[Age-related interrelation of vibration perception thresholds and somatosensory evoked potentials in normal subjects].

In order to establish the age-related changes of vibration perception thresholds (VPTs) and somatosensory evoked potentials (SEPs), which are known to share a common afferent pathway in the central nervous system, 119 normal subjects ranging in age from the 20s to the 90s were studied. The VPTs of the index fingertip were measured at 63 Hz, 125 Hz and 250 Hz by a Rion vibrometer, nd the latencies of the SEPs to the median nerve stimulation (N9, N13 and N20) were simultaneously evaluated. The VPT, tended to elevate from the 40s, became increased towards higher frequencies (63 Hz less than 125 HZ less than 250 Hz) in the 80s to the 90s, whereas the VPT in younger ages showed just an opposite pattern (63 Hz greater than 125 Hz greater than 250 Hz). None of the height-corrected latencies of N9, N13, N20 and central conduction time (N20-N13) revealed a significant correlation between the VPTs at any frequencies. The age-related decline of vibration sense, therefore, could not be explained solely by the degenerative changes of he posterior column and/or the delay of the peripheral nerve conduction velocity. This phenomenon was rather thought to be compatible with an age-associated characteristics of Pacinian corpuscles.

Immobilization of fatty acid synthetase from Mycobacterium smegmatis by radiation-induced polymerization.

Fatty acid synthetase of Type I from Mycobacterium smegmatis was immobilized by radiation-induced polymerization of 2-hydroxyethyl methacrylate (HEMA) in the presence of trimethylolpropane trimethacrylate (TMPTMA). The stability of immobilized synthetase toward low ionic strength increased in comparison with the free form, but the stabilities of immobilized preparations assessed by pH and temperature were identical to those of the free form. The apparent Km of immobilized enzyme for acetyl-CoA and malonyl-CoA were both 6 microM, essentially the same as those of the free form; acetyl-CoA, 5 microM and malonyl-CoA, 6 microM.

[Coronary artery bypass in dextrocardia with situs inversus totalis--a case report].

A 71-year-old woman was admitted to our hospital by ambulance, because of right chest pain and hypotension. Chest X-ray and standard 12-lead ECG showed mirror-image dextrocardia with situs inversus totalis. ECG with right-left reversal of all leads showed acute inferior myocardial infarction. The patient underwent coronary bypass surgery due to postinfarction angina, and she is now doing well 2 years following the operation. Mirror-image dextrocardia with situs inversus totalis is very unusual. But the patients are believed to have normal longevity, and presumably have an incidence of atherosclerotic coronary artery disease similar to the general population. To our knowledge, this is the first case of coronary bypass surgery on a patient with mirror-image dextrocardia in Japan.

[Coronary artery bypass grafting for a very rare run off case of superdominant left anterior descending artery].

A superdominant left anterior descending artery (LAD) was found in a 59-year-old man who had electrocardiographic evidence of acute anterior and inferior wall myocardial infarction. Coronary cineangiography showed 95% stenosis at segment 7 of LAD which extended around the apex, ascending into the posterior interventricular sulcus to the crux and ran to the left into posterior atrioventricular sulcus. Posterior descending artery was not provided by the left circumflex artery or the right coronary artery. Multiple infarction due to proximal disease of such a superdominant LAD is very rare in pathoanatomically. Coronary artery bypass grafting to the LAD using left internal mammary artery was performed. The patients is now doing well with no anginal attack, 18 months following the operation.

[The effect of smoking and drinking habit on the process from liver cirrhosis to liver cancer].

A retrospective cohort study of liver cirrhosis cases was carried out. 270 cases of liver cirrhosis which were confirmed histologically from 1972 to 1981 at Juntendo University Hospital, were followed up until the end of 1988. The average observation period for these cases was 74 months or 6.2 years, and we found 141 death cases including 46 of primary liver cancer (PLC). The analysis of survival rate revealed that the smoking group had a slightly higher death rate of PLC than non-smokers, and the ex-smoker group had a much higher survival rate than the continuous smoking group. On the other hand, it was found that the alcohol-drinking group had a much lower death rate of PLC than the non-drinkers group, and that there was no difference of the survival rate among drinkers, non-drinkers and ex-drinkers.

[A case control study of breast cancer, mammary cyst and dietary, drinking or smoking habit in Japan].

A matched pair analysis was done among patients with breast cancer, those with mammary cyst and those without any abnormality in mammary gland by ultrasonic echogram (U.S.) to investigate the relationship between dietary or smoking habit and mammary disease. Breast cancer was diagnosed pathologically, and mammary cyst by U.S.. 49 cancer-normal pairs and 36 cyst-normal pairs were obtained. Breast cancer had significant relationships with a large intake of bread (risk ratio (RR) = 1.60, 95% confidence interval (CI) = 1.03-2.49), margarine (RR = 2.17, CI = 1.20-3.49), cakes (RR = 1.83, CI = 1.18-2.84), soft drinks with sugar (RR = 8.00, CI = 1.33-48.29), meat (RR = 1.48, CI = 1.04-2.10) and a small intake of alcohol (RR = 0.20, CI = 0.06-0.63). Mammary cyst was significantly related with a large intake of fat (RR = 1.56, CI = 1.02-2.38). The result indicates that in Japan women with westernized dietary habit have a larger risk of breast cancer.

