Comparison of selective and nonselective media for recovery of Campylobacter pylori from antral biopsies.

Antral biopsy specimens from patients with acid peptic disease were cultured in parallel on Skirrow medium (SM) and sheep blood agar (SBA) for Campylobacter pylori. In institution 1, 23 of 88 (26.1%) endoscopies were positive on SBA compared with 37 of 88 (42.0%) on SM (P = 0.0001). In institution 2, 29 of 49 (59.2%) endoscopies were positive for C. pylori on SBA and 27 of 49 (55.1%) were positive on SM (P = 0.50). The addition of SM optimizes the recovery of C. pylori from antral biopsy specimens.

Campylobacter pyloridis is associated with acid-peptic disease in Toronto.

Campylobacter pyloridis has been associated with acid-peptic disease in centres outside Canada. The authors conducted a pilot study to see if this association existed in Toronto. Patients in whom esophagogastroscopy was indicated on clinical grounds were arbitrarily selected for determination of the presence of C. pyloridis. Included in the study were 100 patients who underwent 105 endoscopies. In 75 patients (80 endoscopies) there was some form of acid-peptic disease (inflammation or ulceration of stomach or duodenum). Of those with acid-peptic disease, 34% had C. pyloridis compared with 4% among patients without acid-peptic disease (p less than 0.01). Men with acid-peptic disease were more likely to harbour C. pyloridis than women (48% versus 16%, p less than 0.01). The organisms were curved gram-negative rods that appeared as small colonies after 4 days of incubation under microaerobic conditions. They were strongly urease positive. There was considerable heterogeneity of endoscopic diagnoses. Future clinical studies of C. pyloridis need careful endoscopic and histologic classification.

Intraoperative bacterial contamination of vascular grafts: a prospective study.

The establishment of graft infection depends on host response, an appropriate field and bacterial contamination. Intraoperative bacterial contamination of prosthetic graft material was studied prospectively in 77 patients. Vascular reconstruction was indicated for abdominal aortic aneurysm (15%), claudication (42%), rest pain (25%) and ulceration or gangrene (18%). In 78% of cases the procedure was elective. Staphylococcus epidermidis was isolated in 80% of cultures; mixed flora were more frequent in patients with rest pain (60%) and ulceration or gangrene (45%) than in those with aneurysms (22%) or claudication (16%). Grafts became contaminated in 56% of cases using standard techniques; this was lowered to 35% when the surgeon changed gloves before preclotting the graft. There was no significant difference with respect to the surgeon who performed the operation, the indication for operation, primary versus secondary repair or the use of skin barriers. One patient (1.3%) had an established graft infection. It is concluded that the incidence of contamination is high but may be decreased by glove changing.

Susceptibilities of 45 clinical isolates of Proteus penneri.

Patterns of susceptibility of 45 Proteus penneri clinical isolates to 14 antimicrobial agents were evaluated by a macrobroth dilution method. All strains were highly susceptible to ceftizoxime, ceftazidime, moxalactam, cefoxitin, gentamicin, tobramycin, netilmicin, and, with few exceptions, to amikacin, piperacillin, and cefoperazone. Most strains were susceptible to cefotaxime and ceftriaxone. All strains were resistant to cefazolin and cefsulodin.

Proteus penneri and urinary calculi formation.

The clinical significance of Proteus penneri, a newly described species, is unknown. A case report is presented, which is to the best of our knowledge the first description of this organism causing a urinary tract infection and bladder calculi.

Treatment of tuberculous masses in the neck.

Tuberculous cervical lymphadenitis was diagnosed in 23 patients from 1970 to 1980; 21 of them were immigrants. The masses ranged in diameter from 1.5 to 5.5 cm. The posterior triangle was involved in 11 patients, the anterior triangle in 7 and both triangles in 5 (3 bilateral). Excisional node biopsy was performed in all. Acid-fast bacilli were identified on the smear in only 12 patients. Caseation necrosis was present in 19 specimens; the other 4 were comparable with sarcoidosis. However, all cultures grew Mycobacterium tuberculosis, which was corroborated in all by guinea-pig inoculation. No chest roentgenograms showed active pulmonary tuberculosis. Chemotherapy was given for 12 to 18 months. In three patients masses continued to enlarge for 5 months after initiation of chemotherapy, but then regressed and disappeared completely within 12 months. No recurrences were present in any patients (follow-up more than 2.5 years, mean 5.4 years).

