Increased compliance of niceritrol treatment by addition of aspirin: relationship between changes in prostaglandins and skin flushing.

The relation of plasma levels of prostaglandins to the occurrence of flushing induced by niceritrol was investigated. Niceritrol increased plasma levels of PGE2 (p less than 0.01) and 6 keto-PGF1 alpha (p less than 0.05) in 10 male subjects and aspirin reduced the level of PGE2 (p less than 0.01). Five of 10 subjects had flushing, and aspirin reduced flushing in 4 subjects. On the basis of the above study, we treated 35 hyperlipidemic patients with niceritrol in combination with aspirin, investigating the effect of the treatment of serum lipids and postheparin lipolytic activity. None of the 12 cases given aspirin from the start of the treatment experienced flushing, whereas 9 of the 23 cases not given aspirin experienced flushing, which was suppressed by adding aspirin in prescription in all cases except one. Niceritrol decreased serum cholesterol, triglyceride and atherogenic index. It also increased HDL2 cholesterol and decreased HDL3 cholesterol. The LPL activity in postheparin plasma increased in all cases after niceritrol treatment. In conclusion, aspirin increased compliance of niceritrol by reducing the occurrence of flushing probably due to the decreased levels of prostaglandins, yielding favorable results for the long-term treatment of hyperlipidemia with a sufficient doses of niceritrol.

Nasal allergy and nasal polyp with special reference to the mucosal IgE antibodies.

Immunologic conditions were studied in 8 patients with nasal allergies and nasal polyps and in 20 patients with nasal polyps. The present study showed the following: Nasal polyp mucosa contains a low level of mucosal IgE antibodies; nasal polyp mucosa is devoid of or deficient in the capacity of producing IgE antibodies; the presence of mucosal IgE antibodies in nasal polyp mucosa does not always imply the clinical manifestation of nasal allergies, and the antigen and antibody interaction does not always result in nasal symptoms.

[A case of fibrous hamartoma of infancy and a review of the Japanese literature].

A case of fibrous hamartoma of infancy in the left axilla of an 18-month-old boy, is reported with some discussion (citations of references). The tumor was situated in the subcutaneous fatty tissue, and its border was unclear. It was removed easily and found to measure 2.5 X 1.5 X 1.5 cm. Histologically, in addition to the three main components, i.e., trabeculae of fibrous tissue, immature mesenchymal cells with a mucoid matrix and interspersed mature fat, a capillary vasoformative proliferation, peripheral nerve fibers and lymph follicle-like structures mingling with and transforming into one another were noted throughout the tumor. On the basis of these findings, immature mesenchymal cells were considered as having very important roles with regard to the histogenesis of fibrous hamartoma of infancy.

Topical immunology of nasal allergy and mucosal IgE antibodies.

In the present study, the in vitro tissue-radioallergosorbent test (t-RAST) was performed in two groups of patients: one with perennial attacks of sneezing, serous hypersecretion and nasal congestion, the other with nasal congestion only. The results obtained were compared with those obtained by a series of conventional allergy tests. We then found that t-RAST provided objective data comparable to those obtained with serum-RAST and that the t-RAST is a reliable means of quantitatively detecting specific IgE antibodies in the nasal mucosa. t-RAST is of special value to diagnosticians because it is able to discern unequivocally and easily those patients with localized nasal allergy.

Ciliary activity in patients with nasal allergies.

Nasal allergy is a disease ensuing from type-I allergic reactions in the nasal cavity. A correct understanding of this process requires further investigation of the non-allergic pathology present. In the allergic state, the function of the nasal cilia is considered to be involved in the invasion of allergens into the nasal mucosa, and may also be involved with the manifestation and progression of the disease produced. In the present study, we examined the ciliary activity of the nasal mucosa of 35 patients with nasal allergies by using the photo-electric method of Ohashi and Nakai to better understand the non-allergic pathologies present. The nasal cilia of four healthy volunteers served as controls. In addition, a multivalent analysis was made according to quantifying theory I in order to define the effects of various factors on ciliary activity in the nasal mucosa during the allergic state. The following results were found in our present study. Ciliary activity was observed in 32 of 35 patients with nasal allergy. The mean value of the ciliary activity in these patients was 498 +/- 195 beats/min, while that of the normal controls was 753 +/- 46 beats/min. The ciliary activity in the nasal mucosa in the allergic state declined as the morbid period was prolonged; this decline in ciliary activity was relatively great in cases of perennial allergies aggravated by season-specific allergens.

The ciliary activity of the middle ear lining--functional and morphological observation.

Since the middle ear lining is an extension and a modification of the respiratory mucosa, it is conceivable that it has a mucociliary system and it plays an important role in clearance of the tympanic cavity. It was already noted in morphological studies that the middle ear lining had ciliated cells. To our knowledge, however, ciliary activity has never been observed directly. In the present paper, the ciliary activity of the middle ear mucosa was investigated directly and quantitatively by the photo-electric method of Ohashi and Nakai. In this work, special reference was made to the frequency of ciliary beating at various sites within the tympanic cavity. Ciliary activity was found to exist in the Eustachian tube and middle ear same as in other respiratory epithelium and this activity by ciliated cells was more active as their location became more distal to the Eustachian tube. Although the present results are preliminary, the techniques used in this study will provide a new avenue of the investigation of pathological middle ear lining.

The ciliary activity of the middle ear lining in some pathological states.