[A case of malignant mesenchymoma of the maxilla].

The case of a 48-year-old male with a malignant mesenchymoma of the maxilla is reported. The patient had complained of bloody rhinorrhea and cheek pain, and a total maxillectomy was performed. The tumor consisted of a rhabdomyosarcoma, an osteosarcoma and a chondrosarcoma, which were relatively differentiated. Later, recurrence of a tumor and a lung metastasis occurred, however the tumor revealed undifferentiated mesenchymal cells. This led us to suspect the origin of the malignant mesenchymoma had been a primitive mesenchymal cell. To our a knowledge, the present case is the first reported case of a malignant mesenchymoma with a maxillary origin in the world.

[A study on the endometrial movement and serum hormonal level in connection with uterine contraction].

By means of transvaginal ultrasonography, endometrial activity was investigated in its relationship with uterine contraction and ovarian hormones. The following results were obtained. (1) Endometrial movements were classified into two types: Peristaltic movements and undistinguished ones. Peristaltic movements were rhythmic and mainly found in the proliferative stage, directed from the internal uterine os to the fundus. (2) The detection rate and the frequency of peristaltic movements increased towards ovulation, with the highest values at 88% and 3.9/min, respectively. (3) Peristaltic movements rapidly ceased soon after ovulation and there were practically none after the mid-secretory stage. (4) All endometrial movements decreased in the secretory stage and became markedly quiescent after the mid-secretory stage. (5) In the proliferative stage, a positive correlation was found between the frequency of peristaltic movements and the serum estradiol level (p less than 0.001). In the secretory stage, the detection rate was lowered significantly when the serum progesterone level was more than 3.8 ng/ml (p less than 0.01). (6) Peristaltic movements coincided with uterine contractions both in rhythm and duration. As stated above, endometrial movements were strongly affected by hormonal condition, so they could be of value in the prediction and confirmation of ovulation and in the diagnosis of luteal insufficiency and dysfunctional bleeding as well.

The concepts, procedures, and problems related in endoscopic laser therapy of early gastric cancer. A retrospective study on early gastric cancer.

We report 34 cases of early gastric cancer in which endoscopic laser therapy was performed at the Kitasato University Hospital. The problems encountered were the size and location of the tumor, the presence of lymphatic metastasis or concomitant disorders, and technical problems associated with the irradiation procedure. Of the 34 cases, 22 cases were considered as completely cured (no malignancy was detected after laser therapy), 11 cases were incompletely cured (malignant cells were detected after laser therapy), and 1 case was undetermined.

Classification by position of dorsal root ganglia in the lumbosacral region.

In order to determine the incidence of the proximally placed L5 and S1 dorsal root ganglia, 144 patients with lumbosacral radiculopathy were investigated. All of the patients underwent nerve root infiltration for depicting the nerve root. Using some bony landmarks on the anteroposterior radiculogram, the dorsal root ganglia were classified into three types: intraspinal, intraforaminal, and extraforaminal. The intraspinal type was further subdivided into subarticular and sublaminar types. The intraspinal type is more common in the S1 than in the L5 root, regardless of age and sex. Although the proximally placed dorsal root ganglia may offer a locus minoris resistantiae for neural irritation and compression, its role for symptom production of radiculopathy should be further analyzed.

[Study on thermal pulp testing of immature permanent teeth].

The duration of reaction to cool stimulation was investigated in 185 normal maxillary right and left central and lateral incisors from 55 pediatric patients aged from 6 to 15 years, who were brought to the Department of Pedodontics of Nippon Dental University Hospital, and from 9 students aged from 19 to 20 years in the School of Dental Technology and Hygiene affiliated with Nippon Dental University by using Dichloro-Difluoro-Methane coolants [PULPER (G-C Dental Industrial Corp.) and PULPOFLUORANE (Septodont Co.)] in order to determine the efficacy of thermal pulp testing for mature and immature permanent teeth. The results obtained are as follows: 1) The mean duration of reaction to cool stimulation according to the type of teeth was 0.93 sec for the maxillary left lateral incisors, 0.91 sec for the maxillary left central incisors, 0.88 sec for the maxillary right central incisors and 0.95 sec for the maxillary right lateral incisors, showing no statistically significant difference in the duration between the tooth types. 2) The duration of reaction to cool stimulation according to age was shortened with aging except for the 6-year-old patients. 3) The patients were divided into those 10 years old or younger and those older than 10 years because the tooth roots of the maxillary anterior teeth are developed at 10 years of age. The mean duration of reaction to cool stimulation was 1.02 sec in the group with immature permanent teeth, which consisted of patients aged 10 years or younger, while it was 0.82 sec in the group with mature permanent teeth, which consisted of patients aged 11 years or older.(ABSTRACT TRUNCATED AT 250 WORDS)

[Prognosis for the deciduous molars of juvenile dogs after root canal filling using apatite root sealer].

