A case of accessory mitral valve leaflet associated with solitary mitral cleft.

Accessory mitral valve leaflet is a rare congenital anomaly. More than half of the cases show other congenital cardiac defects and almost all of the cases show subaortic obstruction. We report a case of an accessory mitral valve tissue without outflow obstruction associated with mitral cleft of the posterior mitral leaflet. To our knowledge, this is the first reported case of the combination of these two congenital anomalies.

Effects of morphine and D-Ala2-D-Leu5-enkephalin in the seizure-susceptible El mouse.

Opioid agonists were used to investigate the modulation of seizures in the seizure-susceptible El mouse. Morphine and D-Ala2-D-Leu5-enkephalin (DADLE) were injected subcutaneously or intracisternally as prototypic agonists for mu and delta opioid receptors. Systemic or intracisternal injection of both morphine and DADLE decreased the incidence of seizures and the seizure score in El mice in a dose-dependent manner. The anticonvulsant effects of morphine and DADLE were reversed by naloxone (2 mg/kg, s.c.). This implies that opioid agonists have anticonvulsant properties which are mediated by mu and delta opioid receptors. In conclusion, a deficit in endogenous opioid peptides, which act as anticonvulsants may play a significant role in the etiology or pathophysiology of seizures in the El mouse.

[Combined medical and surgical treatment of 74 cases of acute type III aortic dissection].

In the early period up to 1986, our treatment of acute type III dissection was anti-hypertension drug therapy as a rule, and Collins operations were performed in two cases of progressive hemothorax. Among 21 patients receiving medical therapy, five died of rupture, and three operated cases died of multiple organ ischemia, and then the hospital mortality was 40%. Since 1987, we have selected hypotensive treatment of strictly maintaining blood pressure less than 120mmHg for the completely thrombosed type of the dissected lumen, and the emergency operation of ringed intraluminal graft insertion (RIG operation) for the blood-flow type and aneurysm formation type of the dissected lumen, diagnosed by the emergency cine-angiography. As the result, among 51 cases having hypotensive therapy, one died of respiratory failure. In the 23 operated cases, in which RIG operation and/or arterial reconstruction was performed, four died of multiple organ ischemia. The hospital mortality was 8%, which was significantly improved compared with that of the early period.

Crystallization of allosteric L-lactate dehydrogenase from Thermus caldophilus and preliminary crystallographic data.

L-Lactate dehydrogenase of Thermus caldophilus GK24 was purified from Escherichia coli containing an overexpression plasmid. The enzyme was crystallized from polyethylene glycol 6000 solutions without ligands by the hanging drop vapor diffusion method. Two forms of crystals were obtained. The crystals grown at pH 6.0 were characterized by means of an X-ray diffraction experiment, while those grown at pH 6.5 and 7.0 did not give detectable diffraction spots. The crystals grown at pH 6.0 belonged to monoclinic space group P2(1), the cell dimensions being a = 54.8 A, b = 138.2A, c = 86.1 A, and beta = 93.3 degrees. These crystals diffract to beyond 2.5 A spacing and are stable on X-ray irradiation.

Developmental and regional alteration of methionine enkephalin-like immunoreactivity in seizure-susceptible E1 mouse brain.

Methionine enkephalin-like immunoreactivity (ME-LI) in the brain of El mice (seizure-susceptible strain) was measured by radioimmunoassay (RIA) to elucidate the relation between seizures and the opioid system. The lyophilized supernatants of tissue extracts were subjected to ME RIA. The concentration of ME-LI in 25-day-old El mice that had no seizures was significantly decreased in the hippocampus. At the age of 50 days when El mice displayed abortive seizures, the levels of ME-LI in both El(+) and nonstimulated El(o) mice were also significantly reduced in the hippocampus and septal area. It was further shown that the ME-LI concentrations in both 150-day-old adult El(+) during interictal periods and El(o) mice were markedly decreased in the cerebral cortex, septal area, and striatum, as compared with the corresponding regions in ddY mice (seizure-nonsusceptible strain; the mother strain of El). The decrease of ME-LI in the El mouse brain was generally compatible with our previous findings concerning the up-regulation of opioid delta receptors in this species. These results suggest that the reduction of ME-LI in the El mouse brain is not due to convulsions, but could be associated with the pathogenesis of seizure diathesis and seizure manifestations in the El mouse.

