Seborrheic keratoses and trichostasis spinulosa.

In 54 of 325 seborrheic keratoses, trichostasis spinulosa was found within the keratinous cysts and between papillomatous surface keratin, in addition to being present in contiguous hair follicles. Trichostasis in seborrheic keratoses and trichostasis spinulosa are both more frequent in older age groups. Trichostasis spinulosa represents a marker of vellus hair follicles and indicates that some seborrheic keratoses incorporate and may be partially derived from the cells of the infundibular portion of the follicle.

Low-dose chlorambucil in the treatment of generalized granuloma annulare.

5 of 6 adults with generalized granuloma annulare responded favorably to therapy with low-dose chlorambucil. 5 of the patients had lesions that were recurrent and long-standing and that resisted conventional modalities of therapy. Low-dose chlorambucil may be effective in a short-term therapy program but should be used only in patients with persistent, widespread, or unusual forms of generalized granuloma annulare. The program requires close patient and hematologic supervision.

Vascular calcification in dermatopathology.

Calcification in cutaneous blood vessels is an uncommon finding in biopsies submitted for dermatopathological examination. Of 14 biopsy specimens showing the phenomenon that was studied by us, the greater number was from women who had a combination of severe diabetes, hypertension, and atherosclerosis. Unusual clinical syndromes as the bases for the vascular calcification were hyperthyroidism in three patients and arteritis in two patients. Three patients died as a consequence of massive cutaneous infarction and sepsis, probably stemming from cutaneous vascular calcification. Vascular calcification in biopsy of skin may result from metabolic, inflammatory, or degenerative diseases of blood vessels.

Response of generalized granuloma annulare to alkylating agents.

The conditions of three patients who had generalized granuloma annulare responded favorably to treatment with alkylating agents. This mode of therapy in two cases was chosen on the basis of recent evidence that implicates delayed hypersensitivity mechanisms as forming part of the pathogenesis of granuloma annulare. Two patients had rapid clearing of their lesions. The response in one of these patients was observed during therapy for a concomitant plasma cell dyscrasia. The third patient had an unusual form of granuloma annulare that had evolved during a period of 28 years, with hand deformities secondary to fascial and tendon involvement and the production of contractures and lymphedema, which responded partially to therapy. The results lend further support to data implicating cellular immunity to granuloma annulare.

Dermatology in hospital practice.

Modern hospital facilities have proved of great value in the management of the more difficult problems presenting to dermatologists. In a series of 100 consecutive patients admitted to hospital, four died, at least one out of every three was found to have undiagnosed internal disease and the average time to gain a remission was three weeks after outpatient treatment had proved unsuccessful over an average of six months.
