Effects of beraprost sodium on canine cauda equina function and blood flow using a chronic spinal cord compression model.

Changes in blood flow after chronic compression were observed in 19 dogs after 10 mmHg compression for 1 week before and 1 hour after the intravenous administration of one of three doses of beraprost sodium (BPS; 30 ng x kg(-1) x min(-1), n = 7; 100 ng x kg(-1) x min(-1), n = 7; and 300 ng x kg(-1) x min(-1), n = 5). The speed of blood flow was calculated using a specially designed microscope equipped with a video camera. Dogs treated with BPS had lesser degrees of reduction in their nerve conduction velocity compared with controls. A vascular mechanism of injury likely explains why BPS-treated dogs had a lesser degree of reduction in their nerve conduction velocities compared with the control population.

Blood flow measurement in experimental chronic cauda equina compression in dogs: changes in blood flow at various conditions.

The purpose of this study was to present a simple method of measurement of blood flow in the nerve root, assess the reliability of this method, and examine the changes of blood flow in chronic cauda equina compression in an experimental setting. A total of 15 dogs were used to determine the blood flow of cauda equina in normal (n = 5), sham (n = 5), and 10 mm Hg compressed cauda equina for 1 week (n = 5). The speed of blood flow was calculated using a specially designed microscope supplied with a video camera. Blood flow in chronic 10 mm Hg compression decreased compared with the sham and normal groups. The kappa value was between 0.75 and 0.96. We conclude that this method might be useful for measurement of blood flow in the nerve root and for confirming the reduction in blood flow of compressed cauda equina.

Histologic change of the meniscus and cartilage tissue after meniscal suture.

The influence of suturing on cell infiltration into the meniscus and surrounding tissue is not well known. Histologic changes in the meniscus after suturing and prediction of histologic changes using magnetic resonance imaging were studied. Forty knees in 20 mongrel dogs were studied using four types of 4-0 suture material: nonabsorbable monofilament, nonabsorbable braided, absorbable monofilament, and absorbable braided. Each type of suture material was used for meniscal suture on eight knees each. The other eight knees were not subjected to meniscal suture. Specimens obtained at 1 and 3 months were studied by magnetic resonance imaging and light microscopic analysis. The maximum width of change of meniscal tissue was measured. Changes from the nonabsorbable suture group were smaller than that of the absorbable suture group in magnetic resonance imaging. Histologic changes as seen by light microscope were larger in the absorbable group than in the nonabsorbable group at 1 and 3 months after surgery. Injury was found in the articular cartilage of the femoral condyle in the nonabsorbable braided suture group. Histologic changes were significantly greater when changes in magnetic resonance imaging signal intensity were larger. The current study showed that non-absorbable monofilament suture material results in the least damage to the meniscus and surrounding tissue.

Lumbosacral transitional vertebrae and nerve-root symptoms.

Transitional vertebrae (TV) may be one of the risk factors for lumbar disc herniation. It is not clear, however, whether the presence of TV can affect the development of nerve-root symptoms. Our aim was to clarify this relationship. A total of 501 patients with lumbar degenerative disease and nerve-root symptoms was studied in respect of their level and the presence of TV. As a control group, 508 patients without low back pain or nerve-root symptoms were studied to establish the incidence of TV. In patients with disc herniation, the incidence was statistically higher and the mean age lower in patients with TV than in those without. In most patients, the symptomatic disc level was just above the TV. Similarly, in those with stenosis of the spinal canal without spondylolisthesis, the symptomatic disc level was most commonly just above the TV.

Removal of polyA tails from full-length cDNA libraries for high-efficiency sequencing.

We have developed a method to overcome sequencing problems caused by the presence of homopolymer stretches, such as polyA/T, in cDNA libraries. PolyA tails are shortened by cleaving before cDNA cloning with type IIS restriction enzymes, such as GsuI, placed next to the oligo-dT used to prime the polyA tails of mRNAs. We constructed four rice Cap-Trapper-selected, full-length normalized cDNA libraries, of which the average residual polyA tail was 4 bases or shorter in most of the clones analyzed Because of the removal of homopolymeric stretches, libraries prepared with this method can be used for direct sequencing and transcriptional sequencing without the slippage observed for libraries prepared with currently available methods, thus improving sequencing accuracy, operations, and throughput.