A tuberculous pseudoaneurysm of the thoracic aorta presenting as massive hemoptysis--a case of successful surgical treatment.

Tuberculous aneurysms of the aorta need early diagnosis and prompt surgical intervention because if untreated, they lead to severe consequences. We report herein, a case of a 63 year old woman who underwent successful resection of a tuberculous pseudoaneurysm which had ruptured into the left upper lobe of the lung after a punch biopsy, performed under bronchoscopy, had caused severe bleeding. Subsequent CT and MRI examinations suggested a fistula between the mass and the aorta and proved useful in establishing the diagnosis of a pseudoaneurysm. We wish to emphasize the need for bronchoscopy to be done carefully because of the risk of inducing massive hemoptysis.

Resection of the superior vena cava for primary lung cancer: 5 years' survival.

We describe a patient with squamous cell carcinoma of the right lung that required a resection of the superior vena cava combined with a tracheal sleeve pneumonectomy. The superior vena cava was totally replaced with a polytetrafluoroethylene graft with 115 minutes cross-clamping of the superior vena cava. The patient remains healthy and the polytetrafluoroethylene graft remains patent 5 years 4 months after operation.

Cloning and identification of a rabbit sperm membrane protein gene.

Antibodies raised against rabbit sperm membrane protein (rSMP-B) interacted with a rat testis cytosolic protein with an estimated molecular weight of 70 kD. The clone expressing rSMP-B was isolated by epitope selection from a rat testis lambda gt11 vector cDNA library. An Eco R1-cut 2.0 kb cDNA fragment encoding rSMP-B was identified. The cDNA fragment (RSD-1) was isolated, subcloned and restriction mapped. Dot hybridization analysis of mRNA prepared from human, rabbit and rat testes showed that the gene encoding rSMP-B is expressed in the testes of all three mammals.

[Torsion after left lower lobectomy and cerebral infarction following the re-exploratory thoracotomy: a case report].

A 38-year-old man had a left lower lobectomy for pulmonary carcinoid. Following the operation, torsion of the left residual upper lobe occurred. Re-explosive thoracotomy was performed on the second postoperative day. The left upper lobe showed a clockwise 180-degree rotation. Pneumonectomy was not done. After the re-thoracotomy, the patient developed right hemiplegia. Head CT showed a cerebral infarction due to the thrombus of pulmonary vein that was released after the repair of the torsion.

[Fontan-type operation in a 14-month-old patient with tricuspid atresia who had [I, D, D], pulmonary atresia and right pulmonary artery stenosis].

A 14-month-old male with tricuspid atresia who had [I, D, D], pulmonary atresia, right pulmonary stenosis and a Blalock-Taussig shunt successfully underwent a Fontan-type operation accomplished by making a direct right atrial-pulmonary arterial connection with a Gore-Tex roof patch. The patient had an uneventful postoperative course without development of the heart failure.

Conformation of NAD+ bound to allosteric L-lactate dehydrogenase activated by chemical modification.

On modification of arginine residues with 2,3-butanedione, the Thermus caldophilus L-lactate dehydrogenase is converted to an activated form that is independent of an allosteric effector, fructose 1,6-bisphosphate (Fru-1,6-P2). The conformation of NAD+ bound to the modified enzyme in the absence of Fru-1,6-P2 was investigated by means of proton NMR, analyzing the time dependence of the transferred nuclear Overhauser effect (TRNOE) and TRNOE action spectra. The inter-proton distances determined on TRNOE analysis indicated that both the nicotinamide riboside moiety and the adenosine moiety of NAD+ were in the anti conformation, the ribose rings being in the C3'-endo form. This conformation was almost the same as that of NAD+ bound to the native enzyme-Fru-1,6-P2 complex, rather than that of NAD+ bound to the free native enzyme. These results suggest that the C3'-endo-anti form of the enzyme-bound NAD+ is essential for the activation of the T. caldophilus L-lactate dehydrogenase.

