Relationship between forced expiratory flow and tracheal sounds. Possible effect of vortices on flow.

We measured both tracheal sounds and forced expiratory flows simultaneously in 20 normal subjects and 4 patients with stenosis of the main bronchus. Spectral analysis of the tracheal sounds revealed development of discrete line spectra, the frequencies of which ranged between 488 and 942 Hz. There was a positive correlation between these frequencies and the peak flows (r = 0.601). We speculate that the origin of these line spectra are vortices rather than the vibrations of the wall. It is postulated that the pressure drops caused by vortices make the airways more susceptible to collapse and may lead to flow limitation.

Left atrial myxoma with an unusual tumor vascularity demonstrated by angiography--report of two cases.

We describe two cases of left atrial myxoma, demonstrating an unusual tumor vascularity as revealed by coronary angiograms. Both cases had suffered typical episodes of transient ischemic attack (TIA). Coronary angiography revealed tumor blood supply from coronary arteries in both cases, and also a leakage of contrast medium from the surface of the tumor blood vessels to the left atrium in one case. In the medical literature available in English reviewed so far, no report of such a leakage has been reported. Selective coronary arteriography can be a useful diagnostic method of delineating left atrial myxoma and its blood supply.

Importance of cell surface marker to the prognosis of non-Hodgkin's lymphoma.

We studied the correlation between the cell surface markers and prognosis of non-Hodgkin's lymphoma (NHL) patients treated in the Shikoku Cancer Center Hospital from 1980 to 1986. Thirty-one cases were selected on the basis of having a lymphnode as a primary lesion, having been immunophenotyped before chemotherapy, being in the intermediate histologic grade and being in stage II, III or IV. Thirteen cases of the T-cell type (T-lymphomas) and 18 cases of the B-cell type (B-lymphoma) were identified. The complete remission rate was 54% among T-lymphoma patients and 78% among B-lymphoma patients. The median length of survival was 12+ months in T-lymphoma and 26+ months in B-lymphoma. The survival rate of T-lymphoma patients was significantly lower than that of B-lymphoma patients. The importance of making surface marker studies was reappraised in our study.

[Combination intra-arterial chemotherapy with degradable starch microspheres and mitomycin C against inoperable hepatic metastases].

Intra-arterial hepatic infusion chemotherapy combined with degradable starch microspheres (DSM) and mitomycin C (MMC) was performed for 9 patients with inoperable hepatic metastases from alimentary tract primary cancer. DSM, 45 +/- 5 micron in average diameter, produces temporary obstruction of arterial blood flow in the arterio-capillary bed and are subsequently degraded by serum amylase with T 1/2 of about 30 min. This intra-arterial treatment was repeated 2.3 times on the average. The average dose of DSM in a single infusion was 721 +/- 194 mg and the average total dose of MMC was 34.4 +/- 22.3 mg. Antitumor effects were evaluated in terms of tumor regression measured by CT scan and sonography. An objective tumor response was shown in 4/9 patients (44.4%): PR, 2/9; MR, 2/9. Elevated serum CEA levels of more than 3.0ng/ml were decreased in 7/8 patients (87.5%). Marked declines in the CEA level of more than 50% were observed in 4/8 patients (50%). Nausea and vomiting as well as abdominal pain were experienced in 8/21 treatments (38.1%) and 5/21 (23.8%), respectively. Furthermore, fever (2/21 : 9.5%), hepatic dysfunction (2/21 : 9.5%), and leukopenia (1/21 : 4.8%) were observed. All these side effects, however, were mild and transient. Thus, these results suggest that combined intra-arterial administration of DSM and MMC favorably enhances the antitumor efficacy of MMC.

[Statistical survey of malignant skin tumors during the first five years of cases treated at the Dermatological Clinic of Shimane Medical University Hospital].

