[Current trends in therapy of colorectal cancers].

There is an increase of colorectal cancer incidence in the GDR. The rate of early diagnosis has not improved and the 5-year-survival rate has remained constant since 20 years. A multidisciplinary approach is mandatory: modern staging, typing, grading; screening in high-risk groups; therapy of the adenoma-cancer-sequence; adjuvant tumour therapy; improvement in preoperative care and surgical technique; advance in therapy of local recurrences and metastases; improved organization in postoperative care. Own experiences in these different fields with 154 patients suffering from rectal cancer are dealt with.

[Manometry studies in the evaluation of anal continence].

In a complex measuring program performing anorectal perfusion manometry a great number of parameters were determined in incontinent patients, patients after continence improving operations and in healthy persons. In this way, we were able to select 20 parameters in which healthy persons differ from incontinent patients, the difference occurring with a security of 99.0 to 99.9%. Our investigations were carried out with test persons at rest, after distension of the rectal ampulla, during abdominal straining and coughing. Rectoanal resting profiles and stress profiles were recorded and the measuring values obtained compared with an anamnestic score.

[Manometry studies of continence performance in patients following gracilisplasty].

Out of a total of 10 patients who had undergone a Pickrell-procedure of the gracilis muscle from 1970-1980, 6 were now followed-up by clinical examination and perfusion manometry of the anal canal. The manometric parameters were compared with an anamnestic score. For anal continence the degree of resting activity of the transposed gracilis muscle is in our opinion not decisive. For that reason, there is no necessity for a permanent contraction.

[Hypothermic perfusion and cold storage of the dog pancreas].

The results of 18-hours of hypothermic pulsatile perfusion and 18-hours of cold storage of canine pancreas are compared. Both methods of preservation yield the same endocrine pancreas function. However preservation of pancreas by perfusion results in considerable oedema of the gland and high enzyme activity in the perfusate. Therefore, cold storage should be used for the preservation of pancreas.

[Clinical course and therapy of acquired anal incontinence].

43 patients suffering from anal incontinence underwent surgical correction. The late results up to 14 years after surgery are described. Continence was evaluated by inquiries and by manometric readings of the recto-anal stress profile. Prevailing surgical procedures were reconstruction of the anal sphincter (n = 23) and pedicled transplantation of the M. gracilis (n = 10). Both methods yielded satisfactory late results.

[New trends in the therapy of colorectal cancer].

Cancers the sigmoid colon and rectum are detected in an increasing frequency within the population of German Democratic Republic. 6 groups of improvements in prophylaxis and therapy are discussed: modern staging, typing and grading as basis of planning of treatment; purposeful prophylaxis of developing cancers by endoscopic surgery; systematizing of screening examinations; improvements of the preoperative care; innovations of operative technique and finally the possibilities of adjuvant tumortherapy. This survey is based on own experiences in 63 patients with colonic carcinomas and 139 patients with rectal cancers in Greifswald in the years 1977 to 1983, inclusively 55 patients with preoperative irradiation.

Plasma insulin and glucose tolerance in pancreatectomized dogs after autologous partial pancreas transplantation. Loss of the insulinogenic reflex after oGTT and meal feeding.

A transplanted pancreas is not subjected to the influence of external innervation. It can therefore be used to study the mechanism governing the early phase of insulin secretion after oral glucose intake or meals, which is independent of any absorption causing increase in the plasma glucose level. Three totally pancreatectomized dogs had the proc. lienalis of the pancreases (35% of the total pancreas weight) transplanted autologously into the abdominal cavity. Later extirpation of the transplant showed that the transplant had been the only organ secreting insulin in the animals. The transplanted dogs had normal basal plasma glucose and IRI concentrations, a reduced IRI reaction and glucose tolerance after i.v. glucose application. The early reflex increase in IRI concentration after oral glucose intake or meal feeding was observed in all control animals but not, in contrast, in the transplanted animals. The authors conclude that the early insulin secretion phase is entirely due to nerval influences.

[Autogenous heterotopic pancreatic segment transplantation in dogs].

The own animal experimental investigations served the clearing of the most favourable variant of the operative technique in dog including organ perfusion and microsurgery. The middle-term endocrinic effects of a segmental transplant of nearly 35% of the pancreas from the dog were defined and the regulation of the endocrinic function of the denervated transplant was analysed. The morphological transformations in the transplant show new aspects by the exclusion of immunologic events. The evidence of the function of the transplant in vivo was led by the absolute lack of insulin after extirpating the transplant.

[Autogenous heterotopic pancreas segment transplantation in the dog].

In 50 mongrel dogs autogenous heterotopic transplantation of pancreatic segments was performed without duct ligation. Analyses of the endocrine function of the transplant up to 275 days yielded reduced glucose tolerance and simultaneously normoglycaemia as well as stimulation of insulin release following administration of glucose and no early reflex insulin incretion following oral glucose administration. The exocrine pancreas tissue disappeared nearly completely and inside big complexes of isles mitoses could be observed. The A:B-cell relation had changed. The reduction of the transplanted tissue mass to one third was also evident by angiography.

[Therapy of posttraumatic osteomyelitis. Present state (author's transl)].

Between 1977 and 1979 a total of 79 patients with osteomyelitis were seen at the Surgical Dept. of the Greifswald-University. 144 operations were performed, 93 to cure infections, 17 for stabilisation of bones (preferably by "Fixateur externe", 34 for closure of bone and soft tissue defects. Special attention is devoted to transplantation of autologous cancellous bone chips, crossleg-flaps and splitskin grafts for secondary wound cover.

[Differential therapeutic indications for the useof Xenoderm and SYSper-derm in burns and other skin defects. Commentary on the work of J. Maserova: a comparative study of several synthetic skin substitutes].

The experimental studies by Moserová yield an objective judgement of different biological and synthetic wound dressings and underline the importance of evaporation in the progression of dermal necrosis in the postburn period. Basing on the results of these findings and own experimental and clinical results recommandations are given concerning the use of Xenoderm and SYSpur-derm, both temporary skin substitutes of the GDR-production. The different indications for the application of the two materials are described.

[Soft tissue plastics in treatment of chronic osteomyelitis (author's transl)].

Soft tissue plastics are indispensable in the surgical treatment oft chronic osteomyelitis (56 cases in 10 years). Myoplastics (27 operations) are simple, successful, but only suitable with stabile bones. The treatment of osteomyelitis in the distal parts of the extremities often requires additional skin flaps (25 operations), rarely free skin grafts.

[Lyophilized split pigskin for biological wound dressings (author's transl)].

Experimental and clinical investigations of an unviable split pigskin preserve, produced by a fairly simple industrial process, outline the following results: The skin preserve protects a wound surface against infection and prevents loss of fluid. The reaction of the wound surface is very restricted even after repeated application, whilst the viable pigskin releases a more intense host reaction.
