasahara isoenzyme and adult intestinal alkaline phosphatase is discussed. The fetal and adult intestinal alkaline phosphatases differ in electrophoretic mobility, heat stability, and reactivity with concanavalin A. The adult-type enzyme has two components; only the electrophoretically slower, neuraminidase-resistant one is described here.

In this report, the development of false aneurysm of the ascending aorta 8 years following the repair of congenital aortic stenosis is described. A 14-year-old male patient was found to have congenital aortic stenosis in 1966 after medical check-up and subsequently operated upon. In the postoperative period, he was complicated by remittent fever of 1 month's duration, which was finally controlled by antibiotics. Arterial blood cultures taken on a few occasions were negative. Eight years and 5 months after surgery, he was noticed to have a mediastinal mass around the base of the heart and the presence of false aneurysm was confirmed by aortography. Aneurysmectomy was carried out almost 9 years after the previous operation, and it was found that aneurysm was arising from the suture line of the aortotomy with communication to the aorta at the bottom of the aneurysm. Postoperatively, patient's recovery was uneventful and was discharged on the 23rd postoperative day. Etiology, timing of operation and techniques utilizing hypothermia and/or circulatory arrest are discussed.

The Kasahara isoenzyme of alkaline phosphatase was found in cancer tissues from patients with gastric carcinoma, maxillary carcinoma, pulmonary carcinoma, and carcinoma of the urinary bladder, in addition to hepatoma. This fact suggests that the Kasahara isoenzyme may not be a specific marker protein of liver cancer but could occur in a variety of neoplasms.
It was found that a human hepatoma-associated ALP (orthophosphoric monoester phosphohydrolase, E.C. 3.1.3.1) shared electrophoretic mobility, inactivation by urea, inhibition by inorganic phosphate, ethylenediaminetetraacetate, and amino acids (L-phenylalanine, L-leucine and L-homoarginine), heat stability, sensitivity to neuraminidase, pH optimum, Km value, and antigen site with fast moving ALP isozymes of FL cell strain derived from human amniotic membrane. However, 40-week-old fresh amniotic membrane lacked this isozyme. Instead, it had a placental type ALP consisting of minor components. The other ALP isozyme of FL cells had properties common to hepatoma ALP with regard to L-phenylalanine sensitivity, inhibition by ethylenediaminetetraacetate, inactivation by urea, and antigen site, but differed from it in electrophoretic mobility, sensitivity to L-leucine and L-homoarginine, and the presence of another antigen site. It was more heat stable and more sensitive to inhibition by inorganic phosphate than Hepatoma AP. The possible regulatory mechanism between the hepatoma-type ALP and the placental type ALP in the amnion cells is considered.
The electrophoretically faster migrating alkaline phosphatase isozyme in FL cells derived from the human amniotic membrane was similar in some of the enzymatic and immunochemical properties to the variant alkaline phosphatase which occurs in serum and cancerous tissue of some patients with hepatocellular carcinoma. These facts suggest that the hepatoma-associated alkaline phosphatase may be the resurgence of the isozyme originally present in the amniotic membrane.