A statistical survey of malignant skin tumors has been carried out involving the outpatients seen at the Dermatological Clinic of Shimane Medical University Hospital during the period between October, 1979 and October, 1984. Out of 9,702 patients seen, 51 were found to have a malignant skin tumor or, statistically, 0.53% of all patients. The number and type of the malignant skin tumor were as follows: squamous cell carcinoma, 16; basal cell epithelioma, 7; Bowen's disease, 6; Paget's disease, 1; a metastatic carcinoma of the skin, 4; a malignant melanoma, 5; a malignant lymphoma, 10; leukemia, 1 and a multicentric reticulohistiocytoma, 1. reticulohistiocytoma, 1.

[A case of aorto-enteric fistula after reconstruction of an abdominal aortic aneurysm associated with Behçet's disease and special reference to 95 reported cases in Japan].

A 45-year-old male with incomplete type of Behçet's disease was operated on because of abdominal aortic aneurysm. The post-operative course was uneventful. On twenty-one months after discharge, the patient noticed left leg pain and tarry stool, and was referred to our hospital. Computed tomography and angiography showed aorto-enteric fistula at the proximal aortic anastomotic site. Aortic reconstruction was performed, but the patient died on 36th post-operative day. We analysed the reports of 95 cases of aneurysm associated with vasculo-Behçet's disease in Japan. This series comprised 82 men, 9 women and 4 unknowns. The cases were classified to four groups, which were incomplete type (48 cases), complete type (27 cases), suspected type (12 cases) and unknown (8 cases). They had 46 aortic aneurysms and 108 peripheral aneurysms. Operation was performed on 29 aortic aneurysms and 73 peripheral aneurysms. Eleven aortic aneurysms and twelve peripheral aneurysms had anastomotic complication post-operatively. Four cases after reconstruction of abdominal aortic aneurysm was complicated with aorto-enteric fistula. They were all dead post-operatively. Vasculo-Behçet's disease with aortic aneurysm is comparatively a rare entity and morbidity and mortality is high because of its etiological characteristics.

[Repeated intra-arterial chemotherapy combined with mitomycin C and degradable starch microspheres in inoperable metastatic hepatic cancer].

Intra-arterial chemotherapy combined with mitomycin C (MMC) and degradable starch microspheres (DSM) was prescribed repeatedly for 14 patients with inoperable hepatic metastasis. This intra-arterial treatment was repeated 2.7 times on the average. The average dose of DSM and MMC in a single infusion was 685 +/- 201 mg and 13.8 +/- 3.8 mg, respectively. To analyse the degradation time of DSM, MMC concentrations in the peripheral blood were measured by HPLC method. RI-angiography using 99mTc macroaggregated albumin(MAA) was performed to estimate the hemodynamic changes in the liver. Antitumor efficacy was evaluated in terms of tumor regression measured by CT scan. An objective tumor response was observed in 9/14 patients(64.2%): CR 1/14; PR 6/14; MR 2/14. Elevated CEA levels were decreased in 11/12 patients (91.7%). Based on the peripheral MMC blood levels after combined infusion with DSM and MMC, an occlusion of intrahepatic vessels with DSM continued at least for 30 minutes. Again, RI-angiography with 99mTc-MAA plus DSM revealed the increased accumulation in the tumor, compared to 99mTc-MAA only. Side effects possibly attributable to DSM were observed in 14/38 treatments, though they were slight and temporary. Thus, these results indicate that this combined intra-arterial infusion of DSM and MMC achieve, higher regional selectivity.

[The relationship between thermotolerance and the tissue oxygen tension in human gastric cancer tissue].

To study the relationship between thermotolerance and post-thermal phenomenon, tumor pO2 (TpO2) in xenoplanted gastric cancer tissue (H-23) has been measured by a polarographic method. The heat treatment was done in a water bath at 43.5 +/- 0.1 degrees C for 23 minutes. In order to produce thermotolerance, the second treatment was carried out at an interval of 24 hours, or, alternatively at 72 hours, or at 5 days or at 7 days. The thermotolerance of the H-23 tumor reached its maximum at the 72 hour-interval and was seen to disappear completely at 7 days. The TpO2 in the H-23 tumor decreased immediately after a single heat treatment and returned to its pre-treated value after 10 hours. In cases of the second heat treatment, the 72-hour interval group returned to its pre-treated value in 2-6 hours, whereas the 24-hour, 5-day, and 7-day interval groups showed a longer recovery time. The relationship between thermotolerance and TpO2 recovery was inverse at r = -0.858 and p = 0.035. Thus, our study suggests that not only is the thermotolerance affected by the alteration of the intra-cellular components, but also by the post-thermal changes in the tumor vessels.