[Experimental study of periodontal tissue regeneration after the application of enamel matrix derivative in rat periodontal defects].

The purpose of this study was to evaluate periodontal wound-healing after the application of enamel matrix derivative (EMD) in rats. Periodontal defects were surgically created on the mesial side of the first maxillary molar of 24 male Long-Evans rats. EMD was applied to cover the denuded root surfaces in the experimental group. The contralateral molar was used for the control group, which received the same treatment without EMD. The rats were sacrificed at 2 weeks (8 rats), 4 weeks (8 rats), and 8 weeks (8 rats) after the surgery. Demineralized paraffin sections were stained with Masson's trichrome. Histological analysis and histomorphometric measurements were performed on the periodontal sections. Using an immunohistochemical technique, the localization of osteocalcin (OC) was also examined. The formation of new cementum was statistically significant in the experimental group, especially new cementum with extrinsic fiber. Both acellular and cellular cementum were also rather strongly observed in the experimental group. Epithelial down growth was also strongly inhibited in the experimental group at 8 weeks after the surgery. OC-positive cells and matrix were limited at the bottom of the defects in the control group, while positive reaction was detected not only at the bottom but also spreading to the coronal portion of the defects in the experimental group. These results suggest that EMD has potential to promote cementum regeneration, especially fiber-inserted cementum, and to create a favorable environment that will promote periodontal regeneration.

[Efficacy of modified ultrafiltration in reoperation for valvular disease].

We evaluated the efficacy of modified ultrafiltration (MUF) in reoperation for valvular disease. Fourteen patients were divide into two groups consisting of a control group (n = 6) and a MUF group (n = 8). MUF was carried out for fifteen minutes immediately after the completion of cardiopulmonary bypass. The blood flow through the ultrafilter was 300 ml/min and about 1,200 ml of water was removed. The hematocrit elevated significantly from 25% to 31% in the MUF group (p < 0.05). The percentage of the increase in body weight after the operation in the MUF group was significantly less than that in the control group (3.3 +/- 3.1% vs 8.3 +/- 4.3%, p < 0.05). The PaO2/FIO2 after the operation in the MUF group was significantly higher than that in the control group (376 +/- 125 mmHg vs 242 +/- 79 mmHg, p < 0.05). The duration of mechanical ventilation in the MUF group was significantly less than that in the control group (1.1 +/- 1.1 days vs 5.3 +/- 3.3 days, p < 0.05). In conclusion, MUF is useful to hemoconcentrate, reduce postoperative body weight gain and promote early recovery of pulmonary function in reoperation for valvular disease.

[Risk factors of stomach cancer].

The risk factors for stomach cancer are reviewed. A family history of stomach cancer, smoking habit, highly salted foods and Helicobacter pylori are risk factors, while fresh vegetables, fruits and green vegetables are preventive ones. Stomach cancer may arise mainly from moderately but not severely atrophic gastric mucosa, where atrophic change is ongoing. New strategies for primary and secondary prevention of stomach cancer should be built utilizing these recent findings.

Anomalous subaortic position of the brachiocephalic vein associated with Tetralogy of Fallot.

The left brachiocephalic vein is found in an anomalous position less frequently than the superior vena cava or azygous channels in thoracic venous systems. We experienced a rare case of anomalous left brachiocephalic vein which was clearly demonstrated by spiral computed tomography (3D-CT). Although the malformation in itself seems to be of no functional importance, we assessed its importance in terms of associated conditions and its relevance to subsequent operations.

[Minimally invasive stereotactic functional surgery using an intravenous anesthetic propofol and applying Image Fusion and AtlasPlan].

Image-guided stereotactic surgery of the ventralis intermedius nucleus of the thalamus, globus pallidus, and subthalamic nucleus is a prevailing modality as a treatment of movement disorders. This technical note describes a method of minimally-invasive stereotactic functional surgery for patients with parkinsonian symptom or various tremors. Patients were administered propofol, an intravenous anesthetic, during placement of a burr hole and a stereotactic frame, but not the period when the patients were necessary to be awake. The intravenous anesthesia was very beneficial to relieve local pain and mental stress of the patients. Radionics ImageFusion and AtlasPlan were used for defining the target localization without an intraoperative ventriculography. ImageFusion efficiently fused high-resolution MR images on CT images. AtlasPlan accurately corrected the localization of the tentative target point after measurements from the midpoint of the anterior commissure-posterior commissure line on the modified MRI, and enabled us to reduce the degree of an intraoperative correction to fix a final target. Stereotactic functional surgery has been thought to be less-invasive, however further modifications of surgical procedure and intraoperative medication can make this type of surgery much more less-invasive.

