Preoperative evaluation by laser interferometry in cataractous eyes with retinitis pigmentosa.

Measurements of visual acuity were carried out preoperatively by laser interoferometry in 16 cataractous eyes of 13 patients with retinitis pigmentosa who were older than 60 years of age. Subsequently, all patients underwent uneventful extracapsular cataract extraction and posterior chamber intraocular lens implantation. Postoperative visual acuity, which was evaluated as the best acuity during the first postoperative year, was improved in all eyes and it correlated well with the preoperative acuity determined by laser interferometry. Laser interferometry can be useful in predicting postoperative acuity and deciding on cataract surgery for elderly cataractous patients with retinitis pigmentosa.

Photopic electroretinogram B-wave implicit time in monkey eye.

Photopic electroretinograms were elicited in anesthetized monkey eyes with stimulus light of various intensities under background light of different luminances. The b-wave implicit time was prolonged with increasing stimulus light intensity and shortened with brighter background luminance. While the change of anesthetic conditions produced by the addition of halothane (0.5% or 1.0%) mildly prolonged the implicit time, it affected neither the waveforms nor the relationship between the b-wave implicit time and background luminance.

[Static perimetry and temporal aspects of the photopic electroretinogram in sector retinitis pigmentosa].

Six cases of sector retinitis pigmentosa were studied by automated static perimetry and photopic electroretinogram. Four cases out of six revealed elevated thresholds in the lower half of the visual field, which corresponded to the normally appearing upper eyeground. They also showed delayed implicit time on the photopic electroretinogram. These results show that sector retinitis pigmentosa is not necessarily a regional retinal dystrophy.

Electroretinogram in cone dystrophy.

Electroretinograms (ERGs) in 10 cases of cone dystrophy were studied with special respect to log(bp/bs) that represents the log of photopic ERG amplitude divided by scotopic ERG amplitude. Photopic ERGs were either greatly diminished or nonrecordable, and scotopic ERGs showed either normal or reduced amplitudes. All cases had a low value of log(bp/bs), less than the lower normal limit, indicating significantly greater impairment in cone function than in rod function. These results have proved log(bp/bs) to be helpful in confirming the diagnosis of cone dystrophy especially in cases with recordable photopic ERG and reduced scotopic ERG. Among other ERG parameters, the photopic ERG b-wave implicit time that was determined with averaging and digital amplification of the responses in 4 cases showed prolongation in 3 cases while normal in one case. Oscillatory potentials were nonrecordable or barely recordable in all cases.

[Retinal arterial obstruction and systemic disorders].

The records of 36 patients (37 eyes) with central retinal artery occlusion (CRAO, 19 patients, 19 eyes), branch retinal artery occlusion (BRAO, 15 patients, 16 eyes), and cilioretinal artery occlusion (CIAO, 2 patients, 2 eyes) were reviewed with respect to underlying systemic disorders. Hypertension was the most common disorder (58%). Half of the patients had cardiovascular disorders including ischemic heart disease, valvular disease, atrial fibrillation, internal carotid artery obstruction, and atrial myxoma. Seventeen percent of the patients had diabetes mellitus, and 8% had hyperlipemia. Cerebral infarction was detected in a quarter of the patients. The incidence of the disease was one CRAO patient per 1,000 outpatients. Age distribution showed a peak in the seventh decade. Men were twice more frequently affected than women. Right eyes were more commonly involved.

Two types of EEG spikes in the kindled rabbit hippocampus.

The characteristics of interictal EEG spikes were studied in the rabbit hippocampus. After kindling for 2-4 weeks, rabbits were anesthetized and curarized for acute experiments. There were two types of EEG spikes in the kindled hippocampus; one (B) had its source in the stimulated side, and the other (A) in the contralateral side. In addition, compound EEG spikes (C) were observed that consisted basically of B and A spikes. Intracellular counterparts of A and B spikes were generally a depolarization-hyperpolarization sequence in the CA1 pyramidal cells. On the other hand, counterparts of the C spikes were initial depolarization with a superposed spike burst followed by relatively shorter lasting hyperpolarization which seemed to indicate an enhancement of excitation during the kindling process.

Technetium-99m pertechnetate and gallium-67 imaging in salivary gland disease.

Thirty-two patients with salivary gland tumors or sialadenitis were studied with Tc-99m pertechnetate and Ga-67 imaging and, in some instances, sialography. The diagnostic algorithm presented allows the correct categorization of the salivary gland pathology in the vast majority of patients. The patients were studied serially with Tc-99m pertechnetate, Ga-67 and in certain situations sialography (or CT-sialography). Use of the algorithm can distinguish benign salivary tumors from malignant tumors and malignant tumors from inflammatory disease. The limitations and pitfalls of interpretation are discussed.

Further phenotypic characterization of target cells for bovine leukemia virus experimental infection in sheep.

To determine the phenotype of target cells for bovine leukemia virus (BLV) infection in sheep, we analyzed blood lymphocytes from BLV-infected clinically healthy and leukemic sheep by use of monoclonal antibodies. In clinically healthy and leukemic sheep that were BLV-infected, the blood concentration of T lymphocytes was within normal values, but the number of B lymphocytes was increased in several cases. In addition, the number of blood lymphocytes expressing the BLV antigen correlated well with that of B lymphocytes. Double immunofluorescence staining demonstrated that lymphocytes expressing BLV antigens bore B-cell but not T-cell surface markers. Moreover, neoplastic cells in the lymph nodes of leukemic sheep were stained immunohistochemically with an anti-B monoclonal antibody but not with any of anti-T monoclonal antibody tested, indicating that tumor cells are of B-lymphocyte origin. Collectively, these results show that BLV antigen-positive cells obtained from BLV-infected sheep that have no clinical signs and BLV-induced lymphosarcoma cells belong to the B-lymphocyte lineage.