The ciliary activity in some pathological middle ear lining was discussed with special reference to the morphology of the epithelial cells in this paper. The changes in ciliary activity due to in vitro Staphylococcus aureus injection did not practically show any differences between the sites. On the other hand, the time-course changes in ciliary activity after in vivo S. aureus injection were found to differ between the sites. Ciliary activity was rather accelerated at the proximal site, while it was decelerated at the distal site. Few changes but for sporadic small compound cilia were observed at the proximal site, and some inflammatory changes were noted at the epithelium of the distal site. SO2 exposure affected the middle ear lining of rabbits. Fourteen days after 4 week's exposure to SO2, the middle ear lining displayed declined ciliary activity and many signs suggestive of secretory asthenia. In addition, the middle ear lining of rabbits given about 600 mg carbocysteine, both during and 2 weeks after the exposure displayed excellent ciliary activity and normal morphology of the epithelial structure.

Electron microscopic observation of gastric mucosal capillaries in diabetics--relationship between diabetic microangiopathy and complications following gastrectomy.

An electron microscopic study was made of the gastric mucosal capillaries (GMC) of diabetic patients. Ultrastructural findings, including diabetic microangiopathy (DMA), and clinical features of these diabetic patients were investigated in relation to anastomotic leakage after gastrectomy. The gastric mucosa of resected stomachs from 12 diabetics and 10 nondiabetics were examined. Ultrastructurally, DMA was demonstrated in the stomachs of diabetics, in which the basement membrane of GMC was thicker. The degree of this DMA in the stomach significantly correlated with the duration of diabetes and the degree of DMA found in the skin, but not with age and fasting plasma glucose (FPG) level. Marked DMA was observed with significant frequency in diabetics with anastomotic leakage after gastrectomy. In our hospital, anastomotic leakage after gastrectomy occurred in 9 out of 53 diabetics. Diabetics with anastomotic leakage had severe FPG, a long duration of diabetes and diabetic retinopathy. It seems reasonable to presume that anastomotic leakage after gastrectomy occurs more frequently in diabetics with ultrastructurally severe DMA, clinically severe FPG and relatively long duration of diabetes.

[Biological and clinical analysis of breast cancer in relation to hormone receptors].

We examined whether or not the presence of hormone receptors (ERC, PRC, and ERN) were related to morphological and functional features in breast cancers. Well-differentiated tumors were more frequently receptor positive while poorly differentiated tumors were generally receptor negative. And labelling index and receptor contents were negatively correlated. There was a significant inverse correlation between the receptor contents and numbers of intracellular organellas in ultrastructural features of breast cancer cells. In LMI tests by 3-M KCl cancer extracts, migration of leucocyte from patients with poorly differentiated tumors were more frequently inhibited than those well differentiated tumors. Patients with receptor positive tumors were more endocrine sensitive and had better prognosis.

Plasma concentrations of lidocaine and its principal metabolites during continuous epidural infusion of lidocaine with or without epinephrine.

BACKGROUND AND OBJECTIVES: The purpose of this study was to evaluate the effect of epinephrine on the absorption of lidocaine and the accumulation of active metabolites of lidocaine during continuous epidural anesthesia. METHODS: Lidocaine was administered as an initial bolus of 5 mg/kg of 2% lidocaine solution followed by continuous infusion at 2.5 mg/kg/h. Patients in group I (n = 10) received lidocaine alone and patients in group II (n = 10) received lidocaine + epinephrine (5 pg/mL). Concentrations of lidocaine and its active metabolites, monoethylglycinexylidide (MEGX) and glycinexylidide (GX), were measured in plasma samples obtained after 15 minutes, 30 minutes, and 1, 2, and 3 hours of infusion using high-performance liquid chromatography with ultraviolet detection. RESULTS: Plasma lidocaine concentrations were higher in group I for the first 30 minutes; however, after 1 hour the levels were the same. Plasma MEGX and GX increased continuously in both groups. MEGX levels the were significantly higher in group I, but there was no significant difference in the sum of lidocaine + MEGX after 2 hours. There was no significant difference in GX levels between the two groups. CONCLUSIONS: With respect to continuous epidural administration, addition of epinephrine to lidocaine solutions is ineffective after 2 hours for reducing the potential for systemic toxicity, because the sum of the plasma concentrations of lidocaine and its principal active metabolite, MEGX, are unaffected.

Effects of soy protein diet on the expression of adipose genes and plasma adiponectin.

Many studies have reported the cholesterol-lowering, anti-lipogenic, anti-obesity and anti-hypertensive effects of soy protein. Adipose tissue-specific plasma protein, adiponectin, has anti-atherogenic and anti-insulin-resistance properties. Here, we investigated the effects of soy protein diet on body fat composition, plasma glucose, lipid and adiponectin levels and expression of genes involved in glucose and fatty acid metabolism in obese KK-A y mice. Body weights and adipose tissue weights of mesenteric, epididymal, and brown fat were lower in mice on calorie-restricted diet containing soy protein isolate. Plasma cholesterol, triglyceride, free fatty acid, and glucose levels were also decreased by this diet. Body fat content and plasma glucose levels in mice on a soy protein isolate diet were still lower than those treated with an isocaloric casein-protein-diet. Among the genes related to glucose and fatty acid metabolism, adiponectin mRNA levels in adipose tissue and adiponectin plasma concentrations were elevated in mice on a calorie-restricted diet, although there were no significant differences between soy protein and casein protein groups. Our results indicate that that soy protein diet decreased body fat content and plasma glucose levels more effectively than isocaloric casein-protein diet in obese mice.
