Allergic predisposition among infants with bronchiolitis.

Allergic predisposition among infants with bronchiolitis was examined. The number of infants with serum IgE exceeding mean +1 SD was 31/70 (44.3%). The rate of positive radioallergosorbent test (RAST) scores of 1 or more to mites, egg white, or milk was 31/71 (43.7%) and that of scores over 2 was 11/71 (15.5%). Eosinophils and/or mast cells were found in their nasal smears on several occasions. These results indicated that allergic predisposition may be observed among infants with bronchiolitis.

Effects of changes in dietary sodium intake and saline infusion on plasma atrial natriuretic peptide in hypertensive patients.

Plasma concentrations of immunoreactive (IR)-atrial natriuretic polypeptide (hANP) were measured by radioimmunoassay in 9 essential hypertensive patients after alteration of salt intake and acute saline infusion. Daily salt intake was altered every one week in the order of 15g/day, 3g/day, and 7g/day. On the last day of the first week, 1500 ml of 0.9% saline was infused intravenously over one hour. Plasma concentrations of IR-hANP tended to decrease, although not significant, by salt restriction. Further, there were significant positive correlations between changes in plasma concentrations of IR-hANP and those of several variables such as body weight, systolic blood pressure, and creatinine clearance. Plasma concentrations of IR-hANP rose significantly (p less than 0.05) from 50.7 +/- 20.1 (Mean +/- SEM) pg/ml to 119.0 +/- 48.8 after acute saline infusion. Although there was significant correlation between mean blood pressure and the increase in sodium excretion by saline infusion, this increase was unrelated to the rise in plasma concentrations of IR-hANP. These results suggest that the release of ANP is stimulated mainly by expansion of extracellular fluid volume in hypertensive patients. However, natriuretic and hypotensive effects attributable to the changes of ANP release could not be elucidated.

A sensitive method for precise measurement of endogenous angiotensins I, II&III in human plasma.

We measured endogenous angiotensins (ANGs) I, II&III using a system of extraction by Sep-Pak column followed by high performance liquid chromatography (HPLC) combined with radioimmunoassay (RIA). An excellent separation of ANGs was obtained by HPLC. The recovery of ANGs I, II&III was 80-84%, when these authentic peptides were added to 6 ml of plasma. The coefficient of variation of the ANGs was 0.04-0.09 for intra-assay and 0.08-0.13 for inter-assay, thereby indicating a good reproducibility. Plasma ANGs I, II&III measured by this method in 5 normal volunteers were 51,4.5 and 1.2 pg/ml. In the presence of captopril, ANGs II&III decreased by 84% and 77%, respectively, while ANG I increased 5.1 times. This method is therefore useful to assess the precise levels of plasma ANGs.

[Attenuation correction of SPECT imaging by dual energy method--quantification of 111In-labeled monoclonal anti-tumor antibody localization].

Application of radiolabeled monoclonal anti-tumor antibodies for diagnosis and therapy has made remarkable progress in the past few years. Quantification of radiopharmaceutical localization is required adequate attenuation correction in SPECT imaging. Attenuation correction by transmission CT (TCT) data is one of the best method at present time. However, if a patient is moved between TCT and SPECT, this method is no more applicable. We developed a new attenuation correction algorithm by dual energy method, using 99mTc and 111In because of similarity of these linear attenuation coefficients. The new algorithm uses data of TCT with an external source of 99mTc, and requires another data from SPECT of 111In labeled monoclonal anti-tumor antibody, which are simultaneously obtained. TCT results in an attenuation map, which then serves as input into the final intrinsic correction algorithm to uncorrected SPECT data. In chest phantom experiment, the attenuation corrected SPECT images revealed nearly same distribution of actual radioactivity of 111In as compared to that of uncorrected one.

[Data processing system of personnel exposure with personal computer].

This paper describes a software package that manages personnel exposure data with a personal computer. The exposure data is automatically transferred to the computer through a floppy disk supplied by film badge service companies. The computer processes data of about 4000 persons who belong to the 11 facilities and prints out the reports on the distribution of personal exposure level. This system made it possible for the radiation protection division to rapidly investigate the actual situation of occupational exposure with less laborious works.

Examination of the direct effects of nicorandil, verapamil, propranolol, and nitroglycerin on the hypoxic canine myocardium.