Radiosensitivity of late recurrences following radiotherapy of murine fibrosarcomas.

Radiosensitivity of late recurrent tumors which emerged after radiotherapy was investigated. Tumors observed were fibrosarcomas. Recurrences emerged in the irradiated area approximately 200 days after a 50% tumor control dose of radiation of 60Co gamma rays or mixed irradiation with fast neutrons and gamma rays. The recurrent and radiation-induced tumors were differentiated by karyotype analysis. Once transplanted into fresh mice, the recurrent tumors grew more slowly than the original tumor. Tumorigenicity of the late recurrences was lower than that of the original tumor. Radiosensitivity of the late recurrences, which was examined using methods to assess control, tumor growth delay, and colony forming assays, was significantly higher than that of the original tumor. D0 values of hypoxic tumor cells were significantly smaller in two of the three recurrences compared to the original tumor. Oxic cells, when irradiated in vitro, also showed smaller D0 values for the recurrent tumors than the original tumor. Hypoxic cell fractions were between 0 and 14% in the late recurrences and 10% in the original tumor. These results are consistent with the hypothesis that radiotherapy causes mutation of tumor cells which results in increased radiosensitivity of surviving tumor cells.

[Effect of combined use of radiation and twice intratumor administration of OK-432 at one week intervals against a murine fibrosarcoma (NFSA)].

Effect of combination use of radiation and multiple administration of OK-432 was studied using a radio-resistant and weakly immunogenic murine fibrosarcoma (NFSa) originated spontaneously in a C3H female mouse. Mice implanted the tumor in the leg were locally irradiated with 40 Gy of gamma rays and locally administered total 8 KE of OK-432. The first group was given 1 KE daily for 8 days, 2 KE every other day in the 2nd group, 4 KE at 1 week interval in the 3rd group and 8 KE immediately after irradiation in the 4th group. The tumor volume at day 19 after irradiation was compared. The tumor growth inhibitory effect was observed in the 3rd group (p less than 0.01) and 4th group (p less than 0.05) compared to the group given radiation alone. This effect was seen at even more smaller dose of OK-432 as 0.5 KE by 2 times administration at 1 week interval. The effect of route of OK-432 administration was studied comparing the tumor volume at day 22. OK-432 was given 2 times at 1 week interval. A significant difference was seen in the group given OK-432 locally compared to the control group given radiation alone (p less than 0.001). There were no significant differences between the groups given OK-432 by the routes of i.p., s.c. i.v. and the control group. TCD50 value was examined giving OK-432 4 KE on the day and 2 KE on day 7 after irradiation. TCD50 value in the group treated with irradiation alone was 83.5 (79.6-87.4) Gy and the group given combination therapy with OK-432 was 60.7 (55.9-65.4) Gy. Two times OK-432 administration at 1 week interval appears to potentiate the radiation response by about 23 Gy.

[Study on postoperative deep venous thrombosis with reference to prevention].

Preventive effect of intermittent sequential pneumatic compression of the legs (ISPC) on the postoperative deep venous thrombosis (DVT) was studied by 125I-fibrinogen uptake test in 64 surgical patients. Furthermore the mechanism of preventive effect by ISPC was analysed from the point of view of coagulation and fibrinolytic activity in 78 patients. Following results were obtained. 1) The incidence of DVT in control group was 18.0 percent, while it was 6.3 percent in ISPC group (p less than 0.01). 2) The euglobulin lysis time in ISPC group was significantly shorter than that of control group on the first postoperative day (p less than 0.01). 3) There were no definite changes in plasminogen and B beta 15-42 peptide between control group and ISPC group. 4) There were no complications in using of ISPC. This study demonstrates that ISPC has a dual action; (1) mechanical prevention of venous stasis and (2) fibrinolytic acceleration. ISPC are effective for prevention of postoperative DVT.