[Efficacy of modified ultrafiltration in coronary artery bypass grafting].

We evaluated the efficacy of modified ultrafiltration (MUF) in coronary artery bypass grafting. Twenty patients were divide into two groups consisting of a control group (n = 11) and a MUF group (n = 9). MUF was carried out for fifteen minutes immediately after the completion of cardiopulmonary bypass. The blood flow through the ultrafilter was 300 ml/min and about 1,200 ml of water was removed. The hematocrit elevated significantly from 25% to 30% in the MUF group (p < 0.01). Postoperative blood loss in the first 24 hours in the MUF group was significantly less than that in the control group (8 +/- 2 ml/kg vs 12 +/- 4 ml/kg, p < 0.01). There was no statistical difference in the percentage of the increase in body weight after the operation, inflammatory reaction and pulmonary function (A-a DO2, PaO2/FIO2 and duration of intubation) between two groups. In conclusion, MUF is useful to hemoconcentrate and reduce postoperative blood loss in coronary artery bypass grafting.

[Long-term results of Rygg's monocusp ventricular outflow patch for the reconstruction of right ventricular outflow tract in tetralogy of Fallot].

Thirty surviving patients after corrective surgery for tetralogy of Fallot with right ventricular outflow tract reconstruction (RVOTR) using monocusp ventricular outflow patch (MVOP) were reviewed retrospectively to determine the long-term results. The age at operation ranged from 2 to 55 years with a mean of 19 years, and follow-up extended to 18.2 years (cumulative: 345.4 patient-year). There were 4 late deaths (1.2% per patient-year), and the cumulative survival rate was 85.3% at 18 years after the corrective surgery. Eight patients (2.3% per patient-year) required intracardiac reoperations mostly resulted from problems after RVOTR with MVOP, such as recurrent stenosis of right ventricular outflow tract (3 cases) or pulmonary valvular incompetence (4 cases). In addition, one patient underwent balloon angioplasty for the recurrent stenosis located in the distal end of MVOP. Freedom from surgical or catheter reintervention for the MVOP-related complication was 60.6% at 18 years after the corrective surgery. MVOP caused compression of the pulmonary artery at the distal end of the anastomosis and reoperation in a younger patients quite early after the corrective surgery. Like other transannular patches, tissue failure and degeneration of MVOP were inevitable, and resulted in severe pulmonary valvular incompetence that required the valve replacement in 4 patients (1.3% per patient-year). Freedom from pulmonary valve replacement was 71.2% at 18 years after the corrective surgery. As long-term results, our experiences emphasize the need for an innovative transannular patch that possesses significantly better long-term durability.

[Low back pain and sciatica].

In order to see the patients with low back pain and/or sciatica, it is most important to consider the pathophysiology of symptoms. We should know that the image findings such as X-ray and MRI do not always show the cause of symptom. Because degenerative changes in lumbar spine are common findings in asymptomatic group compared to symptomatic patients with low back pain and sciatica. According to pathophysiology (nerve root syndrome, cauda equina syndrome, facet syndrome, discogenic pain syndrome and intermittent claudication for low back pain), the treatment for low back pain and sciatica were described.

[Differences in intake of food and nutrients between manual and desk workers].

AIM: To compare intake of food and nutrients between manual and desk workers. SUBJECTS AND METHODS: Subjects were 28,603 residents (13,432 were male and 15,171 were female) in a city. Using a questionnaire, information on frequencies of consumption of different foodstuffs for one year and the type of employment was collected. Intake of nutrients was calculated from the frequencies and compared between manual and desk workers. The comparison was carried out with stratification by gender and adjustment for age and smoking habits. RESULTS: Desk workers took larger amounts of protein and lipids, and smaller amounts of carbohydrates than manual workers. Predominant foodstuffs in the latter was rice wheat products, potatoes and soybean products, while desk workers consumed more meat, processed meat, milk products, fish, vegetables and fruits. CONCLUSIONS: Differences in intake of nutrients and food depending on occupation should be considered, when the relationship between health status and employment is evaluated.