Monoclonal S-100 immunocytochemistry of pigmented naevi with tactile-like corpuscles.

We examined 12 cases of pigmented naevi with tactile-like corpuscles by immunohistochemistry using polyclonal and monoclonal antibodies against S-100 protein. Naevus cells, the tactile-like corpuscles, and the attached lamellae and fibroblastic cells gave a strong reaction with S-100 protein and the beta-subunit antibodies. The same cells gave only a weak reaction with the alpha-subunit antibody with the exception of about 4 per cent of corpuscles and 1-1.5 per cent of naevus cells, whether pigmented or not, which gave a strong reaction with this antibody. These results suggest that the non-pigmented naevus cells and the constituent cells of the corpuscles are closely related and the latter might derive from the former by organoid differentiation.

A marker epitope of attenuated Plasmodium berghei.

Plasmodium berghei XAT is an irradiation-induced, permanent attenuated derivative from high-virulence P. berghei NK65. Monoclonal antibodies against XAT were developed. By immunofluorescence screening, one monoclonal antibody was identified that was reactive with XAT at the schizont stage but not with NK65 nor with any other stage of intra-erythrocytic development of either parasite. The monoclonal antibody precipitated a 240-kD molecule from metabolically labeled XAT antigens. This molecule was thought to be a marker epitope of the attenuated parasite.

Simultaneous recordings from two types of hippocampal nonpyramidal cells during electrically induced paroxysmal discharges.

To reveal the relationship between hippocampal nonpyramidal cells and seizure activity, single-unit activities of nonpyramidal cells were studied during electrically induced paroxysmal discharges in the hippocampal CA1 region of lightly anesthetized rabbits. Type I nonpyramidal cells (N = 21) that were located in or near the pyramidal cell layer tended to cease firing during the early part of paroxysmal discharges, but refired during the late part. In contrast, type II nonpyramidal cells (N = 14) which were found in the inferior subzone of the stratum oriens continued to fire during the entire course of the paroxysmal discharge.

Serum cholesterol level in normal people: association of serum cholesterol level with age and relative body weight.

Serum cholesterol level was studied in normal subjects in Kasakake Village. The serum cholesterol levels increased significantly with age, from the third to the fifth decade in males and from the third to the seventh decade in females. Thereafter, the levels were maintained in males while declined in females. The mean peak values (+/- SD) were 178 +/- 31 mg/100 ml in males and 207 +/- 37 mg/100 ml in females. The presumptive values of the zero-year-old obtained from the regression lines calculated from the plot of serum cholesterol values against age were 129 mg/100 ml and 112 mg/100 ml in males and females respectively. Throughout the age-range examined in females, the serum cholesterol level was well correlated with the relative body weight determined with modified Broca's method. A similar, but less obvious correlation was demonstrated in males. However, there was not comparable change in the relative body weight against the trend of the serum cholesterol level in both sexes. In the babies normally delivered with full term, the mean cholesterol level (+/- SD) in the umbilical cord blood serum was 65 +/- 13 mg/100 ml and it increased to 150 +/- 46 mg/100 ml during one to three months after birth which was very close to the presumptive values obtained from the regression lines in the adults. There also was significant correlation between the cholesterol value and the body weight. It is concluded that in normal people, age and relative body weight are major and independent determinants of serum cholesterol level from the start of their life.

High risk of systemic embolism in hypertrophic cardiomyopathy.

Six of 66 patients with hypertrophic cardiomyopathy had an embolic event during an average observation period of 3.73 years. Five of these had probable cerebral embolism, while 1 patient had a femoral artery embolism. The calculated incidence of systemic embolism was 2.4% per patient year. All embolic events occurred in the patients with atrial fibrillation. In comparison to patients without systemic embolism with or without atrial fibrillation, the cardiothoracic ratio was greater, the cardiac index was lower and the left atrial dimension tended to be greater in those patients with systemic embolism.

[Aneurysm of a diverticulum of the ductus arteriosus in an adult: a case report].

An aenurysm of a diverticulum of the ductus arteriosus in a 33-year-old man was presented. The chest radiography revealed a mass obscurring the aortic window above the hilus of the left lung. His blood pressure was 130/70 mmHg, and there was no difference in pressures between the upper and lower or between the right and left extremities. A CT scan of the chest showed 30 X 34 mm angioma adjacent to the descending aorta and pulmonary artery. Aortography and contrast radiography of the right ventricle revealed a cystic pedunculated aneurysm at the superior portion of the descending aorta. The base of the left pulmonary artery was slightly displaced. There was no communication between the aorta and pulmonary artery, nor was any significant difference in pressure between the ascending and descending aorta. This case was diagnosed as an aneurysm of the diverticulum of the ductus arteriosus, and then it was resected. The abnormality was confirmed by the localization of the aneurysm and histopathologic findings, but the presence of the ligamentum arteriosum was not confirmed. This is the 23rd case of the aneurysm of the diverticulum of the ductus arteriosus in adults, and the first case in Japan in which the diagnosis was made in a living patient, followed by successful surgery.
