Should coronary arteries with less than 60% diameter stenosis be treated by angioplasty?

We evaluated all patients receiving percutaneous transluminal coronary angioplasty (PTCA) in the past year for mild stenosis (60% or less diameter narrowing, n = 64, group 1) and compared them with a random sample of 330 patients with greater than 60% stenosis (n = 66, group 2) treated during the same year. The degree of coronary stenosis before PTCA was 52 +/- 7% (mean +/- SD) in group 1 and 79 +/- 11% in group 2. The primary success rate was 90% (58 of 64 patients) in group 1 vs 86% (57 of 66 patients) in group 2. The incidence of complications requiring coronary surgery after PTCA failed was similar in both groups (3 of 64 in group 1, 4 of 66 in group 2), but there were four occurrences of myocardial infarction in group 1 and none in group 2 (p less than .05). Recurrence of stenosis was judged on the basis of objective data, 76% of which were angiographic data, in 97% of the patients with primary success. At a mean interval of 5 months with a mean follow-up period of 7 months, 17 of 58 patients (29%) with primary success in group 1 and 24 of 57 patients (42%) in group 2 developed restenosis. In group 1, restenosis was markedly more severe (73 +/- 15%) than initial stenosis (p less than .005), which was not the case in group 2. In conclusion, PTCA in mild stenosis has favorable primary and long-term results, yet carries the risk of myocardial infarction and emergency operation and may, in some cases, even accelerate the disease process.

BPD2. An autosomal dominant form of bipolar affective disorder.

Seven pedigrees with bipolar affective disorder (BPD2) have been found to show a pattern consistent with autosomal dominant genetic inheritance in linkage with, but distal to, HLA on the short arm of chromosome 6. Segregation analysis of subjects by narrow criteria yielded lod score 8.02 at theta = 0.15. Four pedigrees of three to five generations contributed most to the total score. Note is made of high concordance of marker HLA haplotype in affected sibs.

The effect of cholestyramine and colestipol on the absorption of hydrochlorothiazide.

The effect of cholestyramine and colestipol on the absorption of a single orally administered dose of 75 mg hydrochlorothiazide was determined. Both urinary excretion and plasma levels of hydrochlorothiazide were measured. Cholestyramine or colestipol administration decreased the total urinary excretion of hydrochlorothiazide by 85 and 43%, respectively. Similar effects were also noted on plasma hydrochlorothiazide levels.

Two genetically distinct forms of bipolar affective disorder?

In five pedigrees in which bipolar affective illness (BPD) can be traced through three generations with no father-son transmission, no linkage to HLA could be established. This group was evidently dependent on a single dominant gene (X-linked?), for which the noncommittal designation BPD1 was proposed. Two pedigrees of BPD, in which father-son transmission occurred at least twice, showed highly suggestive linkage to HLA (lod = 2.61 at theta = 0.15). BPD2 was proposed as the designation for this gene. Loss of BPD in one HLA-A/B cross-over suggests that a single dominant gene in this group has a locus in p2-pter region of chromosome 6.

Effects of pectin and cellulose on growth, feed efficiency, and protein utilization, and their contribution to energy requirement and cecal VFA in rats.

Graded levels of cellulose (0 to 20%) or of pectin (0 to 10%) were incorporated in purified diets with either 22 or 8.5% casein as the protein source. Weanling male rats were fed these diets for 4 weeks. Body weight gain, food consumption, feed efficiency, protein biological value, apparent digestion of food, fiber and protein, cecal weight and cecal volatile fatty acids (VFA) were recorded. Cellulose was largely and inert diet diluent causing increased weight gains when protein was limiting but not when protein was sufficient. The 25% disappearance in the gut was due, at least in part, to compaction in the ceca and in increased cecal VFA. Pectin caused progressively greater depressions in body weight gain, food intake and efficiency and protein digestion at both levels of dietary protein. At least part of the 75% disappearance of pectin during passage through the gut could be attributed to increased cecal VFA. However, a rat growth bioassay for energy showed that pectin as well as cellulose, agar and gum arabic contributed no measurable energy to young rats.

Spontaneous activities of female versus male newborns.

This study explored potential sex differences in all activity spontaneously emitted by newborns. 2 groups of full-term infants (14 uncircumcised males, 15 females), equally distributed along several physical and demographic variables, served as subjects. Each newborn was observed for a total of 8 hours during the 2 days following birth. Observational procedures were stringently controlled and interobserver reliability ranged from .84 to .98. Results showed higher levels of wakefulness, facial grimacing, and low-intensity motor activity for males. Of marginal significance, both low-intensity and high-intensity oral activity was higher for males. In addition, a marginally significant interaction effect (sex X interfeeding interval X day) was found for high-intensity oral activity (crying). Independent of sex, high-intensity activity increased in the evening and over an interfeeding period, and low-intensity oral activity increased over days. The pattern of results indicates that even uncircumcised males may be slightly more irritable than females, and thus more active. Potential implications of these findings are considered.

The effects of discontinuing long-term diuretic therapy in the elderly.

The effects of discontinuing long-term diuretic therapy were investigated by means of a double-blind randomized controlled trial. A total of 141 elderly patients in the long-stay wards of six hospitals were found to be taking maintenance diuretics, and for 33 of these the drugs were judged to be mandatory. Of the remaining patients, 52 continued to receive diuretics while 54 were given matching placebo tablets. Eight in the latter group required diuretic therapy to be resumed during the following 12 weeks. The main change observed in patients who completed the trial was a slight increase of ankle oedema in the placebo group. Plasma potassium levels below 3.5 mEq/1 were found in some patients taking diuretics but not when the drugs had been withdrawn for 12 weeks. Blood pressure rose slightly in the placebo group and plasma urea rose slightly in the diuretic group. It was concluded that old people receiving long-term diuretic therapy without obvious current indication should have the drugs withdrawn under careful supervision so that those needing them could be identified.

Pancreatic lesions induced in rabbits and guinea-pigs with pancreatic antigens.

Rabbits and guinea-pigs were immunized with various pancreatic antigens in Freund's adjuvant. Rabbits received unfractionated bovine insulin and the "A" component and "single peak" insulin separated from it by gel-filtration. All produced antibodies capable of reacting with porcine insulin but none were found to have pancreatic lesions when killed up to 6 weeks after initial injection. Guinea-pigs immunized with bovine "A" component developed pancreatic peri-ductulitis which appeared most frequently (10/20) in animals killed 30 days after a single injection and less frequently in animals killed after 60 (4/10) and 90(1/10) days. Similar lesions were found in only a small proportion of control animals (2/23) or of guinea-pigs immunized with single peak bovine insulin (3/22). Guinea-pigs immunized with homogenates of homologous and heterologous islets of Langerhans developed signs of peri-ductulitis in a high proportion of animals killed up to about 60 days after first injection (18/26). None of these animals exhibited clearly defined signs of diabetes mellitus and the incidence of induced lesions could not be correlated with levels of circulating insulin-binding antibodies.

Echocardiographic manifestations of postinfarction ventricular septal rupture.

The echocardiographic features of three patients with postinfarction ventricular spectal rupture are described. All patients showed a decreased or paradoxical motion on the ventricular septum, and two of the patients demonstrated an unusual motion of the tricuspid valve. There were no abnormalities in mitral valve motion. The echocardiogram can be helpful in the diagnosis of postinfarction ventricular septal rupture and can assist in distinguishing this condition from acute disruption of the mitral valve complex.