Effects of intracoronary administration of verapamil (VER), nitroglycerin (TNG), propranolol (PRO), and nicorandil (NIC) on regional hypoxia were examined in 39 open-chest dogs. Distal left anterior descending coronary artery was perfused for 5 min with deoxygenated Krebs-Henseleit solution (KHS) in group 1 (n = 9), VER-containing (1.3 mg/dl) KHS in group 2 (n = 9), TNG-containing (0.5 mg/dl) KHS in group 3 (n = 7), PRO-containing (0.4 or 0.8 mg/dl) KHS in group 4 (n = 7), and NIC-containing (0.5 or 2.5 mg/dl) KHS in group 5 (n = 7). Transmural biopsy at 5 min revealed a less severe metabolic deterioration in the hypoxic myocardium in groups 2, 4, and 5, as evidenced by higher ATP content (2.99 +/- 0.73, 2.81 +/- 0.35, and 3.14 +/- 1.20 mumol/g in groups 2, 4, and 5, respectively) and lower lactate accumulation (5.16 +/- 0.59, 5.62 +/- 1.44, and 5.05 +/- 1.12 mumol/g in groups 2, 4, and 5, respectively) compared with those in group 1 (2.09 +/- 0.45 mumol/g in ATP and 8.77 +/- 2.34 mumol/g in lactate). Metabolic preservation by VER, PRO, and NIC were not mediated by changes in hemodynamics and regional myocardial contraction. On the other hand, any significant protective effects were not noted in group 3, despite a significant reduction in rate-pressure products. Under an aerobic condition these agents affected regional myocardial contraction in a different manner from each other.

Hypertensive complications and home blood pressure: comparison with blood pressure measured in the doctor's office.

We have compared hypertensive target organ damage with home blood pressure readings (HBPs) and with office blood pressure readings (OBPs) in 100 patients with mild to moderate essential hypertension. The correlation between blood pressure levels and hypertensive target organ damage in HBPs and OBPs were similar (r = .42, p less than 0.001 for systolic HBPs; r = .33, p less than 0.001 for diastolic HBPs; r = .42, p less than 0.001 for systolic OBPs; r = .34, p less than 0.001 for diastolic OBPs). In most instances, HBPs were lower than corresponding OBPs. Among individual patients whose OBPs were identical, HBPs in some instances differed strikingly. Optic fundi abnormalities were significantly more severe in patients whose systolic HBPs were 150 mmHg or greater, than in those whose systolic HBPs were less than 150 mmHg (p less than 0.05). Hypertensive complications did not differ among office hypertensive patients who were normotensive or borderline hypertensive at home, from the differences of OBPs. We concluded that overall hypertensive complications were equally related to HBPs and OBPs, but patients with discrepancies between HBPs and OBPs had fewer hypertensive complications. Thus, both OBPs and HBPs should be considered in deciding therapy.

Hypoxia provokes leukotriene-dependent neutrophil sequestration in perfused rabbit hearts.

Isolated rabbit hearts were perfused with salt solution containing autologous 111In-labeled neutrophils to determine whether 1) hypoxia provoked myocardial neutrophil sequestration, 2) neutrophil accumulation could be suppressed by inhibition of lipoxygenase and 3) hypoxic myocardium generated radioimmunoassayable leukotriene B4 (LTB4). Whereas accumulation of 111In-labeled neutrophils by normoxic hearts was minimal, induction of acute myocardial hypoxia by perfusion with hypoxic medium caused a rapid uptake of 111In-labeled neutrophils. Sequestered neutrophils were not released during a subsequent 20-min normoxic perfusion period. Hypoxia-induced neutrophil uptake was prevented by nordihydroguaiaretic acid (NDGA) or diethylcarbamazine (DEC), two structurally different inhibitors of lipoxygenase. Although no radioimmunoassayable LTB4 could be detected in neutrophil-free perfusate from normoxic or hypoxic preparations, hypoxia caused an approximate 2-fold increase in myocardial tissue levels of LTB4. Tissue levels of LTB4 reverted to control values after 20 min of normoxic perfusion. Infusion of exogenous LTB4 also provoked myocardial neutrophil uptake. Viewed collectively, these observations suggest that in isolated buffer-perfused rabbit hearts hypoxia induces LTB4 release from resident myocardial cells, which promotes avid neutrophil sequestration.

[Prophylactic intravesical chemotherapy in bladder tumors after surgery of upper tract urothelial carcinoma].

Sixteen patients with upper tract urothelial carcinoma underwent intravesical chemotherapy usually at 2 week intervals in the first year and at 4 week intervals in the second year after nephrouretectomy. For bladder instillation 10 mg mitomycin C in 20 ml saline was used on 7 patients, 5 mg carboquone and 100 mg cytosine arabinoside in 40 ml saline on 5 patients and 30 mg adriamycin in 40 ml saline on 4 patients. Two (12.5%) of the 16 patients developed bladder tumors within 2 years after surgery, but 11 (42.3%) of the 26 patients with upper tract urothelial carcinoma who did not receive intravesical chemotherapy suffered from bladder tumor within 2 years after surgery. Prophylactic intravesical chemotherapy reduced significantly (p less than 0.1) the incidence of bladder tumor after the surgery of renal pelvic and ureteral tumors.