Clinical and microbiological features of otitis externa.

A prospective study of 40 cases of acute otitis externa and 99 cases of chronic otitis externa in unselected patients revealed that otitis externa affects males and females with a similar frequency. The peak incidence occurs in the summer and early fall months of the year. Exposure to water, previous use of ear drops, and cotton-tipped applicators predisposed to both acute and chronic otitis externa. Hearing aid ear molds appear to be a predisposing factor in the development of chronic otitis externa. Pain, itching, discharge, and hearing loss were the most common presenting complaints in both acute and chronic otitis externa. The discharge in chronic otitis externa is more commonly purulent, whereas in acute otitis externa it is more commonly mucoid. The tympanic membrane is not frequently involved in acute otitis externa; however, in chronic otitis externa changes in the tympanic membrane were more often encountered. Most infections were of a pure bacterial origin, primarily Pseudomonas aeruginosa and Staphylococcus aureus. Fungi were the causative organisms more commonly in chronic otitis externa than in acute otitis externa (Figure 7). It was found that previous usage of ear drops was more often associated with otomycosis in acute otitis externa and yet was not related to a higher frequency of otomycosis in chronic otitis externa. The presence of a foreign material, such as an ear mold, was associated with a greater frequency of mixed infections (bacteria and fungi) in the group with chronic otitis externa. The presence of a greenish discharge or foul odor was not related to any particular organism.

Blastomycosis: a new endemic focus in Canada.

A survey of the 38 patients resident in Ontario from whose sputum, body fluids or tissues Blastomyces dermatitidis was cultured by our laboratory between 1970 and 1981 revealed a new endemic focus to the north and east of Lake Superior, where 20 of 27 traceable patients lived. Direct microscopy revealed B. dermatitidis in 90% of the cases. A lack of clinical awareness, however, had often resulted in a delay (average 15 weeks) in diagnosis. In two cases the disease was identified only at autopsy. About 80% of the patients survived.

Enterovirus infections in the neonate.

A chart review was made of 24 neonates less than one month of age with culture-proven enteroviral infection. The seasonal distribution was summer and fall. An antecedent illness was common in the mother or other family members. One patient had a mild gastroenteritis. Three categories of severe disease were noted: (a) meningitis accounted for 50 per cent of the illnesses; (b) myocarditis for 25 per cent of the illnesses; and (c) the remainder presented with a severe sepsis-like illness. High mortality rate was associated with low birth weight and low gestational age.

Safety and toxicity of oxamniquine in the treatment of Schistosoma mansoni infections, with particular reference to electroencephalographic abnormalities.

Thirty-seven patients infected with Schistosoma mansoni were treated with oxamniquine. All were cured. The only significant adverse reaction was the development of abnormal electroencephalographic (EEG) findings in 6 (17.6%) of 34 patients whose pretreatment EEG was normal. Management of patients, with particular reference to the EEG abnormalities, is discussed.

Person-to-person transmission of Giardia lamblia in day-care nurseries.

In an outbreak of giardiasis at two day-care nurseries in metropolitan Toranto Giardia lamblia appeared to be transmitted person to person. No common source of infection could be found. The proportions of children infected in the two nurseries were 39% and 17%; infection was spread to 7% and 23% of their household contacts. Of the infected children and household contacts 26% and 30% respectively of those for whom detailed information could be obtained were symptomatic. Canadian children were more likely to be infected and symptomatic than were immigrant children attending the nurseries. The most susceptible ages for infection were 1 to 3 years. The results of this study suggest that all children in day-care nurseries who are infected with G. lamblia should be treated, regardless of whether they are symptomatic.