There are various restrictions on the root canal filling of deciduous roots after root canal treatment unlike in the case of permanent teeth, because deciduous molars undergo physiological root resorption with aging. Among the restrictions, it is most important that the root canal filling material be similarly absorbed and disappear along with the deciduous root so that it does not prevent the eruption of the permanent successors. Hardly any of the root canal filling materials currently on the market, however, are suitable for deciduous molars because some of them disappear earlier than the deciduous root without becoming indurated in the root canal, while some delay tooth replacement because they are resorbed with great difficulty, and continue strongly to stimulate the surrounding tissue of the root apex. Root canal filling materials utilizing alpha-TCP and hydroxyapatite with excellent biocompatibility have recently been put on the market by Sankin Kygyo Co. Type-III, in particular, contains only 5% iodoform, causing only very slight stimulation, and is a relatively good product from a practical standpoint, i.e., filling the root canal using a lentulo. Root canal filling was performed immediately after dental pulp extraction from the deciduous mandibular molars of juvenile dogs to determine the feasibility of filling deciduous teeth with root canal filling materials. Three dogs were then observed for sequential changes in the resorption of the deciduous roots, presence/absence of adverse effects on permanent successors, and influence on the process of eruption by means of radiographic examination every week. The results obtained were as follows: 1. The type-III apatite root sealar or Vitapex used for root canal filling was not absorbed, and there was almost no resorption of the root. Thus, the deciduous teeth on the experimental side were ejected and shed. 2. There was no significant difference in the amount of tooth germ formed between the experimental and control sides. 3. The amount of eruption tended to be delayed because of insufficient resorption of the root of the deciduous predecessor, but this tendency was considered to be due to the influence of the timing of root canal filling and individual difference. In the final analysis, no definite conclusions could be drawn.

Pharmacokinetic analysis in intraperitoneal hyperthermic chemotherapy of far-advanced gastric cancer patients.

The pharmacokinetics of mitomycin C (MMC) has been evaluated in 10 far-advanced gastric cancer patients by means of an intraperitoneal hyperthermic perfusion (IPHP) using MMC to prevent and/or treat peritoneal dissemination. Further, misonidazole (MIS), which was used as a thermosensitizer, also was evaluated. The IPHP was performed for 120 min right after surgical procedure, using a closed-circuit with an inflow perfusate temperature from 46.3 approximately 47.5 degrees C and an outflow temperature of 44.0 approximately 46.0 degrees C. The MMC level in the perfusate was 10 micrograms/ml at the onset of IPHP and thereafter decreased to 1.7 +/- 0.4 micrograms/ml minutes later. The average AUC (area under the curve) in the perfusate was 3.3 micrograms/ml during the IPHP. The MMC level in the peripheral blood was 0.15 +/- 0.04 micrograms/ml 30 min after start of the IPHP and increased to 0.18 +/- 0.05 micrograms/ml at the end of IPHP. Additionally, the MIS level in the peripheral blood was 64.7 +/- 10.3 micrograms/ml 60 min after the IPHP, 60.6 +/- 9.2 micrograms/ml at 120 min, and then decreased to 32.8 +/- 10.3 micrograms/ml at 12 hours after IPHP. The AUC was calculated as being 975 micrograms.hr/ml. Again, the level in the portal vein blood was calculated from the peritoneal permeability and the peripheral blood level of the drug. From data, it was concluded that the perfusate and portal vein blood level of MMC, the peripheral blood level of MIS, as well as the perfusate temperature, are important in providing a favorable, safe, antitumoral treatment.

[Evaluation of digital vibration threshold for asymptomatic ulnar neuropathy in patients with carpal tunnel syndrome].

Quantitative assessment of vibration threshold at 125 Hz of index and little finger tips together with measurements of median and ulnar sensory nerve conduction velocity (SNCV) was performed for 25 limbs of 15 patients with carpal tunnel syndrome (CTS), 25 limbs of 13 disease controls with neurological symptoms similar to CTS and the nearly same number of limbs of normal controls. There was no significant difference in ulnar SNCV among 3 groups. However, ulnar SNCV at wrist was significantly reduced in patients with CTS as compared with normal value. In 17 limbs with CTS, vibration threshold of index finger tips was significantly increased, and 8 of those limbs were found to have concomitant increase of vibration threshold of little finger tips. Most of patients with increase of vibration threshold of both index and little finger tips were those who used to expose to repetitive mechanical stimuli at the wrist in their occupation. It was concluded that digital vibration threshold is simple to measure yet sensitive enough to detect asymptomatic ulnar neuropathy in CTS.
