A new method of external cardiac massage to improve diastolic augmentation and prolong survival time.

Experiments were performed on 50 dogs to determine the reason for unsatisfactory survival rates following closed-chest cardiac massage and to help develop new means for increasing survival following acute myocardial infarction and other causes of cardiac arrest. A new technique, named the countermassage method, was subsequently developed. Coronary and carotid flows after use of countermassage following cardiac arrest were increased 1.4 and 2 times, respectively, in contrast to flows obtained during manual closed-chest cardiac massage. These increases brought flows up to normal levels. We also tried to determine whether survival could be extended by this method to facilitate clinical treatment such as defibrillation or placing a temporary pacemaker catheter under fluoroscopic control, which are hard to accomplish during the brief time permitted by conventional closed-chest cardiac massage.

Therapeutic use of helium-oxygen mixture in continuous positive airway pressure for early weaning from mechanical ventilation after cardiovascular surgery in infants.

Therapeutic use of helium-oxygen mixture in continuous positive airway pressure (He-CPAP) was employed for early weaning from mechanical ventilation of 11 infants who underwent cardiac surgery from August, 1974, to April, 1975. With the use of He-CPAP, a 20 to 30 mm. Hg elevation of PaO2 was usually observed and respiratory distress was reduced, as compared to results obtained with nitrogen-oxygen CPAP. In all cases, He-CPAP resulted in the possibility of relatively early removal of the endotracheal tube. Therefore, He-CPAP is recommendable for intermediate respiratory support between mechanical ventilation and nitrogen-oxygen CPAP.

Prinzmetal's variant form of angina pectoris. Re-evaluation of mechanisms.

Thirty-five patients with typical Prinzmetal's variant angina were studied by coronary cineangiography. There was no demonstrable stenosis of the major coronary arteries in 19 patients. Nine patients with single coronary stenosis underwent aortocoronary bypass and had recurrence of the symptoms postoperatively. Administration of nifedipine effected complete cessation of the symptoms among patients formerly treated medically. Although surgical treatment did not effect permanent relief of pain, all patients initially treated surgically experienced relief of pain when nifedipine was administered. The pathophysiology of variant angina remains obscure. Our results suggest that neurohumoral factors exert more of an effect on the myocardial cell than on the coronary vessels.