Presentation of exogenous protein antigens by dendritic cells to T cell clones. Intact protein is presented best by immature, epidermal Langerhans cells.

The capacity of dendritic cells to present protein antigens has been studied with two MHC class II-restricted, myoglobin-specific, T cell clones. Spleen dendritic cells and cultured epidermal Langerhans cells (LC) presented native myoglobin weakly and often not at all. These same populations were powerful stimulators of allogeneic T cells in the primary MLR. Freshly isolated LC were in contrast very active in presenting proteins to T cell clones but were weak stimulators of the MLR. Both fresh and cultured LC could present specific peptide fragments of myoglobin to the clones. These results suggest that dendritic cells in nonlymphoid tissues like skin can act as sentinels for presenting antigens in situ, their accessory function developing in two phases. First antigens are captured and appropriately presented. Further handling of antigen then is downregulated while the cells acquire strong sensitizing activity for the growth and function of resting T lymphocytes. The potent MLR stimulating activity of cultured epidermal LC and lymphoid dendritic cells probably reflects prior handling of antigens leading to the formation of allogeneic MHC-peptide complexes.

[Pathological findings of the aortic wall at the site of ligation against the rigid rings of an intraluminal graft for acute DeBakey type I aortic dissection].

A ringed intraluminal graft has been utilized in the surgical treatment for dissecting aneurysm since 1978. Histological findings of the aortic wall long after surgery, however, have not been described in detail, and concern remains about possible pressure necrosis at the point of encirclement. The following report is for a case of acute type I aortic dissection which required the use of an intraluminal surgical graft in a patient who died eight months postoperatively of a disease unrelated to previous surgical management. The pathological findings at autopsy were as follows. At the site of circumferential ligations around the aorta, 1) discontinuity of the elastic fibers in the media was found only at the outer surface, 2) there was no compression necrosis in the outer half of the media, although a dark shade of elastic fibers was recognized, 3) there were no pathological changes in the inner half of the media. In addition, complete repair of the intimal tear as well as closure of the false lumen replaced by collagen fibers was confirmed histologically in the whole length of aorta where the intraluminal graft had been placed. We conclude that concerns about the fragility of the aortic wall at the site of circumferential ligation, the migration of prosthesis, and the formation of thrombi is alleviated by these pathological observation.

[Influences of irradiation on the anastomotic sites of artificial grafts].

To examine the acute influences of irradiation on the anastomotic sites of EPTFE vascular grafts, an experimental study was performed using dogs. After replacement of superior vena cava and bilateral femoral arteries with EPTFE grafts, a total of 20 to 100 Gy doses were given at the anastomotic sites of superior vena cava and right femoral artery. There was no degradation of grafts themselves or rupture in the anastomoses at cumulative radiation doses of 20 to 100 Gy. However, in many dogs, receiving over 50 Gy doses at the anastomotic sites of superior vena cava, stenoses and/or overgrowth of granulation tissues were observed, although at those of right femoral artery, no remarkable differences were observed between dogs which received and those which did not receive irradiation.

Induction of interleukin 1 alpha mRNA during the antigen-dependent interaction of sensitized T lymphoblasts with macrophages.

DNA-RNA hybridization with an IL-1 alpha cDNA probe was used to monitor the induction of IL-1 in macrophages that were acting as accessory cells for the proliferation of T lymphocytes. Mouse peritoneal macrophages bound and stimulated T lymphocytes in the presence of the mitogens, Con A, or anti-CD3 mAb, but little or no IL-1 mRNA was detectable. In contrast, if the T cells were first sensitized in a mixed leukocyte reaction with dendritic cells and then added to macrophages, IL-1 mRNA was clearly induced. Induction of the IL-1 alpha gene seemed to require the recognition of class II MHC products on the macrophage because of the following observations: specific rather than third-party macrophages were responsive to the T blast but not to T cell-conditioned media; induction was blocked by an anti-Ia mAb; CD4+ rather than CD8+ blasts were active; and polyclonal Con A blasts were much less efficient than antigen-specific T cells. Our data indicate that the strongest signal for IL-1 production during the macrophage-T cell interaction occurs in the efferent limb of the response, after rather than before the formation of class II MHC-restricted T lymphoblasts.
