[Female hyperprolactinaemia. 19 cases (author's transl)].

The authors were able to confirm the ineffectiveness of vitamin B6. The correction of hyperprolactinaemia by bromocriptine restored ovarian function immediately, or after the failure of surgical treatment. It would still be premature to determine the exact place of surgery in this type of pathology.

Anterior uveitis and multiple sclerosis.

Three patients with clinically definite multiple sclerosis and uveitis were studied. The uveitis presented as inflammation of the anterior segment of the eye (iridocyclitis) and was characterized by large keratic precipitates and posterior synechiae, consistent with a granulomatous iridocyclitis. Clinical and laboratory evidence militated against other central nervous system diseases often associated with uveitis including neurosarcoidosis, Behçet's syndrome, Vogt-Koyanagi-Harada disease, neurosyphilis, and tuberculosis. Further attention should be directed to the possibility of granulomatous anterior uveitis in patients with multiple sclerosis.

Endoscopic patterns of primary gastric MALT lymphoma.

Primary low grade B-cell lymphoma of the mucosa associated lymphoid tissue (MALT), is an uncommon malignancy with a prolonged period of localized disease, and a possible transition to a high grade MALT lymphoma. The endoscopic diagnosis is often difficult, because of the various macroscopy patterns. The endoscopic aspects seen in 14 patients diagnosed and treated for primary gastric MALT lymphomas, were reviewed. Four main patterns were recognized: 1/Large ulcerations (n = 6, 42.9%), 2/Polypoid lesion (n = 2, 14.3%), 3/Gastritis with small ulcerations (n = 4, 28.5%), and 4/Atypical mucosal relief (AMR): erythema, nodularity, edematous or infiltrated folds (n = 2, 14.3%). In 10 cases (71%) AMR was associated with another endoscopic pattern. Histologically the tumors were of low grade (n = 11) and high grade (n = 3). On initial endoscopy a diagnosis of malignancy was done in 6 out of the 7 patients (85.7%) with large ulcers and the large polypoid mass; in contrast, an adequate endoscopic diagnosis was made in only 3 out of the 7 patients (42.8%) with ulcerative gastritis, AMR, and the small polypoid lesion. The endoscopic aspect of the latter group, representing 50% of all patients and 63.6% of the group with low grade MALT lymphomas, was indistinguishable from a benign lesion. Therefore it is important to be aware of this entity and perform biopsies in every patients with abnormal endoscopic findings.

Scanning and light microscopic study of irrigated and nonirrigated joints following burr surgery performed through a small incision.

Research was conducted on young, adult male rabbits where osseous burr surgery was performed through a small incision using two methods of joint debris extrusion. Surgical joint debris was found in all specimens at 1 week following surgery. Also, 1 week postoperatively, the surgically untouched chondral and subchondral joint aspects were intact in both irrigated and nonirrigated specimens. There was, however, subchondral bone inflammation in all specimens but the nonirrigated population had a greater inflammatory subchondral infiltrate. The synovium was more inflamed in the nonirrigated group. Bone particles, observed with scanning electron microscopy, generally appeared to be only slightly more evident in the nonirrigated group. Metal particles were observed in several specimens. By 28 weeks, joint debris was practically undetectable. In the irrigated specimens the chondral and subchondral portions presented with occasional degenerative focal loss of the chondral and subchondral layers. In comparison, the nonirrigated group demonstrated not only occasional but moderate-to-frequent focal loss with fissuring in the chondral and subchondral layers in addition to extrinsic repair of the cartilage surface. Subchondral bone inflammation was also more severe in the nonirrigated group. Observations from this project confirm that joint debris is removed through intrinsic joint processes and further suggests that the postoperative course of inflammation is not a totally abrasion-related phenomenon. The findings support the use of intraoperative irrigation as opposed to debris extrusion without irrigation in order to reduce both immediate and chronic postsurgical inflammation.

[Hormonal profile and HLA typing of adult subjects from 7 families with congenital adrenal hyperplasia caused by 21-hydroxylase deficiency].

Most members of seven families in whom one subject was known to have congenital adrenal hyperplasia due to 21-hydroxylase deficiency were haplotyped for HLA B antigen which was supposed to be linked to the defective gene. Heterozygous males (group 1, n = 9), and females (group 3, n = 8) were recognized. These had no clinical abnormalities. All heterozygotes, as well as eight normal males (group 2) and ten normal females (group 4) received in IM injection of Synacthen. Plasma samples obtained before (PRE) and three hours after (POST) the injection was assayed for cortisol, testosterone, androstenedione, 17-hydroxyprogesterone, and dehydroepiandrosterone. All steroids increased significantly after ACTH except for testosterone. The ratio cortisol/17-OHP (F/17-OHP) decreased in group 1. POST 17-OHP (ng/ml) (x +/- SEM) was higher and F/17-OHP lower in groups 1 and 3 respectively: (5.83 +/- 1.83, 60 +/- 20; 4.53 +/- 1.13, 81 +/- 34) than in groups 2 and 4: (1.95 +/- 0.72, 196 +/- 67; 1.94 +/- 0.65, 269 +/- 167). POST F levels were not different among groups. When the distributions were standardized, individual POST 17-OHP levels of heterozygotes were different from the mean POST 17-OHP of the respective normal group and vice versa. The two normal males from the affected families could also be separated from heterozygous but not from normal males.

[Cis-platinum in the treatment of disseminated testicular neoplasms resistant to classical chemotherapy].

Seven patients with advanced testicular tumors, resistant to existing chemotheurapeutic agents, were treated with a new antitumor agent, the Cisplatinum (associated to mannitol induced hyperdiuresis). There were 3 objective remissions (1 complete and 2 partial). Major toxicity was gastrointestinal. There was little renal and hematologic toxicity in our serie. The drug may have use in combination therapy in advanced non seminomatous testicular tumors.
